Abstract

Myasthenia gravis is one of the most common autoimmune diseases at the neuromuscular junction. Although this disease is rare, the incidence of this disease is believed to have increased in the last seven decades. This community service activity aims to provide education to the public regarding myasthenia gravis. This community service activity was carried out in the form of counseling at the Neurology Polyclinic of UNRAM Hospital on Friday, January 20 2023. All participants were asked to take part in all counseling activities which consisted of pre-test activities, delivery of material on the topic of myasthenia gravis, and post-test. The enthusiasm of the participants in this activity was presented in the form of photo documentation. Statistical analysis using paired t-test was carried out to analyze the difference between the pre-test and post-test mean values ​​and the results were considered significant if a p value <0.05 was obtained. As many as 23 participants who attended the Neurology Polyclinic at UNRAM Hospital enthusiastically participated in the whole series of activities. The average participant post-test score was significantly higher than the pre-test means score (p = 0.025). This activity was able to increase the participants' knowledge about myasthenia gravis.

Abstract

Parkinson's disease is a neurodegenerative disease that is quite common in the elderly population, which is the second most common after Alzheimer's disease. Patients with advanced Parkinson's disease will rely heavily on family members to carry out their basic daily activities. This activity aims to disseminate information related to Parkinson's disease to visitors to the Neurology Polyclinic at the University of Mataram Hospital. The activities carried out included pre-tests, topic presentations, and post-tests. The enthusiasm of the participants in this activity was recorded as a photo documentation. Statistical analysis using paired t-test was carried out to test whether there was a significant difference between the mean pre-test and post-test scores of the participants. A total of 15 participants participated in this whole series of activities with great enthusiasm. There was a significant increase in the post-test mean value compared to the pre-test mean value (p = 0.002). Community service activities in the form of counseling are effective in increasing participants' knowledge about Parkinson's disease.

Abstract

Stroke is the leading cause of death and disability in the world and Indonesia. The standard therapy for acute ischemic stroke is alteplase but only 10-20% of patients receive this standard therapy. Code stroke is quick response for management acute stroke to get alteplase. The success for code stroke requires good communication and cooperation of code stroke team. This training aims to provide code stroke training needed to increase the capacity of stroke services at Mataram University Hospital. The method of training include lecture, patient simulation, video on the code stroke and also preparation of the thrombolysis also discussion and the target of this training are doctors and staffs in the emergency wards, radiology, laboratorium, pharmacy, ICU and inpatient ward. Evaluation of participants' understanding through pre and post tests was assessed with the Kahoot application. Evaluation of the training was also assessed using the Google form. A total of 25 health workers participated in this activity. The mean pre and post test were 34,38% and 68,42% (an increase of 34%). Participants' satisfaction with the activity was good, namely 4.13 (from a Likert scale of 0 – 5). The aspect of material provided in this training had the highest satisfaction score. This education should be given periodically to all health workers to increase the capacity of stroke services.

Abstract

Protein S (PS) is Vitamin K dependant protein which plays a role in regulating the blood clotting process in the body. Protein S deficiency is explained as a condition characterized by reduced levels of protein S in the serum so that it makes the blood clot too easily. The purpose of this literature review is to give an update of protein S defficiency and the mechanism of protein s in hemostasis and atherosclerosis based on publish literature reference from various database. Protein S plays important role in hemostasis by regulating blood cloth process in the initiation phase of the extrinsic pathway as a TFPI cofactor or in the propagation phase, namely as an APC cofactor and independent activity of TFPIα and APC. Protein S deficiency is known to be associated with the occurrence of atherosclerosis via the Tyro3-Axl-Mer (TAM) receptor. The main clinical manifestations of the majority of patients with heterozygous mutations in the protein S (PROS1) gene are deep vein thrombosis (DVT) and pulmonary embolism that precipitate by VTE. Administration of anticoagulant drugs such as heparin in the form of low-molecular-weight heparin (LMWH) or unfractionated heparin, vitamin K antagonists (VKA), namely warfarin, or direct oral anticoagulants (DOACs) can be selected as a management of protein S deficiency with VTE. Protein S has a central role in the coagulation prosses and its deficiency associated with atherosclerotic conditions.

Abstract

Neutrophils, also known as polymorphonuclear leukocytes, play a significant role in fueling systemic inflammation and tissue damage within the context of frailty, a clinical syndrome associated with increased susceptibility to stressors and decreased functional performance typically linked to aging. This literature review delves into the interplay between neutrophils and frailty, shedding light on their contribution to age-related systemic inflammation. The investigation explored the correlation between heightened neutrophil levels, diminished physical activity, increased frailty, and elevated white blood cell counts. These findings underscore the substantial role of neutrophils in the development of frailty, necessitating further research to enhance comprehension of their involvement for more effective prevention and intervention strategies. The implications of this study emphasize the importance of tailored exercise regimens, promoting optimal nutrition practices, efficiently managing chronic inflammation, and improving the control of chronic diseases as essential approaches to address frailty among older individuals.

Abstract

 CORRELATION BETWEEN NEUTROFIL LIMPHOCYTE RATIO AND CEREBRAL INFARCTION VOLUME IN ACUTE ISCHAEMIC STROKEABSTRACTIntroduction: Inflammation plays a key role in stroke pathophysiology. Neutrophils is one of the earliest leucocyte subtypes to infiltrate the ischemia area of the brain. The neutrophil-lymphocyte ratio (NLR) is independent of the severity of coronary heart disease and as a predictor of poor clinical outcomes in patients with coronary heart disease undergoing angiography. The neutrophil-lymphocyte ratio values may be a prognostic factor in ischemic stroke patients. The neutrophil- lymphocyte ratio values can also be used in predicting infarct size, however limited study has been conducted in this area.Aim: To determine the correlation of NLR with cerebral infarct volume in acute ischemic stroke patients.Method: This was an analytical observational study with cross sectional design. The population for this study were acute ischemic stroke patients at NTB Genereal Hospital. The collected data were analyzed by Spearman correlation test.Results: Approximately 52 participants  were enrolled in this study with mean age was 59.79±8.65 years old. Hypertension was a common modifiable risk factor identified in more than 94.2 % participant. The mean of neutrophil lymphocyte ratio was 3.94±2.96 and mean of infarct volume was 13.96±37.26cm3. There was correlation between RNL and cerebral infract volume (r=+0.351; p=0.023).Discussions: Higher lymphocyte neutrophil ratio in acute ischemic stroke patients would result in an increase of brain infarct volume.Keyword: Infarct volume, ischaemic stroke, neutrophil lymphocyte ratioABSTRAKPendahuluan: Inflamasi memegang peranan penting dalam patofisiologi stroke. Salah satu subtipe dari sel leukosit yang paling awal menginfiltrasi ke area iskemia di otak adalah neutrofil. Rasio neutrofil limfosit (RNL) merupakan faktor independen terhadap beratnya penyakit jantung koroner dan sebagai prediktor luaran klinis yang buruk pada penderita penyakit jantung koroner yang menjalani angiografi. Nilai RNL dapat menjadi faktor prognostik pada penderita stroke iskemik. Nilai RNL juga bisa digunakan dalam memprediksi ukuran infark namun belum dilakukan penelitian hingga saat ini.Tujuan: Mengetahui korelasi RNL dengan volume infark serebri pada penderita stroke iskemik akut.Metode: Penelitian analitik observasional dengan desain potong lintang terhadap pasien stroke iskemik akut di RSUP NTB. Data yang terkumpul dianalisis dengan uji korelasi Spearman.Hasil: Didapatkan 52 subjek dengan rerata umur 59,79±8,65 tahun dan faktor risiko utama hipertensi (94,2%). Didapatkan rerata rasio neutrofil limfosit 3,94±2,96 dan rerata volume infark 13,96±37,26cm3. Terdapat korelasi antara RNL dengan volume infark serebri (r=+0,351; p=0,023).Diskusi: Semakin tinggi nilai rasio neutrofil limfosit mengakibatkan semakin luasnya volume infark serebri pada penderita stroke iskemik akut.Kata kunci: Rasio neutrofil limfosit, stroke iskemik, volume infark

Abstract

Cognitive impairment in hypertensive patients is a significant clinical problem. It is a prevalent condition that has a major impact on quality of life. Hypertension, characterized by persistently elevated blood pressure, has been identified as a potential risk factor for cognitive impairment. Assessing cognitive function in hypertensive patients is essential for early detection and intervention. Community participation is crucial to raise awareness about the symptoms of cognitive impairment in hypertensive patients. Therefore, education on cognitive impairment associated with hypertension is necessary to reduce its morbidity. This community service activity aims to provide education on the definition, signs, and symptoms, as well as early detection methods of cognitive impairment in hypertensive patients to patients and their families at the neurology clinic University of Mataram Hospital. The activity was conducted using an interactive method with the aid of PowerPoint slides. The participants were given a pre-test, followed by a presentation on the disease, a discussion session, and finally, a post-test. There were 14 participants in total. The education session began with a pre-test consisting of five questions about cognitive impairment and hypertension. The participants' average score was 74.2. The material covered definitions, symptoms, early signs, and how to conduct early detection of cognitive impairment and hypertension. The education session concluded with a post-test, which revealed that 10 participants (71.4%) showed an increase in their knowledge scores, with a mean score of 88.5. Effective communication, information, and education are essential in improving knowledge about the early detection of cognitive impairment in hypertensive patients.

Abstract

Abstract: Study Literature: Patomekanisme Gangguan Kognitif Pada Pasien Hipertensi. Menurut Konsensus Penatalaksanaan Hipertensi oleh Indonesian Society of Hypertension (INaSH), hipertensi emergensi merupakan situasi hipertensi derajat 3 yang diikuti dengan hypertension-mediated organ damage (HMOD) akut. Atau dapat didefinisikan sebagai peningkatan tekanan darah sistol dan diastol yang berada di atas 180/110 mm Hg. Hipertensi masih menjadi permasalahan global dan diperkirakan akan meningkat sebanyak 29% pada tahun 2025, dengan angka mortalitas mencapai 4,6%. Sekitar 75 juta atau 32% dari orang dewasa di Amerika Serikat mengalami hipertensi dengan sebanyak 1%-3% pasien dapat mengalami krisis hipertensi. Hipertensi memiliki hubungan yang erat dengan peningkatan risiko terjadinya disfungsi kognitif terutama pada individu lanjut usia. Ketika seseorang lansia mengalami hipertensi, akan terjadi gangguan umum pada beberapa mekanisme homeostasis termasuk regulasi aliran darah ke otak dan tekanan mikrovaskular. Salah satu contoh dari gangguan kognitif yang dapat terjadi adalah alzheimer.

Abstract

Highlight: Chemotherapy induces neurotoxicity through DNA crosslink, impaired calcium homeostasis, mitochondrial damage, increased reactive oxygen species, pro-inflammatory cascade, axon degeneration, and programmed cell death. CIPN has a primary impact on the sensory neuron. CIPN is diagnosed based on the patient's history, chemotherapy history, and neurologic examination. Some pharmacological and non-pharmacological treatments are hypothesized to reduce CIPN symptoms, but only duloxetine is recommended.   ABSTRACT Chemotherapy-induced peripheral neuropathy (CIPN) is the most common and severe neurological side effect of many commonly used chemotherapy agents. It affects more than 60% of cancer patients. Approximately 30%–40% of patients have persistent symptoms five months or longer after stopping treatment. Even years after completing chemotherapy, some patients still experience CIPN symptoms. CIPN increases the annual cost of healthcare, leads to detrimental dose reduction and even cessation of treatment, and severely affects cancer survivors’ quality of life. Chemotherapy induces neurotoxicity through a variety of mechanisms that lead to neuronal cell damage or cell death. This mechanism of neurotoxicity varies depending on the specific agent. CIPN is characterized predominantly by sensory axonal peripheral neuropathy. Motor and autonomic symptoms may appear, but less frequently. To diagnose CIPN, a thorough patient's history and neurological examination are required. The current approach to CIPN management focuses on managing the symptoms of neuropathic pain and reducing or stopping the chemotherapy agent when CIPN manifests. There is no proven or advised prophylaxis therapy for CIPN. The point of this review was to talk about how some commonly used chemotherapy agents (such as platinum-based compounds, taxanes, vinca alkaloids, bortezomib, and thalidomide) cause CIPN, how to diagnose it, and the newest treatments that are available.

Abstract

Latar belakang: Decompression Sickness (DCS) adalah penyakit multisistem disebabkan oleh gelembung gas terlarut yang terbentuk dalam darah dan jaringan selama atau pasca penurunan tekanan lingkungan mendadak. Isi: Insiden DCS jarang terjadi, hanya sekitar 3 kasus per 10.000 penyelaman. Insidennya meningkat pada nelayan penyelam tradisional. Inner Ear Barotrauma (IEB) merupakan suatu kerusakan jaringan fisik yang terjadi akibat ketidakseimbangan antara tekanan udara pada ruang telinga tengah dan telinga dalam. Alternobaric vertigo (AV) merupakan suatu kondisi kejadian vertigo sementara yang biasanya akan berkembang saat naik atau melakukan manuver valsava. Faktor risiko lainya pada AV meliputi paparan kebisingan saat melakukan penyelaman. Review ini menggunakan Inner Ear Disorders in SCUBA Divers: A Review sebagai panduan dalam menyusun artikel. Penelusuran menggunakan Google Scholar, Pubmed, dan Plos One. Kesimpulan: Decompression Sickness (DCS) dapat menyebabkan Gangguan keseimbangan pada IEB, Inner Ear Decompression Sickness, dan AV.
