Abstract

Background: Located in remote area, X Hospital is facing various challenges as a private non-profit class C hospital. Therefore, it needs to develop an appropriate strategic plan for the year 2020-2024 to overcome challenges and achieve its vision. This study is aimed at identifying the strengths, weaknesses, opportunities and threats from internal and external strategic factors, determine strategic positioning, and formulate alternative strategies for X Hospital.  Method: This is a preliminary study with qualitative - exploratory design. Data were collected through in-depth interviews, group discussions and observation. The interviews were conducted with 7 key informants holding strategic management positions. This study was conducted in two in-depth stages using Consensus Decision Making Group (CDMG) method to avoid bias. Data were analyzed using Internal Factor Evaluation (IFE) and External Factor Evaluation (EFE) matrixes at the input stage and Internal External (IE) and Threat Opportunity Weakness Strength (TOWS) matrixes at the matching stage. This study was conducted at X Hospital, West Borneo Province, Indonesia, from March to April 2019. Results: The IFE final score was 2.38 with strength (1.68) greater than weakness (0.70) and EFE score of 2.16 with threats (1.10) greater than opportunities (1.06). Such input stage results indicate that X Hospital has average response in utilizing its internal and external strategic factors where strengths are more prominent than weaknesses but the threats are still overshadowing its opportunities. Consequently, the results of matching stage place X Hospital on the position of hold or maintain strategies and formulate 11 alternative strategies that focus on market or product development. Conclusions: X Hospital is currently on a strategic position to develop its services as the basis for the five-year strategic planning. A new policy should be made to encourage the establishment of a network with private hospitals to promote healthcare in remote areas.

Abstract

Background: Stigma, lack of knowledge about HIV risks to oneself, openness, limited access to health services and aspects of gender inequality are determinants that influence the willingness to take HIV tests in the context of VCT. However, studies in the context of PITC have not been widely publicized in Indonesia, particularly studies in private hospitals. Analysis related to the reason that influence the willingness of health workers to carry out PITC in TB patients is important to do. The purpose of this study is to analyze the reason that influence the willingness of health workers to carry out PITC in TB patients. Methods: This study is a quantitative study with cross-sectional design. The study was conducted at a private hospital in Jakarta. The sampling technique used total sampling techniques, where the entire population was sampled. the research sample obtained is 30 doctors who have TB patients in the private hospital during March to April 2019. Results: The majority of doctors (46.7%) were sometimes willing to do the PITC in TB patients. The proportion of doctors who are always willing to do the PITC in TB patients is still low at 20%. On the other hand, there were 33.3% doctors were never willing to do the PITC on TB patients. The reason that influences the doctor's willingness to do PITC on TB patients is the possibility to explain (66.7%), automatically as requested in the rules (20,0%) and the severity of the TB case (13,3%). Conclusions: The biggest reason that influences the doctor's willingness to do PITC on TB patients is the possibility to explain. Doctors are often difficult to explain the importance of HIV testing for TB patients due to the thick negative stigma associated with HIV testing. the government should encourage socialization regarding the importance of HIV testing for both TB patients and high-risk HIV groups. The key challenge to improving health care delivery is to improve the implementation and monitoring of effective interventions.

Abstract

The local government hospital in Serang (RSUD Serang) is a self-financed type B non teaching hospital that has a capacity of 285 beds. It employs 218 nurses who work in the inpatient units, consists of 75 civil servant nurses (34.4%), and 143 (65.6%) contractual based nurses who are placed in variety of units (Department of nursing, 2001). The different of the employment status has produced many problems that has to be explored seriously. Many contractual based nurses perceived that they have no job security and stability. This was demonstrated by the fact that many of them had resigned and moved to another hospitals that could provide them with job security and stability. On the other hand, civil servant nurses who have more job security and stability apparently have not yet demonstrated high productivity as has been expected. Many complaints raised by patients on the staff work performance (Mulyati, 2001). The purpose of the study was to identify the difference in nursing care time productivity between both types of nurses and its relationship with the staff placement system in RSUD Serang. The design was a descriptive comparative using cross sectional approach (76 respondents). A questionnaire was used to collect data on staff placement system, and a series of observation was done to measure workload to determine time productivity for both groups of nurses. The findings demonstrated that there was no significant difference in time utilization between both types of nurses; the productivity score in both groups was low (47.84%). In addition, there was no significant difference in time productivity related to staff placement system for both groups. Some recommendation were contributed to the hospital management, and further study needs to be done to explore some other variables using different design.
