Abstract

Background: Early Initiation of Breastfeeding (EIB) is a crucial practice that supports newborn health and the success of exclusive breastfeeding. Despite its known benefits, the implementation of EIB faces several challenges, particularly in hospital settings. Objective: This scoping review aims to identify and map the factors that delay EIB in hospitals, providing insight into the barriers that hinder its effective practice. Method: A comprehensive search was conducted across four databases—Google Scholar, Semantic Scholar, Elsevier, and PubMed—using terms related to EIB obstacles. Results: Initially, 1,276 articles were identified, and after applying the PRISMA method for screening, 14 articles were included in final analysis. The review highlights several obstacles to EIB, including cognitive factors like mothers’ lack of knowledge and experience, operational challenges such as high workloads and limited resources among health staff, and postpartum factors, particularly discomfort following cesarean sections. These issues were found to be consistent across various settings. Conclusions: To address these barriers, the review emphasizes the need for improved education for mothers and healthcare providers, stronger hospital policies supporting EIB, and better resource availability, such as counseling materials. Implementing these strategies is vital for optimizing EIB practices and achieving better health outcomes for mothers and newborns while supporting global breastfeeding initiatives. Abstract

Kualitas pelayanan kesehatan di rumah sakit merupakan faktor krusial yang mempengaruhi tingkat kepuasan dan loyalitas pasien terhadap institusi tersebut. Penelitian ini bertujuan untuk menganalisis elemen-elemen yang mempengaruhi kepuasan pasien di rumah sakit dengan mengaplikasikan pendekatan SERVQUAL, yang digunakan untuk mengidentifikasi perbedaan antara harapan dan pengalaman pasien dalam lima dimensi: tangible, reliability, responsiveness, assurance, dan empathy. Metode yang diterapkan dalam penelitian ini adalah systematic review, mengacu pada pedoman PRISMA, dengan melakukan pencarian literatur di berbagai database seperti ProQuest, ScienceDirect, Scopus, dan Google Scholar yang mencakup periode 2014 hingga 2024. Temuan dari tinjauan literatur menunjukkan bahwa dimensi responsiveness dan empathy memiliki pengaruh yang paling signifikan terhadap kepuasan pasien, diikuti oleh dimensi tangible, reliability, dan assurance. Layanan yang konsisten, cepat tanggap, aman, dan memperhatikan kebutuhan individu pasien terbukti meningkatkan kepercayaan serta loyalitas mereka, sekaligus memperkuat reputasi rumah sakit. Penelitian ini menekankan pentingnya evaluasi layanan secara terus-menerus untuk memenuhi harapan pasien dan meningkatkan daya saing rumah sakit. Hasil temuan ini diharapkan dapat memberikan kontribusi dalam pengembangan kebijakan yang lebih tepat dalam meningkatkan kualitas pelayanan kesehatan di rumah sakit.

Abstract

Sustainable urban development, healthy cities and rapid urban growth have become a major focus in Indonesia. This research aims to identify the key factors needed to achieve optimal Urban Health in Indonesia. Currently, DKI Jakarta Province is the largest metropolitan city in Indonesia, but in 2022 DKI Jakarta will be ranked 238th in the world and 48th in Asia regarding the Quality of Life Index. A symbol of success and progress in the country of Indonesia, there are 98 cities spread across all provinces, but only 8 cities are included in the metropolitan category. This research uses the PRISMA extension for Scoping Review (PRISMA-ScR) guidelines, 93 articles published between 2000 and 2022 meet the inclusion criteria that look at Urban Health for city development. In an effort to build a healthy city, Urban Health needs to be implemented using several key factors that encourage good urban health. The policy approach in Indonesia must be to improve the quality and accessibility of health services which are important factors in the effectiveness of urban health services, so as to achieve good urban health. well in Indonesia requires a holistic approach that includes green energy, a clean environment, high levels of physical activity and good stress management, adequate health services, good nutrition and equitable health.

Abstract

Coronary heart disease (CHD) remains a leading cause of mortality in Indonesia, and coronary artery bypass graft (CABG) surgery is frequently employed to manage arterial blockages. Despite its efficacy, the recurrence of heart disease post-surgery is a significant concern, highlighting the need for a deeper understanding of its influencing factors. The aim of this study was to examine the factors associated with the incidence of heart disease recurrence after coronary bypass surgery. This study employed a prospective observational design, analyzing hospital claim data from Indonesia's Social Security Agency for Health, known as Badan Penyelenggara Jaminan Sosial (BPJS) Kesehatan, from 2017 to 2022. The analysis included 5,947 patients who survived CABG surgery. Multivariable logistic regression was utilized to assess the relationship between patient demographics, comorbidities, socioeconomic status, and compliance with follow-up visits, as well as their impact on the recurrence of cardiovascular disease. The study found that 24.1% of patients experienced hospitalization recurrence. Patients with irregular follow-ups were less likely to experience recurrence (adjusted odds ratio (AOR): 0.63; 95%CI: 0.51–0.78). Other significant risk factors for recurrence included being self-employed (AOR: 2.09; 95%CI: 1.72–2.55), having comorbid conditions such as disorders of fluid, electrolyte, and acid-base balance (AOR: 3.55; 95%CI: 2.97–4.24), and experiencing cerebral infarction or stroke (AOR: 10.85; 95%CI: 8.24–14.29). In contrast, older age (AOR: 0.89; 95%CI: 0.88–0.91) and the presence of non-insulin-dependent diabetes mellitus (AOR: 0.35; 95%CI: 0.29–0.42) were associated with a lower risk of recurrence. Sex did not significantly influence the risk of recurrence (AOR: 1.18; 95%CI: 0.86–1.62). In conclusion, the study indicates a considerable rate of cardiovascular disease recurrence post-CABG in Indonesia, highlighting several key risk factors. Tailored postoperative management and strict adherence to follow-up protocols are essential for mitigating recurrence. These findings offer crucial insights for improving post-CABG health management strategies in Indonesia.

Abstract

Successfully treating tuberculosis (TB) could significantly help reduce its spread. The aim of this study was to identify factors associated with successful TB treatment. A retrospective study was conducted in West Nusa Tenggara, Indonesia, using data from the National TB Information System (SITB) covering patients from January 1 to December 31, 2022. Patients were classified into two groups: those with successful treatment outcomes (cured or completed treatment) and those with unsuccessful outcomes (including treatment failure, loss to follow-up, or death). Univariate and multivariate logistic regression analyses were performed to identify factors associated with treatment outcomes, providing odds ratios (OR) and adjusted odds ratios (aOR) with 95% confidence intervals (CIs). A total of 2,225 TB patients (1,382 males and 843 females) were included in the study. Of these, 2,048 (92.1%) achieved successful treatment outcomes. Univariate analysis indicated that older age (OR: 0.47; 95%CI: 0.28–0.78) and a high number of AFB in sputum smears (OR: 0.23; 95%CI: 0.09–0.66) were associated with a higher likelihood of unsuccessful TB treatment. In contrast, having no HIV infection (OR: 13.44; 95%CI: 6.22–29.08), clinical TB cases (diagnosed clinically rather than bacteriologically) (OR: 1.50; 95%CI: 1.04–2.20) and longer duration of TB treatments were associated with successful treatment outcomes. Multivariate analysis suggested that the TB treatment durations of 4–6 months (aOR: 1256.95; 95%CI: 431.89–3658.19) and 7–12 months (aOR: 575.5; 95%CI: 99.1–3342.06) were associated with a significantly higher likelihood of success compared to durations of 0–3 months. In conclusion, this study highlights that a minimum treatment duration of three months was crucial for increasing the likelihood of successful TB treatment. These findings emphasize the importance of comprehensive support programs to ensure adherence to treatment guidelines and improve outcomes.

Abstract

Neonatal mortality is a leading cause of infant deaths, particularly in developing countries. In Serdang Bedagai Regency, the coverage of neonatal visits remains low, contributing to the high neonatal mortality rate and causing economic losses. This study aims to identify factors related to the neonatal visit coverage rate in Serdang Bedagai Regency, North Sumatra, Indonesia. This study used a cross-sectional design with multivariate analysis. Data were collected from 200 mothers with babies aged 0-28 days in five health centers in Serdang Bedagai Regency. The variables analyzed include age, education, economic status, knowledge, maternal attitudes, culture, family support, midwife attitudes, access to health facilities, and service quality. The data were analyzed using logistic regression. The results showed that the variables consistently and significantly affecting the increase in neonatal visit coverage were maternal education, knowledge about neonatal visits, local culture, husband/family support, and the quality of visit services. Factors that significantly contributed to increasing coverage of neonatal visits were maternal education (p-value = 0.004, OR = 14.285), maternal knowledge (p-value = 0.000, OR= 75.920), local culture (p-value = 0.000, OR = 57.105), husband/family support (p-value = 0.000, OR = 77.763), and service quality (p-value = 0.000, OR = 47.474). These factors have a major influence in supporting the success of the neonatal visit program, with maternal knowledge and family support being the most dominant factors.. Efforts to increase coverage should involve educating mothers, involving families, and enhancing services while considering local wisdom.

Abstract

According to the 1945 Constitution of the Republic of Indonesia, health is one of the basic human rights. One of the aspects that support health is the aspect of pharmacy. Pharmacy workers consist of Pharmacists and Pharmacy Technical Workers according to Law No. 36/2014 regarding Health Workers. In practice, pharmacy education in Indonesia is considered not sufficient to prepare the pharmacy workforce. There are currently contradictions in the applied pharmaceutical policy, where the number of pharmacy undergraduate programs is 279, while the number of pharmacist study programs is only 45. This results in an imbalance of graduates. There is a tendency that most pharmaceutical graduates will continue their professional education to become a pharmacist so that he can have full authority and is responsible for pharmaceutical work. Differences in curriculum background and type of education will lead to confusion regarding competency assessment. At the Pharmacist education level, there is no nationally applicable curriculum until this date. The difference in curriculum causes variations in the knowledge and abilities of graduates. The method used in this study is a review of articles from various legal rules related to pharmaceutical education in Indonesia. The results of this study indicate a need for synchronization of the pharmaceutical policy into one legal aspect so that there is no disharmony among the applied legal regulations. Eventually synergistic legal regulations will emerge, from education to legal certainty for Pharmacist as health professionals.

Abstract

The prevalence of smoking in Indonesia is so high in all walks of life that the government has made several policies to control it. One of them is the inclusion of health warnings and health information on tobacco product packaging which aims to prevent novice smokers from starting smoking, as well as guaranteeing the public's right to clear, correct and honest information, as well as choosing based on clearer (visual) information that they understand (informed decision). The purpose of this study was to determine the extent of active smokers' perceptions of pictorial warnings of the dangers of smoking on cigarette packs. Subjects and Methods: This study uses a qualitative approach and the Extended Parallel Process Model (EPPM) method. The research subjects consisted of active smokers who were found in Pondok Gede District, Bekasi City. Data collection techniques in this study were in-depth interviews. The research instrument used was the researcher himself who was directly involved in the research. Then the technique of checking the validity of the data uses source triangulation. Results: The results showed that the informants' perceptions of the pictorial warnings of the dangers of smoking on cigarette packs varied quite a lot. Not all informants feel afraid and horrified by the scary pictures on cigarette packs. These results affect their beliefs to be able to reduce or quit smoking. Those who feel afraid and spooky of the pictures on cigarette packs have more confidence to be able to stop smoking than those who don't feel scared or spooky. Conclusion: Based on the results of the analysis it can be concluded that the pictorial warnings on cigarette packs has not been able to reduce the desire to quit smoking among informants.

Abstract

One of the functions of hospital bylaws are a means to provide legal protection for all all parties that are related to the hospital. Legal protection is essential for prosecuted hospital staff members. One of the reasons for indictments is adverse events. The goal of this research is to discover how the hospital by-laws protects the hospital staff in the event of an indictment based on an adverse event and how the by-laws adhere to the guidelines in a comprehensive way. The research design used the qualitative method. The results of this research indicates that the hospital by-laws of RSKO Jakarta, although it has served as a protective method, it still needs further technical-operational policies as a supplement, and there are still discrepancies compared to the guidelines. We suggest that the by-laws of RSKO Jakarta are amended to adhere to the governmental guidelines and to establish supplementing technical-operational policies.

Abstract

Public Health Centers (PHCs) are responsible for organizing public health efforts and individual health efforts. PHCs also serve as FKTP, UPTD, sub-district supervisors, and implementers of policies and programs from the Ministry of Health. The workload of PHCs is increasing because people visit PHCs primarily for treatment, rather than seeking preventive measures to stay healthy. The PHCs have Sub-PHCs to lighten the workload of the PHCs, but there is no policy regulating Sub-PHCs to be used as FKTP. This research aims to analyze the policy of Balancing Community and Individual Health Efforts by utilizing Sub-PHCs such as FKTP. The research uses qualitative methods with a policy analysis process design, using primary data from in-depth interviews and FGDs with 14 informants and secondary data by reviewing documents. The research results indicate that PHCs have a heavy workload, and are more focused on medical services, and access to FKTP for communities in remote areas remains challenging. Sub-PHCs deserve to be used as FKTP on par with Pratama Clinics to increase community access to FKTP, particularly in areas with difficult geographical access, and to help lighten the workload of PHCs. It is recommended that the Ministry of Health conduct a policy analysis to support PHCs becoming FKTP, in line with the Minister of Health Regulation Number 43 of 2019, to change the role of the auxiliary health centers (Sub-PHCs) into FKTP.
