Abstract

Acute myocardial infarction (AMI) is one of coronary artery disease that has high rate of mortality and morbidity. The use of antithrombotic has ben recomended widely as the main therapy of AMI. The benefit from using aspirin in high-risk vascular disease patients comes at the cost of increased gastrointestinal complications, such as gastroduodenum ulcerations. This research aimed to know the monitoring of antithromboticsâ??side effects AMI patients in a private hospital of Yogyakarta from Januari to Desember 2007. The research used descriptive method and the data was collected from patientsâ?? medical records  with AMI as primary diagnose. Result of research showed that antithombotics drugs which applied as therapy at AMI patient were antiplatelets, anticoagulants and thrombolytics. We identified the side effects monitoring such as; haemorrhage risk in 3,90% (2 patients), thrombocytopenia in 3,90% (2 patients) and renal function in 15,58% (12 patients). There were no patients with  gastrointestinal disorder as antithromboticsâ??side effects. Intensive blood pressure monitoring was done for patient who get thrombolytic therapy (21 patients) that was equal to 26,92% from total patients. ABSTRAK Infark miokard akut (IMA) merupakan jenis penyakit jantung koroner yang mempunyai tingkat morbiditas dan mortalitas yang tinggi. Antitrombotik merupakan terapi utama dari IMA. Penggunaan aspirin sebagai terapi IMA sering disertai dengan meningkatnya komplikasi gastrointestinal. Komplikasi tersebut bisa berupa tukak gastroduodenal, dispepsia dan esofagistis. Penelitian ini dilakukan untuk mengetahui tindakan monitoring efek samping antitrombotik yang telah dilakukan pada  pasian infark miokard akut di sebuah rumah sakit swasta di Yogyakarta periode Januari-Desember 2007. Penelitian dilakukan dengan metode non eksperimental dan data monitoring dilihat pada rekam medis pasien IMA. Hasil dari penelitian ini menunjukkan bahwa terapi antitrombotik yang digunakan pada infark miokard akut adalah antiplatelet, antikoagulan dan trombolitik. Monitoring efek samping antitrombotik yang telah dilakukan berupa monitoring perdarahan pada 2 pasien (3,90%), trombositopenia pada 2 pasien (3,90%), fungsi ginjal pada 12 pasien (15,58%). Tidak ada pasien yang mengalami gangguan gastrointestinal akibat efek samping antitrombotik. Monitoring tekanan darah secara intensif hanya dilakukan pada pasien IMA yang mendapat terapi trombolitik yaitu 21 pasien atau sebesar 26,92% dari total pasien IMA.

Abstract

Type 2 diabetes mellitus is a chronic disease that requires lifelong therapy and high service costs so that the use of antidiabetic drugs affects the patient's therapeutic costs. The purpose of this study was to determine the cost of outpatient type 2 diabetes mellitus therapy for National Health Insurance (JKN) participants in Condong Catur Hospital Yogyakarta. The research method used is observational. The parameters measured are calculating the average direct medical costs (drug costs, doctor's examination and consultation fees, laboratory costs, medical treatment costs), and comparison between the costs of oral drugs, insulin, oral and insulin. The results of normality tests with Shapiro-Wilk (microvascular, microvascular and macrovascular), Kolmogorrov-Smirnov (macrovascular) p<0.05 indicate that data are not normally distributed, the results of the Kruskal Wallis test of the three groups (microvascular, macrovascular, microvascular and microvascular) show that differences are not normally distributed significant (p = 0.001).The results showed that the microvascular group therapy costs Rp 3,164,733.58 per month, the macrovascular group Rp 9,984,566.85 per month, the microvascular and macrovascular groups Rp 11,260,372.39 per month. The cost of therapy is based on the microvascular group drug Rp. 408,567.78 per month, the macrovascular group Rp. 1,245,987.48 per month, the microvascular and macrovascular groups Rp. 1,786,270.68 per month so that statistically from the three complications groups show significant differences p = 0,001 (p <0,005). The difference in the difference between the total real costs and the total INA-CBG's rates, the microvascular group Rp -38,741.14 per patient, the macrovascular group Rp -10,914.03 per patient, while in the microvascular and macrovascular group Rp 3,272.90 per patient shows that from to three groups of complications were not significantly different p = 0.207 (p> 0.005). Statistically it can be concluded that the cost of therapy based on antidiabetic drugs in the three groups of complications shows a significant difference. There is no significant difference in the difference between the real costs in the three groups of complications

Abstract

Dengue hemorrhagic fever (DHF) in the community can have an impact of panic, death, and reduced age of hope in the family. Research on the quality of life of DHF pediatric patient who got the treatment of crystalloid and colloid fluids has never been done before. The purpose of this study was to determine the quality of life of DHF pediatric patients who received crystalloids and colloids with PedsQL instruments. The study design was a cohort with block randomization. The inclusion criteria in this study were hospitalized DHF pediatric patients aged one month -18 years from February to May 2018. The exclusion criteria were DHF patients who came with referrals from other hospitals who had received fluid therapy. Research data include the length of stay (LOS) and quality of life score with the PedsQL questionnaire. The statistical analysis was using an independent t-test and the Mann Whitney test. The length of stay for the crystalloid group is five days longer than the colloid group, which is four days. From a total of 48 subjects studied, the score for the physical function of the crystalloid fluid group 84.54 ± 9.90 was higher than the colloid group of 77.58 ± 19.30 (p = 0.125). The emotional functions in the crystalloid group obtained a score of 81.88 ± 12.14 higher than the colloid group, i.e., 79.17 ± 18.5 (p = 0.552). The social function in the crystalloid group had a score of 92.08 ± 8.84 higher than the colloid group of 86.67 ± 13.96 (p = 0.232). The school functions in the crystalloid group had a score of 50.42 ± 33.68 higher than in the colloid group of 37.92 ± 36.62 (p = 0.225). The results of this study showed that the crystalloid group had a higher quality of life score compared to the colloid group, although it was not significant.

Abstract

The research about cancer patients’ quality of life on before and after therapy of high emetogenic cytotostatic in RSUP Dr Sardjito Yogyakarta by EORTC QLQ-C30 has been done.This research used cohort design with cancer patients who just the first time received cisplatin ≥ 50 mg/m2 and or its combination, and also got ondansetron-dexamethasone as anti-emetics. While the patients were given EORTC QLQ-C30 questionnaire before got cisplatin (before chemotherapy) and 5 days after got cisplatin (after chemotherapy). The difference of the quality of the life will be analysed with paired t-test with 95 % level of significancy. The correlation between the domains before and after chemotherapy was analysed by Pearson correlation test with the 95 % level of significancy.We got 38 patients during 2 months with the cancer diagnoses of cervix, nasopharyng, ovarium and lung. Generally, the quality of life was nonsignificantly decreasing, whereas not all the domains were decreasing. The physical, emotions, cognitive and social domains were non-significantly increasing. The decline in the quality of life was more influenced by the role domain of the patients, pain, the decline in appetite and the financial difficulty. The correlation of fatigue, constipation and the financial difficulty were strong between the domain in before and the chemotherapy after was received in the symptom (r ≈ 0.50).Key words : quality of life, cancer, EORTC QLQ-C30.

Abstract

The aim of the research was to know the description of antiemetic usage on the patients with cancer based on the medical records that were available in the hospital. Hereby, the use of antiemetic would be compared to literature for seeing the pattern.The study was a descriptive, analytic and retrospective study. The data taken from the medical records were written by doctor and nurses for cancer in patients hospital during Januari to June 2003. Data were grouped based on cytotoxic treatment with strong, medium and weak emetogenic effects, the condition of cancer patients, and antiemetic usage before and after having cytotoxic treatment.The results showed antiemetic usage before and after strong emetogenic cytotoxic treatment were not suitable because there was no combination with dexametason. Antiemetic usage which was used before and after mild emetogenic cytotoxic treatment were not suitable with ondansetron, metochlorpramide, combination of ondansetron – metochlorpramide. The use of antiemetic on cancer patients who got strong and medium emetogenic cytostatic was not been suitable with literatures because the patients were not given dexamethasone as combination therapy. For cancer patients given before and after medical treatment with weak emetogenic effect weren’t suitable in both of hospitals. Antiemetic usage before cytotoxic treatment that was not suitable was ondansetron and metoclopramide, where as antiemetic usage after cytotoxic that was suitable was metoclopramide.Key words: Antiemetic, Emetogenic Effect, Cytotoxic, hospital

Abstract

Currently, Indonesia is in the fifth rank of highest TB prevalence over the world. One of the TB problem is low patients’ adherence due to the oral antituberculosis induced hepatotoxicity. Polymorphisms of NAT2 and CYP2E1 genes had important role in the isoniazid (INH)-induced hepatotoxicity. The aim of this study was to evaluate the polymorphisms profile of NAT2 and CYP2E1 genes associated with hepatotoxicity induced by INH.We used cohort design in Public Health Centers and Lung Clinics of Yogyakarta and Lampung. The inclusion criteria were adult subjects (> 18 yo), newly diagnosed TB and treated by oral antituberculosis, normal function of renal and live and willingness to participate in this study. Subjects were excluded when having positive reaction of  HbsAg test, history of HIV and abnormality of renal and liver function. The SNPs of NAT2 and CYP2E1 were designed using IPlex method of DNA sequenom.Among 57 TB patients, we found 14 patients with higher INH serum concentration and experienced increase of ALT-AST. Subjects with  SNPs of rs 2070676, rs 1329149, rs 3813867, rs 6413432, rs 8192772, rs 2031920, rs 2515641, rs 8192775, rs 915908 of  CYP2E1  experienced increase of ALT and AST. Subjects with SNPs  of rs 1799930, rs1799931, rs1801279, rs1801280, rs1799929 , rs1208, rs1041983 of  NAT2 are associated with the increase of ALT and AST.The polymorphisms of  CYP2E1 and NAT2 may have a role in the mechanisms of INH induced DILI.Keywords: CYP2E1, NAT2, tuberculosis, isoniazid, Indonesia

Abstract

Herbal medicine has been an alternative treatment, instead of modern medicines. However, some of patients could experience adverse drug reaction (ADR) due to the use of herbal medicines. The ADR can decrease patients quality of life (QoL). Objective. This study was aimed to understand the QoL of patients prescribed with herbal medicines. Methods. Patients prescribed with herbal medicines as complementary treatment, in two Public Health Centers of Yogyakarta were recruited. The patients were prescribed with herbal medicines one year before this study was started. Naranjo algorithm was used to identify the ADR. The SF-36 questionnaire was given to the patients as the instrument to measure patients QoL. Results. There were 47 patients participated in this study. Most of the patients are female (62.5%).Ten patients experienced ADR. There are no significant differences between QoL domains in patients experienced ADR and those without ADR. However, the scores of physical function, role physical, general health, social function, role emotional, and mental health in patients without ADR are higher than those with ADR ( p0.05) . Conclusion. There are no significant differences of QoL between patients with and without ADR. In general, the QoL of patients without ADR is higher than those with ADR.

Abstract

Tuberculosis (TB) is one of the disease as the highest contributor to the disease burden in Indonesia. Tuberculosis can affect the patients’ quality of life, such as psychological, physical, and social functioning. St. George's Respiratory Questionnaire (SGRQ) is a special instrument which was widely used to measure the patients’ quality of life with respiratory disease. The objective of this study was to validate the Indonesian version of the SGRQ as instrument to collect data. A descriptive cross section design with 61 subjects was conducted at the Pulmonary Clinics and Primary Health Centers in the region of Yogyakarta within 3 months. The validation process included the known group validity, convergent and discriminant validity and factor analysis. There were 14 items question numbers which did not meet the criteria for convergent validity and 9 items which did not meet the criteria for discriminant validity. Known group validity analysis on gender showed that of the three domains of SGRQ, the activity domain gave statistically significant result. The factor analysis showed the result of Kaiser Meyer Olkin analysis (KMO) was less than 0.5. With a few modifications, the Indonesian version of SGRQ is valid and reliable for measuring quality of life in tuberculosis patients.

Abstract

Mercaptopurine is a chemotherapy drug that used in the consolidation and maintenance phase in pediatric patients with acute lymphoblastic leukemia ( ALL). Mercaptopurine is supposed to have relationship with the occurrence of toxicity in hepatology. This study was aimed to determine the patient incidence , severity and management of toxicity and risk factors that affect the occurrence of hepatologic toxicity using of 6 - MP in the maintenance phase in pediatric patients with acute lymphoblastic leukemia in Dharmais Cancer Hospital Jakarta. This research was carried out by cross-sectional observational design with data was taken retrospectively and prospectively during the period from March to April 2014. The study subjects were patient who have been getting treatment of maintenace phase in Dharmais Cancer Hospital Jakarta from 2013 until March-April 2014. The Subjects who met the inclusion criteria were 23 patients of ALL received 6 - MP in the maintenance phase. Data was collected by looking at the medical records for getting of the patient's identity and data hepatology. The result showed that there was a correlation between patient characterision on hepatology toxicity, on SGOT age and weight with SGPT both of them has significant value, it was 0,036 (P<0,05)

Abstract

DNA extraction from dried blood spots was used for molecular analysis. Dried blood spots sample usually used FTA cards, but short-term use would come at a cost. This study aims to identify DNA extraction quality from dried blood spots in Whatman filter paper as an alternative storage sample. This study used 15 samples with three different storage time for 1, 3, and 7 days on 4°C. Dried blood samples were extracted using the Chelex-100 method, and qualitative were identified by electrophoresis. The DNA extraction was used as a template on PCR amplification of the gapdh gene. The result showed that DNA extraction showed band from 1, 3, and 7 days, and PCR amplification showed bands in 200 bp. In conclusions, DNA from dried blood samples was stored for 1, 3 and 7 days in Whatman filter paper were successfully extracted by the Chelex-100 method and can be used as a DNA template for PCR amplification.
