Abstract

Single nucleotide polymorphisms (SNPs) are the most common form of genetic variation in humans. Missense SNPs can change protein structure and function. This study aimed to determine missense SNPs of the IRAK1 gene that can affect the amino acid sequence and lead to changes in protein structure and function, as well as their relationship as a risk factor for SLE. In this in silico method, several bioinformatics tools have been used to identify missense SNPs, including their properties and impacts, as well as their interaction networks with proteins. The tools used were PolyPhenv2, SIFT, PhD-SNP, PROVEAN, SNAP, Panthers, I-Mutant 3.0, and GeneMania. Four missense SNPs, rs11465830, rs1059702, rs1059703, and 10127175, were obtained from the NCBI SNP database. The SIFT test results showed that all the SNPs were tolerant. In the test results obtained using PolyPhen, the four SNPs were benign. The results of the probe test indicated that the four SNPs were neutral. When tested with SNAP, one SNP was neutral, and three others had an impact. In the PhD-SNP test, all SNPs were neutral. In the panther test, all SNPs were benign. The I-mutant assay showed that the four SNPs could decrease protein stability. Tests with GeneMania have reported that most interactions between genes were between IRAK1 and MYD88, and physical interactions were the most dominant form of interaction. Conclusion. rs10127175, rs11465830, rs1059702, and rs1059703 are missense SNPs in IRAK1, which can disrupt protein stability and be a risk factor for SLE. Abstract

Children are a crucial population requiring COVID-19 vaccination. With the global rollout of COVID-19 vaccines, the increasing number of unvaccinated children and adolescents has raised concerns. To was translated, adapted, and validate the Health Belief Model instrument for parents of children aged 6–11 years regarding COVID-19 vaccination in the Indonesian language. Cross-cultural translation and adaptation were conducted according to international standards. The instrument testing process, according to Beaton et al., (2020), involves five stages: forward translation, synthesis of the translation results, back translation, review of the back translation, and readability testing. Respondents were purposively selected from the parents of children aged 6–11 years. Face and content validity were achieved for the Indonesian version of Health Belief Model for parents of children aged 6–11 years. The validity of each statement item was assessed using Pearson Correlation with a significance level of 0.05 and 193 respondents, resulting in an r table value of 0.1406. The constructs within the Health Belief Model were deemed valid based on the Pearson Correlation values, as the coefficient values were greater than the r-table value (r > 0.1406; p < 0.05). The constructs in the Health Belief Model were also reliable, with a Cronbach’s alpha value of 0.657. The culturally adapted Indonesian version of the Health Belief Model instrument for parents of children aged 6–11 years regarding COVID-19 vaccination is considered valid and reliable for use in research among the parent population of children aged 6–11 years in the context of COVID-19 vaccination.

Abstract

The incidence of dengue virus infection in the D.I. Yogyakarta reached 3,618 cases in 2020. Based on its clinical manifestations, dengue virus infection is divided into Dengue Fever (DF), Dengue Haemorrhagic Fever (DHF), and Dengue Shock Syndrome (DSS). This study aims to identify the level of knowledge, quality of life, and costs of dengue patients at PKU Muhammadiyah Gamping Hospital from March 2021 to October 2021. This study used a cross-sectional design. The measurement of the patient's knowledge level used a questionnaire knowledge of dengue, while the quality of life was assessed using EQ-5D-5L for adult patients and EQ-5D-Y for pediatric patients. The cost of illness data from the hospital finance database. This study involved 31 respondents with the characteristics aged 16-40 years (54.84%), male (87.1%), using BPJS insurance (80.65%), and an average length of stay of 4 days (51.61%). The patient's level of knowledge about dengue, knowledge of dengue prevention, and dengue prevention behavior reached 74.20% (sufficient), 74.20% (sufficient), and 77.42% (sufficient) respectively. The quality of life of dengue patients reached 100% and 85.2% in DF patients, while the DHF patients felt a few anxious/depressed (sad). The average cost of illness in DF patients was Rp. 2,796,386 and Rp. 3,378,537 in DHF patients.

Abstract

Background: Long-term treatment of tuberculosis can cause side effects that cause concern for patients. The side effects experienced by patients have an impact on treatment compliance and drug withdrawal. Efforts to control drug withdrawal require the role of pharmacists and cadres in monitoring treatment. Objective: to explore the perception of understanding and actions of pharmacists and cadres in monitoring anti-tuberculosis drugs. Method: This study is qualitative study with a case study approach. Participants selection using purposive sampling namely pharmacists, cadres and TB program holders who are involved in monitoring TB treatment. The number of participants was six people. Results: interview transcripts were analyzed thematically using software QSR NVivo 12. We categorized into three themes : drug side effects, medication adherence, and treatment success. Conclusion: pharmacists' understanding and actions in monitoring TB treatment are good. While the cadres' understanding and actions regarding the side effects of drugs are not good enough, the importance of compliance with taking medication and the success of treatment is good

Abstract

Latar belakang: Demam neutropenia merupakan salah satu efek samping kemoterapi yang paling serius, karena adanya infeksi dapat berkembang dengan cepat dan dapat mengancam jiwa. Antibiotik yang diindikasikan untuk profilaksis pada pasien demam neutropenia diantaranya adalah sefepim. Penelitian ini bertujuan untuk mengetahui durasi kebutuhan sefepim terhadap perbedaan rejimen kemoterapi AML, serta luaran terapinya di RSUP Dr. Kariadi, Semarang. Metode: Penelitian ini menggunakan rancangan deskriptif dengan pengambilan data secara restrospektif dari bulan Juni 2018 hingga April 2019 di satu ruangan rawat inap RSUP Dr. Kariadi, Semarang. Kriteria inklusi adalah pasien usia 18-64 tahun dengan diagnosa AML yang mengalami demam neutropenia dengan riwayat pernah menjalani kemoterapi, mendapatkan terapi antibiotik sefepim. Kriteria eklusi dalam penelitian ini yaitu pasien yang alergi terhadap sefepim, yang tidak melanjutkan pengobatannya, dengan program cangkok sumsum tulang dan dengan adanya diagnosa sepsis/septik syok sebelum diberikannya sefepim. Hasil: Total sebanyak 15 pasien memenuhi kriteria inklusi. Durasi pemberian sefepim yang paling banyak adalah kelompok dengan pemberian 8-14 hari (46,7%), selanjutnya ?7 hari (33,3%), sedangkan pemberian selama >14 hari tidak ditemukan. Pasien yang meninggal pada penelitian ini sebanyak 3 pasien (20%), dengan 2 pasien diantaranya teridentifikasi mengalami sepsis karena sefepim yang resisten akibat patogen ESBL E. coli, sedangkan satu lainnya tidak diketahui patogen penyebabnya. Kesimpulan: Pasien dengan rejimen intensif rata-rata memerlukan durasi terapi yang lebih lama. Munculnya patogen ESBL khususnya E. coli mempunyai luaran yang jelek dan angka kematian yang lebih tinggi, sehingga kita sarankan untuk menggunakan karbapenem daripada sefepim untuk kondisi ini. Kata kunci: Demam neutropenia, leukemia myeloid akut, sefepim Backgound: Febrile neutropenia is one of the most serious side effects of chemotherapy, because the infection can develop rapidly which can be life-threatening. Cefepime is drug of choice that indicated for prophylaxis in patients with febrile neutropenia. The aim of this study was to determine the duration of the cefepime needs of differences in AML chemotherapy regimens, and the outcomes in Dr. Kariadi General Hospital Center, Semarang. Method: The study used descriptive design with retrospective data collection from June 2018 to April 2019 in an inpatient room at Dr. Kariadi General Hospital Center, Semarang. The inclusion criteria are patients aged 18-64 years old with diagnosis of AML who has febrile neutropenia with a history of having undergone chemotherapy with cefepime prophylaxis. The exclusion criteria in this study were allergy to cefepime, patients who did not continue their treatment, bone marrow transplant programs and diagnosis of sepsis / septic shock before the administration of cefepime. Result: A total of 15 patients met the inclusion criteria. The most duration of cefepime was in the group with 8-14 days (46,7%), then ?7 days (33,3%), whereas> 14 days were not found. The patients who died in this study were 3 patients (20%), with 2 patients identified as having sepsis due to the pathogen ESBL E. coli cefepime resistant, while the other one did not know the causative pathogen. Conclusion: Patients with an intensive regimen require a longer duration therapy. The emergence of ESBL pathogens especially E. coli has poor outcomes and a higher mortality rate, so we recommend using carbapenem rather than cefepime for this condition. Abstract

Latar belakang: Kanker kolorektal saat ini sudah menjadi penyebab utama ketiga kematian akibat kanker di dunia, penyakit ini membutuhkan terapi yang progresif dimana salah satu terapinya adalah bevacizumab. Namun diketahui bevacizumab dapat menimbulkan hipertensi pada sebagian pasien. Penelitian ini bertujuan untuk untuk mengetahui onset terjadinya hipertensi akibat bevacizumab dengan rejimen kemoterapi apa yang digunakan. Metode: Penelitian ini merupakan sebuah tinjauan deskriptif restrospektif yang dilakukan di RSUP Dr. Kariadi, Semarang. Kriteria inklusi terdiri dari pasien kanker kolorektal metastatik yang berusia ? 18 tahun dengan hipertensi tingkat ?2 berdasarkan NCI CTCAE version 5.0 setelah diberikan terapi bevacizumab 5 mg/kg berat badan di RSUP Dr. Kariadi. Pengambilan data dalam 1 tahun (bulan April 2018 hingga April 2019) melalui peninjauan dari rekam medis, laporan penggunaan obat bevacizumab dan laporan penggunaan obat kardiovaskuler instalasi farmasi. Kriteria ekslusi dalam penelitian ini yaitu hipertensi tingkat ?2 berdasarkan NCI CTCAE version 5.0 yang muncul setelah ?4 kali waktu paruh bevacizumab (t 1/2 = 20 hari) dari kemoterapi yang terakhir. Hasil: Total 95 pasien sesuai kriteria, sebanyak 24 pasien (25,26%) teridentifikasi mengalami hipertensi tingkat ? 2 berdasarkan NCI CTCAE version 5.0. Dengan 20 pasien (83,33%) pasien mendapatkan kemoterapi rejimen FOLFOX4, sedangkan sisanya (16,66%) dengan rejimen de Gramont. Hipertensi ini muncul sebagian besar pada siklus III, diikuti siklus ke II, ke I dan ke VI. Tidak satupun pasien dengan kombinasi FOLFIRI yang terdeteksi mengalami hipertensi ini. Kesimpulan: Penelitian ini menunjukkan onset munculnya hipertensi tingkat ?2 NCI CTCAE version 5.0 paling banyak adalah pada siklus awal terapi yaitu I-III dan dengan rejimen kombinasi FOLFOX4 + bevacizumab. Kata Kunci: Kanker kolorektal, bevacizumab, hipertensi Background: Colorectal cancer nowadays is the third leading cause of cancer deaths in the world, this disease requires progressive therapy where one of the treatments is bevacizumab. But it is known that bevacizumab can cause hypertension in some patients. This study aims to determine the onset of the hypertension with chemotherapy regimen used. Method: This research is a retrospective descriptive review conducted at Dr. Kariadi General Hospital, Semarang. Inclusion criteria consisted of metastatic colorectal cancer patients aged ?18 years old with hypertension level ?2 based on NCI CTCAE version 5.0 after being given bevacizumab therapy dose 5 mg/kg body weight. Retrieval of data in 1 year (April 2018 to April 2019) through a review of medical records, reports on the use of bevacizumab drugs and reports on the use of cardiovascular drugs in Department of Pharmacy. The exclusion criteria in this study were hypertension level on NCI CTCAE version 5.0 which appeared after 4 times the half-life of bevacizumab (t 1/2 = 20 days) from the last chemotherapy. Result: Total 95 patients according to the criteria, 24 patients (25.26%) were identified as having hypertension level ? 2 based on NCI CTCAE version 5.0. With 20 patients (83.33%) patients received FOLFOX4 regimen, while the rest (16.66%) with de Gramont regimen. This hypertension occurs mostly in cycle III, followed by cycle II, to I and to VI. None of the patients with a combination of FOLFIRI. Conclusion: This research shows that the onset of hypertension level ?2 NCI CTCAE version 5.0 mostly in the initial cycle of therapy (I-III) with the FOLFOX4 + bevacizumab combination regimen. Abstract

Latar belakang: Reaksi obat yang merugikan (adverse drug reaction) sering muncul salah satunya pada obat-obat sitostatika. Ifosfamid adalah obat sitostatika golongan pengalkil yang ketika dimetabolisme akan membentuk senyawa aktif nitrogen mustard sitotoksik. Selain itu ia juga membentuk senyawa lain yaitu akrolein dan kloroasetaldehid. Akrolein yang bersifat urotoksik ini dengan mudah akan diikat oleh mesna, sedangkan kloroasetaldehid bersifat neurotoksik yang hingga sampai saat ini masih menjadi masalah karena keterbatasan panduan untuk anti dotumnya. Kasus: Kami mendeskripsikan 3 pasien dengan diagnosis berbeda yang mengalami gangguan neuropsikiatri paska kemoterapi ifosfamid yaitu kanker saluran empedu dengan mengantuk berat, kanker ovarium residif dengan halusinasi, dan tumor ganas pelvis dengan penurunan kesadaran disertai kejang. Pembahasan: Gangguan neuropsikiatri dapat disebabkan oleh neurotoksisitas dari hasil metabolisme dari ifosfamid yaitu kloroasetaldehid yang secara langsung maupun tidak langsung merusak neuron (neurotoksik). Manifestasi klinisnya dapat bertingkat mulai dari kebingungan dan mengantuk hingga kejang, koma dan kematian. Onset munculnya gejala terutama hitungan jam hingga 2 hari setelah kemoterapi selesai. Terdapat faktor resiko munculnya neurotoksisitas yang teridentifikasi pada laporan kasus ini yaitu gangguan fungsi ginjal dan hipoalbumin. Beberapa penelitian mengatakan bahwa toksistas ini berifat reversible, demikian juga dengan pasien kasus pertama yang membaik setelah 24 jam, dan kasus kedua yang membaik setelah 5 hari, namun pemberian terapi spesifik tetap direkomendasikan pada NCI stadium III atau IV. Kesimpulan: Gangguan neuropsikiatri paska kemoterapi ifosfamid dapat disebabkan oleh neurotoksisitas pada SSP akibat kloroasetaldehid dengan faktor resiko yaitu gangguan fungsi ginjal dan hipoalbumin. Diperlukan penelitian lebih lanjut untuk memastikan bahwa gejala neuropsikiatri yang muncul adalah murni karena ifosfamid, misalnya dengan melakukan pengukuran kadar obat di dalam darah.

Abstract

Diabetic foot ulcer (DFU) has been associated with genetic and environmental factors, which could potentially have a role in DFU development. Single nucleotide polymorphisms (SNPs) in genes linked to DFU, including inflammation. Bibliometric studies on the SNP on genes affecting DFU still have not been evaluated. This study aims to depict bibliographically and understand the topic trend of genetic polymorphism and the risk of DFU publications. A bibliometric methodology was applied in this study. The data were extracted through the Scopus database from 2011 to 2021. VOS viewer was used to classify and summarize Scopus articles. The 35 articles were evaluated. India topped the list of countries with the most publications, and Tehran University of Medical Sciences was the primary institution. Singh K and his team were the first contributing authors with 44 citations. Keywords analysis indicated that the research hotspots were DFU, type 2 diabetes mellitus (T2DM), diabetic foot ulcers, polymorphisms, hypoxia, vascular endothelial growth factor (VEGF), diabetic foot, diabetes mellitus, and oxidative stress. This study summarizes the current state, trends in genetic polymorphism and risk with DFU research. It may provide researchers with insight into the genetic polymorphism and risk associated with DFU research, as well as useful information for identifying possible collaborators and partner institutions.

Abstract

Timely detection of patients with a high mortality risk in coronavirus disease 2019 (COVID-19) can substantially improve triage, bed allocation, time reduction, and potential outcomes. A potential solution is using machine learning (ML) algorithms to predict mortality in COVID-19 hospitalized patients. The study's objective was to create and verify individual risk assessments for mortality using anonymous demographic, clinical, and laboratory findings at admission, as well as to assess the possibility of death using machine learning. We used a standardized format and electronic medical records. Data from 2,313 patients were collected from two Muhammadiyah hospitals from January 2020 to July 2022. Utilizing each patient's clinical manifestation state at admission and laboratory parameters, 24 demographic, clinical, and laboratory results were studied. The algorithms analyzed were AdaBoost, logistic regression, random forest, support vector machine, naïve Bayes, and decision tree, which were applied through WEKA version 3.8.6. Random forest performed better than the other machine learning techniques, with precision, sensitivity, receiver operating characteristic (ROC), and accuracy of 78.6%, 78.7%, 85%, and 78.65%, respectively. The three top predictors were septic shock (OR=21.518, 95% CI=4.933–93.853), respiratory failure (OR=15.503, 95% CI=8.507–28.254), and D-dimer (OR=3.288, 95% CI=2.510–4.306). Machine learning–based predictive models, especially the random forest algorithm, may make it easier to identify patients at high risk of death and guide physicians' appropriate interventions.

Abstract

COVID-19 is a disease that requires antivirals, such as favipiravir and remdesivir, which have been recommended in Indonesia. Choosing the right antiviral agent can affect the length and total cost of hospitalization. The impact of COVID-19 can cause a decline in health and the economy, if not treated effectively. This study aimed to determine the average length of hospitalization and total cost of hospitalization for COVID-19 patients based on the selection of favipiravir and remdesivir antivirals at the PKU Muhammadiyah Gamping Hospital from August 2022-August to 2023 based on real world data. The research method was Observational research with a descriptive research design and retrospective data collection was used. During this period, 306 patients were diagnosed with COVID-19. A total of 171 patients met the inclusion criteria, including 130 patients using favipiravir antivirus and 41 patients using remdesivir antivirus. The results of this study are the average length of hospitalization of the patient group with antiviral favipiravir was 4.1 days and the patient group with antiviral remdesivir was 5 days and the average total cost of hospitalization of the patient group with antiviral favipiravir was IDR 12,377,415 and the average total cost of the patient group with antiviral remdesivir was IDR 14,428,132. The conclusion of this study is that the use of favipiravir antiviral therapy is more effective when viewed from the perspective of length of hospitalization and total cost of hospitalization; therefore, these results can be applied under the same conditions in the treatment of COVID-19.
