Abstract

The aim of this study was to analyse the risk factors of hypertension among adult in rural Indonesia. This cross-sectional study included 112 married couples (112 men and 112 women) aged 45-59 years old and was conducted in Cianjur District, West Java, Indonesia. The measurement of body weight and fat composition used Bioelectrical Impedance Analysis (BIA) monitor (OMRON® Karada Scan Body Composition Monitor HBF-358-BW). Body height was measured by stadiometer, meanwhile waist circumference was measured by measuring tape. Data on the blood pressure measured by an automatic blood pressure monitor (OMRON® Automatic Blood Pressure Monitor Model HEM-7200). All measurement were done twice in repetition. Chi-square was used to analysed the association between variables and logistic regression was used to analyse the risk factors. The prevalence of hypertension was 49.1% (women 54.5%; men 43.7%). Women had a higher proportion of overweight, obesity, abdominal obesity, and hypertension stage 2 than men. BMI (p<0.05), waist circumference (p<0.05), total body fat (p<0.05), and visceral fat (p<0.05) were significantly higher in hypertension subject than not hypertension. Nonetheless, only waist circumference became the risk factor for hypertension (OR=4.005; 95%CI:1.47-10.914 for high risk). There is high prevalence of hypertension among adult in rural area. Adult, particularly women need to pay attention on their body fat to reduce the risk of hypertension.

Abstract

The purpose of this research was to analyse nutritional status, physical activity, and sedentary activity in school children in West Java, Indonesia. The study used a cross-sectional design and was conducted from October to December 2016. Samples were taken using simple random sampling technique with a total sample of 300 subjects taken among fifth-grade students in seven elementary schools located in Bandung city, West Java. The data collected in this research were anthropometric data such as physical activity and sedentary activity. Data analysis was carried out using descriptive statistics, analysis of variance (ANOVA) and Pearson test. The results showed that 21.7% of school children were overweight, while 19.7% were obese. The results of the subject activity, based on nutritional status category, were not significantly different. All subjects with underweight nutritional status were having light activity (100%), the percentage of subjects with normal nutritional status, overweight, and obese were 71.6%, 75.4%, and 74.6%, respectively. The average duration of underweight subjects performing sedentary activities on holidays was 4.2±3.0 hours, 5.4±2.7 hours for normal subjects, 5.8±2.7 hours for overweight, and 5.4±2.5 hours for obese subjects. The proportion of sedentary activity time on holiday among underweight, normal, overweight, and obese subjects were 17.6±12.4%; 22.3±11%; 24.1±11.3%; and 22.3±10.5%. The sedentary activity of children on holiday was not significantly different, but there was a significant difference between the proportion of sedentary time in a day between groups of nutritional status (p<0.05), i.e., normal children and overweight children. The correlation test results indicated a negative correlation between physical activity and sedentary activity, meaning that higher sedentary activity of subjects both in school time and holiday resulted in lower physical activity (p<0.01). The nutritional status, physical activity, and subject sedentary activity based on nutritional status were not significantly different.

Abstract

This study aimed to investigate the prevalence and determinants of hypertension in rural women of reproductive age. The cross-sectional study was conducted in Cianjur district on August 2019. It involved 193 married rural women aged 20–49 years old. Independent variables investigated were socio-economic characteristics, anthropometry, fat distribution and nutrient intake. The prevalence of hypertension in the study population using the new American Guideline (ACC/AHA) was 58% in contrast to 23.8% using the European (ESC/ESH) cut offs. Socioeconomic characteristics and nutrient intake showed no significant association with hypertension (p>0.05) while, Body Mass Index (BMI) (p>0.012), Visceral Fat (VF) (p>0.013) and Waist Circumference (WC) (p>0.010) were significantly associated with hypertension. Hence, result of binary logistic regression showed waist circumference is the strongest factor to determine hypertension in our study (OR=2.2; 95% CI: 1.20–4.01). Overall, the use of the American guideline increases the sensitivity of hypertension screening. The use of simple anthropometry measurement of WC combined with BMI can be applied for risk screening of hypertension in the primary health care setting including in the integrated community health post to improve preventive measure. Due to several limitations in the current study, future study should consider larger sample size and addresses history of hormonal contraceptive use as well as physical activity

Abstract

Metabolic syndrome (MetS) is a growing health concern in Indonesia, particularly in rural communities with limited healthcare access. Behavioral risk factors such as smoking, coffee consumption, and poor health-seeking behavior (HSB) may increase MetS risk in vulnerable populations. This study aimed to examine the association between lifestyle behaviors—smoking, coffee consumption, and HSB—and the risk of MetS among fishermen and oil palm farmers in Aceh, Indonesia.  A case-control study was conducted among 240 adults, comprising 120 couples,  recruited from rural fishing and farming communities. Cases met the IDF 2020 MetS criteria, while controls did not. Data on sociodemographics, smoking, coffee intake, and HSB were collected via structured interviews and clinical assessments. Multivariate logistic regression was used to identify independent risk factors, adjusting for age, gender, and education. Smoking among husbands (AOR = 2.89; 95% CI: 1.19–7.02; p = 0.019), coffee consumption (AOR = 2.40; 95% CI: 1.00–5.79; p = 0.049), and low HSB (AOR = 3.10; 95% CI: 1.35–7.11; p = 0.008) were significantly associated with higher odds of MetS. While age and education were not independently significant, they were retained in the model as potential confounders. This study highlights the need for tailored health promotion targeting smoking cessation, improved health service engagement, and dietary awareness in rural Indonesian populations. Interventions should consider sociocultural practices such as sweetened coffee consumption to reduce MetS risk effectively.

Abstract

Background: At present, many female adolescents experience faster growth and faster development phase, including menstruation (menarche). According to 2010 Riskesdas data, 21.3 % of female adolescents in Jambi experienced menarche at the age of ≤12 years, and disorder during menstrual cycle often experienced by female adolescents is premenstrual syndrome (PMS) with a prevalence ranging from 80-90 %.  Objective: This research was conducted to analyze any differences from energy intake, nutritional status, age at menarche, and PMS incidence along with finding dominant factors of age at menarche and PMS in female adolescents in urban and rural areas of Jambi.Method: This research employed a cross sectional study design involving 200 female adolescents selected from State Junior High schools in urban and rural areas of Jambi province by a simple random sampling technique. The energy intake data collected by 2 x 24 food recall, whereas nutritional status data was collected through anthropometric measurements using Bioelectrical Impedance Analysis (BIA) and a microtoise, and age at menarche and PMS data were collected by a questionnaire. Bivariate data analysis was taken by Independent t-test, Mann-Whitney test and Spearman Rank test, while the multivariate data analysis was taken by the Logistic Regression.    Results: There were significant differences found in age at menarche, energy intake, and nutritional status (HAZ and BAZ) of female adolescents in urban and rural areas (p<0.05). A significant negative relationship was found between nutritional status and age at menarche (p<0.05) and a significant positive relationship was found between nutritional status and the incidence of PMS (p<0.05). The Logistic Regression Test revealed that residence area was the dominant factor related to age at menarche (OR: 2.591 CI 95%: 1.415-4.744) and body fat percentage was the dominant factor related to the incidence of PMS (OR: 2.468 CI 95%: 1.159-5.254) .Conclusion: There are differences found in age of menarche, energy intake, and nutritional status (HAZ and BAZ) between female adolescents from urban and rural areas. There is a significant relationship between nutritional status and age at menarche to the incidence of PMS. Residential area becomes the dominant factor related to age at menarche and body fat percentage becomes the dominant factor related to the incidence of PMS. KEYWORDS: age at menarche; energy intake; female adolescents; nutritional status; PMS

Abstract

Introduction: Food beliefs and cultural practices have a significant influence on food choices, recommendations, and taboos, with variations shaped by the context of ecological, social, and local knowledge. Rural populations generally adhere to long-established traditional practices, whereas urban environments undergo more rapid dietary transitions driven by globalization and the commercial food industry. This cultural framework shapes individual interpretations of nutritional information, often leading to the rejection of health messages that contradict established belief systems. In Indonesia, considerable ethnic and geographical diversity gives rise to culturally embedded diets that can impact nutritional status; however, the dynamic interactions between food choices, recommendations, and taboos remain insufficiently studied. To address this gap, a qualitative study that explores regional variations in food beliefs and practices in Indonesia, focusing on urban–rural differences across diverse socio-ecological settings was conducted. Methods: In-depth interviews with 28 respondents and focus group discussions with 42 respondents were conducted in four provinces (West Java, Central Sulawesi, East Kalimantan, and West Nusa Tenggara) under the Socio-ecological Model (SEM) framework to investigate how dietary choices, food recommendations, and food taboos are shaped. The study was conducted in 2022. Ethical approval was obtained and all participants provided consent. Results: The findings reveal three main insights: (1) a paradoxical decoupling between dietary diversity and nutritional outcomes, wherein greater dietary diversity in Eastern Indonesia does not directly correlate with better nutrition outcomes; (2) food recommendations for mothers and infants vary, with urban areas favoring commercial products and rural areas relying more on local knowledge; and (3) food taboos persist across regions, particularly in rural Eastern Indonesia, often affecting infant and maternal nutrition. Conclusion: This study contributes to a nuanced, culture-sensitive framework to inform health communication and nutrition policies in Indonesia. These findings highlight the need for culturally sensitive and context specific nutrition policies.

Abstract

Latar Belakang: Budaya berpengaruh terhadap keputusan dan perilaku terkait gizi-kesehatan suatu masyarakat adat.  Seperti yang diketahui perilaku gizi-kesehatan yang salah dapat berdampak buruk seperti kurang gizi bahkan sampai mengakibatkan kematian ibu dan anak.Tujuan: Penelitian ini bertujuan untuk menganalisis tingkat pendidikan dan perilaku gizi-kesehatan ibu balita pada masyarakat adat Samin di Jawa Tengah.Metode: Penelitian deskriptif dengan mixed method ini melibatkan 88 ibu balita masyarakat adat Samin di Kabupaten Kudus, Pati, dan Blora, Jawa Tengah. Tingkat pengetahuan, dan persepsi gizi-kesehatan diperoleh dari hasil persentase jawaban benar pada kuesioner yang kemudian di kategorikan ke dalam tiga tingkatan. Uji korelasi Spearman juga dilakukan untuk mengetahui hubungan antar variabel.Hasil: Hasil menunjukan 98% ibu balita pada kelompok Kudus-Pati tidak bersekolah sedang 68,4% ibu balita di Blora telah mengenyam pendidikan dasar. 50% ibu balita di Kudus-Pati memiliki tingkat pengetahuan yang rendah, namun 55,3% ibu balita di Blora memiliki tingkat pengetahuan sedang. 68% ibu balita di Kudus-Pati memiliki persepsi negatif, namun 73,7% ibu balita di Blora telah memiliki persepsi gizi-kesehatan yang positfi. Uji beda juga menunjukan perbedaan signifikan (p<0,05) pada kedua kelompok terkait dua variable tersebut. Uji korelasi juga menunjukan adanya hubungan antara tingkat pendidikan, pengetahuan, dan persepsi ibu balita (p<0,05).Simpulan: Simpulan yang dapat diambil dari hasil penelitian ialah terdapat perbedaan antara tingkat pendidikan, pengetahuan, dan persepsi ibu balita antar kedua kelompok di mana ibu balita di Blora lebih baik dibanding ibu balita di Kudus-Pati.

Abstract

Low birth weight (LBW, <2,500 g) remains a critical public health problem, contributing to neonatal mortality and long-term risk of non-communicable diseases, and national estimates in Indonesia indicate a prevalence ranging from approximately 6.2% to 11.7%. Parity and interpregnancy interval (IPI) are important reproductive determinants of perinatal outcomes, and their effects may be modified by socioeconomic status (SES). This study aimed to investigate the association between parity and IPI with LBW and to assess whether SES modifies these associations in a prospective cohort of pregnant women in Yogyakarta, Indonesia. A total of 120 women (poor households n=48; non-poor households n=72) were recruited purposively from community health centers and a maternity hospital/clinic. Inclusion criteria were gestational age 26–35 weeks, maternal age 20–35 years, parity 1–2, non-smoker, and provision of informed consent. Pre-pregnancy BMI was obtained through maternal recall of the last measured height and weight before conception. Data were analyzed using Pearson’s χ² tests for baseline comparisons and stratified logistic regression models to estimate adjusted odds ratios (aORs) with 95% confidence intervals. The prevalence of LBW was higher among women with non-optimal IPI (<24 or >60 months and primigravida) compared to those with optimal IPI (24–60 months). Multiparity showed a protective tendency against LBW, particularly among non-poor women. SES significantly modified the association between IPI and LBW (p for interaction <0.05). These findings suggest that multiparity may reduce LBW risk, whereas non-optimal IPI increases it, with SES acting as an effect modifier. Strengthening preconception counseling on birth spacing and targeted support for low-SES families are warranted

Abstract

Stunting remains a major public health problem, particularly in coastal communities where poverty and limited dietary diversity are prevalent. Children in fishing families are vulnerable due to low household income, inadequate food intake, and exposure to infections. This study aimed to identify and analyze the risk factors associated with stunting among children aged 6–23 months in fishing families in Brebes Regency, Central Java. A matched case-control study was conducted from April to May 2025 in three coastal villages (Kluwut, Grinting, and Pulogading), Bulakamba Subdistrict, Brebes. A total of 132 subjects were enrolled, consisting of 44 stunting cases and 88 controls matched by age. Data were collected on child, maternal, and household characteristics, dietary intake, and health status. Statistical analyses included Chi-square test and Binary Logistic Regression to determine significant risk factors (p<0.05). The results showed that children with a history of pulmonary TB had an 9.622 times higher risk of being stunted (OR = 8.120; 95% CI: 1.009–91.735; p = 0.049). Inadequate energy intake (≤89% RDA) had an 8.12 times higher risk of stunting (OR = 8.120; 95% CI: 2.552–25.839; p = 0.001). Birth length <48 cm increased the risk of stunting by 4.105 times ((OR = 4.105; 95% CI: 1.593-10.575; p = 0.003). Meanwhile, age 6-11 months showed a protective effect (OR=0.343; 95% CI: 0.135-0.869; p=0.024).

Abstract

Poor dietary patterns and poor nutritional status increase the risk of morbidity and mortality. Obesity and overweight are major nutritional issues in Kediri. The family environment, particularly the mother's role as a food gatekeeper, is crucial for shaping children's eating habits. This study applies Social Cognitive Theory (SCT), which emphasizes the interaction between individuals, behavior, and the environment. Social media platforms, such as WhatsApp, can be utilized to promote health. This study examined the effect of SCT-based nutrition education via WhatsApp on mothers' knowledge, self-efficacy, self-regulation, family support, and outcome expectations in improving children's dietary intake. A pre-post quasi-experimental design was used with 80 mothers of elementary school children in Kediri City. The intervention group (n=40) received WhatsApp-based nutrition education for 20 days, whereas the control group (n=40) received an e-booklet at the end of the study. Data were analyzed using Spearman’s test, Mann-Whitney test, and Wilcoxon Signed Rank test. The results showed that WhatsApp-based nutrition education significantly improved knowledge (p=0,005) and outcome expectations (p=0,039) in the intervention group, whereas no significant effects were observed on self-efficacy and self-regulation. WhatsApp-based nutrition education effectively increased mothers' knowledge and outcome expectations. Future studies should consider the duration of the intervention, including follow-up assessment, refining educational media, combining educational methods, and considering additional SCT components.
