Abstract

Smoking is one of the periodontal risk factors. Nicotin in cigarettes alters the immune response system and blood vessels in the dental tissue. Tar in cigarettes facilitates the accumulation of plaque. This combination causes periodontal tissue damage. The aim of study is to know the relationship of smoking and periodontal health status. This study was cross sectional study involved 80 smokers who were selected based on simple random sampling. This study used Community Periodontal Index (CPI). Data was analyzed using Chi-Square. The result showed that none of respondents with healthy periodontal health status. Smokers with duration of smoking <10 years, amount of smoking <20 rods/day and white cigarette smokers showed moderate periodontal health status. Smokers with duration of smoking >10 years, amount of smoking  >20 rods/day and kretek cigarette smokers showed poor periodontal health status. The conclusion of this study are significant relationship between amount of cigarette (p=0,000), duration of smoking (p=0,000, type of cigarette(p=0,006) and periodontal health status.

Abstract

Abstrak: Meningkatnya jumlah lansia seiring pula dengan penyakit yang diderita lansia. Menurunnya kondisi fisik, psikologis, dan sosial yang dialami lansia sehingga lansia harus tinggal bersama keluarga. Di sisi lain keluarga harus mengurus keluarga inti dan bekerja di luar rumah. Jika keluarga tidak bisa merawat lansia dengan baik maka lansia akan terabaikan sehingga keluarga perlu memberikan dukungan kepada lansia. Keluarga perlu diberikan pelatihan sehingga keluarga tahu, paham dan dapat mempraktikannya ketika mendampingi lansia. Agar materi yang disampaikan dapat berkelanjutan maka sebelum diberi pelatihan kepada keluarga terlebih dahulu diberikan pelatihan kepada kader sebagai pendamping tenaga kesehatan untuk sustainable.  Tujuan pengabdian masyarakat adalah meningkatkan pengetahuan dan pemahaman keluarga tentang pencegahan pengabaian lansia. Metode pelatihan yang diberikan melalui  ceramah dan tanya jawab,diskusi, dan demonstrasi uji keterampilan fisik tentang cara perawatan dasar pada lansia di rumah. Alat bantu yang digunakan modul, LCD proyektor, dan buku catatan. Hasil pelatihan didapatkan antusias serta keterampilan kader dan keluarga tentang pencegahan pengabaian lansia pada keluarga. Keterampilan yang didapat yaitu keterampilan perawatan demam pada lansia, pengukuran aktifitas fisik lansia, pengukuran aktifitas sosial lansia, dan pengukuran keseimbangan tubuh lansia.Abstract: The increasing number of elderly is also in line with the illness suffered. The reduced physical, psychological, and social conditions experienced by the elderly make them live with their family. On the other hand, their family has to take care of the main family and goes to works. If the family cannot take care of the elderly well, the elderly will be neglected.Familiesneed to provide support to the elderly, sotheyneed to be given training so that they know, understand, and can practice it when accompanying the elderly. To make the training material delivered sustainable, cadres as a companion for health workers will also be given training for sustainability. The aim of the communityservice is to increase families’ knowledge and understanding of the elderly neglect prevention. Training methods are provided through lectures, questions and answers, discussions, and demonstrations of physical skills tests on how to care for the elderly at home. Tools used are modules, LCD projectors, and notebooks. The result of the training shows increased knowledge and skills of cadres and families about preventionof the elderly neglect in families.The acquired skills are such as fever care skill on the elderly, measurement of elderly physical activity,measurement of elderly social activity, and measurement of body balance.

Abstract

Komunikasi operan merupakan transfer informasi terkait pasien untuk kesinambungan asuhan yang diberikan. Kesalahan dalam transfer informasi pasien dapat mengakibatkan kelalaian pengobatan, asuhan tidak tepat dan kematian, untuk itu perlu kerangka komunikasi agar informasi yang disampaikan cepat, tepat dan akurat. ISOBAR merupakan kerangka komunikasi operan yang lebih terstruktur, lengkap dan akurat sehingga menunjang keselamatan pasien. Penelitian ini bertujuan untuk mengidentifikasi pengaruh pelatihan komunikasi ISOBAR terhadap pengetahuan dan kualitas pelaksanaan operan di RSUD Padang Pariaman. Penelitian ini adalah penelitian kuantitatif dengan desain Quasy Experimental Pre – Post Test With control Group. Teknik pengambilan sampel denga systematik random sampling. Yang menjadi sampel pada penelitian ini adalah ketua tim dan penanggung jawab shift dengan jumlah 23 kelompok intervensi dan 18 kelompok kontrol. Pengumpulan data dilakukan dengan kuisioner dan observasi. Analisa data yang digunakan adalah uji statistik Man-Whitney untuk melihat perbedaan kelompok intervensi dan kontrol. Hasil penelitian ini membuktikan bahwa terdapat pengaruh pelatihan komunikasi ISOBAR terhadap pengetahuan (p=0,000) dan kualitas pelaksanan operan (p=0,000) pada kelompok intervensi. Diharapkan pihak Rumah Sakit dapat menggunakan komunikasi ISOBAR dalam operan untuk meningkatkan kualitas SDM dan membuat kebijakan tentang penerapan komunikasi ISOBAR dalam upaya meningkatkan keselamatan pasien

Abstract

Background Pneumonia is a major cause of morbidity andmortality in children under five years of age. Pneumonia can be ofbacterial or viral origin. It is difficult to distinguish between thesetwo agents based on clinical manifestations, as well as radiologicaland laboratory examinations. Furthermore, bacterial cultures taketime to incubate and positive results may only be found in 10-30%of bacterial pneumonia cases. Procalcitonin has been used as amarker to distinguish etiologies, as bacterial infections tend toincrease serum procalcitonin levels.Objective To determine the sensitivity, specificity, positivepredictive value and negative predictive value of procalcitoninin community-acquired bacterial pneumonia.Method This cross-sectional study was conducted in thePediatric Health Department of Dr. M. Djamil Hospital, Padang.Subjects were selected by consecutive sampling. Procalcitoninmeasurements and PCR screening were performed on bloodspecimens from 32 pneumonia patients and compared.Results Of the 32 subjects, most were boys (56.25%), under 5years of age (99%), and had poor nutritional status (68.75%).Using a cut-off point of 0.25 ng/mL, procalcitonin level hada sensitivity of 92%, specificity 50%, positive predictive value 88%, and negative predictive value 60% for diagnosing bacterial pneumonia. Using a cut-off point of 0.5 ng/mL, procalcitonin level had a specificity of 46%, specificity 83%, positive predictive value 91%, and negative predictive value 25%.Conclusion A cut-off point of 0.25 ng/mL of procalcitonin level may be more useful to screen for bacterial pneumonia than a cutoff point of 0.5 ng / mL. However, if the 0.25 ng/mL cut-off point is used, careful monitoring will be required for negative results, as up to 40% may actually have bacterial pneumonia. [PaediatrIndones. 2015;55:65-9.].

Abstract

Background Antibiotic prescription for the management of all acutepharyngitis seems to be increasing recently. Streptococcal pharyngitisis the one that has strong indication for antibiotic prescription. Itis quiet hard to distinguish the clinical features of streptococcalpharyngitis from non-streptococcal one.Objective To determine specifically clinical features of streptococcalpharyngitis and distinguish them from non-streptococcal one.Methods We conducted a cross-sectional study on children withacute pharyngitis at Pediatric Outpatient Department Dr. M.Djamil Hospital, Padang from November 2006 until March 2007.Data on clinical features and pharyngeal swab culture results wereanalyzed using chi-square test for clinical predictors. All were thenreanalyzed using multivariate logistic regression.Results Ninety-five children aged 3-13 years were enrolled andpharyngeal swab culture was performed. Group A β -haemolyticusstreptococcus was found in 13 children (14%). Absence of cough,sore throat, tonsillar exudates and tender anterior cervicaladenopathy were the clinical predictors for streptococcalpharyngitis and the last two shared highest risk (OR 55.05;31.82). Combination of tonsillar exudates, tender anterior cervicaladenopathy and absence of cough contributed 99,3% probability.Conclusions Streptococcal pharyngitis includes a small part of allchildhood with acute pharyngitis. High grade fever, sore throat,absence of cough, tonsiller excudates and tender anterior cervicaladenopathy were considered as clinical predictors for childhoodstreptococcal pharygitis. Combination of some clinical predictorswill strengthen the probability of streptococcal pharyngitis.

Abstract

Providing education on the prevention of pneumonia is an effort to reduce morbidity and mortality among children under five. However, after education on mothers of children under five, the incidence of pneumonia is still high. This research is a descriptive study that aims to explore the characteristics and behavior of mothers regarding the prevention of pneumonia. This is done to be able to plan the provision of further interventions for mothers of children under five. The sampling technique used non-probability methods with total sampling techniques totaling 76 respondents. The research instrument used KAP(Knowledge, Attitudes, and Practices) with a total of 32 questions, Cronbach alpha knowledge (0.844), attitude (0.807), and action (0.826). Data analysis using analysis univariate. Characteristics of respondents aged 18-40 years (90.8%), most education was high school (65.7%), and non-working / housewives (85.5%). Mean knowledge value7.55,  attitude with the mean value 26.16, and measures with a mean value of 27.22. Based on the conclusions of the results of research on maternal behavior regarding the prevention of pneumonia in children under five associated with inadequate risk factors for pneumonia, it can cause toddlers to have a risk of pneumonia.

Abstract

The Covid-19 virus is transmitted from human to human through close contact with infected people through their respiratory droplets. A person can also be infected by touching an object contaminated with SARS-CoV-2 and then followed by reaching the eyes, nose, or mouth. Implementing the health protocol is necessary to reduce the number of virus transmissions. In addition, to increase the community's immune against Covid-19, WHO and the government have been developing a vaccination program. However, the recommendation from the latest research stated that people still have to obey the health protocol even after being vaccinated. The purpose of this community service was to help the government to increase people's understanding of the importance of implying health protocols, even after vaccination, to suppress Covid-19 cases. The method used is by holding an online event called a Webinar. It followed by a statement with audiences to commit to implementing the health protocol led by the Rector of Andalas University, then called a SIAGA SAKATO declaration. The activity began with a pre-test questionnaire for initial information collection, followed by education about Covid-19 prevention from the experts to the 512 students. The activity ended with the final data collection using a posttest questionnaire to test the student knowledge after the education. Paired t-test analysis showed a significant increase in student commitment after attending the Webinar (P≤0.05). Based on these results, the Webinar increases students' understanding of the importance of the health protocol in reducing the Covid-19 cases.

Abstract

Latar belakang: Rinosinusitis kronis (RSK) adalah penyakit inflamasi mukosa hidung dan sinus paranasal yang berlangsung lebih dari 12 minggu. Berbagai kondisi telah dikaitkan dengan patogenesis penyakit ini, seperti infeksi bakteri, jamur, superantigen, dan biofilm. Banyak penelitian telah menunjukkan terdapatnya biofilm bakteri pada pasien dengan RSK. Biofilm bakteri dapat memfasilitasi terjadinya resistensi pada antibiotik. CT Scan sinus paranasal (SPN) merupakan pemeriksaan penunjang pilihan untuk diagnosis radiologik RSK. Lund dan Mackay telah mengembangkan suatu sistem berdasarkan skor dari CT Scan SPN untuk menilai kuantifikasi proses peradangan pada sinus paranasal. Beberapa penelitian menunjukkan bahwa pasien dengan biofilm bakteri memiliki skor Lund-Mackay CT Scan SPN yang lebih tinggi pada saat pra operatif. Tujuan: Penelitian ini bertujuan untuk mengetahui peran biofilm bakteri terhadap derajat keparahan RSK berdasarkan skor Lund-Mackay. Metode: Penelitian ini adalah penelitian cross-sectional dengan jumlah total sampel adalah 48 orang pasien RSK. Sekret hidung diambil menggunakan kapas lidi steril dengan swab pada meatus medius lalu dilakukan identifikasi bakteri dan pemeriksaan biofilm dengan tube method. Skor Lund-Mackay dihitung dari CT Scan SPN potongan koronal. Data dianalisis dengan uji Fisher. Hasil: Proporsi pasien RSK dengan skor Lund-Mackay yang tinggi lebih banyak pada pasien dengan biofilm (46,2%), dibandingkan tanpa biofilm (44,4%). Secara statistik tidak terdapat perbedaan bermakna pada skor Lund-Mackay antara pasien dengan biofilm dan tanpa biofilm (p=1,000). Kesimpulan: Tidak terdapat hubungan antara biofilm bakteri dengan derajat keparahan RSK berdasarkan skor Lund-Mackay. Kata Kunci: Rinosinusitis kronis, biofilm bakteri, tube method, skor Lund-Mackay ABSTRACT Background: Chronic rhinosinusitis (CRS) is inflammation of the nose and paranasal sinuses with the symptoms duration more than 12 weeks. Many conditions have been linked to its pathogenesis such as bacterial and fungal infection, superantigens and biofilm. Many studies showed the presence of bacterial biofilms in patients with CRS. Bacterial biofilms can facilitate the resistance to antibiotics. Paranasal sinuses (PNS) CT scan is the method of choice for radiological diagnosis of CRS. Lund and Mackay has developed a scoring system based on the CT finding to assess the quantification of inflammatory process in PNS. Some research suggested that patients with bacterial biofilms have higher Lund-Mackay score pre-operatively. Purpose: To determine the role of bacterial biofilms to the severity of CRS according to Lund-Mackay score. Methods: This was a cross-sectional study with 48 CRS patient’s sample. Nasal discharges were taken by swab in middle meatal using sterile cotton buds, followed by identification of bacteria and detection of bacterial biofilms using tube method. Lund-Mackay score was counted from coronal section of PNS CT Scan. Data was analyzed by Fisher’s exact test. Results: Proportion of patients CRS with high Lund-Mackay score was more common in patient with biofilm (46.2%) compared to patients without biofilm (44.4%). Statistically, there was no significant difference of Lund-Mackay score between patient with biofilm and without biofilm (p=1.00). Conclusion: There was no relationship between the bacterial biofilm with the severity of CRS according to Lund-Mackay score. Abstract

Sebagian besar keganasan kepala dan leher terdiagnosis sudah dalam stadium lanjut dan sering sudah inoperable. Kemoradiasi merupakan pengobatan pada keadaan seperti ini. Kemoterapi neoadjuvan (induksi) dengan cisplatin pada karsinoma kepala leher telah terbukti dapat mempreservasi organ dan menurunkan angka metastasis jauh, namun di sisi lain cisplatin menyebabkan berbagai efek samping berupa toksik terhadap berbagai organ, salah satunya ototoksikPenelitian ini bertujuan untuk mengetahui gambaran audiogram nada murni penderita karsinoma kepala dan leher yang mendapat kemoterapi cisplatin.Penelitian ini merupakan uji potong lintang untuk mengetahui gambaran audiogram nada murni terhadap 25 orang penderita karsinoma kepala dan leher yang mendapat kemoterapi cisplatin 100 mg/m2 di bagian Telinga Hidung Tenggorok Bedah Kepala Leher (THT-KL) rumah sakit (RS) Dr. M. Djamil Padang, mulai bulan April sampai bulan Desember 2009.Dari 50 audiogram yang diperiksa didapatkan nilai rerata perubahan nilai ambang dengar hantaran tulang pada masing-masing frekuensi yaitu 3,4±5,84 dB pada frekuensi 250 Hz, 2,0±3,91dB (500 Hz), 2,6±4,87 dB (1000 Hz), 3,4±5,10 dB (2000 Hz), 4,9±9,23 dB (4000 Hz), 4,7±5,84 dB (6000 Hz) dan 6,5±9,85 dB pada 8000 Hz.Setelah pemberian satu siklus kemoterapi cisplatin 100 mg/m2 sebagian besar sampel tidak mengalami perubahan ambang dengar pada frekuensi 250, 500, 1000, 2000, 4000 dan 6000 Hz, sedangkan pada frekuensi 8000 Hz, sebagian besar mengalami kenaikan nilai ambang dengar. Namun demikian nilai rerata hantaran tulang sebelum dengan setelah kemoterapi terdapat perbedaan yang bermakna pada semua frekuensi.

Abstract

High Maternal Mortality Rate (MMR) is mostly caused by a lack of information and a bad habit of pregnant women in high-risk pregnancy treatment. This condition causes an impact to high maternal mortality during childbirth. This study has aims to identify factors as the main construction of the Participatory Asset Community Development Research Action Model (YUDHIA). This study used a qualitative approach. The location of the study is at Cimanuk, Pandeglang, Banten. Informants were chosen based on purposive sampling and snowball techniques. Key informants were four pregnant women that had complication risk. Supporting informants were eight-person. Data collecting used in-depth interviews and recorded by a tape recorder. Analysis data used RAP techniques and released 12 themes that described the main factors in the construction of the YUDHIA model. The description was (1) development of community assets, such as physical, economic, environmental, human, and social assets. (2) Community participation, such as the dimension of community contribution, community development, and organization. This study is community participation has been already existed by forming desa siaga (standby village). The most crucial asset in the community is human assets, midwifery, and dukun (traditional birth attendant), therefore create a strong commitment to giving priority to the needs and lives of pregnant women.
