Abstract

            Currently, Indonesia is in the fifth rank of highest TB prevalence over the world. One of the TB problem is low patients’ adherence due to the oral antituberculosis induced hepatotoxicity. Polymorphisms of NAT2 and CYP2E1 genes had important role in the isoniazid (INH)-induced hepatotoxicity. The aim of this study was to evaluate the polymorphisms profile of NAT2 and CYP2E1 genes associated with hepatotoxicity induced by INH.                         We used cohort design in Public Health Centers and Lung Clinics of Yogyakarta and Lampung. The inclusion criteria were adult subjects (> 18 yo), newly diagnosed TB and treated by oral antituberculosis, normal function of renal and live and willingness to participate in this study. Subjects were excluded when having positive reaction of  HbsAg test, history of HIV and abnormality of renal and liver function. The SNPs of NAT2 and CYP2E1 were designed using IPlex method of DNA sequenom.             Among 57 TB patients, we found 14 patients with higher INH serum concentration and experienced increase of ALT-AST. Subjects with  SNPs of rs 2070676, rs 1329149, rs 3813867, rs 6413432, rs 8192772, rs 2031920, rs 2515641, rs 8192775, rs 915908 of  CYP2E1  experienced increase of ALT and AST. Subjects with SNPs  of rs 1799930, rs1799931, rs1801279, rs1801280, rs1799929 , rs1208, rs1041983 of  NAT2 are associated with the increase of ALT and AST.             The polymorphisms of  CYP2E1 and NAT2 may have a role in the mechanisms of INH induced DILI. Keywords: CYP2E1, NAT2, tuberculosis, isoniazid, Indonesia

Abstract

The research about cancer patients’ quality of life on before and after therapy of high emetogenic cytotostatic in RSUP Dr Sardjito Yogyakarta by EORTC QLQ-C30 has been done.This research used cohort design with cancer patients who just the first time received cisplatin ≥ 50 mg/m2 and or its combination, and also got ondansetron-dexamethasone as anti-emetics. While the patients were given EORTC QLQ-C30 questionnaire before got cisplatin (before chemotherapy) and 5 days after got cisplatin (after chemotherapy). The difference of the quality of the life will be analysed with paired t-test with 95 % level of significancy. The correlation between the domains before and after chemotherapy was analysed by Pearson correlation test with the 95 % level of significancy.We got 38 patients during 2 months with the cancer diagnoses of cervix, nasopharyng, ovarium and lung. Generally, the quality of life was nonsignificantly decreasing, whereas not all the domains were decreasing. The physical, emotions, cognitive and social domains were non-significantly increasing. The decline in the quality of life was more influenced by the role domain of the patients, pain, the decline in appetite and the financial difficulty. The correlation of fatigue, constipation and the financial difficulty were strong between the domain in before and the chemotherapy after was received in the symptom (r ≈ 0.50).Key words : quality of life, cancer, EORTC QLQ-C30.

Abstract

The aim of the research was to know the description of antiemetic usage on the patients with cancer based on the medical records that were available in the hospital. Hereby, the use of antiemetic would be compared to literature for seeing the pattern.The study was a descriptive, analytic and retrospective study. The data taken from the medical records were written by doctor and nurses for cancer in patients hospital during Januari to June 2003. Data were grouped based on cytotoxic treatment with strong, medium and weak emetogenic effects, the condition of cancer patients, and antiemetic usage before and after having cytotoxic treatment.The results showed antiemetic usage before and after strong emetogenic cytotoxic treatment were not suitable because there was no combination with dexametason. Antiemetic usage which was used before and after mild emetogenic cytotoxic treatment were not suitable with ondansetron, metochlorpramide, combination of ondansetron – metochlorpramide. The use of antiemetic on cancer patients who got strong and medium emetogenic cytostatic was not been suitable with literatures because the patients were not given dexamethasone as combination therapy. For cancer patients given before and after medical treatment with weak emetogenic effect weren’t suitable in both of hospitals. Antiemetic usage before cytotoxic treatment that was not suitable was ondansetron and metoclopramide, where as antiemetic usage after cytotoxic that was suitable was metoclopramide.Key words: Antiemetic, Emetogenic Effect, Cytotoxic, hospital
