Abstract

Berangkat dari permasalahan belum optimalnya penerapan penggunaan obat secara rasional pada pelayanan kesehatan di Indonesia maka telah dikembangkan indikator khusus yang menjadi dasar penilaian kinerja pelayanan kefarmasian di puskesmas. Penelitian ini bertujuan untuk mengevaluasi penggunaan obat di puskesmas wilayah Kota Kupang berdasarkan indikator hasil metode Delphi yang dikembangkan oleh Satibi dkk. Jenis penelitian yang digunakan adalah deskriptif dengan pendekatan kuantitatif, menggunakan data retrospektif tahun 2018 dan melalui observasi langsung. Pemilihan sampel puskesmas dilakukan dengan purposive sampling. Data hasil penelitian berupa data kuantitatif, dianalisis secara deskriptif dan 6 indikator diantaranya dibandingkan dengan standar Kemenkes RI dan WHO. Hasil penelitian menunjukkan bahwa penggunaan obat belum rasional sesuai standar WHO ataupun Kemenkes RI pada indikator item per resep, penggunaan obat generik dan pemberian oralit dan zink sedangkan indikator antibiotik pada diare non spesifik, antibiotik pada ISPA non pneumonia dan penggunaan injeksi pada myalgia telah memenuhi standar yang ada. Indikator biaya obat per kunjungan resep sebesar Rp.9.394 ± Rp.1.341 digunakan sebagai parameter dalam penetapan alokasi dana pengadaan obat tahun berikutnya dan indikator kejadian medication error tidak dapat dievaluasi karena tidak tersedianya dokumentasi tersebut. Kesimpulan menunjukkan puskesmas wilayah Kota Kupang masih mengalami masalah dalam jumlah item obat per resep, penggunaan obat generik, pemberian oralit dan zink dan pendokumentasian kejadian medication error. Hal ini menunjukkan adanya kebutuhan untuk meningkatkan pengetahuan tenaga kefarmasian yang dapat mendukung penggunaan obat rasional di puskesmas.

Abstract

Bacterial meningitis-encephalitis (ME) management therapy is critical to appropriately manage clinical outcomes. This study aims to provide recommendations on appropriate empiric antimicrobial to support the healing period and reduce the risk of disease severity. A cross-sectional study, including inpatients without comorbid diagnosed with bacterial ME, was conducted, and records of antimicrobial prescriptions were obtained. Sociodemographic, clinical (diagnostic), and pharmacological (antimicrobial) variables were assessed. Through multivariate analysis, variables associated with the use of antimicrobials for bacterial infections were identified. A total of 45 patients with ME. The patients, who were from surrounding Center Java and Yogyakarta, had a mean age of 11.27 ± 16.93 years and a male predominance of 56.9% (n = 23). The most frequent bacterial infections were caused by: S. haemolyticus and S. epidermidis (25.93%). A total of 100% the patients (n = 45) received a prescription for empiric antibiotics, predominantly 3rd generation cephalosporin e.c ceftriaxone (35.56%) and cefotaxime (13.33%). Empiric antimicrobials are frequently prescribed for the first management of bacterial ME, are considered an inappropriate practice due to a lack of clinical benefits, increased generation of antimicrobial resistance, and risk of adverse reactions due to the use of medications that patients do not require. Drug utilization studies are a great tool for monitoring how antimicrobial is being used and planning interventions to improve their use.

Abstract

Suatu terapi pengobatan yang tepat akan sangat menguntungkan bagi pasien, terutama bagi pasien yang harus mengkonsumsi obat dalam waktu lama, bahkan seumur hidupnya, seperti penyakit hipertesis. Penelitian ini dilakukan untuk mengetahui efektivitas-biaya penggunaan ACE-inhibitor (ACEI) dibandingkan dengan calcium channel bloker (CCB) pada pasien hipertensi rawat jalan. Penelitian yang bersangkutan merupakan suatu survei epidemiologik, data di ambil secara prospektif untuk mengetahui efektivitas terapi hipertensi dengan ACEI .Pasien hipertesis rawat jalan di RS.Dr.Sardjito Yogyakarta yang mendapatkan terapi dengan ACEI atau CCB, baik sebagai terapi awal maupun terapi tambahan atau pengganti, diobservasi selama 2 bulan. Observasi dilakukan pada kunjungan pertama, satu, dan dua bulan setelah di berikan terapi. Efektivitas ditentukan dengan mengukur perubahan tekanan darah setelah menggunakan ACE dan CCB. Parameter toleransi yang diukur adalah jumlah dan timbulnya efek obat yang tidak dikehendaki oleh pasien. Biaya yang di ukur adalah biaya medik langsung, meliputi biaya obat (test dan medikasi yang di lakukan), biaya kunjungan, dan biaya perawatan terkain dengan hipertesis, termasuk biaya efek samping. Hasil penelitian menunjukkan pada pasien hipertensi tanpa penyulit, calcium channel bloker lebih cost-effective, sedangkan pada pasien hipertensi dengan diabetes melitus, ACE- inhibitor lebih cost-effective.

Abstract

Diabetes mellitus menunjukan ketidakmampuan tubuh dalam memproduksi insulin atau dalam pengaturan kadar gula darah pada tingkat normal di saat kondisi tersebut menjadi lebih buruk, dapat ditandai dengan terjadinya hiperglikemia yang cepat, aterosklerosi, mikroangiopati, dan neuropati. Penelitian ini merupakan penelitian deskripstif yang dilakukan menggunakan metode retrospektif dengan melihat catatan rekam medis dari pasien rawat inap yang menderita diabetes mellitus tipe II di Rumah Sakit Panti Rapih pada tahun 2004. Selanjutnya dilakukan deskripsi mengenai penggunaan antidiabetes. Penelitian ini dapat digunakan oleh peneliti lain sebagai evaluasi penggunaan antidiabetes dan studi farmakoepidemiologi. Hasil penelitian ini menunjukan bahwa dari 186 pasien diabetes mellitus tipe II, terdapat 146 pasien yang didiagnosa menderita diabetes mellitus tipe II dengan penyakit penyerta lainnya. Penggunaan antidiabetes yang terbanyak adalah golongan sulfonilurea yaitu sebanyak 164 kasus (88,17%). Biguanida digunakan oleh 119 pasien (63%) dan insulin sebanyak 94 kasus atau 50,54%. Antidiabetes tersebut digunakan sebagai obat tunggal maupun sebagai obat kombinasi.

Abstract

Drugs are one of the important components of the Back-Referral Program (BRP), so the guarantee of the availability of BRP drugs in pharmacies is a factor in the success of BRP implementation. The purpose of this study was to determine the consumption of antidiabetic drugs using ATC/DDD and 90% DU methods, analyze the cost of consuming antidiabetic drugs, and determine the suitability of antidiabetic drugs with the National Formulary. This study was an observational study with a cross-sectional design and retrospective data collection. The study was conducted at 6 BRP pharmacies, namely Pharmacy A, Pharmacy B, Pharmacy C, Pharmacy D, Pharmacy E, and Pharmacy F in the area of Demak Regency. The antidiabetic drug consumption data was obtained from BRP patient prescriptions from July 2020 to June 2021. The cost data were obtained based on prices in the JKN drug e-catalog system at BRP pharmacies in the area of Demak Regency. In analyzing the data, the researcher calculated DU at 90%, the cost of drugs included in the DU segment at 90%, and the suitability of drugs with the National Formulary. The results showed that the highest consumption of antidiabetic drugs in Back-Referral Program (BRP) pharmacies in the area of Demak Regency was glimepiride (54.72%) and metformin (31.01%). The highest cost of consuming antidiabetic drugs per DDD was insulin (Rp.17,639.73), the lowest cost was glimepiride (Rp.65.35), the total cost of antidiabetic drugs was Rp.53,509,090 and an incompatible drug with National Formulary was pioglitazone. In general, the highest consumption of antidiabetic drugs in Back-Referral Program (BRP) 6th pharmacies in the area of Demak Regency, which was included in the DU segment 90%, were glimepiride, metformin and the suitability of drugs with the National Formulary has not reached 100%.

Abstract

ABSTRACTDiabetes Mellitus is a chronicle disease that gives effect to patient well being. Moreover, a complication suffering gives negative effect to the patient. The aim of this research is to identify the life quality of Diabetes Mellitus (type 2) sufferer and the characteristic relating to it quality of life. This research applied descriptive design and performed on the visitors of Policlinic of the interior disease on Tidar Hospital of Magelang during Juli – August 2007. The inclusion criterion is the visitors when this research performed, entering 18 years old or more, of Diabetes Mellitus Type 2, any agreement of informed consent and filled up the questionnaire. The sample taking is done by accidental sampling. To measure the life quality is used a questionnaire of Diabetes Quality of Life (DQOL), then it continued by measure the quality of life of any patients that involved therapeutic satisfaction, the effect of therapy, the anxiety of diabetes, the anxiety of social problem and the quality of life in general. Every question is measured by the Likert Scale of 5. Of the 50 analyzed patients, 46% is female and 54% is male, with the biggest rate of age is 60-69 years old.The treatment of DM type 2 in Tidar Magelang general hospital for 50 patients who do treatment in the house have good rate life of score. It is 0,599 (it means the quality of life is higher). The lowest score of therapeutic effect is 0,588. Moreover, the highest score of anxiety to the diabetes is 0,616. The male patients and the patient with long duration show the low quality of life. Accordance with the correlation test of Spearman and Kendall is obtained that there was no significant correlation between the patient characteristic and it quality of life. Abstract

ABSTRACTThe research about rationality of using over the counter drugs common cold for self medication in district of Gajahmungkur Semarang has been carried out. Data gotten from the result of field survey which is using the questioners that has been filled by Gajahmungkur’s people. Data will be analyzed with non analytical descriptive method. Based on the survey result, knowing that people in Gajahmungkur’s district choosing OTC drugs especially analgetic, antihistamin and decongestan (66,4%). The evaluation of rationality of using drugs for common cold has result: 1) Drugs of choice for common cold is correct. 2) Correct patient are 92,1%. 3) Correct dosage form 96,0%. 4) The most adverse drugs happens is sleepy (53,4%). Abstract

ASTRACTDiabetes melitus is metabolic disease characterized hiperglikemia that caused disparity of insulin secretion, insulin activity or both. WHO had predicted there were increasing of diabetes mellitus patients in next time, included in Indonesia. This study aim to know and evaluate the rational of in patients diabetes mellitus type 2 medication in Bhakti Wira Tamtama Hospital Semarang at 2006. This study is non experimental research that analyzed by non analytical descriptive method. Data were collected retrospectively from medical record comprised patient identity, diabetes melitus type 2 medication, and rational antidibetic usage. Data obtained were analysed with Standard of Perkeni 2006. The result of this study showed that 34 in-patient diabetes mellitus type 2 in Bhakti Wira Tamtama Hospital Semarang at 2006 were 40-50 years old (47,06%). Diabetes mellitus type 2 patients comprised 18 men and 16 women, 8 cases (23,52 %) were diabetes mellitus type 2 without additional diseases and 26 cases (76,47%) diabetes mellitus type 2 with additional diseases. Patients had diagnosis appropriate were 94,12 %, drug choise appropriate were 100 %, which biguanid metformin usage were 15 patients (46,87%). Dosis appropiate 100%, patient appropriate 100%, while drug interaction had happened to 1 case, that was antidiabetic and thiazid diuretic. Abstract

This research to determine the average cost of illness of inpatient pediatric meningitis patients hospitalized in the hospitals in Yogyakarta. It was a pharmacoeconomic study using the cost of illness method. The calculation of the cost in the care of inpatient pediatric meningitis patients according to a societal perspective was conducted observatively with a cross sectional design. Cost of illness analysis included calculating the costs of direct medical, direct non-medical, and indirect cost. Data were collected from a type-A public hospital and a type-B privatehospital in Yogyakarta by taking medical records, detailed data on meningitis patient care costs, and interviews using questionnaires with caregivers. Analysis of cost of illness calculations applied descriptive analysis methods. In this study, 11 inpatient pediatric meningitis patients were obtained. The average cost of illness for the treatment of inpatient pediatric meningitis patients was IDR 26,224,586±12,814,789, consisting of the average direct medical cost of IDR 23,831,813±12,222,885 (90.88%), the average direct non-medical cost of IDR 1,787,955±832,353 (6.82%), and the average indirect cost of IDR 604,818±337,389 (2.31%). Components of direct medical cost with the largest percentage were drugs and medical supplies (26.46%); while the largest direct non-medical cost were the expenditure of additional needs for the patient's family (72.18%). The indirect cost component came from the reduction of the patient's parents' income while the patient is hospitalized.

Abstract

Hemodialysis (HD) patients with chronic kidney disease takes a long time and requires patient compliance. This will give the patient a psychological stressor which will then affect the quality of life. Apart from having an effect on quality of life, therapy hemodialysis also used a high cost, especially if the patient has complications. The purpose of this study was to determine the amount of hemodialysis costs, utility index, and the effect of complications on costs and utility index in hemodialysis patients. This study was a cross-sectional analytical research with the perspective of hospital. The research subjects were outpatient CKD who underwent hemodialysis at the HD Unit of Adji Batara Agung Dewa Sakti Hospital in September-Oktober 2020. Data were obtained use the SF-6D instrument, medical records, and financial financing. Data analysis used independent sample t-test and Kruskal-Wallis.The results showed that the average real cost of therapy in hemodialysis patients for a month was Rp. 12,014,488±0, while the real cost per hemodialysis episode is Rp. 1,316,722±273,989. The mean value of patient utility index as measured by the SF-6D was 0.717±0.143. The statistical test results show a significant value of 0.049 <0.05 on the utility index, which means that there is a relationship between complications on the utility index and a significant value of 0.125> 0.05 on costs, which means that there is no relationship between complications and costs in CKD patients with hemodialysis Keywords: Hemodialysis, Cost analysis, Index utility, SF-6D
