Abstract

Bipolar affective disorder is an episodic mental disorder characterized by manic, hypomanic, depressive, and mixed episodes, usually recurring and can last a lifetime. Patients with bipolar disorder have annual medical costs that were four times those of patients without bipolar disorder. The aim of this study is to analyze the cost-effectiveness of combination therapy between mood stabilizers and antipsychotics in patients with the bipolar affective disorder at Grhasia Mental Hospital Yogyakarta. This study is a retrospective with cohort study design using medical records and direct medical cost data during Januari-December 2018 period. Subjects are patients who were diagnosed with bipolar affective disorder manic episode (F31.2) and received a combination of mood stabilizer and antipsychotic therapy. The results were obtained in 46 patients with the affective disorder who met the inclusion criteria. The average direct medical cost of the sodium divalproate group was IDR 6.319.933 per day and in the lithium group was IDR 5.705.953 per day. The average length of stay in the sodium divalproate group was 25,79 days and in the lithium group was 25,75 days (P = 0,991). The Average Cost Effectivity Ratio (ACER) sodium divalproat group is lower than the lithium group (IDR 221.246 per day and IDR 245.434 per day, respectively). The conclusion of this study is that the combination of sodium divalproate-antipsychotic therapy is more cost-effective than lithium-antipsychotic therapy in a patient with bipolar affective disorder manic episode

Abstract

The World Health Organization (WHO) has announced that Severe Acute Respiratory Syndrome Coronavirus‐2 (SARS‐CoV‐2) and Coronavirus disease (COVID‐19) is considered a worldwide pandemic. Rapidly rising numbers of patients have been reported in almost every country, along with the growing mortality rates. Uncontrolled growth in patient numbers may be due to reasons such as treatment options and vaccine availabilities and unidentified targets of SARS‐CoV‐2. Previous study has revealed that the molecular target of SARS‐CoV‐2 is analogous to SARS (2003), i.e. angiotensin‐converting enzyme‐2 (ACE‐2). Therefore, the determination of ACE‐2 may enrich existing information and facilitate development of drugs targeted toward SARS‐CoV‐2. This study aims to screen the expression of ACE‐2 genes and their relationship to the types of SNP variants in SARS‐CoV‐2. We explored a series of observations using powerful databases, e.g. GTEx portal, HaploReg, 1000 Genome and Ensembl, to identify the gene variant of ACE‐2. We showed that ACE‐2 is highly expressed in the testes and small intestine, while its lowest level is observed in lymphocytes. Subsequently, we observed 17 gene variants containing a missense mutation potentially damaging protein level. Among these genes, single nucleotide polymorphism (SNP) rs370187012 shows the highest damage‐level score, while the lowest effect is in SNP rs4646116. The highest frequency of the C allele was observed in European populations (1%). In addition to showing that ACE‐2 is expressed in several organs, we concluded that the ACE‐2 gene variation can be found in African, American, Southeast and East Asian, and European populations. The polymorphisms of ACE‐2 impact on the ACE2 protein structure and the binding capacity of the ACE‐2 receptor with the S‐Protein of SARS‐CoV‐2.

Abstract

Polymorphisms of proteins which have an important role in drug transport, metabolism, and disposition in the bodycould affect the drugs efficacy and toxicity. The organic cation transporter 1 (OCT1), encoded by SLC22A1 gene,has been known as one of the polyspecific protein transporters which could affect some of the cationic drugresponse such as metformin, levodopa and imatinib. However, the findings of many studies an association of OCT1polymorphisms and drug response both in Caucasian and Asian were still inconsistent. Moreover, this study’s topicin Asians was still uncommon. This review was aimed to explore the polymorphisms of OCT 1 in Asians and Caucasiansand to find the challenges of the next studies in Asians. The articles about OCT1 polymorphisms were searched inthe PubMed with the keywords; OCT1 or SLC22A1, polymorphisms. There were ten articles of OCT1 polymorphismswhich are related to the drug response and most of the studies were performed in Caucasian subjects. In Caucasians,the rs622342 variant might associated with the response of metformin and levodopa. Moreover, the R61C and420del variants still showed the inconsistent findings associated with metformin response. The non-synonymousvariants which were found in Caucasians were not found in Asian. However, the new non-synonymous variantswere found in Japanese, Chinese, Indian and Korean population and some of them were associated with the metforminresponse. The recent findings found in Caucasians cancer patients, were related to the association of non-synonymousvariants haplotype and the 5-Hydroxytriptamine Receptor Antagonists drug response. The inconsistent results ofOCT1 polymorphisms studies could be related to the study’s sample size and design of the studies. Further studieswhich exploring the association of OCT1 polymorphisms and drug pharmacokinetic profiles and/or drug response,which were adjusted by genetic variants of proteins involved in drug tansport, metabolism and disposition are stillneeded in both Caucasians and Asians. Additional large studies also considering non-genetic risk factors are warranted,to implement the results of the various studies into clinical practice.Key words: OCT1- polymorphisms – Asians – Caucasians - drug response

Abstract

The Home Health Care program is getting popular as well. One of the outcomemeasured in this program is patient’s quality of life. Home Health Care program is amultidisciplin program, which need the pharmacist role. The aim of this study was tofind out the potencial pharmacist role in increasing elderly patients’quality of lifeparticipated in Home Health Care program. This study used descriptive design withprospective data sampling. (World Health Organization Quality of Life) We usedWHOQoL as the instrument. The study period was conducted for 13 week in 2009 byvisiting the patients at their homes. The patients were participated in Home HealthCare program of Dr Sardjito hospital, Yogyakarta. Data was descriptively analized.We're recruited seven subjects in this study. In average, their quality of life was low(46,1±4,4). All of the patients received polypharmacy and were found the patientsincompliance in taking the medicines. The role of pharmacist were important in HomeHealth Care program for elderly patients to increase their quality of life. Thepharmacists are supposed to do the drug used monitoring, especially to the elderlypatients with polypharmacy. The advance technique for communication, information,and education is suggested to the pharmacist in this program.

Abstract

Type 2 diabetes mellitus (T2DM) is a complex metabolic disease characterized by high blood glucose levels due to impaired insulin secretion or insulin resistance. Polymorphisms in the ABCC8 gene rs1799854 are widely found to have an association with T2DM, where the ABCC8 gene encodes the SUR1 protein from the K-ATP channel that plays a role in insulin secretion in cells β pancreas. Mutations in the ABCC8 gene can cause potassium channels and insulin secretions problems that possibly decrease the effectiveness of the drug. Polymorphisms in some populations have been reported, but similar research on Solo's population has never been conducted. The purpose of this study is to identify the genotype of the ABCC8 gene in rs1799854 and the therapeutic outcome of blood glucose levels in T2DM patients. This research is cross-sectional research conducted prospectively at Moewardi Hospital Surakarta, Solo. Blood samples were collected from 10 T2DM patients who received sulfonylurea monotherapy taken through veins for genotype examination by DNA isolation, PCR amplification, and sequencing. Parameters of fasting blood glucose (FBG), 2-hour postprandial blood glucose, and HbA1c are measured as therapeutic outcomes. We found the dominant results in a mutant homozygote (TT) 40%; while wild type (CC); and a mutant heterozygote (CT) was 30%. Single Nucleotide Polymorphisms (SNP) of ABCC8 gene rs1799854, found in T2DM patients at Moewardi Hospital Surakarta, Solo, who received sulfonylurea therapy.

Abstract

Hypertension is a major risk factor for the occurrence of deaths from non-communicable diseases in the world. In the World Health Statistics 2012, WHO reported that approximately 51% of deaths from stroke and 45% of coronary heart disease due to hypertension. Patients with obsesitas, hypertension, and diabetes can worsen patient’s quality of life both physically and mental status. This study aims to assess the quality of life by the European Quality of Life-5 Dimensions (EQ-5D) questionnaire. This study aims to determine the validity and reliability of the EQ-5D in Indonesian version as an instrument for measuring the quality of life of patients with hypertension in the Health Center Kotagede II Yogyakarta. The method of this study was descriptive observational. Data were collected using a questionnaire conducted at a particular time. The instrument used in this study is the EQ-5D which has been translated into Indonesian. Affordable population in this study were all patients with hypertension in Health Center Kotagede II Yogyakarta at May to June 2014. The sample in this study are in accordance with the criteria for inclusion and exclusion criteria. Analysis of the validity was construct's validity with Pearson method. Test reliability was Cronbach's alpha coefficient. From the data analysis for 51 patients Hypertension Pearson correlation coefficient obtained results are above 0.30, so it can be concluded instrument EQ-5D questionnaire Indonesian version is valid. Results Cornbach'sα is 0718 so it can be concluded instrument EQ-5D questionnaire Indonesian version is reliable.

Abstract

Diabetes mellitus is chronic disease that needs the complex and a long term medical treatment. Patient adherence and level of distress can affect patient quality of life. The purposes of this study is to examine patients adherence, distress, and quality of life patients type 2 diabetes mellitus in Kabupaten Pati. This study used cross sectional design and data patients collection techniques were conducted prospective data. The subjects of research were patients with Type 2 diabetes amounted to 126 people. Instruments used in this research is Medication Adherence Respon Scale (MARS) questionnaire to measure patients adherence, Diabetes Distress Scale (DDS) to measure distress, and EQ-5D-5L questionnaire to measure quality of life. Data analysis statistic used Mann Whiney test, and linier regression. This study was dominated by female patients 78.6%. The mean age of DMT2 patients was 54.49 ± 6.19 years. The average value of adherence (MARS) was 21.33 ± 2.90 including in moderate adherence. The mean total of DDS score was 1,56±0,42 including not distress level and the highest is the emotional burden domain. The mean value of EQ5D5L index was 0.729 ± 0.161. The results showed a significant association (p <0.05) between patient characteristics age and duration of sick with adherence. There were significant association (p <0.05) between age to physician distress, gender with emotional, marital status with emotional and interpersonal distress, duration of sick to physician distress and management therapy distress. Marital status and income had significant relationship to quality of life. Emotional burden domain had significant relationship to quality of life. Multivariate analysis showed that the relationship of adherence and distress by the emotional burden to quality of life. This study concluded the significant relationship between adherence, distress and quality of life.

Abstract

Diabetes Mellitus (DM) is a metabolic disease that developed due to the pancreas does not sufficient to produce insulin or the body cannot effectively use the insulin it produces. Genetic factors have an essential role in the development of Type 2 Diabetes Mellitus (DMT2), which impaired insulin production by pancreatic β cells, insulin resistance, and action. The single nucleotide polymorphisms (SNP) in the KCNJ11 rs5219 affected the pancreatic β cell activity that can inhibit insulin release, thus causing a decrease in therapeutic effectiveness. The purpose of this study is to identify the SNP rs5219 of the KCNJ11 gene and measure patient blood sugar levels as the outcome of therapy. A cross-sectional study was conducted prospectively at Moewardi Hospital, Surakarta, involving 10 patients with DMT2 who received sulfonylureas therapy. DNA was isolated from the whole blood sample of DMT2 patients. PCR amplification was performed to amplify the KCNJ11 gene, and followed by PCR sequencing. The 2-H PP, FPG, and HbA1c parameters were measured as therapeutic outcomes. The results showed that the genotype frequencies (AA-AG-GG) were 10%, 50%, and 40%, while the allele frequency (A-G) in the sample was 35% and 65%. The uncontrolled values for 2H-PP on genotype (AA and AG + GG) were 10% and 20%; uncontrolled FPG on genotypes AA and AG + GG were 10% and 40%; and uncontrolled HbA1c on genotype AA and AG + GG were 10% and 80%. This study conclusion is the presence of the SNP rs5219 KCNJ11 gene with A>G base change in DMT2 patients who received sulfonylurea therapy.

Abstract

The incidence of type 2 diabetes mellitus (DM) has been predicted to increase until 2045 in the world. Furthermore, long-term treatment and lifestyle factors affect the quality of life. This study aims to determine the models that can be used to predict the quality-of-life domains in prediabetes patients by using Artificial Intelligent (AI) devices. This is a cross-sectional design in which the inclusion criteria were individuals of age above 18 years and has never been diagnosed with diabetes mellitus (both type 1 DM and type 2 DM), fasted for at least 8 hours, and are willing to sign an informed consent after having received an explanation. Participants were asked to fill out two questionnaires, namely the Indonesian version of the Finnish Diabetes Risk Score (FINDRISC) and the EuroQoL-5 Dimensions-5 Level (EQ-5D-5L). The AI application uses Orange® machine learning with three models used in predictive analysis, such as Logistic Regression, Neural Network, and SVM. In addition, the model was evaluated using the sensitivity, precision, and accuracy of the AU-ROC parameters. The results showed that the neural network model based on the AUC value, precision, accuracy, and also the ROC analysis, was the best for predicting the utility index of domains in the EQ-5D-5L questionnaire, based on demographic data and the FINDRISC questionnaire.

Abstract

Diarrhea is the second leading cause of children’s death worldwide, including in Indonesia.With regard to reducing the number of death, it is highly needed to provide a quick and accurate treatment. In clinical practice, prebiotics has been widely used as a treatment, particularly in the case of acute diarrhea in children. However, it is still not recommended by WHO. This study is aimed to determine the treatment pattern of diarrhea in children’s ward at PKU Muhammadiyah Bantul Hospital and determine whether probiotic supplementation on standard therapy results a decrease in the duration of diarrhea which is better than only the standard therapy in the management of acute diarrhea in children. This study used retrospective cohort design using data from medical records with diagnose acute diarrhea or acute gastroenteritis (ICD A09). The data was collected from January to December 2014, at children’2 ward, PKU Muhammadiyah Bantul Hospital, Yogyakarta. Data was divided into three groups: (i) Group I those who were treated by using standard therapeutic with rehydration and zinc, (ii). Group II those who were treated by combination of rehydration, zinc and probiotics, (iii) Group III those who were treated with rehydration and probiotics. Data duration of diarrhea as an outcome of the effectiveness of probiotics analyzed statistically to determine differences in outcomes between groups. Among the three groups, Group I, Group II and group III gained an average duration of diarrhea 66.33 (SD 21.66) hours, 55.32 (SD 19.07) hours and 54.79 (SD 17.69) hours, respectively. The statistical test shows that there is significant different from diarrhea duration among the three groups (p = 0.014). The test results between probiotic groups (II and III) and the control group is significantly different with p-value 0.018 and 0.011, respectively. While the group II and group III do not differ significantly (p = 0.898). Treatment pattern therapy of diarrhea in PKU Muhammadiyah Bantul Hospital divided into three kinds of therapy. The first is rehydration therapy and zinc supplementation, the second is rehydration therapy and zinc supplementation and probiotics, the third is rehydration therapy and probiotics. Probiotic supplementation is proven effective in reducing the duration of diarrhea significantly compared to patients who were received standard therapy.
