Abstract

Pendahuluan: Nyeri punggung bawah atau low back pain (LBP) adalah gangguan kesehatan yang sering dan memilki dampak sosial ekonomi yang berat. Kurang lebih 40% dari nyeri punggung bawah diakibatkan oleh herniasi diskus. Selain herniasi diskus, perubahan endplate tipe Modic juga telah diasosiasikan dengan kejadian nyeri punggung bawah. Meskipun memiliki insidensi yang relatif tinggi dan sering dihubungkan dengan LBP, hubungan antara perubahan endplate tipe Modic dengan penyebab lain LBP seperti herniasi diskus masih belum diketahui dengan jelas. Penelitian ini bertujuan untuk mengetahui hubungan antara derajat herniasi diskus dengan masing-masing tipe perubahan endplate Modic pada pasien yang menjalani pemeriksaan MRI lumbosakral. Metode: Penelitian ini dilakukan dengan desain observasi potong lintang menggunakan data sekunder pada 488 sampel. Derajat herniasi diskus intervertebrais dan tipe perubahan endplate Modic dinilai menggunakan pemeriksaan MRI lumbosakral. Hasil: Derajat herniasi diskus yang paling banyak ditemukan adalah protrusio sebanyak 199 sampel (40,8%) diikuti dengan bulging diskus sebanyak 154 sampel (31,6%). Tipe perubahan Modic yang paling banyak ditemukan pada level terjadinya herniasi diskus adalah perubahan Modic tipe II sebanyak 168 sampel (34,4%). Menggunakan analisa Chi-Square, didapatkan hubungan yang bermakna antara derajat herniasi diskus dengan tipe perubahan Modic dengan nilai p<0,001. Simpulan: Perubahan Modic tipe 1 lebih sering terjadi pada bulging diskus, sedangkan perubahan Modic tipe 2 dan 3 lebih banyak didapatkan pada derajat herniasi yang lebih berat seperti protrusio, ekstrusio dan sekuesterasi.

Abstract

Introduction: Post-stroke depression often causes problems. Depression can slow motor recovery. Giving antidepressants takes one month, and thus Transcranial Magnetic Stimulation (TMS) was developed, especially repetitive Transcranial Magnetic Stimulation (rTMS). This study aims to assess the effectiveness of rTMS in improving depressive symptoms and motor strength in ischemic stroke patients. Methods:  An experimental study with a randomized pretest-posttest control group design was conducted at Wahidin Sudirohusodo Hospital and a network hospital in Makassar from August to October 2022. The treatment group received standard therapy for ischemic stroke, antidepressants, and rTMS; the control group received standard therapy for ischemic stroke and antidepressants for ten days. In this study, the Hamilton Depression Rating Scale (HDRS) and the Medical Research Council (MRC) scale were used and measured in the treatment and control groups on the first and tenth days. Results: A total of 40 subjects met the criteria and were divided into a treatment group (n = 20) and a control group (n = 20). The HDRS score in the treatment group was smaller than in the control group. The value of left extremity motor strength in the treatment group was greater than that of the control group using the Wilcoxon Test. The correlation of the HDRS score to motor strength in the treatment and control groups showed a negative correlation of the HDRS score to the left extremity using the Spearman Test.   Conclusions:  rTMS effectively improves depressive symptoms and motor strength in ischemic stroke, given standard ischemic stroke therapy, antidepressants, and rTMS.

Abstract

Introduction: The survival rate of cancer patients was high due to chemotherapy, but it can cause cardiotoxicity and increase morbidity and mortality. Most studies on cancer therapy-related cardiac dysfunction (CTRCD) focus more on the left ventricle. This study assesses chemotherapy's effect on the right ventricle, namely the right ventricle free wall longitudinal strain (RVFWLS). Methods: This study is a prospective cohort conducted from January to December 2022 on thirty-four cancer patients undergoing chemotherapy at Dr Wahidin Sudirohusodo Hospital Makassar. The RVFWLS was assessed before and after chemotherapy using echocardiographic. Result: The RVFWLS value after one cycle chemotherapy decreased compared pre-chemotherapy (4.40±4.84; p=0.001). Conclusion: The effect of chemotherapy is a decrease the RVFWLS value after the first cycle of chemotherapy. It should be considered for echocardiographic evaluation, especially the RVFWLS value every cycle of chemotherapy.

Abstract

Introduction: Obesity poses a severe threat to global health, making it a significant epidemiological concern that requires special attention. It is associated with several risk factors for various diseases. Alarmingly, one in three Indonesian adults is obese, with the obesity rate steadily increasing. Studies have shown that excess fat mass is linked to higher mortality and morbidity from cardiovascular disease, which is currently Indonesia's leading cause of death. To assess a person's functional capacity and ability for basic physical activity, the 6-minute walking test (6MWT) is utilized, as it proves to be a powerful predictor of morbidity and mortality rates. Therefore, this study aims to analyze the association between VO2 max using 6MWT in obese patients of Makassar City. Methods: This study was a cross-sectional study. The samples comprised all obese individuals who provided signed approval letters and were registered at Makassar City's 20 Public Health Centers. Consecutive sampling was used to gather the samples. Subsequently, the samples were examined and categorized based on their level of obesity.   The 6MWT results are then used with Nury's Formula to produce an estimated VO2max result. To analyze the association between VO2 max using 6MWT and obesity, statistical analysis was conducted using the Mann-Whitney test, with significant results indicated by (p < 0.05). Results: A total of 163 participants comprised the study's sample, with the majority being female and aged 50 years or older. After filling out the questionnaire, data show that most of the obese participants had a family history of cardiovascular disease (CVD), experienced significant stress levels, and had average BMIs falling into the category of obesity. According to the study, the average distance covered in the 6-minute walking test was 340.18 meters (±82.6 meters). When the study participants were ranked by their estimated VO2 max, the average value was 15.10±5.52 for those with first-degree obesity. Following them were participants with second-degree obesity, with results averaging 15.03±5.39 (p = 0.021). Conclusion: A significant inverse association exists between obesity and VO2 max, which means that the more obese a person is, the lower their VO2 max tends to be.

Abstract

Methicillin-Resistant Staphylococcus aureus (MRSA) dapat menyebabkan infeksi yang didapat di rumah sakit (HA-MRSA) atau yang didapat di masyarakat (CA-MRSA). Dengan dampak morbiditas dan mortalitas yang lebih tinggi. Dalam kondisi sumber daya yang terbatas, ketersediaan deteksi cepat masih belum memadai di beberapa fasilitas laboratorium di Indonesia. Seringkali, membutuhkan waktu lebih dari 48 hingga 72 jam untuk mengidentifikasi MRSA di laboratorium. Oleh karena itu, deteksi cepat menggunakan metode alternatif sangatdiperlukan untuk mempersingkat waktu yang diperlukan untuk mengidentifikasinya. Semakin cepat teridentifikasi saat skrining nares anterior pada pasien pre operasi ortopedi, maka semakin cepat pula tindakan pencegahan maupun pengendalian infeksi yang dapat dilakukan di Rumah Sakit. Penelitian ini merupakan penelitian observasional dengan desain cross sectional study. Pada penelitian ini dilakukan pengambilan sampel swab nares anterior pasien rencana bedah ortopedi di RSUP Dr. WahidinSudirohusodo dan RS Universitas Hasanuddin Makassar menggunakan medium transport Amies gel agar. Setelah itu dilakukan inokulasi langsung ke media CHROMOGENIC AGAR MRSA, hasil identifikasi dapat diketahui segera setelah 18-24 jam inkubasi pada media tersebut. Hasil penelitian menunjukkan dari 184 sampel yang diperoleh, proporsi temuan karier MRSA dari swab nares anterior pasien pre-operasi ortopedi di RSUP Wahidin Sudirohusodo Makassar adalah 66 pasien (38.4%), dan 8 pasien (4.6%) MSSA, sedangkan 98 pasien (57%)terdeteksi non aureus. Adapun karakteristik pasien yang mempunyai hubungan bermakna dengan temuan MRSA pada swab nares anterior pasien pre-operasi ortopedi yaitu karakteristik administrasi, riwayat rawat inap RS 3 bulan sebelumnya, dan riwayat pengunaan antibiotik 6 bulan terakhir (p value :0.001; 0.015; 0.002). Hasil penelitian disimpulkan bahwa pemanfaatan media CHROMOGENIC AGAR MRSA sebagai salah satu metodekultur untuk skrining MRSA pada nares anterior pasien pre-operasi ortopedi dapat menjadi pilihan yang efektifdan efisien dalam aplikasi klinis di lingkungan Rumah Sakit. Hal ini mengurangi penggunaan banyak bahan dilaboratorium, memudahkan dalam identifikasi langsung koloni oleh karena kekhasan koloni MRSA padamedia CHROMOGENIC AGAR, serta mempersingkat waktu yang diperlukan untuk mengidentifikasi MRSApada pasien, sehingga dapat memudahkan tindakan pencegahan dan pengendalian penyebaran MRSA dilingkungan Rumah Sakit, membantu mengurangi lama rawat inap pasien maupun mengurangi kemungkinanpenggunaan antibiotik jangka panjang pada pasien.

Abstract

ABSTRAK Meningioma merupakan tumor jinak pada otak yang paling sering terjadi di Amerika. Identifikasi karakteristik klinis maupun histopatologi masih jarang dilakukan di Indonesia. Tujuan penelitian ini untuk melihat karakteristik klinis dan histopatologi meningioma di Makassar. Penelitian kami menggunakan metode deskriptif, yang dilakukan di Makassar dari Januari 2015 hingga Desember 2021. Karakteristik yang dinilai adalah usia, jenis kelamin, lokasi tumor, gejala klinis, serta tipe meningiomaAnalisis data menggunakan software SPSS v.20. Dari 65 subjek, usia >47,5 tahun paling banyak ditemukan yaitu 42 subjek (64,6%), paling banyak jenis kelamin wanita yaitu 51 subjek (78,5%). Lokasi meningioma paling banyak berada di regio temporal yaitu 20 subjek (30,8%), nyeri kepala sebagai gejala klinis umum yaitu sejumlah 31 subjek (31,1%). Meningioma atipikal merupakan subtype terbanyak, yaitu 19 subjek (29,2%).

Abstract

WHO introduced Covid-19 as a global health problem on January 30, 2020, and currently here are more than 6.05 million cases of Covid-19 reported from 188 countries and resulted in 368,000 deaths. One of the efforts recommended by the Indonesian Ministry of Health to prevent the spread of this infection is to limit social contact and practice clean and healthy living. This study develops a communication strategy in education on the prevention and handling of COVID-19 in South Sulawesi. Through a qualitative approach, data collection was carried out through focus group discussions to identify a picture of the community's response in dealing with this pandemic. This study resulted in infographics and guides containing information related to the prevention and handling of COVID-19. In addition, voice messages have been developed about 3M's behavior and the correct use of masks. A communication strategy by considering the local situation in the form of community responses is important when conducting education during a pandemic. This is due to the large amount of information spread in the mass media that is easily accessible by the public. In addition, the ever-changing situation also requires us to be able to continue to innovate in developing health messages that will be disseminated to the public. Messages created can include new information, confirmation or refutation of hoaxes that may develop.

Abstract

Introduction: Hirschsprung's disease (HD) is characterized by the absence of ganglion cells in Meissner's plexus and Auerbach's plexus which used to be assessed by histopathological examination using Hematoxylin and Eosin (H&E).  In some cases is difficult to assess ganglion cells, thus requiring additional examination in the form of calretinin immunohistochemistry to detect the presence of ganglion. Methods: The 59 samples of suspected HD  were analyzed by H&E staining followed by calretinin immunohistochemical staining. This study aimed to assess the sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of calretinin in HD diagnostics. Results:  The immunohistochemistry of calretinin showed a good diagnostic value in detecting aganglionosis in patients with suspected HD with a sensitivity value = 97.7%; Specificity = 100%; Positive Presumptive Value (PPV) = 100%; Negative Presumptive Value (NPV) = 93.7%; Accuracy= 98.3%. Thus, calretinin immunohistochemistry can be relied upon as an additional method in the diagnosis of HD

Abstract

Introduction:  Hepatitis B virus infection in fetal and newborn may result from the intrauterine vertical transmission. The purpose of our study is to determine the association between HBsAg expression in the placental and HBsAg serum in pregnant women. Methods: Total 67 placental tissues and serum were obtained from HBV-infected mothers. This study was conducted in various hospitals in Makassar, Indonesia from December 2017 until August 2018. Maternal serum and placental tissues were tested by ELISA and Immunohistochemistry staining. Results: 67 samples from placental tissues and serum were categorized into two different levels of serum titer and staining positivity. Twenty nine samples (43.3%) were categorized in low titer of HBsAg positive serum and 38 samples (56.7%) in high titer of  HBsAg positive serum. From the total of 67 placental samples, 43 (64.2%) samples had positive expression of HBsAg immunohistochemistry staining and 24 (34.8%) samples had negative expressions. Conclusions: There was a significant correlation between the expressions of HBsAg in the placental tissues and serum titers of HBV-infected mothers that is important in predicting the risk of transplacental transmission. The results of this study can be used as guidelines for the prevention and therapeutic approach of mother-to-child HBV infection.

Abstract

Introduction: Hepatitis C and B are two infectious diseases that increase distribution and high mortality and morbidity. Conditions that exacerbate the economic burden of these two viral hepatitis infections are complications of cirrhosis and gastrointestinal bleeding. This study sought to assess the risk levels of hepatitis C and B across various districts and cities in South Sulawesi. It involved identifying spatial clusters of affected individuals through an analytical model, employing Generalized Poisson Regression to evaluate the potential impacts of these viral infections. Methods:  The natural break method is a quantifiable technique for identifying value clusters indicative of data distribution and for detecting geographical illness clusters, using Global Moran’s I statistics. This research utilizes data from health insurance members diagnosed with hepatitis C and B between January 2020 and December 2022, including 24 districts and cities in South Sulawesi Province. Results:  According to the results of the Local Morans I analysis, there is no significant clustered hepatitis C and B in all districts and cities in South Sulawesi from 2020 until 2022. Conclusions: In South Sulawesi, the incidence of hepatitis C and B cases reported to National Social Health Insurance Administration Body (BPJS Kesehatan) appeared to manifest randomly or sporadically.
