Abstract

Background: Severe Cutaneous Adverse Reactions (SCARs), including Stevens-Johnson Syndrome (SJS), Toxic Epidermal Necrolysis (TEN), and Drug Reaction with Eosinophilia and Systemic Symptom (DRESS), were the idiosyncratic reactions most commonly caused by drugs which have an impact on increasing the patient's financial burden. Objectives: This study aims to determine the highest cost component and the illness cost in patients suffering from Severe Cutaneous Adverse Reactions (SCARs). Methods: This study was an analytic observational study with a cross-sectional design. We collected direct medical costs, direct non-medical costs, and indirect costs for inpatients with SJS, TEN, and DRESS at Dr. Sardjito Yogyakarta Hospital from 2014-2018, retrospectively. Based on the societal perspective, we defined the highest cost component and the illness cost in patients with SCARs. Results: The highest cost component for SJS, and TEN patients were obtained in the health professional services (24,1% and 25,2%, respectively). Meanwhile, the highest cost component for DRESS patients was the cost of drugs (20,2%). The total illness cost of 47 SCARs patients (22 SJS patients, 6 TEN patients, and 19 DRESS patients) was IDR 666.615.321, with the highest average cost for TEN patients, followed by SJS and DRESS (IDR 16.510.595, IDR 14.205.545, and IDR 13.445.555, respectively). Conclusions: The highest cost component was the health professional services in SJS and TEN patients and the cost of medicine for DRESS patients. The management of SCARs requires considerable cost, and it is still being a financial burden on the patients.

Abstract

Kasus demam berdarah (DB) atau Dengue Fever masih banyak dijumpai di daerah tropis dengan lingkungan yang kurang baik. Penelitian tentang DB telah dilakukan berkaitan dengan mekanisme peradangan dan terbentuknya interleukin-6 akibat respon tubuh terhadap infeksi dengue. Sebanyak 571 artikel berbahasa inggris yang diperoleh dari database scopus tahun 1992 hingga 2022 digunakan dalam analisa bibliometrik tentang dengue yang berkaitan dengan interleukin-6. Analisa bibliometrik digunakan untuk melihat tren penelitian meliputi kata kunci yang banyak digunakan, jurnal penerbit terbanyak, penerbit yang paling banyak dikutip, instansi penulis dan kolaborasi antar penulis digambarkan dalam visualisasi menggunakan aplikasi VOSViewer. Analisa bibliometrik menunjukkan tren penelitian semakin meningkat dari tahun 1992 hingga tahun 2022. Kata kunci yang paling banyak digunakan adalah dengue virus, dengue, cytokines, cytokine, dengue hemorrhagic fever dan inflammation. Jurnal penerbit yang paling banyak menerbitkan artikel adalah Jurnal Frontiers in Immunology dengan 34 artikel dan yang paling banyak dikutip adalah dari Jurnal Immunology sebanyak 1078 kali. Penulis terbanyak berasal dari National Cheng Kung University Taiwan sebanyak 103 penulis, diikuti oleh Mahidol University Thailand sebanyak 71 penulis. Kolaborasi antar penulis Single Country Publication (SCP) maupun Multiple Country Publication (MCP) yang paling banyak adalah Amerika Serikat diikuti negara China kemudian Brazil. Hasil analisa menunjukkan bahwa penelitian mengenai demam dengue yang berkaitan dengan interleukin-6 menunjukkan tren yang semakin meningkat dari tahun ke tahun. Kontribusi penulis didominasi oleh negara-negara maju seperti Amerika dan China. Peran penulis dari negara-negara Asia Tenggara dengan kasus dengue yang tinggi diharapkan dapat semakin berkembang.

Abstract

Gagal ginjal adalah penyakit yang disebabkan oleh kerusakan fungsi ginjal yang ditandai dengan penurunan Glomerolus Filtration Rate (GFR) yang kurang dari 60% dan disertai dengan adanya peningkatan kadar urea dan kreatinin serum. Pengobatan pada pasien penyakit ginjal kronis yang paling umum adalah terapi Hemodialisis. Kualitas hidup yang baik menjadi salah satu indikator keberhasilan terapi hemodialisis yang dilakukan. Tujuan dari review artikel ini untuk mengetahui hubungan terapi hemodialisis dengan kualitas hidup pasien pasien gagal ginjal kronik. Metode yang digunakan yang digunakan dalam penyusunan artikel ini yaitu studi literature review. Data yang dikumpulkan dengan mengakses database online dengan penelusuran elektronik pada google scholar berdasarkan PICO, didapatkan 3 artikel yang memiliki titik akhir sama. Hasil review artikel didapatkan pasien yang tidak patuh melakukan terapi hemodialisa memiliki kualitas hidup yang kurang baik. Pasien yang tidak patuh mayoritas mempunyai kualitas hidup kurang baik karena pasien merasa bosan, durasi waktu yang cukup lama dan kurangnya dukungan dari keluarga sehingga membuat pasien tidak termotivasi menjalani terapi hemodialisa. Rata-rata pasien yang patuh sebanyak 61,3%, dan pasien yang memiliki kualitas hidup yang baik sebanyak 44,86%.

Abstract

Tuberculosis (TB) disease is one of the chronic diseases which could influence the patients’ quality and quantity of life. The quality and quantity of life may be affected by the disease itself or the disease’s treatment due to the adverse effect. The purpose of this research is to predict Quality Adjusted Life Years (QALYs) value before and during the treatment using Indonesian version of St George Respiratory Questionnaire (SGRQ) instruments which has been validated. This study used a cross sectional design conducted prospectively in 91 new patients diagnosed as pulmonary TB in Pulmonary Clinics, Province of Yogyakarta on November 2013 until April 2015. Patients filled out the SGRQ and the total score was counted  to predict the QALYs.We found that the predicted QALYs before and during the treatment significantly different with value was 0,691 before the treatment and they were 0.805; 0.846; 0.877; 0.892 and 0.899 during the first until fifth month of treatment respectively (p<0,05). The TB treatment can improve health status from index number 0.691 to 0.899 which is 0.208 during the treatment will produce 2 QALYs.  We conclude that the predicted QALYs of TB patients during the treatment are good and show that the patients experience significant improvement of quality of life.

Abstract

DAGUSIBU is an acronym of to get, to use, to keep and to throw the medicine away appropriately in Indonesia. The Indonesia Pharmacist Association already launched this acronym to support the appropriate use of medicine. However, some efforts are needed to be conducted to understand about the dissemination of DAGUSIBU in community. This study is aimed to define the effectiveness of the health promotion in the increase of knowledge of the community cadres in one of community district in Yogyakarta, IndonesiaThis study used quasi experimental, one group pre-posttest design. The respondents were all community cadres in Muja Muju, Yogyakarta Indonesia. The health promotion with DAGUSIBU topic by the pharmacist was conducted as the health promotion media. The questionnaire was given to the respondents before and after the health promotion.We recruited 39 community cadres from Muja Muju Yogyakarta. Most of the respondents are female (94.9%), with the average of age is 54.5 years old. The community cadres’ knowledge increased significantly from 38.4 to 80.1 (p<0.001). Respondents’ characteristics, such as age, sex and education did not predict the increase of community cadres’ knowledge about DAGUSIBUThe health promotion of DAGUSIBU   conducted by pharmacist can improve the community cadres’ knowledge. This model can be used to disseminate DAGUSIBU effectively in community level.

Abstract

T2DM sufferers during treatment will face stressors that will affect the outcome of therapy and psychological factors. The purpose of this study was to determine the relationship between perception and distress of T2DM patients on adherence, the relationship between adherence to therapy outcomes and quality of life. This study used an observational method with a cross-sectional approach, data was collected prospectively in T2DM patients in Abdul Azis Singkawang Hospital, Dok II Jayapura Hospital and Meranti Hospital. The questionnaires used in this research were the Diabetes Distress Scale (DDS) questionnaire, the Brief Perception Questionaire (B-IPQ), the Morisky Medication Adherence Scale (MMAS-4), and the European Quality of Life-5 Dimensions (EQ-5D). Data analysis using variance based SEM (Structural Equation Modeling), Partial Least Square (PLS) 3.0. The results of the study were 217 DMT2 patients who fulfilled the inclusion criteria dominated by female patients (59%) while men (41%). The index value of quality of life was 0.78±0.26 and visual analog scale was 71.46±14.45. The highest mean perception value was the treatment control domain (8.11±1.89). The mean value of compliance was 2.27±1.36. The results showed that perceptions and stress levels did not have a significant effect on adherence (p>0.05). Compliance did not have a significant effect on therapeutic outcome and quality of life (p>0.05). Stress levels and perceptions did not correlate with outcomes of therapy through adherence (p>0.05) and with quality of life through adherence (p>0.05). The highest mean score on DDS-17 is the therapeutic management domain (2.55±1.08). CSSRM concept conclusion in this study was that adherence did not have a significant effect on outcomes of therapy and quality of life. Perceptions and stress levels did not have a significant effect on compliance. ABSTRAK Penderita DMT2 selama dalam pengobatan akan menghadapi stressor yang akan berpengaruh terhadap luaran terapi dan faktor psikologis. Tujuan penelitian ini untuk mengetahui hubungan antara persepsi dan distress pasien DMT2 terhadap kepatuhan, hubungan kepatuhan terhadap luaran terapi dan kualitas hidup. Penelitian ini menggunakan metode observasional dengan pendekatan cross-sectional, pengambilan data dilakukan secara prospektif pada pasien DMT2 di RSUD Abdul Azis Singkawang, RS Dok II Jayapura dan RSUD Meranti. Kuesioner yang digunakan adalah Diabetes Distress Scale (DDS), Brief-Illnes Perception Questionaire (B-IPQ), Morisky Medication Adherence Scale (MMAS-4), dan European Quality of Life-5 Dimensions (EQ-5D). Analisis data menggunakan SEM (Structural Equation Modeling) yang berbasis varians yaitu Partial Least Square (PLS) 3.0. Hasil penelitian terdapat 217 pasien DMT2 yang memenuhi kriteria inklusi didominasi oleh pasien perempuan (59%) sedangkan laki-laki (41%). Nilai indek kualitas hidup adalah 0,78±0,26 dan visual analog scale 71,46±14,45. Rerata nilai tertinggi persepsi adalah domain kontrol pengobatan (8,11±1,89). Nilai rerata kepatuhan yaitu 2,27±1,36. Hasil penelitian menunjukan bahwa persepsi dan tingkat stress tidak terbukti berpengaruh signifikan terhadap kepatuhan (p>0,05). Kepatuhan tidak berpengaruh signifikan terhadap luaran terapi dan kualitas hidup (p> 0,05). Tingkat stress dan persepsi tidak berkorelasi dengan  luaran terapi melalui kepatuhan (p>0,05) dan dengan kualitas hidup melalui kepatuhan (p>0,05). Nilai rerata tertinggi pada DDS-17 adalah domain manajemen terapi (2,55±1,08). Kesimpulan konsep CSSRM pada penelitian ini adalah kepatuhan tidak terbukti berpengaruh signifikan pada luaran terapi dan kualitas hidup, persepsi dan tingkat stress tidak terbukti berpengaruh signifikan terhadap kepatuhan.

Abstract

Diabetes Mellitus (DM) merupakan suatu krisis global terutama pada fase urbanisasi yang cepat, perubahan gaya hidup, dan pola makan yang tidak sehat. Prevalensi Diabetes Mellitus Type 2 (DMT2) di Indonesia pada tahun 2018 sebanyak 8.5% yang artinya sebanyak 20.4 juta masyarakat Indonesia menderita DMT2. Berbagai pengobatan yang efektif telah sering didapatkan pasien DMT2 tetapi kadar gula darah masih belum terkontrol sehingga akan mempengaruhi kualitas hidupnya. Tujuan penelitian ini yaitu mengetahui hubungan luaran terapi dengan kualitas hidup pada pasien DMT2 yang menggunakan insulin. Penelitian dilaksanakan  dengan desain cross-sectional, menggunakan instrumen kuesioner Diabetes Quality of Life Clinical Trial Questionnaire (DQLCTQ) dan data rekam medis pada pasien DMT2 yang menggunakan insulin di Apotek X Kota Yogyakarta. Hasil penelitian menunjukkan bahwa dari 50 responden yang memenuhi kriteria inklusi, sebanyak 48% berjenis kelamin laki-laki dan 52% berjenis kelamin perempuan dan didominasi umur >60 tahun sebanyak 60%. Gambaran luaran terapi gula darah puasa (GDP) menunjukkan hasil terkontrol sebanyak 22% dan tidak terkontrol 78%. Kualitas hidup baik pada domain fungsi fisik sebanyak 72%, energi 22%, tekanan kesehatan 92%, kesehatan mental 56%, kepuasan pribadi 44%, kepuasan pengobatan 54%, efek pengobatan 20% dan frekuensi gejala penyakit 60%. Hasil uji chi-square menunjukkan tidak ada hubungan yang bermakna antara luaran terapi dengan kualitas hidup (p>0.05). Abstract

Diabetes Mellitus Tipe 2 (DMT2) yaitu penyakit kronis yang dapat menyebabkan komplikasi serius. Hal inilah yang membuat penderita DMT2 mengalami distress. Diabetes distress merupakan kondisi yang menggambarkan stress psikologi yang dialami penderita sebagai manifestasi dari rasa takut mengenai hal negatif akibat penyakit diabetes mellitus. Penelitian ini bertujuan untuk mengetahui gambaran distress pada pasien DMT2 yang menggunakan insulin di Apotek X. Penelitian dilakukan menggunakan rancangan cross sectional selama bulan Juni-September 2022 di Apotek X, Yogyakarta. Kriteria responden penelitian adalah semua pasien DMT2 yang datang ke Apotek X dan sesuai dengan kriteria inklusi.  Kriteria inklusinya yaitu: pasien >18 tahun yang terdiagnosa DMT2, telah menggunakan insulin (baik tunggal maupun kombinasi dengan insulin atau oral antihiperglikemia) minimal 3 bulan sebelum rekrutmen penelitian dan bersedia mengikuti penelitian ini. Instrumen penelitian menggunakan kuesioner Diabetes Distress Scale (DDS). Dari 75 responden mengalami diabetes distress tingkat rendah (94,67%) dan tingkat sedang (5,33%). Domain distress yang berpengaruh pada pasien DMT2 yaitu beban emosional (1,30), pengobatan (1,29), interpersonal (1,18), dan dokter/tenaga kesehatan (1,14). Disimpulkan bahwa pasien DMT2 yang menggunakan insulin di Apotek X mengalami diabetes distress tingkat ringan dengan domain yang paling berpengaruh adalah distress beban emosional. Abstract

The Coronavirus Disease 2019 (COVID-19) pandemic has not yet been fully under public health control, which is still currently impacting a large number of people worldwide in 2023. Since the pandemic emerged, the growing number of publications related to TMPRSS2 as a SARS-CoV-2 receptor worldwide has increased rapidly with various findings and qualities. It is important to determine the trend of TMPRSS2 publication as no such studies currently exist that represent the publication trend related to this critical field of study. Here, we employed a bibliometric-based approach to evaluate the research trends of TMPRSS2 mechanistically as the SARS-CoV-2 receptor. We identified 1012 research documents published between 2020 and 2022 for this study. The most common document category was "Research Article" (646 articles, 63.84%) followed by "Review Article" (261 articles, 25.79%), and letters to editors (57 articles, 5.63%). Germany was the most cited country with a total of citations (9400 citations), followed by the USA (6409 citations) and China (1788 citations), respectively. In conclusion, given the impact of COVID-19, this study indicated TMPRSS2 as a SARS-CoV-2 receptor as a timely and highly relevant research topic.

Abstract

Until presently, tuberculosis has been a burden to Indonesia, placing the country third among countries with the heaviest tuberculosis burden in the world. Tuberculosis treatment takes a long period and may cause side effects. Therefore, it requires sufficient tuberculosis knowledge on the patients' part for the patients to know of the side effects of anti-tuberculosis and how to deal with them. This research aimed to obtain a short questionnaire of tuberculosis patients' knowledge about anti-tuberculosis and hepatotoxicity (SQ-KSH-TB) which determines patients' knowledge of anti-tuberculosis side effects and how to handle them. This research employed a cross-sectional design and involved 17 public health services (puskesmas) and three hospitals in Yogyakarta Province. The inclusion criteria were patients diagnosed with pulmonary tuberculosis and being in a tuberculosis treatment period with a public health service or a pulmonary hospital in Yogyakarta Province. The exclusion criteria were the patients passing away, having completed a six months treatment or having been receiving treatment for over six months, and being unwilling to cooperate in the research. The questionnaire used in this research was a questionnaire that was developed based on prior research work, containing eight questions on a Guttman scale. Validity and reliability analyses were carried out with Pearson's product-moment correlation and the Kuder-Richardson-20 test, respectively. As many as 194 subjects enrolled in this research. The eight questions in the questionnaire had r count values greater than the r table, hence meeting the validity criterion. Meanwhile, the KR-20 value obtained was 0.721, indicating a good level of reliability. The SQ-KSH-TB was able to meet the validity and reliability criteria, showing applicability in the Indonesian context.
