Abstract

Introduction: Hirschsprung's disease (HD) is characterized by the absence of ganglion cells in Meissner's plexus and Auerbach's plexus which used to be assessed by histopathological examination using Hematoxylin and Eosin (H&E).  In some cases is difficult to assess ganglion cells, thus requiring additional examination in the form of calretinin immunohistochemistry to detect the presence of ganglion. Methods: The 59 samples of suspected HD  were analyzed by H&E staining followed by calretinin immunohistochemical staining. This study aimed to assess the sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of calretinin in HD diagnostics. Results:  The immunohistochemistry of calretinin showed a good diagnostic value in detecting aganglionosis in patients with suspected HD with a sensitivity value = 97.7%; Specificity = 100%; Positive Presumptive Value (PPV) = 100%; Negative Presumptive Value (NPV) = 93.7%; Accuracy= 98.3%. Thus, calretinin immunohistochemistry can be relied upon as an additional method in the diagnosis of HD

Abstract

Introduction:  Hepatitis B virus infection in fetal and newborn may result from the intrauterine vertical transmission. The purpose of our study is to determine the association between HBsAg expression in the placental and HBsAg serum in pregnant women. Methods: Total 67 placental tissues and serum were obtained from HBV-infected mothers. This study was conducted in various hospitals in Makassar, Indonesia from December 2017 until August 2018. Maternal serum and placental tissues were tested by ELISA and Immunohistochemistry staining. Results: 67 samples from placental tissues and serum were categorized into two different levels of serum titer and staining positivity. Twenty nine samples (43.3%) were categorized in low titer of HBsAg positive serum and 38 samples (56.7%) in high titer of  HBsAg positive serum. From the total of 67 placental samples, 43 (64.2%) samples had positive expression of HBsAg immunohistochemistry staining and 24 (34.8%) samples had negative expressions. Conclusions: There was a significant correlation between the expressions of HBsAg in the placental tissues and serum titers of HBV-infected mothers that is important in predicting the risk of transplacental transmission. The results of this study can be used as guidelines for the prevention and therapeutic approach of mother-to-child HBV infection.

Abstract

Background: This study aimed to evaluate the effects of neuromuscular taping decompression technique (NMT) on modifying pain scale in diabetic patients with painful distal symmetrical polyneuropathy (DSPN) at Wahidin Sudirohusodo Hospital, Makassar, Indonesia. Using a pure experimental design with a pre-test and post-test control group method, 30 patients were included: 15 in the treatment group and 15 in the control group. The treatment group received decompression NMT on the dorsal and plantar aspects of the foot twice a week for four weeks, along with vitamin B complex supplementation. The control group received only vitamin B complex. Result: The Mann-Whitney test showed significant improvement (p <0.05) in the pain scale of the treatment group on day 28, with the Numeric Pain Rating Scale (NPRS) score being lower than that of the control group. Conclusion: This study demonstrated the significant pain-reducing effect of NMT in DSPN patients. The combination of vitamin B1, B6, and B12 supplementation with 8 NMT sessions over 28 days reduced pain to 0 on the NPRS scale. Treatment twice a week for 4 weeks, alongside vitamin B, led to near-zero pain, with pain-free results expected after 5 treatments over 17 days. NMT combined with vitamin B reduced pain approximately 2 times faster than vitamin B alone. Abstract

Pemerintah Daerah Khusus Ibukota Jakarta yang mengeluarkan beberapa kebijakan penanggulangan pencemaran udara menghadapi banyak kendala implementasi. Tujuan penelitian ini mengetahui implementasi kebijakan pengelolaan kualitas udara perkotaan terkait transportasi di provinsi tersebut dengan pendekatan model sistem. Faktor yang diamati meliputi instrumen kebijakan, sumber daya dan manajemen. Penelitian ini menggunakan metode penelitian kualitatif yang menggunakan sumber data primer dengan metode wawancara mendalam dan sumber data sekunder telaah dokumen. Data primer digali dari berbagai informan yang berkompeten meliputi Dewan Perwakilan Rakyat Daerah, Biro Hukum, Badan Pengelola Dampak Lingkungan Daerah, Dinas Perhubungan, Dinas Kesehatan dan Samsat. Penelitian ini menemukan bahwa instrumen kebijakan telah ada, tetapi dipersepsikan hanya berlaku untuk BPLHD. Penegakan hukum belum dilaksanakan secara semestinya karena sistem dan koordinasi belum maksimal; sumber daya manusia dan sumber dana masih kurang; rencana strategis belum ada, serta manajemen dan koordinasi belum maksimal. Untuk implementasi kebijakan pengelolaan kualitas udara perkotaan yang efektif, pembuat dan pelaksana kebijakan perlu memperhatikan beberapa faktor tersebut. The Government of DKI Jakarta Province had released some policies regarding to remedy air pollution. However, there are much problems related with urban air quality in DKI Jakarta Province. So that, purpose of this study will know implementation of urban air quality management policy related to transportation by system model approach. The matter will be studied are policy instruments, resources and management. This study is qualitative study. In this study, primary and secondary data will be used. Primary data are collected from in depth interview with competent sources such as Regional Parliamentary, Law Bureau, Regional Authority for Environmental Impact, Transportation Department, Health Department and Samsat. Secondary data are collected by conducting documents. The study result showed that policy instrument has existed, but they are perceived only effective for BPLHD, real law enforcement has been not implemented because system and coordination are not optimal, human and money resources are minimal, and there is no strategic planning so that management and coordination are not optimal. Therefore, to make implementation of urban air quality management policy become effective, the factors should be respected by related parties especially policy makers and policy implementers.

Abstract

Cognitive decline is a significant complication that affects most stroke survivors. Early detection of cognitive decline in ischemic stroke patients and identification of risk factors improves their clinical outcomes. This study aimed to determine the characteristics of cognitive status in the sub-acute phase of ischemic stroke. A cross-sectional study was conducted on 89 sub-acute ischemic stroke patients in three hospitals in West Nusa Tenggara recruited consecutively from August 2019 to April 2020. The data collected were demographic and clinical characteristics, cognitive status, and functional outcome. The association between clinical and demographic characteristics and cognitive decline was analyzed using logistic regression. In addition, the relationship between cognitive status and functional outcomes of these patients was examined using the chi-square test. This study revealed that the prevalence of cognitive decline in these subjects was 71.9%. Multiple logistic regression showed that age was the only characteristic associated with cognitive decline in the subjects (OR = 5.12,95% CI = 1.08-24.28). Furthermore, the frequency of cognitive decline in these subjects was significantly associated with functional outcomes (p-value =0.014). Thus, there was a high prevalence of cognitive decline in sub-acute ischemic stroke patients associated with increasing age and poor functional outcomes.

Abstract

Introduction: The global prevalence of allergic diseases has increased significantly, particularly among young adults. Immunoglobulin E (IgE) plays a central role in the pathophysiology of allergic responses and is considered a crucial biomarker in evaluating allergic sensitivity. However, the influence of environmental exposures and clinical complaints on serum IgE levels in adult populations remains understudied, especially in tropical developing countries. Methods: This study aimed to analyze the relationship between environmental factors and clinical complaints with serum IgE levels in a population of healthy young adults in Makassar, Indonesia. A cross-sectional study was conducted involving 80 healthy adults aged 18–40 years. Environmental exposures and clinical symptoms were analyzed against serum IgE levels. Non-parametric tests (Mann-Whitney U and Kruskal-Wallis) showed no significant associations. However, in the multivariate regression model, humidity was the only environmental factor significantly associated with serum IgE concentration. Results: The mean serum IgE level among participants was 176.8 IU/mL (SD ±196.9), with a wide distribution. No statistically significant associations were found between serum IgE levels and clinical complaints such as asthma, allergic rhinitis, and atopic dermatitis (p > 0.05). Similarly, environmental exposures including cigarette smoke, dust, pet dander, and poor ventilation were not significantly related to IgE levels. Notably, high environmental humidity was negatively associated with serum IgE levels (p = 0.047). Most environmental factors and clinical complaints assessed in this study were not significantly associated with serum IgE concentrations in healthy young adults. Conclusion: The observed inverse association with high humidity suggests a potential modulatory role that warrants further investigation. Future longitudinal studies with larger samples are recommended to confirm these findings and provide stronger evidence to inform public health strategies in allergy prevention.

Abstract

Pendahuluan: Stroke merupakan penyakit neurologis utama di usia dewasa, berdasarkan tingginya angka kejadian, kegawatdaruratan, penyebab utama kecacatan, dan kematian. Brain derived neurotrophic factor (BDNF) merupakan salah satu neurotrophin golongan neurotrophic. BDNF terlibat dalam perkembangan sinapsis, platisitas sinapsis, dan fungsi kognitif dan meningkat pada stroke iskemik. Sehingga perlu dilakukan analisis antara kadar BDNF dengan fungsi kognitif. Metode: Penelitian cross-sectional yang dilakukan pada 64 orang pasien stroke iskemik dengan onset 15-30 hari dengan usia 18-70 tahun. Pasien dengan gangguan fungsi kortikal luhur sebelumnya, buta huruf, afasia, Alzheimer, Parkinson dan depresi merupakan kriteria eksklusi. Usia, jenis kelamin, tingkat pendidikan, lokasi infark, kadar BDNF dan fungsi kognitif merupakan variabel yang akan dianalisis. Hasil: Tingkat pendidikan berpengaruh pada fungsi kognitif dan terdapat korelasi kuat positif bermakna antara kadar BDNF dengan fungsi kognitif. Kadar cut off BDNF ≤ 1,3 ng/mL memiliki sensitivitas 86,11% dan spesifitas 96,43% sebagai prediktor gangguan kognitif dan bermakna pada variabel memori, visuospasial dan eksekutif. Kesimpulan: BDNF serum dapat diukur untuk memprediksi terjadinya gangguan fungsi kognitif.

Abstract

Introduction Stroke is an acute disorder of cerebral perfusion or vascularization. Stroke patients commonly encounter numerous functional disorders, such as motor, psychological or behavioral disorders, where the most typical symptom is hemiparesis which greatly affects their ability to carry out daily life activities. Repetitive Transcranial Magnetic Stimulation (rTMS) is one of the interventions proposed to improve stroke recovery. Constraint-Induced Movement Therapy (CIMT) is a neurological rehabilitation treatment designed to improve upper extremity motor function after stroke Objective: This study aims to compare the effectiveness of rTMS with CIMT in improving upper extremity motor function in ischemic stroke patients. Research Methodology: This is an experimental clinical trial study with a randomized pre-test post-test control group design on 30 ischemic stroke patients at Wahidin Sudirohusodo General Hospital Makassar and other network hospitals. Repetitive Transcranial Magnetic Stimulation was carried out for 10 stimulations, CIMT was carried out for 30 days. Motor improvement was evaluated by measuring the Fugl Mayer Assessment Upper Extremity (FMA-UE) score Results: The Friedmann test showed that rTMS and CIMT provided significant changes in FMA-UE scores (p: 0.000). Mann Whitney test showed there was a significant difference in FMA-UE scores in the rTMS group compared to CIMT (p: 0.000) Conclusion: There was a significant difference in the FMA-UE score in the rTMS group compared to CIMT. Recommendation: Further research is needed with longer observation and stimulation time and looks at the effectiveness of these two therapeutic modalities through a biomarker so that more objective results can be obtained.

Abstract

WHO introduced Covid-19 as a global health problem on January 30, 2020, and currently here are more than 6.05 million cases of Covid-19 reported from 188 countries and resulted in 368,000 deaths. One of the efforts recommended by the Indonesian Ministry of Health to prevent the spread of this infection is to limit social contact and practice clean and healthy living. This study develops a communication strategy in education on the prevention and handling of COVID-19 in South Sulawesi. Through a qualitative approach, data collection was carried out through focus group discussions to identify a picture of the community's response in dealing with this pandemic. This study resulted in infographics and guides containing information related to the prevention and handling of COVID-19. In addition, voice messages have been developed about 3M's behavior and the correct use of masks. A communication strategy by considering the local situation in the form of community responses is important when conducting education during a pandemic. This is due to the large amount of information spread in the mass media that is easily accessible by the public. In addition, the ever-changing situation also requires us to be able to continue to innovate in developing health messages that will be disseminated to the public. Messages created can include new information, confirmation or refutation of hoaxes that may develop.

Abstract

Diabetic Painful Neuropathy (DPN) is caused by the progressive loss of nerve fibers due to hyperglycemia, oxidative stress, andhomocysteinemia. Mecobalamin supplementation has shown potential in improving peripheral neuropathy symptoms by enhancing nerve conduction and alleviating neuropathic pain. However, its efficacy data remains limited. This research to investigated the effect of mecobalamin supplementation on the severity of Painful Diabetic Neuropathy in type 2 diabetes mellitus patients. This research is a non-randomized clinical trial, 16 patients were given 500 ?g of intramuscular Mecobalamin 8 times over 30 days (n=8) and 5 times over 10 days (n=8). Subjects underwent measurements VAS, TSS, BDNC and ENMG before and after intervention. VAS (p-value=0,022), TSS (p-value=0,020) and BDC (p-value=0,003) was significant improved in 8 times administration group. VAS (p-value=0,045) dan BDC (p-value=0,014) was significant improved in 5 times administration group, where TSS improved but not significant. The 8 times administration group was better compared to the 5 times administration group in improvement of VAS (p-value=0,016). Based on this research, The addition of mecobalamin 8 times and over 5 times in the management of Diabetic Neuropathic Pain can be useful in reducing neuropathic pain.
