Abstract

The National Health Insurance (JKN) requires efficient, equitable, and sustainable healthcare services. However, various studies indicate discrepancies between INA-CBGs tariffs and actual hospital service costs. One of the procedures with the highest claim rates is Cesarean Section (CS), which exhibits significant disparities. This study aims to analyze the average unit cost associated with Cesarean Sections (CS) across Indonesia using the Activity-Based Costing (ABC) method. A systematic literature review (SLR) approach was employed, analyzing articles published between 2017–2024. Relevant articles were retrieved through the Google Scholar database. Of the 79 articles initially identified, 37 passed the preliminary screening, and 12 articles met all inclusion criteria. Results indicate that INA-CBGs tariffs for CS procedures only cover 25%–54% of actual costs, varying by service class, hospital location, and operational efficiency. This mismatch contributes to financial deficits in hospitals, potentially compromising service quality, restricting medical equipment procurement, and increasing healthcare professionals’ workloads. Moreover, regional cost variations demonstrate that the current tariff system inadequately considers geographic factors and service complexity. Hospitals in major cities like Bandung report nearly double the unit costs compared to regions such as Yogyakarta or Lampung. Therefore, findings highlight the necessity of reformulating INA-CBGs tariffs to better reflect hospitals' actual conditions. Without updates based on cost-of-service, the financing system risks perpetuating service disparities and reducing the quality of national healthcare. The review findings indicate that the actual cost of Cesarean Section (CS) procedures in Indonesian hospitals is significantly higher than the INA-CBGs tariffs reimbursed by BPJS, with disparities ranging from 46% to 75%.

Abstract

Fraud poses a major threat to the integrity of Indonesia’s National Health Insurance–Healthy Indonesia Card (JKN-KIS) program, affecting health financing, hospital quality, and institutional reputation. Despite continuous efforts to combat fraud, allegations persist, particularly in hospitals in Palembang. This study aimed to identify potential fraud in hospitals in Palembang and analyze management countermeasures. Using a descriptive qualitative design with a phenomenological approach, the study was conducted from March to July 2022 and involved three hospitals in Palembang, the City Health Office, and the local branch of the Social Security Administrative Body for Health (BPJS Kesehatan). Data were collected through in-depth interviews with 16 informants representing hospitals, BPJS Kesehatan, and anti-fraud teams, supported by observations and document reviews. Thematic analysis revealed several fraud risks, including diagnosis manipulation, unnecessary treatment, and overcharging. Weak internal and external monitoring mechanisms fostered opportunities for fraud, often driven by financial pressure, revenue targets, and personal motives, and influenced by tenure and position within the organization. Fraud countermeasures remain suboptimal, although BPJS Kesehatan’s management shows potential that requires further improvement. Applying the Hexagon Vousina theory, this study emphasizes the need for a more comprehensive and integrated approach to fraud risk management. Strengthening oversight, professional ethics, inter-agency coordination, and policy enforcement is crucial to maintain the sustainability and effectiveness of the JKN-KIS program.

Abstract

Despite the strategic intent of Indonesia’s National Health Insurance (NHI) to strengthen chronic disease management through the Referral Back Programme (PRB), its implementation at primary health centres (Puskesmas) continues to face operational and systemic barriers. This study aimed to analyse the implementation of the PRB for chronic non-communicable diseases in Palembang City. The research team employed a qualitative case study approach to explore key challenges in programme execution. The research team collected data from six purposively selected Puskesmas between July and September 2022, using in-depth interviews, focus group discussions, observations, and document reviews. They selected twenty-one informants—including BPJS Kesehatan officials, physicians, programme coordinators, and patients—using maximum-variation sampling to ensure diverse stakeholder representation. The team conducted a thematic analysis, identifying three key themes. First, organisational capacity constraints, such as staff shortages, dual workloads, inadequate training, and limited funding. Second, there are infrastructural and systemic barriers, including the lack of dedicated service corners, underdeveloped digital systems, medicine stockouts, and restricted distribution of referral tools. Third, communication and coordination gaps exist, including weak inter-agency reporting, inadequate feedback mechanisms, and limited outreach to JKN participants. These themes align with the input–process–output framework: input barriers reflected fragmented resources; process barriers involved absent guidelines and reactive supervision; output challenges included low patient adherence and insufficient reporting. These interlinked factors undermine the gatekeeping function of Puskesmas and disrupt the continuity of care for chronic conditions. Strengthening institutional capacity, aligning resources, and integrating digital infrastructure are crucial for enhancing the effectiveness and sustainability of the PRB, thereby supporting Indonesia's progress toward universal health coverage.

Abstract

The oil and gas industry is a sector with a very high risk of occupational accidents due to its operational characteristics, which involve hazardous environments and unsafe worker behavior. Although Basic HSSE Training has become a mandatory standard in the oil and gas industry, evaluating its effectiveness remains urgent to ensure that the training is not merely a formality for regulatory compliance, but is truly aligned with the specific operational risks, work environment, and safety culture of Company. This study uniquely combines a pretest–posttest evaluation of Basic HSSE Training with an analysis of key organizational factors using multivariate statistical methods to identify dominant drivers of knowledge improvement among upstream oil and gas workers. This study aims to evaluate the effectiveness of Basic HSSE Training by measuring differences in workers' knowledge levels before (pre-test) and after (post-test) the training, as well as examining the influence of organizational factors management commitment, safety policies, regulations and procedures, HSSE communication, work environment, and worker involvement on knowledge improvement. A quantitative approach with a pretest–posttest design was employed, involving 303 workers at Company X in the upstream oil and gas sector in South Sumatra Province. Data were collected using Likert-scale questionnaires and multiple-choice tests. The Paired Samples t-test results indicate that Basic HSSE Training is effective in improving workers' knowledge, while Chi-Square and logistic regression analyzes show that organizational factors, particularly communication and work environment, play a dominant role in influencing the effectiveness of the training.

Abstract

Dental practices have a high risk of infected Covid-19 when providing dental and oral health services. The highest infections among all health care professions are dentists, dental assistants, and dental hygienists, because these professions are associated with patients and exposure to salivary secretions and aerosols. To be able to reduce the risk and the transmission of Covid-19 virus in dental practice, it is necessary to prevent and control infection during the Covid-19 pandemic. Therefore, the purpose of this study is to analyze infection control prevention in dental practices during the Covid-19 pandemic. This type of research is a literature review with a qualitative approach through descriptive analysis. The selection of articles according to the specified inclusion and exclusion criteria. The search process for this article was carried out using Google Scholar, Pubmed, DOAJ and Springerlink databases using the keywords "infection control, dental practice, and covid-19". The articles obtained from research year 2019 to 2021. From various literatures it is found that prevention efforts can be carried out in several ways, including through the application of triage to patients, patient management, use of personal protective equipment for dentists and clinical staff, and management of the practice environment.

Abstract

Kelompok rentan memiliki risiko dampak kesehatan yang berat dibandingkan dengan populasi lainnya apabila terinfeksi COVID-19. Langkah pencegahan penyebaran COVID-19 harus diterapkan dengan baik pada kelompok rentan, akan tetapi, kelompok rentan terdapat keterbatasan dari segi fisiologis dan psikologis sehingga dibutuhkan suatu dukungan eksternal untuk dapat memastikan dilakukannya langkah-langkah pencegahan transmisi COVID-19 pada kelompok tersebut. Salah satu bentuk dukungan ini adalah peran serta keluarga. Oleh karena itu, penelitian ini bertujuan untuk menganalisis pengetahuan masyarakat tentang melindungi diri dan keluarga di era new normal. Desain penelitian ini adalah quasi experimental pre and post-test design. Populasi pada penelitian ini adalah seluruh peserta seminar online, yaitu sebanyak 254 orang. Sampel penelitian adalah sebanyak 75 responden, dimana responden ini merupakan peserta seminar online yang mengisi quasi pre dan post-test melalui fitur aplikasi Google Form dan telah memenuhi kriteria penelitian. Hasil analisis deskriptif didapatkan karakteristik usia, jenis kelamin, serta pekerjaan. Distribusi berdasarkan usia terbanyak ditemukan pada kelompok usia yaitu 17-26 tahun yaitu 49 responden (65,3%), distribusi berdasarkan jenis kelamin banyak ditemukan pada wanita, yaitu 46 responden (61,3%), dan pekerjaan tertinggi didapatkan pada mahasiswa sebanyak 30 orang (40,0%). Hasil uji statistik Wilcoxon menunjukan bahwa ada perbedaan yang bermakna pada pengetahuan sebelum dan setelah mengikuti seminar online via WhatsApp terhadap keinginan melindungi diri dan keluarga di era new normal (p-value = 0,000) p<α.

Abstract

Telemedicine adalah salah satu bentuk pelayanan jarak jauh yang memerlukan teknologi sebagai salah satu upaya untuk mempercepat peningkatan akses dan kualitas pelayanan kesehatan. Telemedicine dipilih sebagai salah satu upaya untuk percepatan peningkatan akses dan kualitas pelayanan kesehatan di Indonesia. Penggunaan telemedicine merupakan salah satu kebijakan strategis untuk meminimalisir kontak fisik antara pasien dan petugas medis dalam upaya penanganan melawan wabah pandemi COVID-19. Hingga saat ini strategi tersebut ditujukan agar layanan kesehatan masyarakat tetap terjaga, tanpa meningkatkan potensi penularan COVID-19 di lingkungan fasilitas kesehatan. Penelitian ini berupa literature review dengan cara merujuk pada beberapa Jurnal dan artikel ilmiah sebagai data sekunder. Hasil menunjukkan bahwa Telemedicine mempunyai dampak positif bagi kesehatan masyarakat. Telemedicine semakin diminati oleh penggunanya karena mampu memudahkan tenaga medis maupun pengguna yang membutuhkan pelayanan kesehatan selama masa pandemi. Telemedicine memungkinkan untuk diterapkan di Indonesia dalam upaya peningkatan kualitas pelayanan kesehatan. Kesuksesan pelayanan telemedicine dipengaruhi oleh Pemerintah sebagai stakeholder dalam menerbitkan regulasi, konektivitas dan aksesibilitas guna mnejangkau layanan di seluruh wilayah dan sumber daya manusia yang handal dan berkompeten.

Abstract

Abstrak Konsumsi sayur dan buah diperlukan tubuh sebagai sumber vitamin, mineral, serat dan antioksidan sehingga dapat mengurangi terjadinya penyakit tidak menular seperti obesitas, kanker, kardiovaskular dan diabetes. Pemanfaatan tanaman pangan pada rumah tangga merupakan sayah satu upaya meningkatkan konsumsi sayur dan buah. Pengabdian masyarakat ini bertujuan untuk memberdayakan ibu rumah tangga melalui pemanfaatan tanaman pangan pada rumah tangga Kelurahan Plaju Ulu Palembang. Kegiatan yang dilakukan adalah bimbingan teknis cara penaman hidroponik, penyuluhan tentang manfaat sayur dan buah, Emo-Demo (Emotion-Demonstration) porsi sayur, dan demo masak pengolahan sayur. Ada perbedaan yang signifikan skor ibu sebelum dengan setelah diberikan edukasi dengan p value 0,000 dan tanaman hidroponik tumbuh subur. Pelatihan dan penyediaan alat dan bahan media tanam hidroponik mampu memberikan sumbangsih kepada masyarakat dalam penyediaan bahan pangan yang sehat bagi anak dan keluarga serta adanya peningkatan pengetahuan ibu terkait manfaat sayur dan buah setelah diberikan edukasi. Kata kunci: hidroponik; sayur; buah; emo-demo Abstract Vegetables and fruits are needed as source of vitamins, minerals, fibers, and antioxidants. These reduce the incidence of non-communicable disease such as obesity, cancer, cardiovascular, and diabetes. Utilizing food crops in the household is one effort to increase vegetable and fruit consumption. This community service aims to Empowering housewives through utilization of food crops in households in Plaju Ulu sub-district, Palembang. The activities carried out were technical guidance on how to grow hydroponics, counseling about the benefits of vegetables and fruits, Emo-Demo (Emotional-Demonstration) on vegetable portions, and a cooking demonstration on processing vegetables. There was a significant difference in the mother's scores before and after being given education with p value 0,000  and the hydroponic plants grew well. Training and providing tools and materials for hydroponic planting media could contribute to the community. These activities has provided healthy food for children and families as well as increasing mothers' knowledge regarding the benefits of vegetables and fruit after being given education. Keywords: hydroponic; vegetables; fruits; emo-demo

Abstract

Female sex workers carry out sexual transactions that pose a high risk to their health. They experience a high risk of health problems, especially transmission of sexually transmitted infections. There are various factors that female sex workers (FSW) carry out sexual transactions with frequent frequency, such as poverty, gender inequality, and limited access to productive assets. This study aims to determine the associated between the frequency of sex transactions and health risks. This research uses a quantitative approach with a cross-sectional design. The population of this study was all female sexual workers (FSW), with a sample size of 162 taken by purposive sampling. The results of this study show that the majority of FSWs have had a sexual transaction frequency more than two times in the last week, amounting to 79.5%, with a prevalence of high health risks of 75.9%. Based on the multivariate test, it is known that the frequency of sexual transactions is associated with health risks after controlling for the variables age and length of time as a FSW with an Adjusted Odds Ratio of 3.467 (1.415-8.494). Health risks FSW who engage in sexual transactions more than two times have a risk of 3.467 (1.415-8.494) times experiencing high health risks compared to female sex workers who engage in sexual transactions less than two times after controlling for age and length of time as female sex workers. When carrying out work in sexual exploration, FSW should continue to use sexual protective equipment and carry out regular health checks, especially for sexually transmitted infections.
