Abstract

Background: Lack of knowledge and limited access to information about reproductive health make street children vulnerable to complex reproductive health problems, one of the problems of uncontrolled reproduction is free sex, drinking alcohol, unwanted pregnancy, abortion, Sexually Transmitted Infections (STIs), HIV/AIDS.Objective: This study aims to conduct a literature study to obtain the input on improvements in dealing with reproductive health problems in street children.Methods: This research was conducted in April 2021 by conducting literature review on various research results that have been published during January 2014 to December 2020 in national and international journals and obtained 10 research journals.Results: The results of several studies to develop reproductive health on street children from various sectors illustrate that the knowledge of street children about reproductive health is still very lacking. PKPR as the basic sector in the development of reproductive health has not touched street children. The health department as a pioneer of health reproduction has also not launched a reproductive health program for them. Social services as the father of street children have not touched on his reproductive health problems, so far social services are still focusing on the social welfare of street children.Conclusion: Health as a component is no less important in the problem of street children, especially in their reproductive health. Therefore, we need cross-sector collaboration to manage reproductive health problems of street children.

Abstract

AbstrakAIDS (Acquired Immunodeficiency Syndrome) merupakan kumpulan penyakit yang disebabkan oleh Virus HIV (Human Immunodeficiency Virus) yang menular dan mematikan. Berdasarkan data Kementrian Kesehatan pada tahun 2015 jumlah anak usia ≤ 4 tahun yang terinfeksi HIV sebanyak 795, meningkat pada tahun 2016 menjadi 903. Pencegahan Penularan HIV dari Ibu ke Anak dapat dilakukan melalui 4 (empat) prong/kegiatan, yaitu : 1) Pencegahan penularan HIV pada perempuan usia reproduksi, 2) Pencegahan kehamilan yang tidak direncanakan pada ibu HIV positif, 3) pencegahan penularan HIV dari ibu hamil HIV positif ke bayi yang dikandung, dan 4) pemberian dukungan psikologis, sosial dan perawatan kepada ibu HIV positif beserta anak dan keluarganya. Pencegahan Penularan HIV dari Ibu ke Anak (PPIA) atau Prevention Mother to Child Transmission (PMTCT) merupakan program pemerintah untuk mencegah penularan virus HIV/AIDS dari ibu ke bayi yang dikandungnya. Tujuan dari literature review ini adalah untuk mengetahui konsep, model atau teori yang efektif digunakan untuk mengeketahui gambaran indeks keluarga sehat di beberapa daerah Indonesia. Metode yang digunakan dengan cara melakukan pencarian beberapa studi yang diterbitkan melalui database Google Scholar, Science direct, dan Scopus. Studi yang dipilih diterbitkan dari tahun 2017-2021. Setelah dilakukan pencarian artikel dengan kata kunci tersebut maka total artikel yang di review dalam tinjauan literatur ini sebanyak 5 (lima) artikel. Faktor yang mempengaruhi pelaksanaan PMTCT adalah kelengkapan data, kualitas SDM, gender, serta dukungan keluarga. The purpose of this literature review is to know the concept, model or theory that is effectively used to know the picture of healthy family index in some areas of Indonesia. The method is used by searching for several studies published through the Google Scholar, Science direct, and Scopus databases. Selected studies are published from 2017-2021. After searching for articles with these keywords, the total articles reviewed in this literature review as many as 5 (five) articles. Factors that affect the implementation of PMTCT are the completeness of data, the quality of human resources, gender, and family support.Keywords: Evaluation, Implementation, PMTCT

Abstract

AbstrakBerdasarkan data jumlah kasus HIV/ AIDS pada ibu hamil dari Dinas Kesehatan Kabupaten Demak pada tahun 2020 yaitu dari 21.980 ibu hamil yang dilakukan pemeriksaan sebanyak 17.045 (77%). Didapatkan hasil bahwa Jumlah kasus HIV/ AIDS pada ibu hamil di Kabupaten Demak tahun 2020 adalah sebanyak 13 orang (16,3%). Sejak masa pandemi COVID-19 tahun 2019 kasus HIVAIDS pada ibu hamil sebanyak 5 orang (9,8%) dan tahun 2020 lebih banyak menjadi 13 orang (16,3%) yang tersebar di 9 Puskesmas. Sedangkan pada tahun 2021 dalam kurun waktu 5 bulan pertama ditemukan kasus baru HIV pada ibu hamil sebanyak 5 orang yang tersebar di 4 puskesmas. Berdasarkan data penularan HIV pada ibu kepada bayi diperoleh bahwa terdapat penularan HIV pada ibu kepada bayi pada tahun 2020 terdapat 2 bayi. program PMTCT di Kabupaten Demak dilakukan melalui 4 prong. Selama masa pandemi Covid-19 ini, dalam kurun waktu tahun 2019 hingga 2021 kurang maksimal, sehingga diperlukan penguatan program baik dari segi health promotion, konteks, input, proses, dan output. Tujuan penelitian ini menjelaskan untuk mengevaluasi Pelaksanaan Pelayanan Kegiatan PMTCT Pada Ibu Hamil dengan HIV di wilayah kerja Dinas Kesehatan Kabupaten Demak. Metode dalam penelitian ini adalah studi kualitatif dengan rancangan studi kasus. Subjek dalam penelitian ini merupakan petugas yang terlibat dalam pelayanan PMTCT pada ibu hamil dengan HIV dan sudah mendapatkan pelatihan PDP meliputi 6 Puskesmas yang terdapat kasus ibu hamil HIV positif dalam kurun waktu 2020 hingga 2021. Pengumpulan data menggunakan wawancara mendalam pada informan utama pemegang program HIV dari 6 puskesmas, dan informan triangulasi Kepala puskesmas, Pemegang program HIV Dinas Kesehatan Kabupaten Demak, pemegang program Kesehatan Ibu Dinas Kesehatan, Bidan Koordinator Rumah Sakit, dan ibu hamil HIV. Hasil penelitian yang dilakukan menunjukkan sebagian besar pelayanan PMTCT masih berjalan meskipun ditemukan beberapa kendala, hal ini dapat dilihat berdasarkan variabel health promotion, media promosi yang kurang menarik dan belum maksimalnya kunjungan rumah oleh petugas di masa pandemi dalam melakukan promosi kesehatan sehingga ibu hamil kurang mengerti. Pada variabel konteks masih adanya stigma negative terhadap ibu hamil HIV. Pada variabel input yang terdiri dari sumber daya manusia masih ditemukannya petugas yang berpindah tempat kerja sehingga petugas yang baru belum mendapatkan pelatihan, pada aspek money berasal dari dana BOK yang penganggarannya tergabung dengan program TB dan penyerapannya belum maksimal, pada aspek sarana dan prasarana mengenai ketersediaan reagen mengalami keterlambatan dalam pendistribusiannya, pada aspek method masih ditemukan pada beberapa puskesmas yang belum memiliki SOP. Pada aspek pencatatan dan pelaporan sering terjadi keterlambatan dalam mengirimkan laporan selama masa pandemi Covid – 19. Pada variabel produk masih ditemukan adanya penularan HIV dari ibu ke anak sebanyak 1 orang. Dengan demikian, perlu adanya perencanaan pelayanan PMTCT baik dari tingkat puskesmas maupun Dinas Kesehatan kabupaten Demak sehingga tidak ditemukan adanya penularan HIV dari ibu ke anak.Kata Kunci: Evaluasi, PMTCT, ibu hamil, Pandemi COVID – 19 AbstractBased on data on the number of HIV/AIDS cases in pregnant women from the Demak District Health Office in 2020, from 21,980 pregnant women, 17,045 (77%). Meanwhile, in 2021, within the first 5 months, 5 new cases of HIV were found in pregnant women spread over 4 public health centers. Based on data on HIV transmission from mother to baby, it was found that there were 2 babies transmitting HIV from mother to baby in 2020. PMTCT program in Demak Regency is carried out through 4 prongs. During the Covid-19 pandemic, the period from 2019 to 2021 was not optimal, so program strengthening is needed both in terms of health promotion, context, input, process, and output. The purpose of this study is to evaluate the implementation of PMTCT activities for pregnant women with HIV in the working area of the Demak District Health Office. The method in this research is a qualitative study with a case study design. The subjects in this study were officers who were involved in PMTCT services for pregnant women with HIV and had received PDP training covering 6 public health centers with cases of HIV positive pregnant women from 2020 to 2021. Data collection used in-depth interviews with key informants holding HIV programs from 6 public health centers, and triangulation informants from the Head of public health centers, HIV program holders from the Demak District Health Office, holders of the Maternal Health program at the Health Office, Hospital Coordinator Midwives, and HIV pregnant women. The results of the research conducted showed that most of the PMTCT services were still running even though several obstacles were found, this could be seen based on the health promotion variable, promotional media that were less attractive and home visits by officers during the pandemic were not maximal in conducting health promotion so that pregnant women did not understand. In the context variable, there is still a negative stigma against HIV pregnant women. In the input variable consisting of human resources, there are still officers who change places of work so that new officers have not received training, in the money aspect it comes from BOK funds whose budgeting is combined with the TB program and the absorption has not been maximized, in the aspect of facilities and infrastructure regarding the availability of reagents experienced delays in its distribution, in the method aspect it was still found in several health centers that did not yet have SOPs. In the aspect of recording and reporting, there are often delays in submitting reports during the Covid-19 pandemic. In the product variable, it is still found that there is 1 person transmitting HIV from mother to child. Thus, there is a need for planning for PMTCT services from both the public health centers level and the Demak district health office so that there is no mother-to-child transmission of HIV. Keywords: Evaluation, PMTCT, pregnant women, Covid-19 pandemic

Abstract

Background : Gonorrhoea is a health problem with increasing rates worldwide. The phenomena of increasing and spreading gonorrhoea cases affect the high risk groups including Indirect Female Sex Workers (IFSWs). The aim of this study was to explain some host factors as the risk factors of gonorrhoea in IFSWs in Wonosobo.Methods : This is an observational research with control case design. The study population was IFSWs who worked as lady companions in karaoke under the guidance of NGO Wonosobo Youth Center and had done the STI examination by STI clinic. The subjects were 84 consist of 42 case and 42 control. Samples were collected by consecutive sampling technique. Data analysis used chi-square, Odd Ratio (OR) and logistic regression.Results : Influencing factors to the occurence of gonorrhoea in IFSWs were history ofcombination sexual activity (p=0,013; OR=3,843; 95% CI= 1,323-11,63), history of STI (p=0,013; OR=3,863; 95% CI= 1,328-11,240), and consistency of condom use (p=0,009; OR=5,778; 95% CI=1,553-21,491).Conclusion : Factors that had been proven to influence the occurence of gonorrhoea in ISFWs were history of combination sexual activity, history of STI, and consistency of condom use.

Abstract

Waria menjadi kelompok dengan risiko tinggi penularan HIV AIDS disebabkan banyaknya daerah pangkalan bagi waria untuk melakukan transaksi seksual dengan pasangan seks. Risiko penularan HIV AIDS besar terjadi pada waria pekerja seks, sehingga untuk mencegah penularan HIV AIDS dapat dilakukan melalui seks yang sehat dengan pemakaian kondom dan pelicin secara rutin dan benar. Perubahan perilaku dikalangan waria pekerja seks masih sangat sulit karena masih rendahnya pemakaian kondom dikalangan waria. Tujuan penelitian adalah menganalisis hubungan persepsi manfaat yang dirasakan dengan konsistensi penggunaan kondom pada Waria Pekerja Seks untuk pencegahan HIV AIDS di kota Makassar. Jenis penelitian ini adalah penelitian mix methods dengan pendekatan kuantitatif dan kualitatif menggunakan desain sequential explanatory (model kombinasi). Teknik pengumpulan data kuantitatif menggunakan desain penelitian cross sectional dan data kualitatif menggunakan wawancara mendalam dengan rancangan penelitian menggunakan studi kasus untuk memperkuat hasil kuantitatif yang diperoleh dari Waria Pekerja Seks yang HIV positif. Jumlah responden dalam penelitian ini sebanyak 60 orang. Hasil penelitian menunjukkan bahwa ada hubungan persepsi manfaat yang dirasakan (p=0,013) dengan konsistensi penggunaan kondom pada Waria Pekerja Seks. Dapat disimpulkan bahwa persepsi manfaat yang dirasakan berhubungan dengan konsistensi penggunaan kondom pada Waria Pekerja Seks untuk pencegahan HIV AIDS.Kata Kunci: Konsistensi, HIV AIDS, Waria Pekerja Seks.

Abstract

A breastfeeding father is a father who takes part in supporting his wife to breastfeed the baby. The success of exclusive breastfeeding can be achieved if there is husband involvement in it. Breastfeeding really requires a stable emotional condition. The husband’s role in the breastfeeding process in Indonesia is still lacking so that the husband has to understand the importance of support for a mother. This study aims to analyze the factors of father support related to exclusive breastfeeding behavior in Semarang City. This study was an analytical study with a Cross-Sectional design using a Simple Random Sampling technique. The data were collected using a questionnaire. The Chi-Square Test was used for bivariate analysis and the logistic regression test for multivariate analysis. The study results showed that variables of knowledge, attitude, emotional support, and instrumental support were related to exclusive breastfeeding in Semarang City. The most dominant factor influencing breastfeeding behavior was instrumental support with an OR value = 52.044. Father awareness regarding the important role of the breastfeeding father in supporting exclusive breastfeeding is expected to be improved. Family and health workers must collaborate to encourage the important role of the breastfeeding father.

Abstract

Semarang Health Office in 2019 recorded the age group of 25-49 years old as the most group of people with HIV. Ironically, the productive age group is the asset of the nation, also considered as the ideal group for marriage so that premarital preparation is required to result in a healthy household. On the other hand, Catholic Church has a requirement for brides and grooms to participate in Wedding Preparation Course (KPP) and is expected to increase knowledge about the household readiness as well as encourage a willingness to do the premarital HIV test as a step of wedding preparation. This study aims to analyse factors influencing the intention of catholic brides and grooms to do the premarital HIV test. The results of the study showed that the variables that influenced the intention to do premarital HIV test were knowledge (α=0.005), behavioural beliefs (α=0.004), attitude (α=0.002), subjective norms (α=0.002), and behaviour control (α=0.007). The results of multivariate analysis showed that the most influential variables were knowledge (OR=5.222), continued by subjective norms (OR=4.631), and behavioural beliefs (OR=3.825). It is suggested that KPP can collaborate with Health Office, AIDS Commission (KPA), or Non-Governmental Organization (LSM) by conducting a sharing session.

Abstract

Background: Teenagers who live on the streets are groups in a society prone to sexual violence and reproductive health risks. Sexually active street teenagers often engage in sexual activity in any place, change partners, and do not use contraceptives to prevent pregnancy and the risk of sexually transmitted diseases. Objective: This study aimed to analyze factors related to risky sexual behavior of street teenagers in Semarang. Methods: This study used a cross-sectional design with a research population of 10-18 years old street teenagers who became street teenagers assisted by the Social Office of Semarang. A sample of 81 teenagers was selected using the total population sampling technique. Data analysis used the Chi-square test and logistic regression with the 95% confidence level. Results: The study results found a significant correlation between age, parental status, permissive attitude, risky health behavior, friends’ attitude, and friends’ risky reproductive health behavior with the risky sexual behavior of street teenagers in Semarang City (p<0.05). Multivariate analysis showed that the variables of friends’ attitudes and friends’ risky behaviors had simultaneous effects on street teenagers’ risky sexual behavior. Street teenagers who had friends with permissive attitudes were more at risk of engaging in sexual behavior 13 times than those who did not have permissive friends. Street teenagers who had friends with high-risk sexual behaviors were also ten times more likely to have risky sexual behaviors than those who had friends with low-risk behaviors.Conclusion: This study concluded that the variable of friends’ attitude had the most significant influence on risky sexual behavior of street teenagers in Semarang. The efforts were necessary to establish peer-educators related to reproductive health education for street teenagers to reduce reproductive health risks.

Abstract

Background: Dietary habits formed during adolescence have been shown to track into adulthood. Establishing healthy eating behaviours among adolescents, therefore, has both short- and long-term health benefits..Objectives: This study aims to identify dietary habits among adolescent girls aged 15-19 in rural area and its influence factorsMethod: This is a quantitative study with cross-sectional approach, involved 93 adolescent girls aged 15-19 in Jatipurwo village who were selected randomly. Data was collected by enumerators using structure questionnaires that has been validated in other village before.  Univariate, bivariate using Chi-Square and multivariate (logistic regression) were employed to analyze data.Results: The eating behavior of adolescent girls were categorized as unhealthy and unbalanced diet. It was shown with insufficient level of energy including low adequacy of carbohydrate, protein and fat. The average height of adolescents was 155 cm. Most adolescents have LILA less than 23.5cm. As Low socioeconomic family income tends to have eating habits with low nutritional value both in quantity and quality as well as food variations. Lack of food availability at the family level makes teens have poor diet. It is recommended that the need for increased food security at the family level and provide awareness to adolescents about healthy eating.

Abstract

Risiko terhadap kesehatan wisatawan dapat dipengaruhi oleh gaya hidup dan perilaku wisatawan Penelitian ini bertujuan untuk mengidentifikasi dan menganalisis faktor yang berpengaruh terhadap perilaku kesehatan pada wisatawan di kawasan Candi Borobudur Jawa Tengah. Penelitian ini menggunanakan desain Cross sectional. Pengambilan sampel dilakukan dengan menggunakan metode accidental sampling. Sampel dalam penelitian ini adalah wisatawan domestik yang berkunjung ke Candi Borobudur yang berusia > 17 tahun sebanyak 100 orang. Hasil penelitian menunjukan karakteristik wisatawan domestik sebagian besar berusia <30 tahun sebesar 64%, sebagian besar berjenis kelamin perempuan sebesar 71%, dan sebagian besar berasal dari Pulau Jawa sebesar 83%, faktor yang berhubungan dengan perilaku kesehatan wisatawan adalah pendidikan wisatawan dengan p-value 0,000, pengetahuan wisatawan dengan p-value 0,000, sikap wisatawan dengan p-value 0,000 dan budaya masyarakat sekitar p-value 0,000, faktor yang tidak berhubungan dengan perilaku kesehatan wisatawan adalah kepemilikan asuransi dengan p-value 0,097. Sedangkan faktor yang yang paling berpengaruh adalah budaya masyarakat sekitar dengan OR 64,264. Disimpulkan bahwa pendidikan, pengetahuan, sikap wisatawan serta budaya masyarakat sekitar berhubungan dengan perilaku keseahtan wisatawan, sedangkan kepemilikan asuransi kesehatan tidak berhubungan dengan perilaku wisatawan.
