Abstract

Background: Dissatisfaction, additional work, and desired compensation are related to the needs and perceptions of civil servant health workers to increase their income. However, the probability and influencing factors regarding the income adequacy of civil servant health workers in Indonesia are still unknown. Aims: to find out the factors related to the income adequacy of health workers. Methods: This observational study with a cross-sectional design uses data from the Employment Research in the Health Sector (RISNAKES) 2017 in Public Health Centers in Indonesia. Probit analysis was conducted to assess the probability of income adequacy. Results: A total of 963 respondents were analyzed. The study found that changes in each variable, including income, official residence facilities, training facilities/ seminars/ training, motivation, and job satisfaction of a unit, would increase the probability of income adequacy ranging from 2% - 81%, except for the age variable. By referring to high job satisfaction of > 75% and taking into account the probability of 80%, the presumed adequate income is no less than IDR 28,800,000. Conclusion: The probability of the monthly income adequacy of civil servants in the health sector is influenced by job satisfaction, ability to save, official residence facilities, facilities to attend seminars, age, and income. Increasing and reformulating the amount of income must be done to optimize the performance and productivity of health workers. Keywords: health workers, income adequacy, public health center, saving

Abstract

ABSTRACTBackground: Laryngopharyngeal reflux (LPR) is a condition caused by retrograde flow of gastric contents to the larynx, pharynx, trachea, and bronchus, which can affect patients’ quality of life (QOL). Purpose: To evaluate the effect of proton pump–inhibitor (PPI) therapy on the quality of life of patients with laryngopharyngeal reflux (LPR). Methods: This randomized controlled trial study involved 52 patients with dysphagia. The short-form REFLUX-QUAL® questionnaire was used to assess QOL. The patients were given lansoprazole twice daily as early diagnostic method as well as a therapy for LPR. If improvement found, PPI therapy was continued for two months. Afterwards patients was randomly divided into continuous therapy group (n = 25) or an on-demand therapy group (n = 27). Results: There was no significant difference in gender, age group, or economic level among subjects in both groups. Treatment with PPIs both continuously and on-demand significantly improved patients’ QOL from 44.83 ± 19.11 to 83.37 ± 11.51 (p <0.001). Significant improvements occurred in almost all of eight QOL domains as compared at the start of and six months into PPI therapy (p <0.001); discomfort remained the exception as patients with LPR must continue to avoid certain foods, (p = 0.233). However, there was no significant difference between the continuous and on-demand therapy groups concerning improvements in the quality of life (p = 0.281). Conclusion: Treatment with PPIs either continuously or on-demand for six months significantly improved patients’ quality of life, with no significant difference between the two treatment groups.Keywords: proton pump inhibitor, PPI, quality of life, laryngopharyngeal reflux, LPRABSTRAKLatar belakang: Refluks laringofaring (RLF) adalah kondisi terjadinya aliran balik isi lambung ke laring, faring, trakea, dan bronkus yang dapat mempengaruhi kualitas hidup pasien. Terapi empiris Proton Pump Inhibitor (PPI) merupakan pendekatan diagnosis dan metode terapi untuk RLF. Tujuan: Penelitian ini bertujuan untuk mengevaluasi efektivitas pengobatan PPI terhadap kualitas hidup pasien RLF Metode: Penelitian secara uji acak terkendali ini melibatkan 52 pasien dengan keluhan disfagia. Digunakan Reflux-Qual Short form (RQS) untuk menilai kualitas hidup subjek. Pasien diberikan lansoprazole dua kali sehari sebagai diagnosis dini dan terapi untuk refluks. Apabila terdapat perbaikan, PPI diteruskan selama 2 bulan, kemudian pasien dibagi menjadi 2 grup secara acak. Kelompok pasien yang mendapatkan terapi secara kontinu terdiri dari 25 pasien, sedangkan kelompok terapi on-demand terdiri dari 27 pasien. Hasil: Tidak terdapat perbedaan signifikan pada jenis kelamin, usia, tingkat ekonomi antara pasien pada kelompok terapi kontinu dan on-demand. Pengobatan PPI secara kontinu dan on-demand secara signifikan meningkatkan kualitas hidup pasien dari 44,83 ± 19,11 menjadi 83,37 ± 11,51 (p<0,001). Selain itu, terdapat perbaikan yang signifikan pada hampir semua dari delapan domain kualitas hidup pada bulan keenam terapi PPI dibandingk an awal terapi, kecuali pada domain ketidaknyamanan pasien karena pasien harus menghindari makanan tertentu (p = 0,233). Namun, tidak ditemukan perbedaan yang signifikan pada kelompok terapi secara kontinu dan on-demand pada peningkatan kualitas hidup (p = 0,281). Kesimpulan: Pengobatan PPI secara kontinu dan on-demand selama 6 bulan secara signifikan meningkatkan kualitas hidup pasien RLF. Namun, tidak ada perbedaan yang signifikan antara kedua kelompok tersebut terhadap peningkatan kualitas hidup.Kata kunci: penghambat pompa proton, PPI, refluks laringfaring, refleks laringofaring, kualitas hidup

Abstract

Patient satisfaction depends on the quality of service and the Emergency Department (IGD) is one of the components of the hospital that is most concerned about the quality of service. The purpose of writing this article is to analyze the impact of patient satisfaction on the quality of emergency room services. The research method used is qualitative method, this study is descriptive analytic, this is to describe and analyze the system of Islamic financial institutions with the system of conventional financial institutions. The results of this study concluded that the impact of patient satisfaction on the quality of emergency room services is to increase the popularity of hospital emergency room services, increase patient loyalty, become the best means of promotion and improve the reputation of hospital service quality. And the satisfaction of health service users can be concluded as the difference in the performance of health service institutions with patient expectations, patient satisfaction can be formulated by, 1) Performance < Expectation which means patients feel less satisfied with the services received, 2) Performance = Expectation, meaning patients are satisfied with the services received, 3) Performance > Expectation, meaning patients feel very satisfied with the services received

Abstract

Program Jaminan Kesehatan Nasional (JKN) melalui BPJS Kesehatan memberikan akses pelayanan kesehatan bagi seluruh masyarakat Indonesia, salah satu layanan yang paling banyak digunakan adalah layanan rawat inap. Tujuan penelitian ini adalah untuk menganalisis manajemen kendali biaya pelayanan rawat inap pasien BPJS Kesehatan di rumah sakit. Penelitian ini menggunakan metode penelitian kualitatif. Teknik pengumpulan data pada penelitian ini adalah studi literatur. Data yang telah terkumpul kemudian dianalisis dalam tiga tahapan yakni reduksi data, penyajian data dan penarikan kesimpulan. Hasil penelitian menunjukan bahwa manajemen kendali biaya pelayanan rawat inap pasien BPJS Kesehatan merupakan usaha bersama antara tiga komponen penyedia layanan kesehatan yaitu Kemenkes, BPJS, dan fasilitas kesehatan. Prinsip yang digunakan oleh BPJS Kesehatan dalam pembiayaan kesehatan adalah kendali mutu dan kendali biaya yang bertujuan agar rumah sakit memiliki kualitas yang tinggi dengan biaya yang efektif. Manajemen kendali biaya pelayanan rawat inap pasien BPJS Kesehatan meliputi kendali mutu, kendali biaya, komitmen kesehatan dan pengawasan. Manajemen kendali biaya pelayanan rawat inap pasien BPJS Kesejahteraan sangat penting untuk memastikan bahwa sistem pembiayaan kesehatan BPJS Kesehatan berjalan dengan efektif dan efisien, serta memastikan kualitas pelayanan kesehatan yang tinggi

Abstract

Digital transformation in healthcare is needed to continuously enhance healthcare services in Indonesia, but all the changes happened especially after the Covid-19 pandemic has made many healthcare facilities to face difficulties on prioritizing development and their operational services. The Government thru the Ministry of Health have already defined the direction of where the transformation is expected to be, but to fulfill that expectation will still be a huge challenge for all healthcare facilities, both government and private. This research is intended to bring perspective on the situation and challenges for private healthcare facilities on facing digital transformation. The research method is conducted using cross sectional survey and literature study to several private hospitals. From the results it can be seen that there are various levels of readiness between the hospitals in order to face these challenges, which in some cases still supported by skepticism of the stakeholders on the need for digital transformation. But, at the end it can be concluded that digital transformation is needed and eventually it will come, therefore all hospitals must be ready to face the transformation and changes. All of this are happening for the good of healthcare in Indonesia

Abstract

This paper analyzed the Policy Implementation of Community Health Center (CHC) as Report Obligation Recipient Institution for Narcotic Addicts in Jakarta Province in 2014. The design of the study employed a mixed methods approach of Sequential Explanatory. The implementation analysis employed the model developed by Donald Van Meter and Carl Van Horn. Within the model, policy implementation was associated with performance. The relationship was influenced by six variables: standards and targets, resources, communication among implementing agencies, implementing agency characteristics, attitudes of the implementers, as well as social, economic, and political factors. We found that variations of those six variables influenced policy performance linearly.

Abstract

The number and types of hospitals in Jakarta are constantly increasing, causing competition for customers or patients. Hospitals owned by the DKI Jakarta provincial government face challenges in providing specialist doctor services with the best performance for the community, this is closely related to the rewards offered to specialist doctors. One of the regional government's efforts to improve the performance of specialist doctors is by issuing regulations on remuneration for hospitals. This research aims to determine the relationship between remuneration policy and the performance of specialist doctors at Kramat Jati Regional Hospital. The research was conducted using a case study method with a qualitative approach. The research was conducted on 11 specialist doctors using questionnaires and in-depth interviews with informants. The results of the study found that the implementation of the remuneration policy at Kramat Jati Regional Hospital increased the average income of specialist doctors by 42.9% of the income of specialist doctors before the implementation of remuneration. This study also found that the implementation of remuneration policies at Kramat Jati Regional Hospital was related to the quantity performance of specialist doctors.

Abstract

The quality of transportation in big cities, especially in DKI Jakarta faces many challenges, such as high population creating traffic congestion, air pollution, and access to public transportation. Currently, DKI Jakarta has a population of 10.67 million in 2022 and is ranked 38 out of 60 countries with good public transportation, the development of health-based transportation that is effective in reducing health impacts, infrastructure and urban design are important in supporting high mobility. An important aspect of improving transportation in big cities is the development of sustainable public transportation systems. Using the PRISMA extension for Scoping Review (PRISMA-ScR) guidelines, 93 articles published between 2000 and 2022 met the inclusion criteria looking at Health-based transport, Urbanization, and Policy. Health-based transportation is very important to be addressed by encouraging sustainable development, green transportation, good urban planning to data-driven policy making, utilizing data and technology, can contribute to public health and a healthier and more livable urban environment, this still requires collaborative efforts from the government, policy makers, and the community to create an efficient and sustainable transportation system in DKI Jakarta

Abstract

Dalam era digital saat ini, menjaga kerahasiaan dan keamanan sistem informasi merupakan sebuah tantangan tersendiri. Industri terutama kesehatan saat ini melakukan investasi besar dalam teknologi informasi. Masih banyak kasus yang terjadi di lapangan, terutama terkait perilaku kesadaran pengguna informasi kesehatan. Tenaga medis menjadi salah satu faktor besar dalam insiden keamanan yang terjadi di layanan kesehatan. Saat ini belum banyak yang membahas mengenai faktor-faktor individu yang mempengaruhi perilaku kesadaran keamanan sistem informasi terutama di layanan kesehatan. Oleh karena itu, tujuan penulis melakukan penelitian ini adalah untuk memberikan tinjauan mengenai perilaku kesadaran kemanan sistem informasi pada sumber daya manusia (SDM) kesehatan di layanan kesehatan, mengumpulkan metode dan faktor penentu yang dapat meningkatkan perilaku kesadaran. Metode penelitian dengan kuantitatif yang disajikan dalam bentuk Narrative Litterature Review, diambil dari jurnal ilmiah untuk ditinjau. Hasil penelitian menunjukan pengaruh dari pelatihan yang telah diberikan kepada tenaga medis, lama pengalaman kerja serta aturan dan hukuman berlaku yang ada di layanan kesehatan terhadap perilaku keamanan sistem informasi pada SDM kesehatan di layanan kesehatan.

Abstract

Elderly individuals are the most vulnerable group affected by Covid-19. Anxiety among the elderly regarding Covid-19 is an important mental health factor that needs to be addressed. There are obstacles to meeting the basic needs of the elderly, so they require family assistance. The experience of families in caring for elderly individuals exposed to Covid-19 is very useful for obtaining information related to their care. The purpose of this study is to gain in-depth information about the experiences of family members when accompanying elderly individuals exposed to Covid-19 during isolation in hospitals. This research is qualitative, with a phenomenological approach. Participants consisted of 11 family members selected through purposive sampling, and in-depth interviews were conducted for data collection using five open-ended questions recorded via audio. Data analysis employed content analysis. The findings revealed seven themes: 1) Knowledge of caregivers about the purpose of assisting elderly Covid-19 patients during hospital isolation, 2) Family feelings when their parent is diagnosed with Covid-19, 3) The condition of the elderly after being diagnosed with Covid-19, 4) Activities of caregivers while accompanying elderly Covid-19 patients, 5) Moments when nurses motivate elderly patients with Covid-19, 6) Challenges faced by caregivers during the isolation of the elderly in hospitals, 7) Expectations for caregivers to assist elderly Covid-19 patients during hospital isolation. It is crucial to explore the experiences of families in supporting elderly Covid-19 patients during hospital isolation to provide psychological comfort, which can accelerate their recovery. The presence of family support for elderly Covid-19 patients not only alleviates anxiety and fear but also fosters hope and motivation when they learn of their positive Covid-19 status, especially if they have underlying health conditions. Positive and impactful activities by family members during isolation include continuous communication with the elderly, providing motivation for their recovery, monitoring their nutritional intake, praying, watching shows together, and communicating through phone calls or video calls with other family members at home.
