Abstract

There are two problems with partners, they do not yet have a teaching factory program that must exist in vocational schools or Vocational High Schools and networks with the Business World and the Industrial World (DUDI), which are still limited. Solution from the community services team namely training and assistance for establishing a teaching factory program. This is also the implementation of SDGs Number 4: the quality of education. The implementation method is observation, socialization, training, mentoring, and initiation to build networks for partners. Observations are carried out to obtain existing information. Teaching factory socialization and training is carried out by giving pre-tests and post-tests to participants. The result is increased participants' understanding of the teaching factory training material. Assistance has been carried out since the establishment of the teaching factory theme, namely taxation and computer accounting, determining the location of the internship at the Accounting and Tax Management Institute “Dharma Plus” Sidoarjo, and the launching of the teaching factory program. To increase the partnership with the Business World and the Industrial World, the community service team coordinated with KADIN Sidoarjo, and as a result, SMKN 2 Buduran Sidoarjo was invited to a meeting and met with KADIN Sidoarjo management. The output of this community service is that SMKN 2 Buduran already has a teaching factory program, "Taxation and Accounting Computers," and partnerships with member companies of the KADIN Sidoarjo and also a place for fieldwork practice of taxation, namely at the Accounting and Tax Management Institute “Dharma Plus” Sidoarjo.

Abstract

Palliative care is start from diagnosis of serious illness until death and bereavement care. But, the term “Palliative Care” often mistakenly regarded identical to “End of Life Care” without any treatments. It’s caused dilemma among nurses to give maximum care of palliative care needs or let the patients get minimum of care. Methods: The aim of this article is to evaluate the dilemma palliative care needs in Intensive Care Unit. This is a case study report used a qualitative approach of 3 palliative patients in ICU used descriptive case study as well as in depth-interviews with patient’s family, nurses and internist. Results and Discussion: We present a case report of 3 patient who had been admitted to hospital with cancer and need palliative care in ICU. Patients subsequently suffered a pulmonary metastase with multiple organ failure. However the goal of end of life care for dying patients is to prevent or relieve suffering as much as possible while respecting the patient’s desires. But it is still a dilemma in Indonesia, especially palliative patients in ICU. According to medical decision, patients in ICU should receive maximum treatment, but if patient is facing multiple organ failure nurse should consider to providing palliative care. Conclusions: Palliative care approaches and provision within intensive care units can significantly impact care outcomes and increase the quality-of life people with end-of life period. Palliative care not to reduce the dose of drugs and patient care.

Abstract

Dementia is defined as a decrease in intellectual abilities which can cause changes in behavior, disruption in social life, and disruption in daily activities. Palliative care is a useful solution in the future to improve the quality of life for dementia sufferers during the COVID-19 pandemic. This systematic review aims to find out how the implementation of home telemedicine can become the main choice for elderly people with dementia during the COVID-19 pandemic. A database search was carried out by the PRISMA protocol focusing on Scopus, Proquest, Sage, ScienceDirect, and CINAHL identified based on MeSH-adjusted keywords, and A total of 2440 article journals were identified. Then deletion so that it remains 1086 article journals were reviewed. Then excluded (238) articles from journals based on 5 years, experimental test method with Randomized Controlled Trial design (RCT) and Experiments in language English, full text to be reviewed for feasibility so that 10 articles were obtained for review. From all articles, media innovation is very useful in implementing palliative care because of the various impacts of the global pandemic through the experiences of people with dementia and their care partners. The role of home-based palliative care using home telemedicine can potentially reduce the long-term impact of palliative care. The important role of palliative care integrated with long-distance services in informing health service priorities is to restore the quality of life and health of elderly people with dementia so that they are better prepared to face the future.

Abstract

Recovery of digestive function after abdominal surgery is often a challenge for patients, mainly due to nutritional disturbances caused by pain and inability to eat. This can slow down the healing process and prolong recovery time. Pain in the surgical area also inhibits patient movement, which worsens gastrointestinal motility. Complications such as difficulty in flatus and defecation can result in abdominal distension and increase length of hospitalization. This study aims to explore the effectiveness of non-pharmacological interventions, namely chewing gum and acupressure, in accelerating the recovery of gastrointestinal function in patients after abdominal surgery. Methods: The search strategy involved a systematic review using four electronic databases: Pubmed, Science Direct, Proquest, Taylor and Francis, and Scopus. The search was conducted on February 20, 2024, and focused on articles published from 2019 to 2024 in English. The search terms “Chewing Gum” AND “Acupuncture” OR “Acupuncture Points” OR “ST 36 Point” AND “Non-pharmacological Therapy” AND “Postoperative Period” AND “Abdominal Surgery” AND “Digestive” AND “System” AND “Function” AND “Recovery”. After removing duplicates and filtering based on title, abstract, and eligibility, 18 articles were selected for the review using PRISMA. Result: The study found 1.098 articles and reviewed 18 articles the study reviewed the effectiveness of non-pharmacological interventions, specifically chewing gum and acupressure, in enhancing digestive recovery following abdominal surgery. Results indicated that both methods significantly improved gastrointestinal function, evidenced by faster bowel motility and reduced hospital stays. The findings suggest that implementing these interventions could provide effective support in postoperative care, addressing common complications such as delayed bowel movement and abdominal distension. Conclusion: The combination of chewing gum and acupressure can significantly accelerate the recovery of digestive function in post-abdominal surgery patients, making it an effective non-pharmacological method to improve post-surgical care.

Abstract

Factors affecting recurrent hospitalization in patients with heart failure are non-compliance with medication, fluids, and diet. Compliance with medication, dietary and fluid restriction, and weight of the body are part of self-care management. The study aims to improve self-care management in heart failure patients. Literature reviews are used to identify articles through Scopus, ProQuest, PubMed, and CINAHL databases. The inclusion criteria are open access, in English, with full-text articles published between 2016 and 2022 with the keywords "self-regulation", "protection motivation theory" and "heart failure". A total of fifteen articles were reviewed to answer research questions. The data is analyzed by the PRISMA chart. The results of the literary study showed that there were 15 articles that met the inclusion criteria. There are eight studies that use randomized controlled trial designs; seven titles use cross-sectional designs; and one study uses qualitative designs. Results show that better self-care can improve outcomes, reduce the incidence of recurring hospitalizations, and reduce the burden of treatment for heart failure patients. Self-efficacy can also be predicted from self-care behavior. It can be concluded that self-regulation is significantly linked to self-management behavior, and self-adjustment programs are known to be effective in improving self-care behaviors in heart failure patients.

Abstract

Shivering is a common issue experienced by patients during surgery, caused by exposure to cold operating room air. This loss of body heat can make patients uncomfortable and require more oxygen. The purpose of this study is to evaluate the management of shivering and hypothermia based on literature review. A systematic review was conducted following the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA). The article selection process followed the PICO framework. The population consists of individual patients undergoing surgery with spinal anesthesia. The study included patients who experienced shivering and underwent non-pharmacological interventions for temperature management. Articles published between 2018 and 2024 were searched in four databases: Scopus, EBSCO, ProQuest, and PubMed. The quality of the articles was evaluated using the Joanna Briggs Institute (JBI) format. The initial search identified 270 articles, which were narrowed down to 12 after removing duplicates and reviewing titles and inclusion criteria. In this study, 1326 individuals undergoing surgery were surveyed. To prevent hypothermia and shivering, several non-pharmacological methods can be used, such as pre-warming using forced-air warming, which increases body temperature. Isothermic irrigation was found to reduce the decrease in core temperature and the incidence of shivering. Preoperative warming can be done for durations of 30, 15, and 10 minutes before induction. To conclude, shivering can be prevented by non-pharmacological methods to increase core body temperature so that it will increase patient comfort during surgery. One of the innovations that can be done is by making warm vests
