Abstract

ABSTRAK Prevalensi hipertensi di Yogyakarta semakin meningkat, maka diperlukan suatu alat untuk mengukur kualitas hidup pasien hipertensi sebagai salah satu luaran terapi hipertensi. Telah dilakukan validasi instrumen SF-36 pada pasien hipertensi di puskesmas di Yogyakarta. Penelitian ini dilakukan bertujuan untuk mengetahui hasil validasi instrumen tersebut yang telah dirubah susunan pertanyaannya berdasarkan validasi sebelumnya. Penelitian ini menggunakan rancangan cross sectional yang dilakukan secara prospektif. Data diambil melalui wawancara pada pasien hipertensi. Subjek penelitian adalah pasien hipertensi yang menjalani rawat jalan di puskesmas di Yogyakarta periode Februari - Maret 2013. Kuesioner tersebut dianalisis dengan uji reliabilitas dan uji validitas yang menggunakan validitas diskriminan, konvergen dan uji Known Group Validity untuk membedakan kualitas hidup kelompok pasien tertentu. Subyek dalam penelitian berjumlah 30 pasien hipertensi. Kuesioner ini dinyatakan reliabel karena nilai cronbach alpha ≥ 0,70. Hasil validitas konvergen menunjukkan bahwa nilai pada domain yang diujikan ≥ 0,4. Sedangkan hasil validitas diskriminan menunjukkan bahwa tiap domain yang diujikan lebih besar nilainya dari domain lain. Berdasarkan uji Known Group Validity menunjukkan nilai p ≥ 0,05 artinya bahwa kuesioner tersebut tidak dapat membedakan secara signifikan antara pasien hipertensi dan pasien komplikasi, antara laki-laki dan perempuan, dan antara pasien usia 40-49 tahun, 50-59 tahun, 60-65 tahun. Kuesioner SF-36 versi Indonesia dengan susunan pertanyaan yang baru dapat digunakan untuk mengukur kualitas hidup pasien hipertensi. Kata kunci: validasi, SF-36, hipertensi. ABSTRACT The prevalence of hypertension in Yogyakarta has been highly increasing, so it need an instrument to measure hypertension patients’s quality of life regarding the treatment outcome. The study about SF-36 instrument validation has been carried out in hypertension patients in public health center of Yogyakarta. This study was conducted to validate the instruments that have different composition due to the result of previous validation. This study was carried out by prospective cross sectional design. The data will be taken through the interview in hypertensive patients. The subjests were hypertensive patients undergoing outpatient a public health center in the period February to March 2013. Test validity was performed using convergent and discriminant validity was analyzed using person correlation test. We used the Known Group Validity to show differented of quality of life between the groups.The total subjects in this study were 30. The questionnaire revealed reliable due to the Cronbach alpha ≥ 0.70. The convergent validity of the results showed that the value of the domain being tested ≥ 0.4 and the discriminant validity indicated that each of the tested domains had greater value than the other domains. Based on the Known Group Validity test, the p value was ≥ 0.05 means that the questionnaire can not distinguish significantly between hypertensive patients and patient complications, between men and women, and among patients aged 40-49 years, 50-59 years, 60-65 years. The Indonesian new version of the SF36 questionnaire can be used to measure the quality of life patients with hypertension. Key words: validation, SF-36, hypertension.

Abstract

Hypertension is the biggest risk factor for cardiovascular disease which is the first caused of death in the world. Besides lifestyle modification and pharmacological therapy, hypertension can also be treated using traditional medicine. The use of traditional medicine can be influenced by characteristics and perceptions. The purpose of this study was to determine the patient's perception of traditional medicine for hypertension in Banyumas Regency. The research was conducted in April 2019, the subjects were hypertensive patients who become member of PROLANIS in Banyumas Regency. The research design used a cross-sectional study with a purposive random sampling. Perception was measured using a questionnaire with a Likert scale from strongly disagree to strongly agree (scale 1-5) then converted into a percent index score. There were 340 respondents who participated in this study. The majority of respondents were women (56.8%) aged 45-60 years old (52.4%), with secondary education (55.3%), working (85%) and having moderate income (52.6%) . Respondents have a good perception of the safety and effectiveness of traditional medicines for hypertension with an average percent index score of 68.53% for safety and an 67.38% for effectiveness

Abstract

The use of antibiotics as the main standard of pneumonia treatment is usually prescribed in high quantities. Inappropriate selection of antibiotics can lead to bacterial resistance, treatment failure, and the emergence of DRPs (Drug-Related Problems). This study aims to determine the suitability of antibiotic prescribing using the PCNE method and to determine the correlation between the suitability of antibiotic prescribing and clinical response. This study was analyzed using a retrospective analytic cohort study method with data collection of adult Community-Acquired Pneumonia patients medical record, who were hospitalized in private hospital in Yogyakarta from January to December 2019. In addition, this study also assessed antibiotic prescribing according to the PCNE V8.02 method toward patients’ clinical response (the number of leukocyte, temperature, and respiratory rate) after take the antibiotics. The correlation according to antibiotic prescribing analyzed according to chi-square method. The results showed that a total of 52 antibiotic regimens obtained in the evaluation of antibiotic prescribing according to the PCNE method were 21.5% suitable and 78.8% not suitable. Thus, the results of the correlation test for the accuracy of antibiotic prescribing and clinical response to the number of leukocytes showed a significant relationship (p<0.05). However in this study, none significant correlation was found in antibiotic prescription and clinical response in terms of temperature and respiratory rate (p<0.05).

Abstract

Tujuan penelitian ini adalah untuk mengetahui hubungan medication beliefs (MB) terhadap kepatuhan, hubungan antara MB, diabetes distress, persepsi dan karakteristik terhadap kualitas hidup dan luaran terapi pada pasien DMT2 di RSUD Kota Yogyakarta. Desain penelitian cross sectional. Kuesioner dalam penelitian ini : (BMQ), MARS), (EQ-5D), (DDS), (B-IPQ). Luaran terapi : GDP, G2PP, HDL, LDL, TD, Tg, dan total kolesterol. Jumlah pasien 50 terdiri dari pria sebanyak 42,0% dan perempuan 29,0%, rerata umur 46-79 tahun (63,84%). Hasil penelitian menunjukan adanya korelasi MB terhadap kepatuhan (P<0,000, P<0,000), korelasi MB terhadap luaran terapi pada pasien DMT2-Hipertensi (P<0,075), dan tidak ada korelasi pada pasien DMT2-Hiperlipidemia (P>0,335), korelasi kepatuhan terhadap luaran terapi (P<0,012, P<0,014), korelasi persepsi terhadap luaran terapi (P<0,005, 0<0,061), korelasi MB terhadap luaran terapi melalui kepatuhan (P<0,019, P<0,020), dan korelasi distres terhadap luaran terapi melalui kepatuhan (P<0,035) pada pasien DMT2-Hipertensi, pada pasien DMT2-Hiperlipidemia tidak ada korelasi (P>0,173). Konsep CSSRM penelitian ini adalah MB dan persepsi positif pasien terhadap pengobatan dapat meningkatkan kepatuhan, dengan kepatuhan yang tinggi berpengaruh pada hasil luaran terapi terkontrol baik, distres dan kualitas hidup kurang berimplikasi dalam penelitian ini.

Abstract

Background: Diabetes mellitus patients may have a different response tosimilar medication. Therefore, individualized treatment and monitoringof the therapy is needed to ensure the patients’ response. Alternativemedication or dose adjustment should be considered in order to achievethe therapeutic goal.Objective: The aim of this study was to find out patients’ response tomedication variation or dose adjustment.Methods: This study used descriptive design in PKU MuhammadiyahHospital and Panti Rapih hospital period October-December 2014.Participants are all patients who got pharmacological therapy for treatmentof diabetes mellitus type 2, including oral antidiabetics and/ insulin andmet the inclusion criteria. The response of therapy was checked 2 times,during check-in and check-out.Results: Analysis of response variation showed that individualizedresponse existed, in which positive response occurred in 10 patients andthe other had a negative response. The changes were dose adjustmentand medication alteration or medication addition. From 6 patients whoneeded medication adjusment, 5 patients had a positive response, andothers had a negative response. Whereas from 6 patients whose dosage ormedication was not changed, 5 of them had a positive response while onehad a negative response.Conclusion: Individualized treatment is needed to achieve a therapeuticgoal. Individualized treatment can be done through dose adjustment andmedication alteration, or medications addition.

Abstract

ABSTRACT Breast cancer is the most common cancer found in women and is the dominant cancer in Indonesia. Cancer treatments such as chemotherapy can affect the quality of life of patients including physical, psychological, and social. The purpose of this study is to measure the quality of life of breast cancer patients. This research used cross sectional study design. The patient quality of life was measured using the EORTC QLQ-C30 questionnaire. The subjects of this study were breast cancer patients in RSUP Dr. Kariadi Semarang meeting the inclusion criteria. The cognitive function domain, social function, emotional function, role function, and physical function were 86,15 ± 17,82; 84,10 ± 21,13; 82,56 ± 17,72; 77,95 ± 29,77; 76,72 ± 22,95, respectively. Whereas in symptom scale group from pain, fatigue, insomnia, appetite loss, financial difficulties, nausea and vomiting, constipation, dyspnea and diarrhea were 32,82 ± 30,33; 32,65 ± 21,14; 31,79 ± 34,58; 31,79 ± 32,51; 30,77 ± 32,44; 23,59 ± 25,67; 17,95 ± 30,9; 4,62 ± 14,28; 4,10 ± 13,83, respectively. For global health status / quality of life obtained 72.18 ± 18.94. The highest score on the functional scale is the cognitive function domain and the lowest score is the physical function.On the scale of symptom the domain that has the highest score is the pain domain and the lowest score is the diarrhea domain. Abstract

ABSTRACT Tuberculosis (TB) is an infectious disease caused by the bacillus Mycobacterium tuberculosis. It typically affects the lungs but can affect other sites as well. TB disease could influence the TB patients’ quality of life such as psychology of health, physical function and the leading role in society. The purpose of this research is to measure the differed TB patients’ quality of life before and during the treatment using Indonesian version of St George Respiratory Questionnaire (SGRQ) instruments that have been validated. The research used a cross-sectional design conducted prospectively in 91 new patients diagnosed as lung TB in BP4 province of Yogyakarta in November 2013 until April 2015. Patients filled out the questionnaires, and its obtained Total Score values were analyzed with Friedman's test to understand the quality of life. The results from this research showed that the quality of life of TB patient before, and during the treatment differed significantly. The Total score on the first month treatment was 43.57% and the second until the sixth months treatment was 30.40%; 25.22%; 20.50%; 18.45% and 16.51% respectively (p<0.05). The worst of quality of life was occurred in the first month treatment as the initial period before the treatment and continues to improve until the sixth-month treatment. In conclusion, the quality of life TB patients’ before and during the treatment is more increasing. Key words: Tuberculosis, SGRQ, Quality of life.

Abstract

Tuberculosis is a widespread disease listed from the top 10 triggers of death in the world (ranked above HIV/AIDS. According to the Sleman Health Office, the number of TB services at the Sleman regional health center in 2019 was 5466 cases with the cure rate for pulmonary TB with positive smear is 87 ,88%. After a person is diagnosed with Tuberculosis, it will be continued with TB treatment for 6 months, this makes the patient bored and does not comply with taking medication. Based on previous research, it was found that the adherence rate in patients undergoing treatment was 72.7%. Considering that TB is an infectious disease so that adherence to pulmonary TB treatment is important to analyze, this study aims to determine the level of adherence to taking anti-tuberculosis drugs in TB patients in several Puskesmas in Sleman area. Patients compliance was measured by MARS. This study used a descriptive observational design applying the cross-sectional design. The subjects were pulmonary TB patients registered as TB sufferers based on secondary data at the Sleman District Health Office, aged 15-70 years old, seeking treatment in March-April 2020, receiving treatment undergoing OAT therapy category 1 for at least one month and willing to be respondents. The results of the study revealed that 54% were male and 46% were female, dominated by 62% aged >25 years of 13 subjects. The score of MARS questionnaire obtained was <25. The results of the study concluded that the patient complied with 60.82% and the non-adherent patient was 39.19%.

Abstract

Yogyakarta is one of the top 10 cities with the highest number of dengue cases in Indonesia. One of the reasons for this is the lack of knowledge about the dengue virus in the community. To find out the quality of life of dengue patients in the Bantul Penembahan Senopati Public Hospital.This research uses the descriptive method with a cross-sectional design. The sampling technique utilized was the purposive sampling method. 18 research subjects were chosen as the sample with an inclusion criteria of patients undergoing inpatient treatment and aged > 4 years of age. This study used the EQ-5D-5L and EQ-5D-Y.The quality of life of 70% of the adult patients experienced pain of EQ-VAS 67 ± 17.191, while all child patients experienced pain of EQ-VAS 54.375 ± 11.783. The quality of life of dengue patients was measured with the EQ-5D-5L for adults and EQ-5D-Y for children and obtained a poor score due to a decrease in the dimension of pain felt and the usual activities done such as working, studying, and playing.

Abstract

Diabetes mellitus (DM) requires long-term therapy. Selection of the right therapy was very important to provide optimal results at an affordable cost. One of the main standards of oral antidiabetic therapy prescribed for type 2 DM (T2DM) is metformin, either alone or in combination therapy. The purpose of this study was to describe the direct medical costs in type 2 DM patients between metformin and metformin-glimepiride therapy. This research was an observational and descriptive. Demographic and clinical data were taken prospectively from the complete medical records of data patients with the main diagnosis of DM who fulfilled the inclusion criteria in the period January 2019–November 2020 at RSU Nene Mallomo Sidenreng Rappang, South Sulawesi and the two Public Health Centers namely Puskesmas Galur. II and Panjatan II Kulon Progo Yogyakarta. Cost data includes total medical costs from the hospital/ healthcare perspective (cost of drugs, doctor's services and checkups, fees for checking Fasting Blood Glucose/ Gula darah Puasa (GDP) and Blood Glucose Level While/ Gula Darah Sewaktu (GDS)) and total costs from the Badan Penyelenggara Jaminan Sosial (BPJS)/ payer perspective (INA-CBG's rates) were covered by insurance. The results showed that the average direct medical costs at the hospital and the two Public Health Centers respectively were metformin therapy in IDR (Indonesian Rupiah) namely IDR 120.736 and IDR 9.072,- and metformin-glimepiride therapy of IDR 126.298,- and IDR 18.634,-. It can be concluded that the average direct medical cost of metformin therapy is lower than metformin-glimepiride therapy.
