Abstract

The level of public health is determined by the quality of public health services, as stated by WHO, the quantity of health personnel contributing as much as 80% of the success in health services. According to Undang-Undang No 36/2014 regarding Health Worker article1, health works improvement is directed to improve the quality and career of health worker. One of the current problems for health worker is the career path / career improvement. There is still no regulation regarding health worker’s career development aside from health civil worker, especially for pharmacist, that also suffer from the lack of synchronization of the career development program between the one from the government and the one from the pharmacist organization. Qualitative method is used in the research of the process of formulating the related regulations and stakeholder analysis. Primary data is acquired by doing in-depth interview with chosen informant, and literature research is used as secondary data. The result shows that all the interviewed stakeholders are supporting and committing in the Pharmacist’s career development program. Pharmacist organization along with the government are of two of the most important actors regarding the implementation of this program. The competency certification and specialization education program are expected to be the right options for the pharmacist career development program. On the other side, the interaction, advocacy and communication process between the stakeholders in pharmacist organization and the government still need improvement, and there are many of the pharmacist itself that not aware of the career development program. Therefore improvement in stakeholder’s coordination and communication is a must.

Abstract

Rumah Sakit Universitas Indonesia sudah bekerja sama dengan BPJS Kesehatan cabang Depok sejak akhir tahun 2020. Selama bekerja sama dengan BPJS Kesehatan jumlah pasien yang dirawat semakin meningkat, terutama di tahun 2023 jumlah pasien rawat inap di akhir tahun meningkat 71% dibandingkan dengan di awal tahun 2023, dan nilai BOR (Bed Occupancy Ratio) mencapai 76,2%. Dengan meningkatnya jumlah pasien rawat inap JKN ini memerlukan adanya strategi untuk melakukan kendali mutu kendali biaya agar pelayanan yang diberikan bisa efektif dan efisien. Metode penelitian ini menggunakan metode kualitatif studi kasus dengan analisa data primer dari data klaim JKN tahun 2023. Berdasarkan data klaim tahun 2023, total pasien rawat inap JKN di RS Universitas Indonesia mencapai 9.819 pasien dengan selisih antara tarif INA CBGs dengan tarif RS sebesar 23%. Kasus rawat inap terbanyak yaitu Penyakit Dalam, Anak, Bedah Mulut, Syaraf, dan Jantung. Dari kelima kasus terbanyak tersebut dianalisa berdasarkan komponen tarifnya, dan yang menduduki komposisi tarif terbanyak dari kelima kasus tersebut adalah kamar akomodasi disusul kemudian obat dan bmhp. Dengan tingginya tarif kamar dan penggunaan obat serta BMHP, maka strategi untuk mengefisienkan biaya adalah dengan memperpendek lama rawat (LOS) dan efisiensi penggunaan obat serta BMHP.

Abstract

Butterfly pea flower (Clitoria ternatea) may serve as an alternative anti-dandruff treatment; however, its effects on Malassezia spp. remain unexplored. The aim of this study was to explore the effects of C. ternatea as an herbal-based anti-dandruff treatment on Malassezia spp. DNA expression, plakoglobin levels, IL-8 levels, sebum levels, dandruff severity scores, adverse effects, and patient satisfaction. An experimental study with a pretest-posttest control design was conducted at Outpatient Clinic of Dermatology and Venereology, Arifin Achmad Hospital, Pekanbaru, Indonesia, from November 2023 to January 2024. The flower of C. ternatea was used to formulate the shampoo. The study involved 70 female patients aged 18–25 with dandruff, who were divided into two groups: (a) experimental group using 20% C. ternatea shampoo; and (b) control group using 2% ketoconazole shampoo. The present study found that 2% ketoconazole shampoo significantly reduced Malassezia spp. DNA expression compared to 20% C. ternatea shampooo (Clitoria ternatea: ΔCq=1.76±3.18; ketoconazole: ΔCq=3.77±2.90; p=0.008). No significant difference was observed in plakoglobin levels (C. ternatea: ΔCq=1.98±3.63; ketoconazole: ΔCq=2.50±2.36; p=0.427) or IL-8 levels (C. ternatea: ΔCq=3.46±4.00; ketoconazole: ΔCq=4.16 ± 3.62; p=0.459). C. ternatea significantly reduced sebum levels more than ketoconazole (C. ternatea: 1.16±0.98%; ketoconazole: 0.22±0.38%; p<0.001). Dandruff scores and patient satisfaction were similar for both shampoos (p=0.115 and p=0.336, respectively). Adverse effects were more common in the 2% ketoconazole shampoo group, affecting 21.2% of the patients. In conclusion, 2% ketoconazole shampoo is more effective in reducing Malassezia spp. DNA expression, while 20% C. ternatea shampoo offers better sebum control. Both shampoos are similarly effective in ameliorating dandruff severity and are well-tolerated, with fewer adverse effects reported for C. ternatea.

Abstract

The advent of the Fourth Industrial Revolution has catalyzed transformative changes in the healthcare sector, highlighting the need to optimize Artificial Intelligence (AI) to improve patient satisfaction. This research examines the role of AI in revolutionizing the healthcare market and its potential to improve patient outcomes. Using a systematic literature review, we collected and analyzed studies published from 2017 to 2022 from various databases such as ProQuest, Science Direct, PubMed, CINAHL, and Scopus, focusing on AI applications in healthcare and its impact on patient satisfaction. Inclusion criteria for the initial screening were articles that were freely available in full text, had open access, and were published in English or Indonesian, as well as ensuring no duplication in the records found. Significant findings were synthesized through thematic and descriptive analysis to ascertain the efficacy of AI in patient care. Of the 25 articles relevant for further evaluation, 12 studies met the inclusion criteria and were included in the qualitative analysis. The chosen methodology allowed the authors to conduct a comprehensive and systematic review. A practical suggestion that can be drawn from these findings is the importance of investing in the development of artificial intelligence (AI) in healthcare. This requires adequate training for staff to understand and use these technologies effectively. In addition, healthcare institutions should ensure that the use of AI remains compliant with applicable privacy and ethical regulations. Collaboration between various parties such as healthcare institutions, technology providers, and researchers is also necessary to accelerate innovation and knowledge exchange. By utilizing real-time data analysis powered by AI, healthcare can be improved, and governments can support innovation in AI by providing incentives, supportive regulations, and adequate research funding.  Keywords: Artificial Intelligence, Digital Transformation, Healthcare Revolution, Patient Satisfaction, Service Optimization

Abstract

Hospital X is a private hospital that has executive polyclinic services. Medical record data shows that in the last four years, the number of visits from the executive polyclinic unit has never reached the target set. This research is to prepare the strategic plan for improving the executive polyclinic of Hospital X's performance for 2024-2027. The types of data in this research are primary data and secondary data. Primary data is qualitative data from in-depth interviews, Consensus Decision Making Group (CDMG) with informants, and direct observation. Secondary data consists of quantitative data and qualitative data. Strategy implementation will be described as Key Performance Indicators (KPI) using the Balanced Scorecard (BSC) approach. The Matching Stage results in the IE matrix show the position of the hospital in cell 1 in the IE matrix, namely grow and build. The QSPM results combined with existing theory in determining priority strategies produce the following strategy sequence: optimizing promotion, optimizing facilities and facilities, optimizing products and services, optimizing technology, optimizing human resources, and optimizing service excellence. The conclusion is that the strategic plan, made complete with indicators according to the BSC and targets, will be able to serve as a guide for Hospital X in realizing executive polyclinic services as superior services.

Abstract

Background: Early Initiation of Breastfeeding (EIB) is a crucial practice that supports newborn health and the success of exclusive breastfeeding. Despite its known benefits, the implementation of EIB faces several challenges, particularly in hospital settings. Objective: This scoping review aims to identify and map the factors that delay EIB in hospitals, providing insight into the barriers that hinder its effective practice. Method: A comprehensive search was conducted across four databases—Google Scholar, Semantic Scholar, Elsevier, and PubMed—using terms related to EIB obstacles. Results: Initially, 1,276 articles were identified, and after applying the PRISMA method for screening, 14 articles were included in final analysis. The review highlights several obstacles to EIB, including cognitive factors like mothers’ lack of knowledge and experience, operational challenges such as high workloads and limited resources among health staff, and postpartum factors, particularly discomfort following cesarean sections. These issues were found to be consistent across various settings. Conclusions: To address these barriers, the review emphasizes the need for improved education for mothers and healthcare providers, stronger hospital policies supporting EIB, and better resource availability, such as counseling materials. Implementing these strategies is vital for optimizing EIB practices and achieving better health outcomes for mothers and newborns while supporting global breastfeeding initiatives. Abstract

Kualitas pelayanan kesehatan di rumah sakit merupakan faktor krusial yang mempengaruhi tingkat kepuasan dan loyalitas pasien terhadap institusi tersebut. Penelitian ini bertujuan untuk menganalisis elemen-elemen yang mempengaruhi kepuasan pasien di rumah sakit dengan mengaplikasikan pendekatan SERVQUAL, yang digunakan untuk mengidentifikasi perbedaan antara harapan dan pengalaman pasien dalam lima dimensi: tangible, reliability, responsiveness, assurance, dan empathy. Metode yang diterapkan dalam penelitian ini adalah systematic review, mengacu pada pedoman PRISMA, dengan melakukan pencarian literatur di berbagai database seperti ProQuest, ScienceDirect, Scopus, dan Google Scholar yang mencakup periode 2014 hingga 2024. Temuan dari tinjauan literatur menunjukkan bahwa dimensi responsiveness dan empathy memiliki pengaruh yang paling signifikan terhadap kepuasan pasien, diikuti oleh dimensi tangible, reliability, dan assurance. Layanan yang konsisten, cepat tanggap, aman, dan memperhatikan kebutuhan individu pasien terbukti meningkatkan kepercayaan serta loyalitas mereka, sekaligus memperkuat reputasi rumah sakit. Penelitian ini menekankan pentingnya evaluasi layanan secara terus-menerus untuk memenuhi harapan pasien dan meningkatkan daya saing rumah sakit. Hasil temuan ini diharapkan dapat memberikan kontribusi dalam pengembangan kebijakan yang lebih tepat dalam meningkatkan kualitas pelayanan kesehatan di rumah sakit.

Abstract

Sustainable urban development, healthy cities and rapid urban growth have become a major focus in Indonesia. This research aims to identify the key factors needed to achieve optimal Urban Health in Indonesia. Currently, DKI Jakarta Province is the largest metropolitan city in Indonesia, but in 2022 DKI Jakarta will be ranked 238th in the world and 48th in Asia regarding the Quality of Life Index. A symbol of success and progress in the country of Indonesia, there are 98 cities spread across all provinces, but only 8 cities are included in the metropolitan category. This research uses the PRISMA extension for Scoping Review (PRISMA-ScR) guidelines, 93 articles published between 2000 and 2022 meet the inclusion criteria that look at Urban Health for city development. In an effort to build a healthy city, Urban Health needs to be implemented using several key factors that encourage good urban health. The policy approach in Indonesia must be to improve the quality and accessibility of health services which are important factors in the effectiveness of urban health services, so as to achieve good urban health. well in Indonesia requires a holistic approach that includes green energy, a clean environment, high levels of physical activity and good stress management, adequate health services, good nutrition and equitable health.

Abstract

Coronary heart disease (CHD) remains a leading cause of mortality in Indonesia, and coronary artery bypass graft (CABG) surgery is frequently employed to manage arterial blockages. Despite its efficacy, the recurrence of heart disease post-surgery is a significant concern, highlighting the need for a deeper understanding of its influencing factors. The aim of this study was to examine the factors associated with the incidence of heart disease recurrence after coronary bypass surgery. This study employed a prospective observational design, analyzing hospital claim data from Indonesia's Social Security Agency for Health, known as Badan Penyelenggara Jaminan Sosial (BPJS) Kesehatan, from 2017 to 2022. The analysis included 5,947 patients who survived CABG surgery. Multivariable logistic regression was utilized to assess the relationship between patient demographics, comorbidities, socioeconomic status, and compliance with follow-up visits, as well as their impact on the recurrence of cardiovascular disease. The study found that 24.1% of patients experienced hospitalization recurrence. Patients with irregular follow-ups were less likely to experience recurrence (adjusted odds ratio (AOR): 0.63; 95%CI: 0.51–0.78). Other significant risk factors for recurrence included being self-employed (AOR: 2.09; 95%CI: 1.72–2.55), having comorbid conditions such as disorders of fluid, electrolyte, and acid-base balance (AOR: 3.55; 95%CI: 2.97–4.24), and experiencing cerebral infarction or stroke (AOR: 10.85; 95%CI: 8.24–14.29). In contrast, older age (AOR: 0.89; 95%CI: 0.88–0.91) and the presence of non-insulin-dependent diabetes mellitus (AOR: 0.35; 95%CI: 0.29–0.42) were associated with a lower risk of recurrence. Sex did not significantly influence the risk of recurrence (AOR: 1.18; 95%CI: 0.86–1.62). In conclusion, the study indicates a considerable rate of cardiovascular disease recurrence post-CABG in Indonesia, highlighting several key risk factors. Tailored postoperative management and strict adherence to follow-up protocols are essential for mitigating recurrence. These findings offer crucial insights for improving post-CABG health management strategies in Indonesia.

Abstract

Successfully treating tuberculosis (TB) could significantly help reduce its spread. The aim of this study was to identify factors associated with successful TB treatment. A retrospective study was conducted in West Nusa Tenggara, Indonesia, using data from the National TB Information System (SITB) covering patients from January 1 to December 31, 2022. Patients were classified into two groups: those with successful treatment outcomes (cured or completed treatment) and those with unsuccessful outcomes (including treatment failure, loss to follow-up, or death). Univariate and multivariate logistic regression analyses were performed to identify factors associated with treatment outcomes, providing odds ratios (OR) and adjusted odds ratios (aOR) with 95% confidence intervals (CIs). A total of 2,225 TB patients (1,382 males and 843 females) were included in the study. Of these, 2,048 (92.1%) achieved successful treatment outcomes. Univariate analysis indicated that older age (OR: 0.47; 95%CI: 0.28–0.78) and a high number of AFB in sputum smears (OR: 0.23; 95%CI: 0.09–0.66) were associated with a higher likelihood of unsuccessful TB treatment. In contrast, having no HIV infection (OR: 13.44; 95%CI: 6.22–29.08), clinical TB cases (diagnosed clinically rather than bacteriologically) (OR: 1.50; 95%CI: 1.04–2.20) and longer duration of TB treatments were associated with successful treatment outcomes. Multivariate analysis suggested that the TB treatment durations of 4–6 months (aOR: 1256.95; 95%CI: 431.89–3658.19) and 7–12 months (aOR: 575.5; 95%CI: 99.1–3342.06) were associated with a significantly higher likelihood of success compared to durations of 0–3 months. In conclusion, this study highlights that a minimum treatment duration of three months was crucial for increasing the likelihood of successful TB treatment. These findings emphasize the importance of comprehensive support programs to ensure adherence to treatment guidelines and improve outcomes.
