Abstract

Basically the medical record is an important part of health care for patients in the hospital. Medical decisions taken by a doctor based on the diagnosis made will greatly affect the actions of patients in treatment. An accurate diagnosis is based on history, physical examination, supporting examination and written in the medical record file. The purpose of making a medical record to support the achievement of orderly administration in order to improve health services in the hospital. This study aims to analyze and determine the completeness of filling inpatient medical record files at the General Hospital Putri Hijau Medan . This type of research is descriptive with an approach Qualitative using the in-depth interview method from statements to six informants consisting of one pulmonary specialists, one nurse, three hospital management, chief medical records officer. The results of the study generally showed that from 40 medical record files by pulmonary specialists obtained the complete entry date (100%), the percentage of complete entry time (100%), the percentage of complete anamnese recording (80%) completed by the doctor, complete physical examination (55%), complete diagnosis as much as (95%), treatment and complete action as much as (100%), consent and complete action as much as (100%), clinical observations as complete as much as (67.5%), the return summary is completely filled (50%), and the doctor's name and signature are completely filled (90%). The incompleteness of filling medical records is due to doctors feeling the time is limited because of the large number of patients, lack of communication between doctors and nurses in completeness of completing record files, medical lack of monitoring conducted by the hospital. It is expected that doctors, nurses improve communication for the completeness of the medical record, the doctor as the person in charge of the medical record to be more concerned with completing the medical record. Hopefully the hospital management will be more assertive to provide sanctions to the doctor to be able to increase the completeness of the medical record and further enhance monitoring.

Abstract

A medical record is part of the archive that describes all activities by an agency within a certain period. Hospitals must have medical records as a service standard in the health sector that is useful for improving quality in providing optimal service to patients. The problem that often arises in filling out medical records is that in the process of filling it is incomplete, and the doctor's writing is less specific about the diagnosis. This situation has an impact on internal and external hospitals because the results of data processing are the basis for making hospital internal reports and hospital external reports. This study aims to analyze the effect of job demand control-support and protection motivation on Obedience with filling in inpatient medical record files at Batubara Hospital in 2021. This type of research is analytical quantitative research. The sample size in this study was 105 people. Data analysis used Univariate, Bivariate, and Multivariate. The results showed Job Demand Control-Support inpatient nurses at Batubara Hospital in 2021 were good as many as 94 respondents and 11 respondents were not good, Protection Motivation at Batubara Hospital in 2021 was good as many as 95 and 10 were not good, Obedience in filling out files inpatient medical records at the Batubara Hospital in 2021, which complied with as many as 94 respondents and 11 respondents who did not obey, Job Demand Control-Support affects Obedience with filling out medical record files for hospitalization at Batubara Hospital in 2021, Protection Motivation affects Obedience in filling out files Medical records for hospitalization at Batubara Hospital in 2021. The variable that has the most influence on Obedience with filling in inpatient medical record files at Batubara Hospital in 2021 is the Protection Motivation variable

Abstract

In today's growing medical environment, the efficiency of health services and patient satisfaction are often considered as important indicators of the quality of health services. However, the absence of patients according to the specified schedule is a common problem. Short Message Service (SMS) as a reminder and motivation is able to influence changes in patient behavior. The formulation of the problem in this study is how the effect of providing intervention in the form of SMS on the rate of follow-up visits in chronic patients in the internal medicine clinic of Binjai Latersia Hospital. The research design used was Quasi-Experimental with One-Group Pre-Post Test Design. The population in this study were all patients with chronic diseases in outpatient installations who underwent control at the internal medicine clinic of the hospital as many as 42 people for 3 months, with a total sample of 126 people. The results of the study show 5. There is an effect of providing intervention in the form of Short Message Service on increasing the number of repeat visits in doing a treatment.

Abstract

Nurse performance is a determining factor in the success of achieving hospital goals in nursing services. Various studies on the performance of nurses in Indonesia show that the performance of nurses is still relatively low between 47.6% - 53.4%. Nurse performance is thought to be influenced by 3 factors, namely individual factors, psychological factors and organizational factors. The purpose of this study was to analyze the effect of characteristic, psychological, and organizational factors on nurse performance. This research is a quantitative research with a cross sectional study approach. The research was conducted at the Royal Prima Hospital, Medan. The research population was 367 people, the sample obtained was 115 people. Sampling by purposive sampling. Data analysis was univariate, bivariate with chi-square test, and multivariate with multiple logistic regression at 95% confidence level ( a=0.05). The results showed that the factors that influence the performance of nurses at the Royal Prima General Hospital Medan are psychological factors (job satisfaction) (p=0.011), organizational factors (leadership and organizational culture) respectively p = 0.004 and p = 0.002. Individual characteristic factors had no effect, namely age (p=1,000), gender (p=0.661), education (p=0.454), and years of service (p=0.090). The most dominant variable influencing the nurse's performance is the leadership variable, Exp(B)/OR = 6.633, meaning that the nurse who states leadership is in a good category has a 6.6 times higher chance of having a good performance than the nurse who states that leadership is not good. It is recommended that the leadership of the Royal Prima General Hospital Medan to provide guaranteed incentives to all nurses so as to increase satisfaction. Head of the room to demonstrate leadership that is acceptable to all parties. Hospitals should maintain and improve the organizational culture that has been good so far.

Abstract

The need to improve the quality of health services in hospitals by eliminating waste in order to achieve an efficient health service process through a Lean Management approach. Methods using descriptive qualitative analysis where data is compiled, analyzed and then explained through data collection, problem solving, planning improvement ideas and by providing design improvement proposals. The application of this lean hospital reduces the service time for Covid-19 patients at Deli Serdang Hospital by 467.4 seconds in the service flow and the application of this lean hospital can reduce the service time for Covid-19 patients at the Haji Hospital by 377 seconds in the service flow. The conclusion of the study is that the application of the Lean Management approach at Deli Serdang Hospital and Haji Hospital is able to increase the effectiveness and efficiency of health services.

Abstract

Mother's Milk (ASI) is the main and best food for babies that have no equal. The benefits of exclusive breastfeeding are enormous, especially for the baby's growth and development, immunity, psychology, and the economy of the baby's parents. Even though the benefits of exclusive breastfeeding are clear for mothers and their babies, the coverage of exclusive breastfeeding for babies is still low. The purpose of this study was to analyze the increase in exclusive breastfeeding for working mothers using the Health Action Process Approach (HAPA) in Batu Bara Regency. The sample of this research is working mothers who have babies 0-6 months in Batu Bara Regency. The sampling technique is Non-Probability Sampling using Total Sampling, where the number of samples is the same as the population of 100 people. The data processing method used Univariate, Bivariate, and Multivariate tests. Theresults showed that self-efficacy, outcome expectations, risk perception, planning, the effect of intention, maintenance, and planning strategies to overcome barriers had a significant effect on breastfeeding practices for working mothers in Batu Bara Regency as evidenced by the p-value of each variable 0.000 <0.05. The multivariate test results show that together the independent variables have a significant effect on the dependent variable, with the most dominant variable being the maintenance variable with an average value of 10.66. The frequency of exclusive breastfeeding and the duration of breastfeeding has no effect on the practice of breastfeeding for working mothers in Batu Bara Regency.

Abstract

Basic Health Research Report shows that an average prevalence of diabetes mellitus (DM) in urban areas is 5.7% in the population aged over 15 years. Many studies have been conducted to explore the pharmacological effects of snake fruit skin as different preparations against DM. In this study, the effect of snake fruit skin extract (SFSE) on body weight and blood glucose levels induced by alloxan was tested on 25 male Wistar rats. All rats were initially induced with 5% alloxan (150 mg/kg BW) then after 48 hours were grouped into: control (Na-CMC 0.5%), standard (Metformin), SFSE-I (60 mg/200 gBW), SFSE -II (120 mg/200 gBW), and SFSE-III (240 mg/200 gBW). The treatment was given for 14 days. The results showed that the tendency for the highest blood glucose levels of about 333 mg/dl (after 28 days of treatment) was found in the control group, followed by SFSE-I (222 mg/dl), -II (205 mg/dl), -III (138 mg/dl), and the lowest in the standard group was about 129 mg/dl. Hence, it can be concluded that SFSE which has the best antihyperglycemic effect was obtained with the highest dose of 240 mg/200 gBW.

Abstract

The Use of shoes with height-elevating insole lately has been rife in the community. Use sol high this can affect the function of static and dynamic body, where physical changes in the body. As a result, generate a posture which is not stable, not balanced, and less comfortable when used in daily activities. Have many kinds of shoes with high soles with distinctions high pedestal the soles of shoes that allow the existence of different effects for the body. Objective, Prove the change of gait, balance and comfort in use of the shoe with a height-elevating the insole at the height of 5, 7, 9 cm. Material and Methods, experimental Research with pre and post test design. Subjects are men aged 20-22 years with a Body Mass Index (BMI) normal. Thirty people who are willing to follow the study and meet the inclusion and exclusion criteria were divided randomly into 3 groups, group a 5 cm, 7 cm and 9 cm, which respectively amounted to 10 people. Each group performed a walk test to test gait, one leg stands and tandemstand to test the balance with the barefoot/barefoot, shoes with height- elevating the insole with the same high. Results, the Results of this study show a change in gait and balance that are significant between barefoot and wearing shoes high soles (p=0.005). But the difference between the shoes with the height-elevating insole with a height of 5, 7 and 9 cm is not significant on the parameters of gait (p=0.329) but there is a change of balance in between shoes high soles 5, 7, 9 cm on the parameters of the balance (p=0.000). Conclusion, there are differences in the balance and there was no difference in gait on the use of the shoe’s height-elevating insole different (5, 7, 9 cm).

Abstract

Clinical pathways are an important part of documents and tools in realizing good clinical governance in hospitals. In Indonesia, this document is also one of the requirements that must be met in the KARS version of the Hospital Accreditation Standard. However, the process of implementing this Clinical Pathway itself also has various problems. Things that might hinder the implementation of this clinical pathway could come from the staff and hospital members themselves because they have not adapted to this Clinical Pathway. This study aims to analyze the implementation of the Clinical Pathway at the Royal Prima Hospital in Medan. This study uses a quantitative design with a cross sectional approach. The research sample was all doctors, nurses, nutritionists, pharmacists at the Royal Prima Hospital Medan who filled the Clinical Pathway as many as 83 people. The results of the Fisher's Exact test showed that there was a significant effect between regulation (p=0.000), compliance (p=0.004), supporting factors (p=0.025), and inhibiting factors (p=0.002). By using the t-test at a significance level of 0.05, the results obtained that regulation is the variable that has the main influence on the implementation of the Clinical Pathway at Royal Prima Hospital Medan (Sig.0.001). Suggestions to hospitals are expected to strengthen policies regarding the implementation of Clinical Pathway to improve health services in hospitals. Caregivers are expected to maintain compliance with the implementation of the Clinical Pathway to improve health services in hospitals.

Abstract

Since the emergence of the first confirmed case of Covid19 in Wuhan at the end of 2019, much attention has been focused on how the patient's health care system to the development of vaccines. However, the discussion about how the struggle of health workers, especially nurses, seems to have received less attention. Therefore, this study was conducted to find out how the anxiety level of nurses for Covid-19 patients is in terms of the influence of organizational support, social support, workload and personal resilience. The results showed that workload and personal resilience had a positive and significant effect on anxiety levels (p < 0.005) but organizational support and social support had no significant effect on anxiety levels (p > 0.005). From the results of the multivariate test, it was found that the p value was 0.000 (p < 0.005) which stated that together or simultaneously, the independent variables in this study had a significant influence on the dependent variable, namely the level of anxiety.
