Abstract

Background: Ulcerative colitis (UC), a chronic inflammatory disease that can cause bloody diarrhea, remains a major global disease burden. While Helicobacter pylori infection is postulated to be able to reduce the occurrence of UC, its role in the disease itself remains contentious. Hence, this meta-analysis aims to examine whether H. pylori infection can lower the chance of developing UC.Method: A systematic search was conducted through three electronic databases, namely Cochrane, PubMed, and Embase, with the addition of individual hand searching to analyze the association between ulcerative colitis and H. pylori infection in the adult population. Relevant articles selected through eligibility criteria were assessed for quality by using the Newcastle-Ottawa Scale. Furthermore, a random-effects meta-analysis was conducted to estimate the pooled odd ratios (ORs) along with their 95% confidence intervals (CIs). Higgins test  and funnel plots were also conducted.Results: A total of 11,498 patients with UC and 356,130 controls from 22 studies were included in the meta-analysis. Included studies showed fair or good quality. Good quality was achieved with the minimum score of 3 stars for selection, 1 star for comparability, and 2 stars for outcome/exposure, while fair quality was achieved with the minimum score of 2 stars for domain, 1 star for comparability, and 2 stars for outcome/exposure. Our findings indicated that H. pylori infection was associated with lower odds of UC [pooled ORs 0.51 (95% CI: 0.46-0.56)]), albeit moderate heterogeneity (I2= 54%, p = 0.002). Furthermore, publication bias was not found.Conclusion: The present study adds to the growing body of evidence supporting the potential protective effects of H. pylori infection on the occurrence of UC. However, further primary research with prospective study design needs to be conducted to confirm our findings.

Abstract

Background: Endoscopic retrograde cholangiopancreatography (ERCP) alone may not be sufficient for treating difficult common bile duct stones. To address this challenge, several other modalities exist, including extracorporeal shock wave lithotripsy (ESWL) and biliary stent placement. This study aims to investigate the effectiveness of ESWL following biliary stent placement, in comparison to biliary stent placement alone , in the treatment of difficult common bile duct stones.Methods: A retrospective cohort study was conducted using medical record data from the Gastrointestinal Endoscopy Center Registry of Dr. Cipto Mangunkusumo General National Hospital. This study evaluated 126 subjects with difficult bile stones, who were divided into two groups: one group received biliary stent placement alone, while the other underwent extracorporeal shock wave lithotripsy (ESWL) following biliary stent placement. The effectiveness of each treatment was assessed based on the rate of complete clearance of bile stones.Results: Of the total participants, 72 underwent ESWL following biliary stent placement, while 54 received biliary stent placement alone. The rate of complete stone clearance was 69.1% in the ESWL group and 64.8% in the stent-only group. However, the addition of ESWL as an adjuvant therapy did not result in a statistically significant improvement (p = 0.703; 95% CI: 0.816–1.351). Normal body mass index was a factor that influences the effectiveness of ESWL after biliary stent placement (p = 0.002).Conclusion: The ESWL procedure as adjuvant therapy following biliary stent placement has the same level of effectiveness as biliary stent placement alone.

Abstract

H. pylori is a common human pathogen and it is estimated that approximately 50% of the world's population are infected. Furthermore it's prevalence infection in Indonesia is 20% but much higher among several ethnic groups (Papuans 42.9%, Batak 40.0%, and Bugis 36.7%). H. pylori’s growth and survival has been shown to be sensitive to a variety of antimicrobial agents. The success of the treatment depends on susceptibility, dosage, formulation, dose frequency, the use of adjuvants such as anti-secretory drugs, antacids or probiotics, and duration of treatment. The treatment for H. pylori infection keep evolving and the triple therapy, levofloxacin, was replaced by CLR in triple therapy for 14-day with eradication rates over 90%. Sequential therapy, also achieved a higher cure rate against clarithromycin-resistant strains than a 7 and 10 day triple therapy. Triple bismuth therapy and quadruple bismuth therapy are used less frequently due to their inherent complexity, the large number of tablets four times a day, side effects, and lack of support from pharmaceutical companies. Inclusively, vonoprazan is also a good choice that is fully effective from day one. The role of the probiotics is unclear and is not recommended in consensus groups. Two mucolytic agents, erdosteine and N-acetylcysteine (NAC), were found to increase it's eradication efficiency clinical trials when administered in supplementation with triple therapy but are not commonly used because of the need for high doses, and increased medical costs. therefore, H. pylori resistant management should be adapted to the results of the the culture of resistance and the guidelines of existing resistance patterns.

Abstract

Background:  Inflammatory bowel disease (IBD) is idiopathic disease characterized by chronic inflammation of the gastrointestinal tract. Polysaccharide peptide of Ganoderma lucidum mycelium extract (PPGL) is considerably a good option for adjunctive therapy of IBD. This study aims to evaluate the benefit of PPGL in patients treated with 5-ASA.Method: A retrospective observational cohort study was conducted to examine the medical records of 124 ulcerative colitis patients. There were 80 patients in intervention group who were treated with a combination of 5-ASA and PPGL, and there were 44 patients in the control group who were treated with 5-ASA only. Clinical and laboratory endpoints were observed at the baseline and after 30, 60, and 90 days. Clinical endpoints included abdominal pain, bloody diarrhea, aphthous stomatitis, and polyarthritis; meanwhile, laboratory endpoints included hemoglobin level, ESR, CRP, fecal calprotectin, M2-pyruvate kinase (MP2K), fecal culture, C. difficile culture, and colonoscopy results. Clinical trials conducted after patient enrollment with registry number ClinicalTrials.gov NCT04029649.Results: On day-30 there was a significant difference between intervention group 45.6%  and control group 2.3% in abdominal pain complaints (p 0.001). Moreover, laboratory parameters of fecal calprotectin (p 0.001), fecal MP2K (p = 0.015), and hemoglobin (p 0.001) were considerably better in intervention group on day-30. These differences were consistently found on day 60 and 90.Conclusion: The study implies potential correlation between PPGL administration and improvement of clinical and laboratory endpoints up to 90 days. A larger randomized, blinded, prospective study is required to confirm these effects in ulcerative colitis.

Abstract

Background: Inflammatory bowel disease (IBD) is an autoimmune disease that is influenced by food, an important factor in accelerating its clinical disease activity because of intestinal inflammation trough formation of antigen-antibody complex. Food-specific IgG examination can identify the types of person foods consumes that are maybe responsible for disease activity. It is useful in treating IBD without risking malnourishment as it is tailored to the individual immune profile.Method: This is a cross-sectional study involving 113 patients diagnosed with IBD by colonoscopy. Examination of serum IgG specific for 220 types of foods was performed using ELISA and immuno-array techniques. Disease clinical activity was assessed using the Mayo index and Crohn's disease activity index.Results: The highest proportion of dietary IgG in Crohn's disease was peas (100%), barley (97.9%), eggs (95.9%), milk (81.6%), and corn (75.5%); while in ulcerative colitis it was barley (98.4%), peas (96.8%), egg whites (92.2%), corn (82.8%), and prunes (78.1%). In ulcerative colitis, there was a weak negative correlation between cashew nuts IgG (r = -0.347; p = 0.041) and chickpeas IgG (r = -0.473; p = 0.017) with clinical disease activity; while in Crohn's disease, a weak positive correlation with disease activity was seen in barley (r = 0.261; p = 0.042).Conclusion: There was a weak negative correlation between cashew and chickpea-specific IgG antibodies with clinical activity of ulcerative colitis, and a weak positive correlation between barley-specific IgG antibodies and Crohn's disease clinical activity.

Abstract

The pancreas is a complex organ in which a suspicious lesion can appear which can be benign or malignant. Clinical manifestation can guide the clinician to choose the appropriate tests such as autoimmune tests, tumor markers, imaging with ultrasonography (USG), computerized tomography (CT), magnetic resonance imaging (MRI), or magnetic resonance cholangiopancreatography (MRCP), and endoscopic or surgical tissue biopsy. Pancreatic nodule therapy is tailored to the nature of the nodule, etiology-appropriate therapy for benign nodules or surgery, chemotherapy, and radiotherapy for cancerous nodules. Knowledge of the diagnostic approach of pancreatic solid mass is important for clinician to make prompt and accurate treatment for patients and to prevent unnecessary examination or intervention.

Abstract

Susceptibility to infection and increasing antibiotic resistance put burn patients at risk of developing infections caused by multidrug-resistant organisms (MDRO). This condition can progress to sepsis, increasing morbidity and mortality. This retrospective cohort study employed the medical record data of patients treated at Public Hospital X in Jakarta, Indonesia, from January 2020 to June 2022. Of 160 subjects, most were aged <60 years (82.5%) and had comorbidities (16.88%). The most common cause of burns was fire (86.25%). The use of medical devices was 90.63%, with a 14-day median length of stay. The most common gram-negative MDRO pathogens were K. pneumoniae (29.91%), Enterobacter sp (22.32%), and Acinetobacter (20.54%); 45% of patients infected with MDRO died. The bivariate analysis found an increased risk of death due to MDRO infection in burn patients (RR 1.103; 95%CI 1.004-1.211, p-value = 0.046). After adjusting for role variables (age, comorbidities, total body surface area, use of medical devices, length of stay) and from multivariate analysis, the confounding variables for MDRO infection and mortality were length of stay and age. MDRO infection increases the mortality rate in burn patients. Mortality in burn patients due to MDRO infection is greater than non-MDRO.

Abstract

BACKGROUND Decreasing the quality of life (QoL) of patients with inflammatory bowel disease (IBD) will increase morbidity and mortality. A valid and reliable instrument is needed to assess the QoL of patients with IBD. This study aimed to analyze the validity and reliability of the Indonesian version of the 9-item Inflammatory Bowel Disease Questionnaire (IBDQ-9). METHODS This cross-sectional study was conducted using the Indonesian version of the IBDQ-9 in adult patients with IBD at the Gastroenterology Outpatient Clinic, Cipto Mangunkusumo Hospital, Jakarta, in November 2022. Patients aged 18 to 59 years who had experienced IBD for at least 2 weeks and provided informed consents were included. The total score of the IBDQ-9 Indonesian version was compared with the 36-item Short-Form Health Survey (SF-36) using the Spearman's correlation test. Reliability tests were examined using Cronbach’s alpha and the intraclass correlation coefficient (ICC). RESULTS A total of 124 participants were included in this study. The Spearman's test showed a high and significant result for the IBDQ-9 Indonesian version and the SF-36 (r = 0.769 and p<0.001). Cronbach’s alpha and ICC coefficient were equal to 0.883. CONCLUSIONS This study provided evidence of the good validity and reliability of the IBDQ-9 Indonesian version for assessing the QoL of patients with IBD in Indonesia.

Abstract

Background: HER2 is a proto-oncogene and therapeutic target in advanced gastric adenocarcinoma. Reports on its association with clinicopathological features and prognosis remain inconsistent. This study aimed to evaluate the relationship of clinical, endoscopic, and histopathological features with HER2 expression and its impact on two-year survival.Methods: A retrospective cohort study was conducted among patients ≥18 years with newly diagnosed gastric adenocarcinoma at four hospitals in Jakarta (2015–2019). Eligible cases had complete records and paraffin blocks. HER2 was assessed by immunohistochemistry and CISH using ToGA criteria. Chi-square/Fisher’s tests evaluated associations, while survival was analysed with Kaplan-Meier and Cox regression. Results: Among 122 patients, 15 (12.3%) were HER2-positive. More frequent expression was observed in liver metastases than in non-liver metastases (17.6% vs. 10.2%), in Borrmann type I/II than in Borrmann type III/IV (16.1% vs. 9.1%), in moderately rather than in poorly differentiated tumors (14.3% vs. 11%), and in intestinal-type rather than in diffuse/mixed-type tumors (13.9% vs. 10%); however, none were statistically significant. Median survival was 5 months in HER2-positive patients and 4 months in HER2-negative patients. HER2 was not significantly associated with two-year survival (HR [95% CI] = 1.12 [0.61, 2.06]). Mortality in patients with palliative therapy was higher than that of those with definitive/therapeutic therapy (76.8% vs 75.8%; HR = 1.547 [1.022, 2.343]). The combination of resection and chemotherapy intervention provided a protective effect on mortality with HR = 0.456 [0.219, 0.95]. There was a significant association between resection-chemotherapy combination therapy and survival in subjects with negative HER2 expression (HR = 0.409 [0.176, 0.947]. Conclusion: In this study, HER2 expression in gastric adenocarcinoma was 12.3%. HER2 expression tended to be higher in liver metastases, Borrmann type I/II, moderate differentiation, and intestinal type histopathology results. Combination therapy with resection and chemotherapy had a protective effect on mortality in this study. Median survival in HER2-positive patients was higher than in HER2-negative patients, but HER2 expression status was not a prognostic factor for two-year survival in gastric adenocarcinoma

Abstract

The management of ulcerative colitis is primarily focused on the prevention of complications, remission sustenance, and enhancement of overall quality of life. However, long-term use of various pharmacological therapies can be associated with several side effects, including myelosuppression, hepatitis, lupus-like syndrome, and an increased risk of malignancy. These limitations have led to the search for safer therapeutic options, including bioactive compounds such as Beta-1,3/1,6-D-Glucan with immunomodulatory properties. Ganoderma lucidum contains Beta-1,3/1,6-D-Glucan as a principal bioactive compound extracted from its mycelium. Despite the broad therapeutic properties, exploration into its role specifically for ulcerative colitis patients remains scarce. Therefore, this case series aimed to explore the potential role of Beta-1,3/1,6-D-Glucan derived from Mycelium extract of Ganoderma lucidum in managing ulcerative colitis.
