Abstract

Terapi utama pada pneumonia adalah antibiotika. Tingginya peresepan antibiotika berpeluang menimbulkan ketidaktepatan pemakaian antibiotika yang dapat menimbulkan Drug Related Problems (DRPs) dan memberikan dampak negatif pada perbaikan klinis pasien. Tujuan penelitian ini untuk mengetahui hubungan ketepatan peresepan antibiotika dengan perbaikan klinis pada pasien pneumonia rawat inap di rumah sakit PKU Muhammadiyah Gamping. Rancangan penelitian observasional analitik dengan studi cohort retrospektif, dimana kriteria inklusi merupakan pasien dewasa pneumonia komunitas atau Community Acquired Pneumonia (CAP) rawat inap yang mendapatkan terapi antibiotika dan kriteria eksklusi pasien memiliki infeksi lain selain pneumonia, pasien dengan gangguan imunitas (immunocompromised), pasien dengan penyakit penyerta berupa keganasan pada paru atau saluran napas dan pasien meninggal <72 jam selama perawatan di rumah sakit. Data diambil dari rekam medik (RM) periode Januari-Desember 2019. Subyek penelitian diperoleh 72 sampel. Analisis penelitian menggunakan uji Chi Square untuk meneliti hubungan ketepatan peresepan antibiotika berdasarkan tools Pharmaceutical Care Network Europe(PCNE) dengan perbaikan klinis. Perbaikan klinis yang diamati meliputi frekuensi pernapasan, suhu, dan jumlah leukosit. Hasil penelitian terdapat 29,2% peresepan antibiotika tepat dan 70,8% tidak tepat. Sebanyak 54,2% mengalami perbaikan jumlah leukosit, 83,3% mengalami perbaikan RR, dan 65% mengalami perbaikan suhu. Nilai signifikansi pada jumlah leukosit p=0,000, pada perbaikan RR p=0,014, dan pada perbaikan suhu p=0,098. Ketepatan peresepan antibiotika berhubungan secara signifikan terhadap perbaikan klinis jumlah leukosit dan RR dengan nilai p ≤ 0,05. Ketepatan peresepan antibiotika dengan perbaikan klinis suhu tidak didapatkan hubungan yang bermakna.

Abstract

Persalinan secara sectio caesarea semakin meningkat di Indonesia. Pelayanan medis dan terapi pada pasien sectio caesarea yang bervariasi dapat menimbulkan outcome terapi yang berbeda-beda yang dapat mempengaruhi biaya perawatan dan lama rawat inap pasien sehingga perlu adanya penerapan clinical pathway karena dapat meminimalkan biaya perawatan dan menurunkan lama rawat inap pasien di rumah sakit. Tujuan  penelitian ini adalah untuk menganalisis kesesuaian penerapan clinical pathway dan efektivitas biaya penerapan clinical pathway pada pasien sectio caesarea di RSUD Sele Be Solu Kota Sorong. Rancangan penelitian berupa deskriptif analitik retrospektif dan jenis penelitian farmakoekonomi yaitu analisis efektivitas biaya pada pasien rawat inap sectio caesarea dengan perspektif rumah sakit (provider). Pada penelitian ini, sampel yang diperoleh dilakukan analisis kesesuaian penerapan clinical pathway berdasarkan kriteria-kriteria yang terdapat pada clinical pathway rumah sakit sehingga diperoleh kelompok sesuai dan tidak sesuai clinical pathway. Data yang diambil pada sampel adalah biaya medis langsung dan length of stay pasien sectio caesarea pada periode Januari-Juni 2019 kemudian dilakukan analisis efektivitas biaya penerapan clinical pathway dengan parameter Incremental Cost Effectiveness Ratio (ICER). Terdapat 74 pasien yang masuk dalam subyek penelitian. Jumlah pasien yang sesuai clinical pathway 1 pasien (1,35 %) dan yang tidak sesuai clinical pathway sebanyak 73 pasien (98,65 %). Rata-rata biaya medis langsung pada kelompok sesuai Rp 20.544.809 dan kelompok tidak sesuai clinical pathway Rp 14.316.469 serta diperoleh ICER sebesar Rp 8.304.453/hari. Disimpulkan bahwa penerapan clinical pathway sectio caesarea di RSUD Sele Be Solu dapat menambah biaya sebesar Rp 8.304.453/hari tiap penambahan satu hari length of stay.

Abstract

Diabetes mellitus (DM) requires long-term therapy. Selection of the right therapy was very important to provide optimal results at an affordable cost. One of the main standards of oral antidiabetic therapy prescribed for type 2 DM (T2DM) is metformin, either alone or in combination therapy. The purpose of this study was to describe the direct medical costs in type 2 DM patients between metformin and metformin-glimepiride therapy. This research was an observational and descriptive. Demographic and clinical data were taken prospectively from the complete medical records of data patients with the main diagnosis of DM who fulfilled the inclusion criteria in the period January 2019–November 2020 at RSU Nene Mallomo Sidenreng Rappang, South Sulawesi and the two Public Health Centers namely Puskesmas Galur. II and Panjatan II Kulon Progo Yogyakarta. Cost data includes total medical costs from the hospital/ healthcare perspective (cost of drugs, doctor's services and checkups, fees for checking Fasting Blood Glucose/ Gula darah Puasa (GDP) and Blood Glucose Level While/ Gula Darah Sewaktu (GDS)) and total costs from the Badan Penyelenggara Jaminan Sosial (BPJS)/ payer perspective (INA-CBG's rates) were covered by insurance. The results showed that the average direct medical costs at the hospital and the two Public Health Centers respectively were metformin therapy in IDR (Indonesian Rupiah) namely IDR 120.736 and IDR 9.072,- and metformin-glimepiride therapy of IDR 126.298,- and IDR 18.634,-. It can be concluded that the average direct medical cost of metformin therapy is lower than metformin-glimepiride therapy.

Abstract

Metformin and metformin-glimepiride are cost-effective therapy options and are most commonly prescribed to treat type 2 diabetes mellitus (DM). This study aims to determine the cost-effectiveness of using metformin and metformin-glimepiride in patients with type 2 DM at Nene Mallomo Sidenreng Rappang General Hospital. This analytical descriptive observational research employed a retrospective cohort design. The data of type 2 DM patients, new and old patients, were obtained from the medical record unit. The target population was data of outpatients for the period January-December 2019. The parameter of the therapeutic effectiveness is the random blood sugar levels of the patients after receiving treatment. The data on cost from the hospital perspective were obtained from the administration and finance unit. Cost-effectiveness analysis employed calculations of the average cost-effectiveness ratio (ACER) and the incremental cost-effectiveness ratio (ICER). Meanwhile, the risk ratio (RR) employed the analytical chi-squared method to determine the relationship between the two types of therapies and their effectiveness. This study has revealed that 30 patients met the inclusion criteria; 14 patients received metformin therapy and 16 patients received metformin-glimepiride therapy. Metformin therapy is more effective (64.29%) and more costly (IDR120,736). The metformin's ACER value is 1877.99, and its ICER value is -3107.26. The cost-effectiveness analysis has revealed that metformin therapy is more cost-effective than metformin-glimepiride therapy. Meanwhile, the chi-squared analysis has discovered no relationship between the two therapies and their effectiveness. The RR value of 1.080 concludes that metformin has 1.080 as much therapeutic effectiveness as the metformin-glimepiride.

Abstract

An assessment of public knowledge about HIV/AIDS is necessary for the development of educational strategies and materials. A valid and reliable instrument is needed to obtain comparable assessment of public knowledge about HIV/AIDS from time to time. This research study aims to determine the validity and reliability of the Indonesian version of HIV-KQ-18 in assessing public knowledge about HIV/AIDS in the Special Region of Yogyakarta. This research study used a cross-sectional design with the inclusion criteria being people living in Yogyakarta who were at least 18 years old and the exclusion criteria being respondents who did not fill out the instrument completely and did not fill out duplicate forms from the same respondent. The instrument was analyzed using product-moment correlation and known-group validity, while its reliability was tested using Kuder-Richardson Formula 20 (KR20). The total number of respondents as the subjects in this research study was 305. The results of the validity test of the HIV-KQ-18 obtained an r-xy value from 0.177-.564, greater than r-table (0.112), and the HIV-KQ-18 instrument exhibited high reliability with a KR20 value of 0.763. The test of known-groups validity showed that there were significant differences in all groups tested, except for the group based on age. The HIV-KQ-18 is a valid and reliable instrument and it can be used to assess public knowledge about HIV/AIDS in the Special Region of Yogyakarta.

Abstract

Yogyakarta is one of the top 10 cities with the highest number of dengue cases in Indonesia. One of the reasons for this is the lack of knowledge about the dengue virus in the community. To find out the quality of life of dengue patients in the Bantul Penembahan Senopati Public Hospital.This research uses the descriptive method with a cross-sectional design. The sampling technique utilized was the purposive sampling method. 18 research subjects were chosen as the sample with an inclusion criteria of patients undergoing inpatient treatment and aged > 4 years of age. This study used the EQ-5D-5L and EQ-5D-Y.The quality of life of 70% of the adult patients experienced pain of EQ-VAS 67 ± 17.191, while all child patients experienced pain of EQ-VAS 54.375 ± 11.783. The quality of life of dengue patients was measured with the EQ-5D-5L for adults and EQ-5D-Y for children and obtained a poor score due to a decrease in the dimension of pain felt and the usual activities done such as working, studying, and playing.

Abstract

An autoimmune disorder is an abnormality that causes a disease. It is caused by a weakened immune system. One of the autoimmune diseases is Sjogren’s syndrome, which affects the salivary and lacrimal glands and causes dry mouth, dry eyes, and dry skin. Sjogren’s syndrome influences humans of every age, with the symptoms occurring at the age of 45–55 years and rarely in children. One of the factors causing Sjogren’s syndrome is genetic disorders. To identify genes that can influence Sjogren’s syndrome in this study, we used several databases, including GWAS Catalog, HaploReg Version 4.1, GTEX portal, and Ensembl, particularly to identify the gene expression profiles of TNIP1, TNFAIP3, and IRF5 and the quantitative properties of locus’ expression. This research showed that the missense variants and splice donor rs2233290, rs2230926, and rs2004640 influenced the susceptibility of autoimmune diseases, especially Sjogren’s syndrome, in the fibroblast tissue, sigmoid tissue, sigmoid colon, skin, esophagus, and adrenal glands. The allele frequency of each variant was then assessed in African, American, European, and Asian populations. Our data showed that TNIP1, TNFAIP3, and IRF5 genes in African and American populations had higher frequencies than in the Asian population. This implies that the last of the aforementioned populations might be relatively susceptible to the autoimmune disease Sjogren’s syndrome.

Abstract

Health professions work together to provide the best service in health care facilities by collaborating with patients, families, worker and the community. Interprofessional education (IPE) were practice of collaboration between two or more students from different health profession programs. The purpose of study was to compare IPE results from first and third year batches. The two batches included 345 first-year students and 460 third-year students, from three different health disciplines, including medicine, pharmacy, and public health. These students met for four weeks to increase interprofessional collaboration, improve communication skills, foster respect and increase knowledge of the various roles each discipline, especially case management, conflict management and team work. Before IPE program, the students were given pre-questionnaire to assess their prior understanding of IPE. Each group of first-year students presented the outcomes of their discussions in the fourth week, while the third-year students created a poster about the subject and presented it in the second week. The students complete the program and post-questionnaire after their presentation. The International Collaborative Competencies Attainment Survey served the development the IPE questionnaire (ICCAS). The result of pre-IPE domains’ score revealed substantial disparities in the team work domain, with third-year students score was lower than the first students, whereas first-year students had the highest score in the most of IPE categories, unless collaboration and conflict management (p>0.05). The post-IPE domains’ score showed significant differences in all of the domains. Most of the IPE domains had higher score in first year students, excluding communication and team work.

Abstract

The assessment of quality of life serves as a critical evaluation of the effectiveness of the administered hemodialysis treatment. The measurement of quality of life can be conducted using the Kidney Disease Quality of Life Short Form (KDQoL-SF™) questionnaire. This research aims to gather information concerning the quality of life among patients with hemodialysis due to chronic kidney failure at the dr. S. Hardjolukito Regional Military Hospital in Yogyakarta. The research employed a cross-sectional design. The respondents consisted of 65 patients selected through purposive sampling based on specific inclusion and exclusion criteria. The Independent T-test, Mann-Whitney test, Chi-Square test and Fischer test were conducted to define the association between quality of life and respondent characteristics. Most of the hemodialysis patients was male (50.8%) with average od age was 54.66 years old. The findings revealed that 64.6% of the respondents experienced moderate/poor quality of life, while 35.4% exhibited good quality of life. Statistical analysis established a significant relationship between respondent characteristics and quality of life based on the variables of age and duration of hemodialysis (p<0.05). Hemoglobin levels, number of prescribed medications, gender, education, occupation and income demonstrated no significant relationship with the quality of life. Among the respondents’ characteristics, age and duration of hemodialysis were associated with the quality of life.

Abstract

Angka kejadian kasus infeksi dengue tahun 2020 di Kota Yogyakarta sebanyak 296 orang. Berdasarkan manifestasi klinis dengue terbagi menjadi 3 diagnosa yaitu dengue fever (DF), dengue haemorrhagic fever (DHF) dan dengue shock syndrome (DSS). Penularan terjadi akibat gigitan nyamuk Aedes aegypti dan mobilitas yang tinggi. Beberapa faktor yang mempengaruhi angka kejadian yaitu kualitas hidup dan pengetahuan terhadapa penyakit yang berdampak pada tingkat kesehatan. Tujuan penelitian untuk mengetahui kualitas hidup dan pengetahuan. Penelitian ini dilakukan secara observasional dengan pengambilan data prosfektif di Rumah Sakit PKU Muhammadiyah Yogyakarta periode April – Desember 2021. Kualitas hidup pasien dinilai menggunakan kuisioner EQ-5D-Y untuk pasien anak dan EQ-5D-5L untuk pasien dewasa dan pengetahuan dinilai menggunakan kuisioner yang terdiri dari pengetahuan penyakit, pengetahuan pencegahan dan prilaku pencegahan. Hasil penelitian di RS PKU Muhammadiyah Yogyakarta pada pasien dengan diagnosa DHF sebanyak 40% merasa amat sangat bermasalah pada kegiataan yang biasa dilakukan dan sebanyak 40% merasa cukup bermasalah pada dimensi rasa cemas, sedih dan depresi sehingga berpengaruh pada kulitas hidup. Pada pengetahuan penyakit, pengetahuan pencegahan dan perilaku pencegahan pasien DF dan DHF tergolong dalam kategori buruk sehingga perlu adanya promosi kesehatan untuk meningkatkan pengetahuan terhadap kesehatan. Penyakit demam dengue berpengaruh pada kulitas hidup. pengetahuan terhadap penyakit demam dengue dalam kategori buruk.
