Abstract

Background: Patient safety is one of the most important dimensions in quality of healthcare. Nurses are the key in care delivery, their compliance toward Patient Safety policy cannot be overlooked. Studies evaluating determinants of nurse’s compliance to Patient Safety policy is still lacking. Aims: To evaluate nurse’s attitude toward Patient Safety policy using Health Belief Model (HBM). Methods: An analytical cross-sectional study was carried on in Bali Royal Hospital, Indonesia, on December 2016. An anonymous, self-administered questionnaire was used to collect the data on socio demographics, knowledge, and attitude toward Patient Safety policy. Attitudes are specifically assessed using HBM. Results: A total of 124 nurses are included in this study. Mean age of the study population was 29.7±6.7 years. Mean score for knowledge about Patient Safety policy was 12.68±3.40 from 18.0- point scale. Nurses perceived a low risk of non-compliance (mean perceived risk 8.07±1.89) and a low barrier to comply with Patient Safety policy (mean perceived barrier 8.66±1.78). Knowledge and attitudes toward Patient Safety policy did not differ by sex and educational background but it did differ according to unit of workplace. Age and length of work correlated positively to knowledge (Spearman’s r=0.439, p-value 0.000; and r=0.400, p-value <0.001, respectively). A positive and moderate correlation was observed between knowledge vs. perceived severity (r= 0.394, p-value <0.001) and knowledge vs. perceived benefit (r=0.422, p-value < 0.001). There was a strong and positive correlation between perceived severity and perceived benefit (r= 0.725, p-value <0.001). Conclusion: Nurse’s attitudes toward Patient Safety policy were generally good and correlate positively with knowledge. The use of HBM provides an increased understanding of how individual perceptions can be influenced to improve nurse’s engagement in promoting safer health care.

Abstract

Backgound: Building a health system is one of the SDG targets – universal health coverage (UHC). Strong primary health care system needs to be in place (4). The critical issue is the quality of health by improving patient safety, effectiveness, focus on patients, timely, efficient and fair (4, 10). Long waiting times decreased utilization of national health insurance, decreased use by the public and the rising cost of health. (5,7,8,9). The aim of this paper is to investigate how the human resources can lead to long waiting times in the registration by using Malcolm Baldrige approach (6). Method: This study used qualitative design to describe waiting time problem and it’s causes. Informants were appropriately and adequately selected in relation to waiting time including patients clinic, staff and management so the information were triangulated by resources,by methods and by data itself. The results were write as transcription and content analysis carefully done to identify themes and problem. This study held at Public Health Center, Cariu, Bogor, Indonesia in December 2016. Results: All the informants expressed their concern related to waiting time and need to formulate effective solutions. The solutions were proposed related to causes to the waiting time ; no monitoring and evaluation effort to control waiting time and it’s impact, clinical safety procedures, unsatisfied patients and ineffective clinical outcomes ; low priority to human resources management; no supervision ; no on the job training; no evaluation of patients need and expectation, the public health center never had services design ; network instability and inflexibility in the provision of facilites. Conclusion: Monitoring and evaluation, human resources management, supervision, on the job training, strategic planning, patients focus , commitment and the leadership itself are needed to decrease waiting time.

Abstract

Background: Inflammatory bowel disease (IBD) in the form of Crohn’s disease (CD) or ulcerative colitis (UC) is chronic IBD which still difficult to diagnose and clinically characterized by exacerbation and spontaneous remission. A precise diagnosis is needed and essential for appropriate treatment. Most of internists in Indonesia have to cope with a condition of poorly equipped endoscopic facilities - especially colonoscopy and subsequently refer their patients to endoscopic centers with colonoscopy facilities as well as sending biopsy specimens for histopathology examination. They also should be concerned that it would be expensive, time- consuming, and patients may suffer from considerable distress while waiting for the results of diagnostic confirmation. Therefore, we were interested in studying the clinical scoring for IBD to determine the diagnosis of possible IBD by assessing the combination of clinical reporting aspects, identification of probable IBD and histopathology examination Method: The study design was cross-sectional. We collected data pertinent to this study from medical records of patients with IBD clinical features at the endoscopic units of four hospitals in Jakarta between 1999 and 2009. Student t-test and logistic regression analysis were used for the statistic examination. Results: Based on colonoscopy examination, there were 213 patients with positive IBD and 173 patients with negative results. Histopathology results were considered as the gold standard in diagnosis of IBD. The mean clinical score was 9 (nine) points. Subjects with positive histopathology findings were significantly had higher average score than those with negative histopathology findings. Using ROC curves, we found cut-off score of 10 points with a sensitivity and specificity of 73% and 88% consecutively; while by applying logistic regression analysis, we found odds ratio (OR) of 20 (9.9-40.0 Conclusion: The clinical scoring for IBD is a valuable diagnostic tool in determining definitive diagnostic of IBD based on histopathology findings Keywords: clinical scoring, histopathology findings, definitive IBD

Abstract

A new stroke-prevention service (ERDANELA model) has been developed at nursing homes by empowering elderly caregivers to become service-implementing cadres. The aim of this study is to investigate the effect of empowerment on the knowledge, attitude, skill, and behavior of elderly caregivers about stroke-prevention services in nursing homes. A quasi-experimental study was conducted on 52 elderly caregivers who were working at 3 nursing homes in West Sumatra Province, Indonesia. The questionnaire design consists of knowledge, attitudes, skills, and behavior variables. Intervention in the form of training was carried out for three days, using pre-test and post-test methods. The results of this research are the majority of caregivers are 40-49 years old (34.60%), male (51.9%), have middle school education (48.10%), have monthly income bigger than the RMW (75.00%), worked in nursing home>5 years (67.30%), and never attended training while working at nursing-home (73,10%). Caregivers knowledge, attitude, skill, and behavior mean was 18.23±1.89, 7.65±1.22, 6.75±1.37 and 6.00±0,91 respectively before training, and significantly it has increased to 25.48±3.38, 10.15±.09, 9.10±0.96, and 7.06±1.43 respectively after training with sig=0.000<0.05. Empowerment has succeeded in increasing the knowledge, attitudes, skills, and behaviors of elderly caregivers to become service-implementing cadres of the new stroke-prevention service at nursing homes

Abstract

In Indonesia, diarrhea is the second leading cause of death for children aged 29 days-11 months (14% of deaths), and the main cause of death in the group under five is 10. 3% of deaths. The study aimed to analyze the determinants of the father's knowledge regarding giving fluids when children have diarrhea. This study was an analytical quantitative method with a cross sectional-design using secondary data in the form of the Indonesian Health Demographic Survey (SDKI) 2017. Involved 2.683 respondents selected by stratified sampling and data collection using the SDKI questionnaire. Data analysis using logistic regression test. The results showed that the percentage of fathers who knew about the number of fluids given when their children had diarrhea in Indonesia in 2017 was 65.7%. Economic status and education level had a significant relationship with the father's knowledge (p-value < 0.05). Economic status is the most related factor to the father's knowledge (p-value < 0.001; OR 3.557 (2.371-5.337), after being controlled by other factors. Father's age and residence are confounding factors. Considering that economic status and education level affected the father's knowledge, diarrhea management needs to be accompanied by interventions to improve the father's economic status and education access.

Abstract

Perencanaan strategis merupakan hal yang sangat menentukan arah perkembangan Rumah Saki tuntuk mencapai visi dan misi rumah sakit. Tujuan penelitian ini adalah untuk mengetahui kondisi internal dan eksternal RSUD Sawah Besar sehingga memiliki rencana strategi pengembangan rumah sakit. Penelitian ini merupakan penelitian deskriptif dengan pendekatan kualitatif dimana pengumpulan data primer dilakukan melalui kuesioner dan Focus Group Discussion (FGD) bersama manajemen RSUD Sawah Besar. Hasil penelitian ini menunjukkan bahwa nilai IFE (Internal Faktor Evaluation) RSUD Sawah Besar sebesar 2.65 yang artinya berada di daerah IFE rata-rata, dan nilai EFE (External Faktor Evaluation) dari RSUD Sawah Besar sebesar 2.69 berada di daerah EFE sedang. Dari hasil tersebut menempatkan posisi RSUD Sawah Besar  di kuadran V (hold and maintain). Kesimpulan dari penelitian ini adalah dengan melakukan analisa situasi  dengan metode SWOT, RSUD Sawah Besar dapat menentukan secara khusus rencana strategis yang disesuaikan dengan kondisi internal dan eksternal sesuai dengan posisi  kuadran yaitu strategi market penetration dan product development.

Abstract

Background: VIA screening is very suitable for developing countries such as Indonesia, because the technique is easy, simple, low cost/cheap, high sensitivity, fast and accurate for early detection of cervical cancer. The coverage of early detection in Indonesia is <5% so that many cases of cervical cancer are found to be at an advanced stage and often cause death in women.Methods: This study uses a mix-method study approach with a sequential exploratory approach. The research was carried out sequentially with qualitative methods first (interviews and FGDs), then continued with quantitative methods. In this study, the sampling technique used was multi-stage random sampling in 7 selected sub-districts.Results: The variable perception of vulnerability with a good perception of vulnerability have a 2.64 times greater risk of carrying out an VIA examination. In multivariate modeling, knowledge is the most dominant variable because it has the largest POR of 6.3, which means the N-Gain value is in the medium category with a fairly effective interpretation of the effectiveness of the module, namely an increase in knowledge of 63.86% after being given education using the OMPHE-VIA module. Conclusion: Based on the implementation of the OMPHE-VIA module, it was found that there was an increase in the average value of knowledge, before and after the education was given. It was also found that there was a significant difference in knowledge, and husband's support, construct between before and after the education was given using the OMPHE-VIA module.

Abstract

Penerapan Artificial Intelligence (AI) pada dunia kedokteran diharapkan memiliki dampak yang signifikan pada praktik klinis. Perusahaan-perusahaan kesehatan dan kelompok akademis di seluruh dunia telah mengakui dan mempelajari lebih dalam potensi teknologi AI untuk meningkatkan bidang kesehatan, dan banyak tim peneliti sekarang berlomba untuk menghasilkan sistem AI agar semakin bertambah, atau bahkan untuk mengganti posisi dokter. AI berpotensi mengungguli kemampuan manusia, namun orang-orang masih enggan menggunakannya. Penelitian ini merupakan Systematic Review dengan menggunakan metode Preferred Reporting Item for Systematic Reviews and Meta-Analyses PRISMA. Pencarian artikel dilakukan dengan menggunakan beberapa database ProQuest dan Scopus dengan kata kunci yaitu artificial intelligence, medical artificial inteligence, dan artificial intelligence in health care. Semua artikel yang diterbitkan dalam bahasa Inggris dari tahun 2017 sampai 2022. Potensi AI untuk berkontribusi dalam mendukung proses diagnostik hingga menggantikan peran dokter semakin besar. Tidak hanya itu, AI juga berpeluang untuk memberikan pengalaman kemudahan dalam akses seperti rekam medis elektronik, pemberian rekomendasi obat, sebagai peringatan untuk keamanan pasien jatuh, dan lain sebagainya. Banyak sekali peran AI dalam perkembangan teknologi di bidang kesehatan, cepat atau lambat semua potensi yang dimiliki AI akan memberikan dampak baik tentunya dengan perbaikan-perbaikan yang agresif dan dukungan dari semua pemangku kepentingan dan juga para praktisi.

Abstract

Background: : Cancer is one of the main causes of death and increased morbidity in every country in the world. According to Global Cancer Observatory (GLOBOCAN) 2020 data, it is estimated that 19.3 million new cancer cases and nearly 10.0 million cancer deaths occurred in the world in 2020. Based on the same data, it is estimated that 396,914 new cancer cases and nearly 234,511 cancer deaths are estimated to occur in Indonesia. Dharmais National Cancer Hospital (DNCH) as a national cancer center is a miniature of cancer data in Indonesia. For this reason, researchers want to compare whether the data on the number and types of cancer in DNCH are the same as those in GLOBOCAN 2020.Methods: This research is a descriptive analysis with secondary data collection methods taken from medical records. The sampling technique was total sampling, univariate data analysis, namely new cases of cancer patients for the January-December 2021 period which are 2,382 cases.Results: The results showed that the same order in both data was only found in the first, second, and seventh order, namely breast cancer, cervical cancer, and non-Hodgkin’s lymphoma, respectively. However, the percentage of new cases of breast cancer in RSKD is much higher (2.5-folds) than in GLOBOCAN.Conclusion: There are several differences between the data on new cancer cases in the two data, which can be caused because DNCH has complete facilities, both for diagnostic and curative purposes, and examinations such as IHC and bone scans are only available at certain hospitals and one of them is DNCH.

Abstract

Background: Patient safety (PS) is a global priority for achieving quality healthcare. Although patient engagement (PE) is a crucial risk-reduction strategy, research on this subject in Indonesia is scarce. Aim: This study aims to explore healthcare recipients’ (HCRs’) perspectives and their potential role in PS. Methods: Exploratory qualitative research was conducted with in-depth interviews (IDIs). This study purposively selected fourteen patients and fifteen caretakers in chronic wards. Content analysis was subjected to the IDIs data that has been transcribed verbatim. Results: HCRs showed inadequate knowledge, perception, and willingness to engage in patient safety. Four themes were identified from data analysis: (1) complexity barriers to PE Implementation; (2) enabling factors for PE; (3) HCRs' expectations; and (4) existing and potential HCRs' roles in PS. HCRs' roles were still limited to communication, positive attitude and behavior, aided healthcare process, and error prevention. Conclusion: The limited roles of HCRs resulted from their unreadiness to participate more in PS. For patients to be engaged in safety measures, it was essential to improve the ability of patients and caregivers and eliminate obstacles encountered by healthcare professionals and the broader health system.
