Abstract

BACKGROUND: Some of colorectal cancers (CRCs) are familial, however, heterozygote relatives have approximately 80% lifetime risk of cancer. Risk assessment of CRC’s family could be calculated by direct measurement of mRNA gene expression and Bayesian theorem which is modifying initial background of pedigree risk with additional conditional information. This application has not been reported.METHODS: The cross-sectional translational sequential studies were performed: (1) adenomatous polyposis coli (APC) and MutS homolog (MSH)2 mRNA quantitative RT PCR gene expressions in tissue and whole blood CRC patients; (2) gene expression was determined in matched controls; and (3) pedigree and Bayesian analysis was calculated in the patient’s family of Proband.RESULTS: Fourty CRC and 31 control subjects were enrolled. The mean blood APC level control’s group was 13,261±670 fold-change (fc) and blood MSH2 level was 12,219±756 fc. The cut-off points for hereditary APC was 12,195 fc and MSH2 was 11,059 fc. The mean APC blood level in CRC subject was 11,578±2,638 fc and MSH2 blood level was 11,411±2,912 fc. There were significant differences APC and MSH level between tissue and blood level in CRC. Eight of 40 CRC subjects had a history of familial CRC. Four patients and 10 Probands were available for recurrence risk evaluation of pedigree analysis, RNA PCR quantitative and Bayesian calculation.CONCLUSION: There was determined a cut-off point of hereditary mRNA quantitative expression. The APC and MSH2 levels in CRC subjects were significantly lower than controls. Bayesian analysis allowed for the calculation of relative risk in CRC family members and considered in clinical practice.KEYWORDS: hereditary CRC, APC gene, MSH gene, Bayesian analysis

Abstract

Background: Student peformance assessment during problem-based learning (PBL) discussion is still debatable. One of the assessments that applies adult learning principle is self-assessment  (SA). In fact, many medical schools prefer to use tutor assessment than SA because it is considered less accurate. Many studies have reported that the accuracy of SA is poor because of their own lack of knowledge and SA skills.  This literature review aims to explore deeper in SA and its basic principles in designing SA instrument for PBL discussion in medical school.Method: This study was conducted using narrative review method. Ten articles were reviewed. Five articles were chosen from google search engine and the other five from medical education textbooks.Results: SA is the ability of a student to observe, analyze and assess his own performance based on the criteria and he determines a way to fix it. SA skills that have been practiced during  PBL discussions will equip the students to become future health professionals who are competent in determining their own continuous professional development (CPD) programs. When designing a SA instrument, one needs to explore these five main issues, including acceptance, accuracy, power, feasibility and context.Conclusion: SA is the most effective assessment to assess a student’s achievement in PBL discussion if implemented properly. The completion of SA should be made in the normal context and one must explore the five main issues constantly so that SA can be done properly and well.

Abstract

Model 4C/ID adalah desain instruksional yang menciptakan lingkungan pembelajaran berbasis keseluruhan tugas. Kuesioner Satisfaction Scale for e-Learning Process merupakan kuesioner untuk mengukur kepuasan peserta didik terhadap proses pembelajaran daring. Penelitian ini bertujuan untuk melakukan adaptasi kuesioner Satisfaction Scale for e-Learning Process versi bahasa Indonesia. Penelitian terdiri atas empat tahap yaitu forward translation, backward translation, expert review, dan pilot study. Forward translation adalah penerjemahan kuesioner ke bahasa Indonesia oleh penerjemah bersertifikat. Backward translation merupakan penerjemahan kembali kuesioner ke bahasa Inggris oleh penerjemah yang berbeda. Expert review adalah proses validasi konten oleh 8 orang ahli menggunakan skala CVI dan S-CVI. Pilot study dilakukan terhadap 30 mahasiswa yang dipilih secara acak. Forward dan backward translation menunjukkan kesesuaian maksud dengan kuesioner aslinya. Nilai I-CVI menunjukkan bahwa 28 butir kuesioner valid (skor >0,75), dan satu butir tidak valid (skor <0,75). Nilai S-CVI (skor ≥0,90) menunjukkan bahwa keseluruhan kuesioner adalah valid. Kuesioner Satisfaction Scale for e-Learning Process versi bahasa Indonesia adalah valid, sehingga dapat digunakan sebagai instrumen untuk mengevaluasi penerapan model 4C/ID pada pembelajaran keterampilan klinis secara daring.

Abstract

It is sometimes hard to establish the diagnosis of colon tuberculosis because the ulceration features on ileocaecal valve and caecum are not specific. Moreover, biopsy of lesion area can only provide 60-80% detection. The lesions usually include linear or transversal ulcer, nodule, abnormal form of ileocaecal valve and caecum, inflammatory polyp and irregular multiple fibrous bands. We report a case of colon tuberculosis in a 32-years-old woman with giant ulcer and pseudopolyps at ileus terminalis and ileocaecal junction detected by colonoscopy, in addition to multiple ulcers along the colon, starting from rectum to ascending colon. By fecal examination, positive Acid Fast Test (AFT) result was found and by chest X-ray, tuberculosis features were also found.   Keywords: tuberculosis colitis, multiple ulcers in colon, giant ulcer, pseudopolyp, fecal AFT, colonoscopy

Abstract

The recent advances of endoscopic examination had proven that source of upper gastrointestinal bleeding in liver cirrhosis is not always caused by esophageal varices rupture but also gastric mucosal lesion. The prevalence of gastric ulcer in patients with liver cirrhosis is higher than healthy individuals. Imbalance of defensive and aggressive factors of gastric mucosa may involve in development of portal hypertensive gastropathy (PHG). Several studies reported hemodynamic changes associated wuth portal hypertension causing decreased mucus layer thickness as one of mechanism of PHG. Other etiologic factors of PHG were hypoacidity, hypergastrinemia, reduced hexosamin concentration, mucus metabolic function associated with decreased prostaglandin E2 (PGE2), and increased nitric oxyde (NO) which had caused mucus wall thickness changes. Gastric mucus damage induced by portal hypertension has important role in the pathogenesis of gastric ulcer in liver cirrhosis. Keywords: Pathogenesis, PHG, liver cirrhosis

Abstract

Background: Malignant and benign lesions may cause obstructive jaundice. The treatment of these conditions includes biliary stenting drainage, percutaneous transhepatic biliary drainage (PTBD), or surgical procedures. In advanced malignant jaundice, stent placement often turns out to be difficult. The aim of this study was to determine the success rates of malignant obstructive jaundice detection utilising endoscopic retrograde cholangiopancreatography (ERCP) and its stent placement procedure. Method: We conducted a retrospective study in 139 patients who undergone ERCP in Cipto Mangunkusumo Hospital between October 2004 and July 2008. Data was analyzed descriptively with SPSS version 17.0. Results: Of 139 study subjects, 131 (94.2%) of them had clinical obstructive jaundice (direct bilirubin indirect bilirubin level). There were 73 (55.7) male patients, with age range of 20-84 years. Among 114 patients with identified cause of obstruction, 57 (50%) patients had undergone stent placement; however, only 32 (56.1%) patients had successful stent placement. Our descriptive analysis showed that age and sex did not affect the stent success rates, and malignancy was showed to be a factor of stent failure. Conclusion: ERCP appears to be reliable enough for identifying the cause of obstructive jaundice in most patients. In this study, the achieved success rate of stent placement is more than 50%. Moreover, such rate is lower in the malignant obstructive jaundice than the non-malignant counterparts. Papillary carcinoma is the most frequent cause of malignant obstructive jaundice. Abstract

Background: The Endoscopy Unit of Indonesian Air Force Central Hospital Dr. Esnawan Antariksa, Jakarta, Indonesia, has just been established in late 2004 and it is still in developing process. This study was aim to depict the profile of patients who underwent esophagogastroduodenoscopy procedure in the unit. Method: This study was a retrospective study that analyzed data in endoscopic registry of patients who underwent esophagogastroduodenoscopy (EGD) examination from September 2004 to December 2007. Result: Total of 108 patients underwent EGD examination, comprised of 66 (61%) males and 42 (39%) females, with average age of 45.4 years old (range 16-77 years old). The most frequent indication for EGD was dyspepsia 63%, followed by hematemesis-melena 26% and dysphagia 5.5%. Among patients with dyspepsia, EGD revealed organic abnormalities in 71% patients, with descriptions of erosive gastritis 35%, gastritis 18%, bile reflux 7%, esophagitis 5% and gastric ulcer 3%. Among patients with hematemesis-melena, EGD revealed abnormalities, which were described as erosive gastritis 48%, esophageal varices bleeding 22%, gastric ulcer 18%, caustic injury 4%, and esophagitis 4%. Conclusions: EGD is an important procedure to ascertain the occurence of organic abnormalities in patient with gastrointestinal symptoms and signs e.g. dyspepsia and gastrointestinal bleeding. More than half of patients with dyspepsia showed organic abnormalities. Among patients with hematemesis-melena, erosive gastritis is more common than variceal bleeding. Abstract

Sedation helps patients to tolerate uncomfortable gastrointestinal endoscopy procedures by relieving anxiety and pain. It also decreases a patient’s risk of physical injury during gastrointestinal endoscopy procedures while providing the endoscopist with a good environment for an optimum examination. Not all sedation in gastrointestinal endoscopy is managed by an anesthesiologist. Using sedation has increases risk of cardiopulmonary complications. Therefore, sedation is an important part in gastrointestinal endoscopy to learn. This review will focus on basic pharmacology of sedatives and analgesic, common used drugs in sedation (Propofol, Midazolam, Fentanyl), and their use in gastrointestinal endoscopy.

Abstract

Ulcerative colitis (UC) is an idiopathic inflammatory disease that affects the colon. Current pharmacological modalities to treat UC have various side effects; therefore, there is a demand to develop a new alternative medicine that can reduce side effects and increase drug efficacy. One candidate for alternative therapy is Polysaccharide Peptide which is extracted from Ganoderma lucidum mycelium. This Polysaccaharide has an active compound of Β-1,3/1,6-D-Glucan which has strong immunomodulatory and anti-inflammatory properties. Various studies have reported that Ganoderma lucidum polysaccharides can reduce inflammatory markers such as TNF-α, IFN-γ, and IL-17A, which is produced by colonic mucosal inflammation. In addition, β-1,3/1,6-D-Glucan has shown improvements in inflammatory parameters and intestinal immunological barrier function animal studies with artificial colitis and requires further research in humans before clinical applications.
