Abstract

Electronic billing simplifies and speeds up patient billing. This study will evaluate the system's performance, information, economics, control/security, efficiency, and service (PIECES) using the analytical model. This study examines RSU Royal Prima Medan's PIECES hospital billing system's outpatient and inpatient payment recording. A mixed-methodologies study uses quantitative and qualitative methods. This study sampled 200 patients quantitatively and seven informants qualitatively. Quantitative data analysis employed univariate analysis, while qualitative used triangulation. This study found that RSU Royal Prima Medan's hospital billing system for outpatient and inpatient payments performed well in performance, information, economics, control, efficiency, and service. The challenges include rare network problems. This study found that RSU Royal Prima Medan's PIECES hospital billing system for outpatient and inpatient payments is working well. RSU Royal Prima Medan should develop the system to properly use it in the future by training officers, evaluating them, and supervising them to reduce errors and maximize patient care.

Abstract

Hypertension causes the deaths of 8 million people every year, of which 1.5 million deaths occur in Southeast Asia. Overall, the prevalence of hypertension is around 30-45% in adults and the prevalence increases progressively to >60% at ages >60 years. Hypertension is one of the main causes of mortality and morbidity in Indonesia, so it is needed at various levels of health care facilities in an effort to reduce the prevalence and incidence of cardiocerebrovascular disease. The aim of this research is to analyze the relationship between smoking, stress, alcohol, obesity, dysplidemia and hypertension in the elderly at Pustu Desa Deli Tua Dusun IV in 2024. This type of research is analytical quantitative research using a cross sectional approach. The population in this study are elderly people who experience hypertension at Pustu Desa Deli Tua IV in 2024, totaling 95 elderly people. Sampling used total sampling with a total sample of 95 elderly people. The data analysis used in this research is univariate, bivariate and multivariate analysis. The results of the research show that there is a relationship between smoking, stress, obesity and dysplidemia on hypertension in the elderly at the Deli Tua Village Dusun IV Pustu in 2024, but there is no relationship between alcohol and hypertension in the elderly at the Deli Tua Village Dusun IV Pustu in 2024. The variables included The most related to hypertension in the elderly at Pustu Desa Deli Tua Dusun IV in 2024 is the obesity variable (X4). From the results of the research that has been carried out, the suggestions that can be given to elderly hypertensive sufferers are to better maintain their lifestyle by reducing stress, reducing smoking for elderly people who still smoke, maintaining their diet for elderly people who are obese and it is also recommended that they do light exercise with as regular as walking in the morning.

Abstract

Patient loyalty is a critical factor in ensuring the sustainability and competitiveness of hospitals, particularly within health services under the National Health Insurance (JKN) program such as BPJS Kesehatan. According to the 2022 report by the Indonesian Hospital Association (PERSI), the patient retention rate in Indonesian hospitals is only 56%, while the remaining 44% switch to other health facilities due to dissatisfaction with service quality, costs, and accessibility. This phenomenon reflects relatively low patient loyalty, especially among BPJS participants. This study employed a quantitative approach with a cross-sectional design and was conducted at Bandung General Hospital Medan, involving 260 inpatients purposively selected from a total population of 341 patients. Data analysis was performed in three stages: univariate, bivariate using the Chi-Square test, and multivariate using multiple logistic regression. The results revealed four factors that significantly influenced BPJS patient loyalty: satisfaction drivers, emotional response, health worker performance, and relationship quality. Among these, satisfaction drivers emerged as the most dominant factor (OR = 7.611). In contrast, perceived value, expectations, service recovery, and brand image did not show a significant effect. These findings suggest that BPJS patient loyalty is more strongly shaped by direct experiences and emotional interactions during the service process than by external attributes such as brand image or initial expectations.

Abstract

Young doctors often have different perceptions of the effectiveness of clinical learning in medical professional education. Clinical learning is a crucial stage in shaping the competence of young doctors, in terms of knowledge, skills, and professionalism. This study aims to explore young doctors' perceptions of the effectiveness of the clinical learning system at Royal Prima General Hospital in Medan. This qualitative study with a phenomenological approach involved six informants (five young doctors and one clinic coordinator) who were selected purposively. Data were collected through in- depth interviews, focus group discussions, and observations. The results of the study revealed four main themes. First, the quality of clinical learning is influenced by the clarity of instructions, tutor involvement, patient availability, and rotation structure. Second, the learning environment is influenced by the relationship with the supervisor and the clinical work atmosphere. Third, the young doctors' self-competence includes self-confidence, readiness to face patients, and stress management strategies. Fourth, supporting factors (senior support, hospital facilities) and inhibiting factors (patient case variations, high workload, differences in tutor supervision styles) also influence the effectiveness of clinical learning. In conclusion, the effectiveness of clinical learning is influenced by the interaction of instructional, personal, environmental, and structural factors. Therefore, improvement efforts are needed in the form of strengthening tutor supervision skills, equalizing practice opportunities, and providing psychosocial support for young doctors in order to optimize the clinical learning system.

Abstract

This study aims to analyze the role of the Medical Committee in improving the quality of health services at Dr. Pirngadi Hospital, Medan City. The Medical Committee plays an important role in maintaining the professionalism of medical personnel and ensuring quality medical services through medical audits, continuing education, scientific meetings, and mentoring new medical personnel. Using a qualitative descriptive method with an in-depth interview approach to informants consisting of medical staff, doctors, patients, and nurses. This study shows that the role of the Medical Committee at Dr. Pirngadi Hospital has been carried out well, especially in medical audits, although there are obstacles in implementing follow-up audit results due to limited time and facilities. The role of the Medical Committee in organizing scientific meetings and continuing education is still limited and not made a routine program, while the mentoring process (proctoring) for new medical personnel has not been well structured. Through this study, it is clear that although the Medical Committee has played a role in improving the quality of services through medical audits, there is still room for improvement, especially in terms of continuing education and proctoring new medical personnel.
