Abstract

Penelitian ini bertujuan melihat pengaruh dan besaran berbagai variabel kesehatan dan kualitas kehidupan kerja terhadap kinerja perawat di Rumah Sakit Umum (RSU) Tangerang. Variabel kesehatan tersebut meliputi sejarah kesehatan keluarga, lingkungan kehidupan, perilaku gaya hidup, dan status kesehatan perawat. Desain penelitian yang digunakan pada penelitian ini adalah desain potong lintang. Data dianalisis dengan pendekatan Structural Equation Model (SEM) menggunakan software Smart-PLS. Populasi yang diamati adalah perawat yang bertugas di semua pelayanan RSU Tangerang dengan 250 perawat terpilih sebagai sampel yang dilakukan secara multistage random sampling. Hasil analisis menunjukkan bahwa kinerja perawat di RSU Tangerang dipengaruhi oleh semua variabel, yang berarti model teoritis yang diusulkan pada penelitian ini dapat digunakan sebagai variabel prediktor. Pada model, variabel kualitas kehidupan kerja perawat merupakan variabel prediktor terbesar yang memengaruhi kinerja perawat. Dengan demikian, variabel kinerja perawat dipengaruhi oleh banyak variabel dengan pengaruh yang relatif kecil (6,16%). Intervensi terhadap semua variabel yang ditawarkan pada penelitian ini akan memberikan hasil yang kecil terhadap kinerja perawat sehingga memerlukan intervensi tambahan pada variabel lain. This study aims to see the effect and magnitude of health variables and variable of the quality of work life on the performance of nurses working in Tangerang General Hospital. Health variables include family medical history, living environment, lifestyle behaviors, and health status. Crosssectional method used in this study. Data were analyzed with the approach to Structural Equation Model (SEM) using software Smart-PLS. In this study nurses who serve in all services at Tangerang General Hospital was chosen as the unit of analysis with 250 nurses was selected as the samples and multistage random sampling were used. The results of the analysis showed that the performance of nurses in Tangerang General Hospital was influenced by all the variables which means that the theoretical model proposed in this study can be used. The quality of work life of nurse was the greatest predictor variables in the model that affect the performance of nurses. Thus, this study concluded that the variable performance of nurses is affected by many variables with its influence was relatively small (6,16%). Therefore all the variables offered in this study will intervene not large enough results to produce an excellent performance of the nurses at Tangerang General Hospital and required additional intervention efforts on other variables.

Abstract

In 2014, the Indonesian Government introduced a social security program in the health sector. However, Indonesia’s out-of-pocket expenses remain high due to a lack of public interest in National Health Insurance services. Financing expensive health services with high out-of-pocket expenses has the potential to cause poverty. Private health insurance is considered a solution to this problem. This study aimed to determine the socioeconomic factors of private health insurance ownership and its potential in Indonesia. This study used secondary data from the 2018, 2019, and 2020 National Socioeconomic Surveys. Logistic regression analysis showed that the variables related to private health insurance ownership were age, sex, education, economic status, employment status, marital status, household status, and location of residence. The most dominant variable in 2018 was per capita expenditure (economic status), while education was the most dominant variable in 2019 and 2020. The result of this study can be used to formulate a strategy for increasing participation in private health insurance. The socioeconomic health sector should use this information to target specific markets for private health insurance.

Abstract

In this era of COVID-19, suspected breast cancer patients experience delay in diagnosis due to the fear of contracting the virus and reduction of non-COVID-19 health services. Furthermore, it may lead to potential increase in the incidence of advanced cancers in the future. Ultrasound-guided (US-guided) percutaneous core needle biopsy (CNB) is a great option for the diagnosis of cancer but it is poorly utilized. This study aimed to prove that the US-guided CNBis accurate when performed in a local setting and a potential solution for diagnosing breast cancer patients in this pandemic. In addition, it was a single health center cross-sectional study, and the participants were all breast cancer patients that had US-guided CNB from 2013-2019. The pathology results from US-guided CNB were compared to specimens from post-CNB surgeries. The data were collected from medical records and the immunohistochemistry (IHC) examinations were carried out for malignancy. There were 163 patients who were included in this study, 86 had malignancies and 77 had benign tumor reported in their CNB results. The US-guided CNB had 100% sensitivity and specificity compared to surgery. With its lower cost, time usage, and patient exposure to the hospital environment, US-guided CNB should replace open surgery biopsy for diagnosing suspicious breast cancers during the pandemic in Indonesia.

Abstract

Diabetes is the “mother” of various diseases increasing the risk of morbidity and mortality. Diabetes self-management, an effort made by patients to control blood sugar levels, is an important part of the management strategy. Therefore, this study analyzed information, motivation, and behavioral skills associated with diabetes self-management. Data were collected in the Special Capital Region of Jakarta, with 277 diabetic patients selected using a questionnaire by a systematic random sampling method. The analyzed variables were information (with indicator variables of information on physical activity, nutritional intake, drug consumption, and blood sugar monitoring); sociodemographic (age, sex, occupation, education level, and duration of diabetes); motivation (barrier, benefit, self-efficacy, severity, and susceptibility); and behavioral skills (new motor, self-regulatory, and social skills), which were analyzed to identify their influence on diabetes self-management using structural equation modeling. The results indicated that information and motivation significantly and positively affected behavioral skills; while, sociodemographic did not. Behavioral skills had a significant and positive effect on diabetes self-management. Accordingly, people with diabetes information, motivation, and behavioral skills need to be improved to increase the success of diabetes self-management.

Abstract

Promotive, preventive, curative, and rehabilitative efforts that are comprehensive, integrated, and sustainable are employed to enhance the health state of the global population. Within this context, however, the quality of primary health care depends on job satisfaction, which leads to the happiness of human resources in the health sector. Therefore, this study aimed to analyze and formulate a job satisfaction model among primary health care midwives in Indonesia. This study was an advanced secondary data analysis of a cross-sectional study conducted in 2017 by the National Institute of Health Research and Development, Ministry of Health of the Republic of Indonesia. A total of 87,341 midwives from all 9,669 primary health cares in Indonesia participated in this study. Data were collected by distributing the Minnesota Satisfaction Questionnaire, elaborating on the satisfaction level and relevant contributing factors. The prefilled Likert scale questionnaire was analyzed using logistic regression. The findings suggested a model indicating that motivation, work area (region), history of salary delay, and training received were important for their job satisfaction, whereas the motivation aspect contributed the most. Therefore, the local and central governments must consider these factors in the human resource policymaking process.

Abstract

Prevention and treatment of diabetes will have a positive influence on tuberculosis (TB) since people may get TB because they have diabetes mellitus (DM). Recording and reporting through the TB Information System are not run optimally because of many factors. The information system must be strengthened to be used by private health facilities. This study used secondary data from the 2013 and 2018 Indonesian Basic Health Research (IBHR). The data was analyzed univariately and analyzed further using Orange Data Mining Tools to test the screening tool model used to predict TB in diabetic individuals. The total sample in this study from each data was 38,136 people. The 2013 IBHR stated that 749 people (2%) were diagnosed with pulmonary TB, while the 2018 IBHR stated that 97 people (0.3%) were diagnosed in the previous six months. The results of the Orange analysis showed that precision and recall calculations in this study were quite good, at 0.9. Therefore, the model would likely predict the occurrence of TB in diabetic individuals. According to Orange, the TB-DM electronic screening tool model tends to estimate the incidence of TB in diabetic individuals.

Abstract

The high comorbidity rate between diabetes mellitus and tuberculosis has a significant impact on the patient's health-related quality of life and eradication of tuberculosis program outcomes, especially in lower-middle-income countries that bear a "double disease burden". This review outlines the health-related quality of life of tuberculosis patients with diabetes. The search, selection, and article collection procedures were unmethodical. The retrieved articles were open-access and full-text in English. Health-related quality of life includes physical, psychological, social, and environmental domains. Health-related quality of life in diabetes, tuberculosis, and tuberculosis-diabetes comorbid patients experienced negative changes in all domains. Commonly used health-related quality-of-life measures can be generic or disease-specific. This review also highlights the importance of collaborative tuberculosis and diabetes management supported by the community, health workers, and stakeholders. Finally, this collaborative management support will enhance patient health-related quality of life and contribute to the accomplishment of the tuberculosis elimination program.

Abstract

The low utilization of antenatal care services has an impact on the condition of the pregnancy not being monitored which will cause danger signs of pregnancy to go undetected and can lead to maternal death. Padang Lawas is the area with the lowest coverage of antenatal care visits in north Sumatra province. The majority of pregnant women in Padang Lawas district do not receive standard antenatal care services. Efforts to increase coverage rates for Antenatal Care visits have been carried out by the Health Service but have not produced any real impact. Objective: This study aims to find out more about the perspective of antenatal care visits for pregnant women. Method: This study was qualitative with a grounded theory approach. The informants in this study were 5 mothers, 5 husbands, 3 midwives, and 1 community figure who were recruited using purposive sampling. Data were collected using semi-structured interviews. Data were analyzed using the Content Analysis. Results: The study emerged 4 themes: (1) Culture about pregnancy, (2) Infrastructure in antenatal care, (3) Economic issues, and (4) Mother's behavior in carrying out antenatal care. Conclusions: The findings show that there are several barriers of antenatal care visits in rural maternal communities, so the government needs to pay attention to them in following up on the barriers found in these areas.

Abstract

Weight gain that is not according to recommendations during pregnancy has become an epidemic throughout the world today and is a clinical problem with high prevalence and can affect the health of mothers and babies. Objective: This study aims to find out more about the perspective of increase in weight of pregnant women in deli serdang regency. Method: This study was qualitative with a descriptive approach. The informants in this study were 3 pregnant women with weight gain above recommendations, 3 pregnant women with weight gain below recommendations, 1 ob-gyn, 1 midwife, and 1 nutrition who were recruited using purposive sampling. Data were collected using semi-structured interviews. Data were analyzed using the Content Analysis. Results: The study emerged 5 themes: (1) Knowledge, (2) Perspective, (3) Behaviour, (4) Family support, and (5) Physical activity. Conclusions: The findings show that several factors influence the increase in weight of pregnant women in Deli Serdang Regency, so the health care provider needs to pay attention to improve their knowledge about weight gain during pregnancy according to the needs of pregnant women.

Abstract

Most informal female workers in traditional markets are in the reproductive age range. One of the causes of low exclusive breastfeeding in women informal workers is the low implementation of policies related to exclusive breastfeeding in the workplace, especially in the informal sector. Objective to analyze the policies carried out by the local government related to exclusive breastfeeding policies for informal workers in traditional markets. Method: the study was qualitative with a phenomenological approach. Participants consisted of 12 employees of related agencies with purposive sampling. Data collection was done through in-depth interviews using voice records. Data analysis used content analysis by the collaizi method. The principle of traceability was applied to ensure rigor in the research. The results of this study obtained two themes related to the analysis of exclusive breastfeeding policies in Informal Workers of Traditional Markets in North Sumatra, namely 1) perception of lactation corner facilities in the markets; and 2) barriers to the certainty of lactation corners in the traditional market. It is essential to analyze the exclusive breastfeeding policy for informal workers in convention markets to create better policies by providing adequate lactation corner facilities for informal workers.
