Abstract

Barrett’s esophagus (BE) is a predisposing factor of esophageal adenocarcinoma. Metaplasia on BE occurs due to imbalance between esophageal defensive and reluxate materials offensive factors. Nowadays, it is believed that gastroesophageal reflux disease (GERD) is one of major risk factors of BE. Patients with BE generally have lower esophageal sphincter (LES) pressure, shorter LES and intra- abdominal esophageal length, and longer acid exposure than patients with severe esophagitis. Acid exposure has pro-proliferation and anti-apoptosis effect which can facilitate BE occurrence. Currently BE management has gone through various advance, especially in its diagnostic section, from the development of sophisticated endoscopic modality to the finding of biomarker to predict cancer occurrence on BE. Therapeutic section has also been progressing, especially with its endoscopic and chemoprevention therapy. Abstract

Background: Acute diarrhea is a common problem in developing country such as Indonesia with quite high morbidity andmortality rate. It was revealed that the level of zinc deficiency in adult acute diarrhea patients in several hospitals in Jakarta was 69.3%. Although zinc has been proven to be beneficial in the treatment of acute diarrhea in pediatric patients, the effect of zinc supplementation is not fully understood. The objective of this study was to discover the effectiveness of zinc supplementation as an adjuvant therapy in acute diarrhea for adult patients.Method: A double blind randomized controlled trial was done to discover the effect of zinc supplementation to the duration, signs and symptoms on acute diarrhea in hospitalized adults patients in Pertamina Central Hospital, Jakarta from January to December 2013. The data was analyzed using Chi-square test to compare the duration of diarrhea and general linear model (GLM) to assess changes of the symptoms accompanying diarrhea.Results: Data analysis from 84 patients, 30 males (19 zinc, 11 placebo) and 54 females (23 zinc, 31 placebo) with p = 0.111 showed that zinc supplementation significantly reduced the duration of acute diarrhea (p = 0.027) and nausea (p = 0.032). In addition there is a tendency of improvement in several acute diarrhea associated symptoms.Conclusion: Zinc supplementation significantly reduces the duration of diarrhea, nausea, as well as improving some symptoms accompanying acute diarrhea. Keywords: zinc supplementation, acute diarrhea, adult patient

Abstract

Background: Inflammatory bowel disease (IBD) in the form of Crohn’s disease (CD) or ulcerative colitis (UC) is chronic IBD which still difficult to diagnose and clinically characterized by exacerbation and spontaneous remission. A precise diagnosis is needed and essential for appropriate treatment. Most of internists in Indonesia have to cope with a condition of poorly equipped endoscopic facilities - especially colonoscopy and subsequently refer their patients to endoscopic centers with colonoscopy facilities as well as sending biopsy specimens for histopathology examination. They also should be concerned that it would be expensive, time- consuming, and patients may suffer from considerable distress while waiting for the results of diagnostic confirmation. Therefore, we were interested in studying the clinical scoring for IBD to determine the diagnosis of possible IBD by assessing the combination of clinical reporting aspects, identification of probable IBD and histopathology examination Method: The study design was cross-sectional. We collected data pertinent to this study from medical records of patients with IBD clinical features at the endoscopic units of four hospitals in Jakarta between 1999 and 2009. Student t-test and logistic regression analysis were used for the statistic examination. Results: Based on colonoscopy examination, there were 213 patients with positive IBD and 173 patients with negative results. Histopathology results were considered as the gold standard in diagnosis of IBD. The mean clinical score was 9 (nine) points. Subjects with positive histopathology findings were significantly had higher average score than those with negative histopathology findings. Using ROC curves, we found cut-off score of 10 points with a sensitivity and specificity of 73% and 88% consecutively; while by applying logistic regression analysis, we found odds ratio (OR) of 20 (9.9-40.0 Conclusion: The clinical scoring for IBD is a valuable diagnostic tool in determining definitive diagnostic of IBD based on histopathology findings Keywords: clinical scoring, histopathology findings, definitive IBD

Abstract

Pancreatic cancer is one with high mortality cancer in the world. Ninety percent of pancreatic cancer is pancreatic adenocarcinoma. Various factors is associated with an increased risk of pancreatic cancer including age, sex, race, genetic, history of chronic pancreatitis, diabetes mellitus, gallstone, obesity, Helicobacter pylori infection, smoking, diet, and polution exposure. A lot of cases were diagnosed in late stadium due to non-specific early clinical symptoms, and also, until now, there is no examination tool that may screen pancreatic cancer in the earliest stage. Total surgery resection is the therapy of choice in the early stadium of pancreatic cancer, and other therapy modalities are chemotherapy, radiotherapy. Combination of these modalities is frequently used in order to increase the effectiveness of therapy. Mesothelin, a surface glycoprotein on normal mesothelial cells, is overexpressed in pancreatic cancer; therefore, although it is not a cancer specific antigen, it can be used in diagnostic and treatment of pancreatic cancer. Several studies about mesothelin application in pancreatic cancer have been performed; however, more studies are needed to improve the application of mesothelin on pancreatic cancer. Abstract

Background: One of the complications of esophageal varices ligation is deep ulcer. Sucralphate has been shown to prevent and heal post ligation esophageal ulcer. Materials and method: All patients with liver cirrhosis and portal hypertension undergoing esophageal varices ligation in the year 2000/2001 were included in this study. Using “double-blind random sampling”, patients were divided into 2 groups, those receiving sucralphate and those receiving placebo for 2 weeks. Endoscopy of the upper gastrointestinal tract was conducted regularly on the 7th, 10th, and 14th day to evaluate the diameter of the esophageal ulcer based on the diameter of forceps biopsy. The data were evaluated using analysis of variance (ANOVA) or kruskal wallis. Results: Statistically, there was no significant difference in the characteristics of group A patients receiving sucralphate and group B patients receiving placebo. Only 17 patients from group A and 20 from group B were allowed to participate in this study. On the 10th day, the average diameter of the esophageal ulcer in the sucralphate group was significantly smaller than that from the placebo group. (4.74 + 2.02 mm vs 5.42 + 2.22 mm; p=0.04). On the 14th day, the average diameter of esophageal ulcer in the sucralphate group was also significantly less than that of the placebo group (0.88 + 1.58 vs 2.99 + 2.04 mm; p < 0.01). Conclusion:Sucralphate is able to accelerate post esophageal varices ligation esophageal ulcer. Keywords: sucralphate, esophageal ulcer, esophageal varices ligation

Abstract

Background: Colorectal carcinoma is a common malignancy with the highest mortality rate. Epstein- Barr virus (EBV) as the virus that most commonly infect humans, also can infect the body in a latent and induce the occurrence of malignancy. This study aimed to prove an association between EBV virus infection with degree of colorectal carcinoma by examining the main EBV oncogene expression, namely Epstein-Barr nuclear antigen-1 (EBNA-1), in low grade and high grade colorectal carcinoma. Method: Cross-sectional study was performed in 14 colorectal cancer patients in Moewardi Hospital, Surakarta between July 2011 and January 2012. The biopsy specimens were stained for EBNA-1 expression using immunohistochemical technique. Statistical analysis was performed using T-test and Mann-Whitney by SPSS software version 19.0 for windows. Results: Of the 14 patients, there were 7 patients with low grade colorectal carcinoma and 7 patients with high grade colorectal carcinoma. EBNA-1 expression was found in epithelium of low grade and high grade colorectal carcinoma with p = 0.01; CI = -5.24-0.88. We also assessed the expression of EBNA-1 on lymphocytes B of low grade colorectal carcinoma and high grade colorectal carcinoma with p = 0.043. Conclusion: Significant differences in the expression of EBNA-1 was found in association with EBV infection either in low grade and high grade colorectal carcinoma. Abstract

The diagnosis of intestinal amebiasis is easily established based on colonoscopy, i.e. there is a specific characteristic of ulcer/lesion “discrete flask-shape ulcer” with normal mucosa among the ulcers. However, most patients with amebic colitis have non-specific clinical manifestations and their colonoscopy findings are hardly distinguished from inflammatory bowel disease. In the present case, the patient had a chief complaint of chronic bleeding diarrhea and abdominal pain. The fecal analysis found trophozoites of Entamoeba histolytica. Serology test (sero-amebic) revealed negative result. The colonoscopy examination reveals hyperemic mucosa, edema, and multiple ulcers with various sizes covered with fragile and easily bleed debris, from the rectum to ascending colon, rigid colon, narrowing lumen and tumor/mass appearance on ascending colon. The differential diagnosis was severe amebic colitis and inflammatory bowel disease. Based on the colonoscopy biopsy, we found an active chronic colitis along with dysplasia. The patient received management and treatment of severe amebic colitis and inflammatory bowel disease. Abstract

Background: This study retrospectively evaluated post-endoscopic retrograde cholangiopancreatography (ERCP) complications at Cipto Mangunkusumo hospital in order to improve management of a subsequent prospective study of post-ERCP complications.Method: The indications, findings, diagnostic or therapeutic procedure, cannulation, devices used during the procedure, and complications of patients treated consecutively with ERCP between January 2004 and November 2008 were evaluated retrospectively.Results: Of 176 ERCP patients who were initially evaluated, 38% had undergone diagnostic ERCP and 62% therapeutic ERCP. The median age of the patients was 49 years (range 18–80 years); 95 (53.9%) were male. Only 54 of these 176 procedures could be evaluated for post-ERCP complications. A computed tomographic abdominal scan or magnetic resonance cholangio-pancreatography was performed in 23 (42.6%) patients and a biliary sphincterotomy in 14 (25.9%) patients. The overall complication rate was 33.3%: 14.8% after diagnostic ERCP and 18.5% after therapeutic ERCP. The complications after diagnostic ERCP were pancreatitis in 3 (15%) patients, cholangitis in 3 (15%) patients, hemorrhage in 1 (5%) patient, pancreatitis and hemorrhage in 1 (5%) patient; the complications after therapeutic ERCP were pancreatitis in 6 (17.6%) patients, cholangitis in 3 (8.8%) patients, hemorrhage in none, and concomitant pancreatitis with hemorrhage in 1 (2.9%) patient. No significant difference was observed between the complication rates and the type of ERCP performed.Conclusion: There were no differences in the complications after diagnostic and therapeutic ERCP. As our study shows the post-ERCP complication rate to be higher than those of other large retrospective and prospective studies, we must evaluate it in a prospective study.Keywords: post-ERCP, complications, therapeutic ERCP, diagnostic ERCP

Abstract

Gastro-esophageal reflux disease is a pathological condition of esophagus which is caused by gastric content reflux into esophagus. There is an increased prevalence of gastro-esophageal reflux disease. The roles of esophageal pH-metry in clinical application include looking for abnormal acid exposure on esophagus with no abnormality found in endoscopy; evaluating patients following the anti-reflux surgery who are being suspected for abnormal esophageal reflux; evaluating patients with normal endoscopic result but still having refractory reflux symptoms against proton pump inhibitor medication; detecting refractory reflux in patients chest pain following the heart evaluation; evaluating patients with otolaryngologic manifestations (laryngitis, pharyngitis, chronic cough) of the esophageal reflux disease after therapeutic failure of 4-weeks proton pump inhibitor treatment; and looking for correlation between adult onset gastro-esophageal reflux disease and non-allergic asthma. Abstract

Upper gastrointestinal bleeding is one of the emergency conditions in the field of gastroenterology and variceal bleeding is the most common cause of it. Gastric varices accounts only 5% to 10% of all causes of upper gastrointestinal bleeding respectively, but it could be fatal and difficult to control despite provision of adequate therapy.
