Abstract

ABSTRACT Hyperglycemia is a global health problem requiring effective management. Given the high risk of side effects and cost of conventional treatment, the search for natural alternatives is relevant. The Meniran plant (Phyllanthus niruri L) is known for its flavonoid content, which has potential as an antihyperglycemic agent. This study aimed to test and compare the effectiveness of Meniran extract with the standard drug Metformin in reducing blood glucose levels. The study employed an experimental pre and post control design on 20 White Rats (Rattus novergicus) whose hyperglycemia was induced using Dexamethasone at a dose of 0.09 mg/kgBW/day. The test animals were divided into four treatment groups, including Metformin positive control and three dose variations of Meniran extract (300mg, 400mg, and 500mg/kgbw) extracted through 96% ethanol maceration. The results showed that both Metformin and Meniran extract were effective in significantly reducing the blood glucose levels of the test subjects starting from the third day. Notably, the Meniran extract proved to be more effective in reducing blood glucose levels than Metformin on the third day, concluding that Meniran extract (Phyllanthus niruri L) has strong and promising potential as a natural antihyperglycemic agent that can be an alternative treatment.

Abstract

ABSTRACT Fatty liver, or Non-Alcoholic Fatty Liver Disease (NAFLD), is a metabolic disorder commonly observed among individuals with obesity. Reduced High-Density Lipoprotein (HDL) levels are linked to disturbances in lipid metabolism and may contribute to a higher risk of developing fatty liver. This study aimed to analyze the relationship between HDL levels and the occurrence of fatty liver among obese students at the Faculty of Medicine, Muslim University of Indonesia. An observational analytic study with a cross-sectional design was conducted. A total of 33 obese students were selected using purposive sampling. HDL levels were measured through blood laboratory examinations, while fatty liver status was assessed using abdominal ultrasound (USG). The association between variables was analyzed using the Chi-Square test with a significance level of p < 0.05. The majority of participants were male (60.6%), classified as obesity class II (54.5%), and had low HDL levels (48.5%). Fatty liver was identified in 11 respondents (33.3%). Bivariate analysis indicated a significant relationship between HDL levels and fatty liver disease (p = 0.022). Individuals with low HDL levels showed a higher proportion of fatty liver (56.3%) compared to those with normal HDL levels (15.4%), while no cases were observed among participants with high HDL levels. The findings demonstrate a significant association between HDL levels and fatty liver in obese medical students at the Faculty of Medicine, Muslim University of Indonesia, indicating that lower HDL levels are related to an increased risk of fatty liver disease

Abstract

ABSTRACT Chronic kidney disease patients undergoing hemodialysis frequently experience psychological distress that may interfere with treatment adherence and quality of life. To analyze the relationship between hemodialysis duration, stress level, and anxiety level among chronic kidney disease patients undergoing hemodialysis at Bhayangkara Hospital Makassar. This analytic observational study used a cross-sectional design in 2025 among 56 chronic kidney disease patients receiving hemodialysis. Stress was measured using the Perceived Stress Scale-10 (PSS-10) and anxiety using the Generalized Anxiety Disorder-7 (GAD-7). Associations were tested using Spearman correlation. Most participants reported moderate-to-severe stress (moderate 42.9%; severe 35.7%) and mild-to-moderate anxiety (mild 44.6%; moderate 46.4%). Longer hemodialysis duration was associated with lower stress (p0.001; r=-0.712) and lower anxiety (p0.001; r=-0.642). Stress level was positively associated with anxiety level (p0.001; r=0.523). Conclusion: Hemodialysis duration is inversely related to stress and anxiety, while higher stress is associated with higher anxiety. Routine screening and integrated psychosocial support are recommended, particularly for patients in the early phase of hemodialysis. Keywords: Hemodialysis, Chronic Kidney Disease, Stress, Anxiety, GAD-7.

Abstract

Type 2 diabetes mellitus is one of the leading causes of morbidity and mortality associated with cardiovascular complications. Advances in antihyperglycemic therapy have demonstrated that sodium-glucose cotransporter-2 (SGLT2) inhibitors and glucagon-like peptide-1 receptor agonists (GLP-1 receptor agonists) not only improve glycemic control but also play an important role in preventing cardiovascular complications. However, evidence regarding their mechanisms of action, clinical benefits, and roles in cardiovascular prevention remains scattered across the literature. Objective of the research to review the current scientific evidence regarding the role of SGLT2 inhibitors and GLP-1 receptor agonists in preventing cardiovascular complications in patients with type 2 diabetes mellitus based on published studies. This study employed a narrative literature review using a systematic literature search approach. A literature search was conducted in the PubMed database for articles published between 2021 and 2026, following the PRISMA 2020 guidelines. A total of 15 eligible articles were included, comprising randomized controlled trials, cohort studies, observational studies, systematic reviews, meta-analyses, and post-hoc analyses. Results of the research showed that the synthesis of the included studies demonstrated that SGLT2 inhibitors consistently reduced the risk of hospitalization for heart failure, major adverse cardiovascular events (MACE), cardiovascular mortality, and the progression of chronic kidney disease. In contrast, GLP-1 receptor agonists were more effective in reducing atherosclerotic cardiovascular events through improvements in vascular function and metabolic risk factors. Several studies also suggested that combination therapy with both drug classes may provide greater cardiorenal benefits than monotherapy. SGLT2 inhibitors and GLP-1 receptor agonists have complementary roles in preventing cardiovascular complications in patients with type 2 diabetes mellitus. Therapeutic selection should be individualized according to patients' clinical characteristics and cardiovascular risk profiles to optimize cardiorenal outcomes.

Abstract

High sugar intake, particularly from added sugars and sugar-sweetened beverages, is known to contribute to the increasing prevalence of obesity and insulin resistance, which are risk factors for various metabolic diseases. The Objective of the review  to determine the relationship between high sugar intake and insulin resistance in obesity based on published research findings. This study used a literature review method by searching articles through the PubMed databases published between 2021 and 2026. Article selection was conducted based on the PRISMA 2020 guidelines, resulting in 12 articles that met the inclusion criteria. Literature synthesis demonstrates that high sugar consumption, particularly liquid fructose, drives insulin resistance through hepatic de novo lipogenesis (DNL), accumulation of toxic plasma ceramides, low-grade systemic inflammation (elevated CRP and IL-6), gut microbiota dysbiosis, and altered expression of lipogenic genes (e.g., MEG3 and FTO). Isocaloric fructose restriction studies showed significant improvements in insulin sensitivity and reduction of hepatic fat, independent of substantial weight loss. Furthermore, a bidirectional vicious cycle exists where higher baseline insulin resistance reinforces sensory preferences for sugar-sweetened foods. There is a strong and causal relationship between high sugar intake and insulin resistance in obesity, mediated by complex molecular and metabolic pathways. Restricting added sugar intake, especially from sugar-sweetened beverages, is highly recommended as a primary prevention and clinical management strategy for obesity.
