Abstract

Patient satisfaction is when hospital services meet patients' needs, desires, or expectations. The government must promote everyone's right to health information and education, especially in hospitals, and guarantee comprehensive health services based on Minister of Health Regulation Number 004 of 2012 for optimal, effective, efficient, integrated, and sustainable hospital health services. The study was carried out using quantitative methods. Primary data was collected through questionnaires from patients at Royal Prima Medan Hospital, and 180 samples were obtained. Data was processed using SEM-PLS. The research results show that all hypotheses are accepted, and all variables have a positive and significant effect. The research results prove that implementing hospital health promotion helps improve the quality of health services (t-statistic value 7.294) and increase patient satisfaction (t-statistic value 4.419). Patients will be more loyal if their expectations are met from the health promotion carried out and the quality of service they perceive (t-statistic value 2.142). So, it can be concluded that government policy is appropriate in providing direction for hospitals to promote hospital health. Patients expect hospitals to have good service quality, cleanliness, and comfortable health facilities for patient satisfaction. Patients will be more loyal if their expectations are met, based on the perception of service quality and promotions carried out by the hospital, through the quality of personnel, and the empowerment of the community around the hospital.

Abstract

Tuberculosis (TB) poses a grave threat to those afflicted, contributing significantly to the global mortality rate. Caused by the infectious bacterium Mycobacterium tuberculosis, TB primarily targets the lungs, with Indonesia ranking second in its prevalence worldwide. This study aims to scrutinize factors associated with TB treatment discontinuation within the Subulussalam City Health Service Work Area in 2023, employing a quantitative, cross-sectional approach. A sample of 203 TB patients under treatment within the area was subjected to logistic regression and multiple logistic regression analyses. Findings revealed a correlation between treatment discontinuation and factors such as inadequate knowledge (p = 0.00; OR = 27.81) and adverse drug reactions (p = 0.02; OR = 13.05). Recommendations include enhancing patient education on TB risks to bolster awareness and deter treatment abandonment, instituting recognition programs within health centers to incentivize treatment completion, and acknowledging the proactive role of healthcare personnel in facilitating patient adherence.

Abstract

Patient safety is an important factor for everyone, especially in the health system. The importance of patient safety, considering the magnitude of the problem that exists for patients and health care professionals. The role of professional health care is very important to improve the quality of services provided and improve or ensure patient safety. The importance of patient safety is increasingly prioritized around the world and every day health care professionals face several challenges in providing safe care to patients. This study aims to analyze the application of patient safety goals in health care professionals at 3M Plus Tembilahan Hospital. The type of research is quantitative analysis using a cross sectional study approach. The research sample was carried out using the total sampling method, namely 201 respondents at 3M Plus Tembilahan Hospital. The data was analyzed using Partial Least Square (PLS). The results of the study showed that there was a relationship of analysis of the implementation of patient safety goals in health care professionals at 3M Plus Tembilahan Hospital.

Abstract

The National Health Insurance (JKN) program is a government initiative aimed at supporting comprehensive health development to ensure that all Indonesians have the right to good health. According to data obtained from BPJS Kesehatan and RS Murni Teguh Medan, the average waiting time for polyclinic services exceeds the national quality indicators, which requires outpatient waiting times should not exceed 60 minutes. This study aims to analyze the challenges in implementing the JKN program, specifically related to patient admission, medical data processing, reporting, and funding at RS Murni Teguh Medan. This is a qualitative descriptive research involving three key informants (Persons-in-Charge or PICs) and six supporting staff members from the Registration, Medical Records, and JKN Billing units. The research identified several challenges: Man: The registration unit's human resources do not meet the required qualifications, as staff should ideally have a medical records background, resulting in reporting issues. Materials: Patient files are incomplete for administrative processes. Machine: The applications used, such as MTHIS (Hospital Management Information System), Mobile JKN, V-Claim, DIVA, and Online Pharmacy, face operational issues, leading to inefficient medical data processing and reporting. Methods: Although procedures are aligned with the SOPs, issues remain include patient overcrowding at registration, outpatient service waiting times exceeding 60 minutes, issues with registration in executive polyclinics, self-registration issues at APM, service penalties, and semi-manual medical data processing and reporting. Markets: Patients have limited knowledge of using Mobile JKN application, and pending claims are still prevalent. Money: Prolonged use of outdated electronic meda reduces systems performance.

Abstract

Objective: To determine the unit cost of lithotripsy for treatment of urinary bladder calculus at Vita Insani Hospital Pematangsiantar. Material & Methods: This study is a descriptive quantitative study, in which the data was obtained from the finance division of Vita Insani Hospital. The research object is the unit cost of accommodation that occurs in patients with urinary bladder calculus with lithotripsy cases from January 2020 to December 2020. Results: The unit cost of lithotripsy for urinary bladder calculus services at Vita Insani Hospital Pematangsiantar which is calculated using the activity-based costing (ABC) method is Rp 4.167.009. There is a difference in cost among the ABC method, hospital calculations, and INA CBG’s rates with the following details: the amount calculated for the hospital is Rp 4.996.718, while the calculation using ABC method is Rp 4.167.009; the difference in costs is Rp 829.709. INA CBG’s rates Rp 6.137.500, and compared to calculation of hospital, the difference cost is Rp 1.140.782. Compared to the INA CBG’s rates, to the unit cost of the ABC method, there is a difference, amounting Rp 1.970.491. Conclusion: The results of unit cost lithotripsy for treatment of urinary bladder calculus through the ABC method are lower compared to hospital calculation and INA CBG’s rates.

Abstract

Diabetes mellitus (DM) is a chronic metabolic disorder characterized by elevated blood glucose levels and often associated with hypercholesterolemia. Garlic (Allium sativum Linn.), rich in bioactive compounds such as allicin, saponins, and flavonoids, has been widely reported to possess lipid- and glucose-lowering properties. This study aimed to evaluate the hypolipidemic and hypoglycemic effects of garlic extract in Wistar rats subjected to a high-fat diet and streptozotocin (STZ) induction. A laboratory experimental design with a post-test-only control group was employed. For cholesterol assessment, hypercholesterolemic rats were administered garlic extract at doses of 0.05, 0.10, and 0.20g/head/day, while diabetic rats induced with STZ received doses of 100, 250, and 500 mg/kg body weight per day, with corresponding control groups. Garlic extract significantly reduced mean cholesterol levels in a dose-dependent manner (130.60mg/dL, 121.80mg/dL, and 112.00mg/dL for 0.05, 0.10, and 0.20g/head/day, respectively). In contrast, mean blood glucose levels showed a marginal decrease with increasing doses (282.80 mg/dL, 271.20mg/dL, and 269.27mg/dL at 100, 250, and 500mg/kgBW/day, respectively). However, no statistically significant differences were observed among groups (p=0.706, one-way ANOVA). These findings suggest that garlic extract exhibits a potent cholesterol-lowering effect in hypercholesterolemic rats but has a limited impact on blood glucose reduction under the tested conditions. Further studies are warranted to determine the optimal dosage and underlying mechanisms of action.

Abstract

Mahkota Dewa (Phaleria macrocarpa) adalah tanaman obat populer di Indonesia yang berasal dari Papua dan tumbuh subur di iklim tropis. Buah tanaman ini kaya akan senyawa antibakteri, termasuk alkaloid, polifenol, flavonoid, saponin, dan tanin, yang tergolong dalam metabolit sekunder. Penelitian ini menyediakan kontribusi baru dalam bidang farmakologi dengan mengeksplorasi efektivitas ekstrak etanol dari daging buah Mahkota Dewa (Phaleria macrocarpa) terhadap Staphylococcus aureus dan Staphylococcus epidermidis pada berbagai konsentrasi. Tujuan penelitian untuk menilai aktivitas antibakteri ekstrak etanol dari daging buah Mahkota Dewa. Metode yang digunakan adalah desain Post Test Only Control Group dengan pengujian in vitro. Ekstrak etanol disiapkan pada konsentrasi 20%, 40%, dan 60% melalui proses maserasi. Uji fitokimia menunjukkan keberadaan flavonoid, alkaloid, saponin, terpenoid, tanin, dan fenol. Aktivitas antibakteri diuji dengan metode difusi cakram, dan zona hambat diukur setelah inkubasi selama 24 jam pada suhu 37°C. Hasil penelitian menunjukkan bahwa ekstrak etanol Mahkota Dewa memiliki aktivitas antibakteri yang signifikan terhadap Staphylococcus aureus dan Staphylococcus epidermidis. Konsentrasi 60% menghasilkan zona hambat yang paling besar, yaitu 1,48 mm untuk Staphylococcus aureus dan 4,08 mm untuk Staphylococcus epidermidis, sedangkan konsentrasi 20% dan 40% menunjukkan efek antibakteri yang lebih rendah. Analisis statistik menggunakan uji Kruskal-Wallis dan Mann-Whitney menunjukkan adanya perbedaan yang signifikan antara konsentrasi ekstrak dan kontrol. Kesimpulan bahwa ekstrak etanol dari daging buah Mahkota Dewa (Phaleria macrocarpa) menunjukkan sifat antibakteri yang signifikan terhadap S. aureus dan S. epidermidis, terutama pada konsentrasi 60%. Namun, efektivitasnya masih lebih rendah dibandingkan dengan kloramfenikol.

Abstract

Stroke is the leading cause of physical and mental disability worldwide. In Indonesia, the incidence of stroke is alarmingly high, affecting approximately 10.9% of the population annually, with around 713,783 individuals experiencing stroke each year. Understanding how to prevent stroke is crucial. Clear and regular information, along with effective educational media, are essential for increasing public awareness of stroke. This study aims to determine the impact of social media and knowledge on stroke prevention. This type of study is a non-experimental quantitative study using a cross-sectional. This study is conducted in portions, with patients who are willing and able to talk effectively. Univariate, bivariate, and multivariate analyses were conducted using Pearson correlation tests and linear regression tests. The results show that among the 100 respondents, most were male, aged 41-60 years, and engaged in the best stroke prevention efforts. The Pearson correlation test showed significant correlations between information accuracy, frequency, and educational media for stroke prevention (Pearson p=0.89; p=0.62; p=0.75). The linear regression test revealed that the clarity of information, frequency of information, and educational media together increased stroke prevention efforts (p=0.037), with the clarity of information being the most significant factor. The conclusion is the study concludes that educational media and accurate information are crucial in preventing stroke. It is recommended that the Kasih Sayang Clinic conducts comprehensive stroke education and screening every six months to enhance prevention efforts.

Abstract

An important indicator to determine the quality of service in a hospital is about the completeness of the hospital's medical record files. There are many problems arise due to incomplete medical record files and do not recorded neatly and regularly. The incident is due to lack of medical team awareness in recording all information or actions that would be given to the patient. The aim of this study is to analyse the effect of Job Demand-Control-Support and Protection Motivation on compliance in filling out Medical Record files at RSU ESHMUN Medan 2023. This study uses quantitative research method which is analytical in nature. The total sampling of this study is 115 active nurses at RSU ESHMUN Medan. The result of the study informs that there are very strong and significant effect among Job Demand-Control-Support and Protection Motivation on compliance in filling out Medical Record files at RSU ESHMUN Medan 2023. The variable that has the most influence on Obedience with filling in inpatient medical record files at RSU ESHMUN in 2023 is the Protection Motivation variable. It is recommended to RSU ESHMUN Medan to improve the administration pattern in order to all of nurses have the same job demands and receive the same support

Abstract

The kidneys are a pair of symmetrical organs that function to filter the blood and are located in the retroperitoneal region on the posterior wall of the abdomen. The kidneys are drained about 25% of cardiac output. Kidney damage due to the accumulation of kidney stones can be caused by ethylene glycol substances to form calcium oxalate crystals (CaC2O4). This crystal is common in urine specimens even in healthy animals. Crystal formation can be accelerated by the administration of ammonium chloride. This study aims to determine the nephroprotective effects of balakka ethanol extract to reduce creatinine and urea levels in rats induced by ethylene glycol and ammonium chloride. The results showed that the balakka ethanol extract with a dose variation of 50 mg/kg body weight, 100 mg/kg body weight, and 150 mg/kg body weight had nephroprotective activity in rats that had been induced by ethylene glycol and ammonium chloride. Where the induction group had the largest serum creatinine mean of 1.43 mg/dl. The EEB treatment group dose of 50 mg / kgBB had a serum creatinine value of 1.01 mg / dl. The EEB treatment group dose of 100 mg / kgBB had a serum value of 0.91 mg / dl. The balakka ethanol extract treatment group dose of 150 mg / kgBB had a serum value of 0.73 mg / dl. The induction group had the largest serum urea serum of 76.56 mg/dl. EEB treatment group dose of 50 mg / kgBB had serum urea value of 63.36 mg / dl. The EEB treatment group dose of 100 mg / kgBB had a serum urea value of 49.73 mg / dl. The balakka ethanol extract treatment group dose of 150 mg / kgBB had a serum urea value of 49.90 mg / dl.
