Abstract

Background: Family planning will help reduce population growth, fertility, improvement quality of life, women's health status, unwanted pregnancy, and abortion. The selection of the use of con­traceptives needed leads to a contraceptive that has higher effectiveness, namely the long-term contraceptive method. This study aimed to analyze contextual effect of posyandu on the decision to use long term contraceptive in Tulungagung, East JavaSubjects and Method: This was a cross-sectional study conducted at 25 integrated health posts (posyandu) in Kauman health service, Tulungagung, East Java, in January 2019. A sample of 200 women of childbearing age was selected by stratified random sampling. The dependent variable was the use of long-term contraceptive method. The independent variables were age, education, knowledge, occupation, husband and family supports, women's perception, local culture, and posyandu. The data were collected by questionnaire and analyzed by a multilevel multiple logistic regression run on Stata 13.Results: The use of long-term contraceptive method increased with age ≥35 years (b= 1.68; 95% CI= 0.51 to 2.85; p= 0.005), education ≥Senior high school (b= 2.22; 95% CI= 0.97 to 3.46; p <0.001), high knowledge (b= 2.05; 95% CI= 0.75 to 3.35; p= 0.002), working outside the house (b= 1.66; 95% CI= 0.54 to 2.78; p= 0.004), strong husband and family support (b= 2.11; 95% CI= 0.87 to 3.34; p= 0.001), positive perception of long-term contraceptive method (b= 1.92; 95% CI= 0.75 to 3.08; p= 0.001), and supportive local culture (b= 1.10; CI 95%= -0.04 to 2.25; p= 0.009). Posyandu had strong contextual effect on the use of long-term contraceptive method with ICC= 26.70%.Conclusion: The use of long-term contraceptive method increases with age ≥35 years, education ≥Senior high school, high knowledge, working outside the house, strong husband and family support, positive perception of long-term contraceptive method, and supportive local culture. Abstract

Background: Postpartum depression is a condition that affects 20% women in the first four weeks of the puerperium. Postpartum depression can be affected by biological, psychological, socio-cultural, and economic factors. This study aimed to analyze biopsychosocial factors affecting postpartum depression in Sleman, Yogyakarta.Subjects and Method: This was a cross-sectional study. This study was conducted at 25 delivery places in Sleman, Yogyakarta, in August-September 2019. A sample of 200 postpartum mothers was selected by multistage random sampling. The dependent variable was postpartum depression. The independent variables were traditional treatment, age, education, income, parity, pregnancy status, type of delivery, complication, marital satisfaction, and contextual delivery place. The data were collected by questionnaire and analyzed by a multilevel multiple logistic regression.Results: The risk of postpartum depression increased with delivery with intervention (b= 3.30; 95% CI= 1.57 to 5.02; p<0.001), delivery with complication (b=3.77; 95% CI= 2.25 to 5.28; p<0.001), and age ?35 years (b=0.93; 95% CI=-0.20 to 2.07; p=0.109). The risk of postpartum depression decreased with traditional treatment (b= -1.33; 95% CI= -2.51 to -0.15; p= 0.027), education ?Senior high school (b=-1.98; 95% CI= -3.59 to -0.38; p=0.015), family income ?Rp 1,701,000 (b= -3.55; 95% CI= -5.08 to -2.02; p<0.001), multiparous (b= -1.25; 95% CI= -2.45 to -0.04; p=0.041), intended pregnancy status (b= -3.11; 95% CI= -4.96 to -1.25; p= 0.001), and happy marital satisfaction (b= -1.18; 95% CI= -2.30 to -0.05; p=  0.039). There was strong contextual effect of delivery place on postpartum depression with intra-class correlation (ICC)= 31.6%.Conclusion: The risk of postpartum depression increases with delivery with intervention, delivery with complication, and age ?35 years. The risk of postpartum depression decreases with traditional treatment, education ?Senior high school, family income ?Rp 1,701,000, multiparous, intended pregnancy status, and happy marital satisfaction. Abstract

Background: Triple elimination is the elimination of transmission of HIV, syphilis, and hepatitis B from mother to child. The Prevention of Mother to Child Transmission (PMTCT) program is a comprehensive activity. The basic condition of the case is carried out because of the importance of health promotion and early detection in strategies for managing HIV, Syphilis, and Hepatitis B infections. This study aimed to analyzecontextual effect of community health service on the use of triple elimination service in pregnant women.Subjects and Method: This was a cross sectional study conducted at 25 community health centers in Sragen Regency, Central Java, from June to July 2019. A sample of 200 pregnant women was selected by stratified random sampling. The dependent variable was the use of triple elimi­nation. The independent variables were age, perceived susceptibility, perceived seriousness, cues to action, attitude, motivation, occupation, income, and distance to health service. The data were collected by a set of questionnaire and analyzed by multilevel multiple logistic regression.Results: The use of triple elimination increased with age ?35 years (b=1.29; 95% CI=0.37 to 2.21; p=0.006), working outside the home (b=1.20; 95%CI=0.17 to 2.23; p=0.012), income by ?Rp 1,600,000 (b=1.52; 95% CI=0.54 to 2.50; p=0.002), perceived susceptibility (b=1.46; 95% CI=0.53 to 2.38; p=0.002), perceived seriousness (b=1.53; 95% CI=0.55 to 2.50; p=0.002), cues to action (b=1.30; 95% CI=0.30 to 2.30; p=0.010), attitude (b=1.10; 95% CI=0.02 to 2.18; p=0.046), and strong motivation (b=1.68; 95% CI=0.66 to 2.71; p=0.001). The use of triple elimination decreased by a distance ?5km (b=-1.36; 95% CI=-2.27 to -0.46; p= 0.003). Community health center had strong contextual effect on the use of triple elimination with ICC= 22.6%.Conclusion: The use of triple elimination increases with age ?35 years, working outside the home, income by ?Rp 1,600,000, perceived susceptibility, perceived seriousness, cues to action, attitude, and strong motivation. The use of triple elimination decreases by a distance ?5km. Abstract

BackgroundThe polycystic ovary syndrome (PCOS) is caused by endocrine system dysfunction in women. MUCIN-1 (MUC-1) expression is found in endometrial tissues, which leads to implantation process dysfunction because of imbalance of trophoblast adhesion process. This study was conducted to compare endometrial MUC-1 expression between PCOS and normal women considering all existing external variables.MethodsThis cross-sectional study was conducted in General Hospital Dr. Moewardi Surakarta. Endometrial samples were obtained from 30 infertile PCOS women based on Rotterdam criteria, and 30 normal women. Life style and reproductive data such as age, menstrual problems, menstrual cycle, age at menarche, and BMI were collected. Subjects underwent endometrial biopsy in luteinizing hormone (LH) secretion phase LH + 5 days to LH + 10 days for immunohistochemistry (IHC) of MUC-1 expression. An independent-t and multiple linear regression test were used to analyze the data at significance level of p&lt;0.05. ResultsMean MUC-1 expression in the PCOS endometrium (49.66 ± 47.79) was significantly higher than in normal women (7.66 ± 14.55) (p=0.03). Multivariate linear regression model of life style and reproductive variables with MUC-1 showed that PCOS (b=29.54; 95% CI 9.57-49.49; p=0.004) and BMI (b= 29.99; 95% CI 5.91-54.07; p=0.001) significantly increase MUC-1 expression. PCOS (Beta=0.37) was more important than BMI (Beta=0.30) in increasing the MUC-1 expression. ConclusionExpression of MUC-1 levels in the PCOS endometrium was higher than in normal women. This suggests that MUC-1 contributes to the unexplained reproductive failure in PCOS.

Abstract

 Preterm delivery is associated with higher mortality and morbidity of neonates, also increasestheir risk of having growth and development impairment.This study aimed toidentify the role ofgenital tract infection in preterm delivery. A retrospective study was conducted on medical records of mothers who had preterm delivery in a tertiary hospital in Surakarta, Indonesia during 2017. The data collected were mothers’ age and their gestational age, the history of current pregnancy, the number of previous abortion(s), mothers’ body temperature, the extent of abnormal vaginal discharge, and laboratory findings (white blood cell count, platelet count, red blood cell count, hemoglobin count, hematocrit level, urinalysis and microbiology results). The statistical differences amongst categorical and numerical data were analyzed using the Chi-Square test and the Mann-Whitney test. Based on the patient’s history and the examination results, we suspected genital tract infections in22.52% (25/111)of subjects. All of them had abnormal vaginal discharge despite only one case had been confirmed as streptococcal infection. This study found that the majority of mothers with probable genital tract infection hadpreterm premature rupture of the membrane whilst preeclampsiawas more evident in those without genital tract infection. We conclude that genital tract infection during pregnancy is a significant contributor to the occurence of premature birth so that microbiological testing is needed to confirm the diagnosis.

Abstract

Background: Polycystic Ovary Syndrome (PCOS) is an endocrine disorder that most often causes infertility. It occurs due to the failure of folliculogenesis, thus causing non-ovulation. Letrozole is a 3rd generation drug of non-ste­roidal aromatase inhibitors, which can increase ovulation rates. Electroacupuncture is the insert­ion and manipu­lation of needles into acupunc­ture points (acu­points) that are selected speci­fically based on the meridian system and con­nected to the electrode stimulator. This study aimed to determine the differences of the effects of letrozole and letrozoleelectroacupuncture (letrozole-EA) on the improve­ment of folliculo­genesis in women with PCOS.Subjects and Method: This was an analytic observational study with a case-control design. This study was conducted at the Gynecology Polyclinic and Sekar Clinic, Dr. Moewardi Hos­pital, Surakarta. A sample of 30 infertile women aged 20-45 years was selected by fixed disease sampling. The criteria of the PCOS were deter­mined based on Rotterdam criteria. The depen­dent variable was the diameter of the follicle. The independent vari­ables were the administration of letrozole and letrozole-EA. Transvaginal ultra­sound of Voluson P6 was used to measure the folliculo­genesis. The data were analyzed by independent t-test.Results: On the second day after treatment, the diameter of the follicles in the letrozole-EA group (mean= 6.92; SD= 1.60) was bigger than the letrozole group (mean=5.66; SD=1.11), and it was statistically significant (p=0.004).Conclusion: Combination of letrozole and letrozole-EA therapy is more effective in increasing follicular diameter development than letrozole therapy only.Keywords: polycystic ovary syndrome, folli­culogenesis, Letrozole, electro-acupunctureCorrespondence: Yulyanti. Abstract

Background: Polycystic Ovarian Syndrome (PCOS) is an endocrine disorder in women cha­rac­terized by increased levels of androgens, ovu­la­tory dysfunction, and Polycystic Ovarian Mor­p­ho­logy (PCOM), as well as a combination of clas­sic clinical features. Women with PCOS expe­ri­ence changes in the endometrium and endo­met­rial receptivity markers such as VEGF (Vascular En­dothelial Growth Factor). VEGF increases the per­meability and dilation of endometrial blood vessels which play an important role in imp­lan­ta­tion. This study aimed to analyze the difference of VEGF expression between women with PCOS and nor­mal women.Subjects and Method: This study was a case-con­trol study design conducted at the polyclinic of the Department of Obstetrics and Gynecology, Dr. Moewardi Hospital, Surakarta, Central Java, in July 2020. There were 30 infertile women with PCOS (diagnosis based on Rotterdam criteria, con­sensus 2003) and 30 women without PCOS as the sample of this study. The dependent variable was VEGF expression. The independent variables were age, BMI, family history with PCOS, menar­che, occu­pa­tion, and education. VEGF expression was ob­tained from an endometrial biopsy on days 19-24 of periods. The differences in VEGF exp­res­sion (in %) were analyzed by the Mann-Whitney test. VEGF determinants were analyzed by a mul­ti­ple linear regression.Results: The mean of VEGF expression in wo­men with PCOS (Mean=64.33; SD=44.43) was high­er than in women without PCOS (Mean­= 7.90; SD=8.73) and the result was statistically sig­ni­ficant (p<0.001). PCOS (b=42.09; 95%­CI­=­64.­2­6 to -19.93; p<0.001) and obese (b=38.63; 95%­CI=-63.44 to -13.82; p=0.003) increased VEGF expression.Conclusion: VEGF expression in the endo­met­rium of women with PCOS is higher than normal wo­men.Keywords: VEGF, endometrium, Polycystic Ova­rian SyndromeCorrespondence: Ricky Bernadi Saputra. Abstract

Background: Health problems in adolescents are among the most important problems in the human life cycle, especially reproductive and sexual problems. Actively conducted premarital sex behavior poses a risk to teenage pregnancy and transmission of sexually transmitted dise­ases. This study aims to analyze the theory of planned behavior and social cognitive theory on the determinants of premarital sex behavior in adolescents.Subjects and Method: This was a cross-sectional study design. The study was con­ducted at 25 high schools/vocational high schools in Gresik, East Java, from September to October 2020. A sampling of 200 adolescents used stratified random sampling and simple ran­dom sampling. The dependent variable was premarital sex behavior. The independent varia­­bles were the intention, attitude, subjec­tive norms, peer support, family intimacy, and self-efficacy. The data were collected by using a questionnaire. Data were analyzed using multi­ple logistic regression with Stata 13.Results: Premarital sexual behavior increased with strong intention (b= 1.06; 95% CI= 0.15 to 1.98; p= 0.015), positive attitude (b= 1.22; 95% CI= 0.38 to 2.13; p= 0.009), subjective norms supportive (b= 1.09; 95% CI= 0.14 to 2.04; p= 0.024), and strong peer support (b= 1.37; 95% CI= 0.44 to 2.30; p= 0.004). Premarital sexual behavior decreased with strong family intimacy (b= -1.32; 95% CI= -2.23 to -0.40; p= 0.005) and strong self-efficacy for abstinence (b=                 -1.03; 95% CI= -2.23 to -1.93; p= 0.025). There was an effect at the school level on premarital sex behavior with ICC 24.6%.Conclusion: Premarital sexual behavior in­crea­ses with strong intentions, positive attitu­des, supportive subjective norms, and strong peer support. Premarital sexual behavior de­crea­ses with strong family intimacy and strong abstinence self-efficacy.
