Abstract

Background: Diabetes mellitus (DM) is a chronic metabolic disease frequently accompanied by complications such as peripheral circulatory disorders and neuropathy, which contribute to decreased lower-extremity function. This condition is still often found in DM patients at Toto Kabila Regional Hospital, highlighting the need for effective non-pharmacological interventions. Purpose: To analyze the effectiveness of buerger allen exercise and diabetic foot exercise in improving peripheral circulation and neuropathic symptoms among patients with diabetes mellitus. Method: A quasi-experimental two-group pretest–posttest design involving 60 patients who were randomly assigned into two intervention groups. Both exercises were administered for four weeks. Data were collected using standardized operational procedures for each exercise along with instruments assessing peripheral circulation and neuropathic symptoms. The Wilcoxon test was used to analyze within-group effects, while the Mann–Whitney test was applied to compare differences between groups. Results: Both interventions significantly improved peripheral circulation and reduced neuropathic symptoms (p = 0.000). Between-group analysis showed that the Buerger Allen Exercise produced greater improvement in peripheral circulation, particularly in the left extremity (p = 0.019), and demonstrated a more substantial reduction in neuropathic symptoms compared to diabetic foot exercise (p = 0.000). Conclusion: Both Buerger Allen Exercise and diabetic foot exercise are effective in enhancing peripheral circulation and reducing neuropathic symptoms in patients with diabetes mellitus. However, Buerger Allen Exercise shows superior effectiveness and may be recommended as a primary non-pharmacological intervention to improve vascular and sensory function in diabetic patients.   Keywords: Buerger Allen Exercise; Diabetes Mellitus; Diabetic Foot Exercise; Diabetic Neuropathy; Peripheral Circulation. Abstract

Background: Patients with chronic kidney failure report the sight of blood-filled dialysis tubes, the burden of routine therapy costs, fear of invasive measures, and the uncertainty of recovery triggers their anxiety. This anxiety also carries over into the home, causing difficulty sleeping at night and ongoing feelings of restlessness. Purpose. The Effect of the Combination of Mindfulness based on Breathing Exercise with Lavender Aromatherapy on Anxiety and Sleep Quality in Hemodialysis Patients Methods: This study employed a quasi-experimental pretest–posttest control-group design and was reported in accordance with the TREND guidelines for non-randomized intervention research. The target population comprised 90 hemodialysis patients at Bhayangkara Kediri Hospital, of whom 40 were selected using purposive sampling. The intervention consisted of a combination of mindfulness-based breathing exercises and lavender aromatherapy, with anxiety and sleep quality as outcome variables. Anxiety was measured using the Self-Rating Anxiety Scale (SAS), and sleep quality was measured using the Pittsburgh Sleep Quality Index (PSQI), both of which have demonstrated acceptable validity and reliability in prior research. Data analysis included descriptive statistics and the Wilcoxon signed-rank test to determine pre-post differences between intervention and control groups, with a significance level set at p < 0.05. Result: The general characteristics of respondents were comparable between the intervention and control groups with respect to age, sex, duration of hemodialysis, and comorbid conditions. Specific baseline characteristics related to anxiety levels and sleep disturbances were also similar across groups, indicating initial equivalence. The Wilcoxon signed-rank test showed a statistically significant improvement in anxiety (p < 0.001) and sleep quality (p < 0.001) following the intervention. These findings suggest that combining mindfulness-based breathing exercises with lavender aromatherapy may reduce anxiety and enhance sleep quality among hemodialysis patients. Conclusion: The combination of mindfulness-based breathing exercises with Lavender Aromatherapy reduces Anxiety and improves Sleep Quality in hemodialysis patients. This intervention is very easy to implement, so it can be used as a nursing intervention in the hemodialysis unit to advance nursing science.

Abstract

Abstract Diabetes Mellitus (DM) is a non-communicable disease with a rising prevalence, including in Indonesia. Effective management of diabetes is crucial to prevent serious complications; however, many patients still have a limited understanding of how to control their blood sugar levels. This community service program aims to assess the effectiveness of an educational program in improving patients' knowledge about diabetes management at Puskesmas Genteng Kulon, Banyuwangi. The academic program is presented in a simple, easy-to-understand approach called DORMAN (Diet, Exercise, and Monitoring Active, Normal). This program utilizes various educational media such as leaflets, posters, and blood sugar monitoring tools. A total of 46 participants who met specific criteria were involved in this program. Program evaluation was carried out using knowledge questionnaires, program satisfaction surveys, and blood sugar monitoring forms. The results of this program showed a significant improvement in patient knowledge: more than half of the patients (52.2%) who initially had insufficient knowledge improved to a good level (80.5%) after participating in the educational program. Additionally, the blood sugar levels of patients who were initially abnormal, totaling 28 patients (60.9%), improved, with 30 patients (65.2%) achieving normal blood sugar levels after the education. These findings emphasize the importance of education grounded in clear, applicable information for self-management of diabetes. This program is expected to have a positive impact on community-level efforts to prevent diabetes complications.

Abstract

Self-management and the Health Belief Model (HBM) have been shown to improve blood pressure control and quality of life (QoL). However, the effectiveness of integration self-management with HBM for hypertension requires further clarification. This study examined the effect of integration self-management with HBM on blood pressure and QoL among individuals with uncontrolled hypertension. A total of 138 eligible participants were allocated into two groups: one receiving self-management integrated with HBM-based education (n = 69) and the other receiving regular education (n = 69). Independent t-tests, Chi-squared tests, and generalized estimating equation (GEE) were conducted. The findings showed a significant interaction between groups in systolic blood pressure (SBP) and diastolic blood pressure (DBP) after 12 weeks compared to the control group (p < 0.001). Importantly, there was a significant increase in QoL across physical, psychological, social relationships, and environmental domains. Specifically, the intervention group demonstrated significantly higher QoL scores than the control group over time (β = 22.756, 95% CI [20.623, 24.901], p < 0.001). The effectiveness of integration self-management with HBM significantly reduced blood pressure and increased QoL.
