Abstract

Background: Antibiotic associated diarrhea (AAD) occurs from the first initiation until 2 months of the end of antibiotic treatment. The aims of this study were to know the incidence of AAD, Clostridium difficile infectionand other gastrointestinal symptoms in hospitalized adult patients.Method: The study is a cross sectional study. We studied the antibiotic associated diarrhea (AAD), Clostridiumdifficile infection and other gastrointestinal symptoms in patients who were admited in Cipto Mangunkusumo Hospital. Inclusion were male or female, age 18-75 years old, patients started receiving antibiotics maximal 2x 24 hours prior to hospitalization, gave written informed consent.Results: The incidence of AAD was 11.5%. The incidence of Clostridium difficile infection was 15.4%. The upper gastrointestinal symptom was present on 20 (38.5%) patients. Lower abdominal symptom was present on 10 (19.2%) patients.Conclusion: The Incidence of AAD and Clostridium difficile infection were 11.5% and 15.4% respectively.The clinical manifestations of AAD were diarrhea, other upper and lower abdominal symptoms.

Abstract

Gastric cancer remains the second most common cancer in the world, and is usually found in men, especially those over 50 years of age. The diagnosis is made by endoscopic biopsy. The high frequency of late diagnosis or advanced stages accounts for the overall poor prognosis for this tumor. Surgery is the most frequently employed modality for both cure and palliation. However, most patients present with advanced disease that is incurable. We reported a rare case of young female patient aged 23 years old with advanced gastric carcinoma. The main clinical features were epigastric discomfort, vomiting, and weight loss. An abdominal mass was palpable on physical examination. Endoscopy showed a giant tumor mass causing gastric outlet obstruction, nodular lesion, ulcerative and hemorrhagic. The histopathologic examination revealed mucinous gastric carcinoma. Palliative resection could not be performed because the tumor tightly adhered to adjacent structures. Jejunostomy was performed to allow enteral nutrition. Best supportive care is very important to improve the quality of life. Keywords: Gastric cancer, young patient

Abstract

Constipation is the most ignored complaint by doctors and unfortunately, it requires urgent management due to the decreased quality of life. Most cases are functional constipation but there is also a great number of constipation due to organic abnormalities. In an attempt to manage the patient properly, we should put our emphasis on excellent knowledge of pathophysiology of constipation.By using the colonic transit time test, we could determine the type of constipation, which would have an effect on therapy. In most cases, constipation has a good response to high fiber diet and additional laxative medication and surgical therapy is seldom required unless complications have occurred. There is also a suggestion for preventing constipation may be caused by opioid usage. Keywords: Constipation, colonic transit time test, high fiber diet, laxatives

Abstract

Background: This is a study to determine if M2 Pyruvate Kinase (M2-PK) can be used to screen the presence of bowel inflammation.Method: The study design being used is diagnostic test. In this study, we recruited 76 participants and performed colonoscopy examination as a gold standard and faecal M2-PK as the examined procedure.Results: From the result of this study, we identified the important role of M2-PK to screen the presence of bowel inflammation with the cut-off point of 1.05 U/mL compared to colonoscopy as gold standard with the sensitivity of 86.2%, specificity 81.8%, positive predictive value 96.6%, and negative predictive value 50%.Conclusion: From the result of this study, we suggested M2-PK examination to screen bowel inflammation in patients with lower gastrointestinal tract problems.

Abstract

Background:  Pancreatic cancer is the fourth leading cause of death associated with malignancy in the United States, and is thought to be the second leading cause of death in 2030 in the United Kingdom. In Asia, pancreatic cancer is the most fatal cancer with the lowest survival of all malignancies, 25-30% survival five years after surgery. Indonesia has no data on the survival of pancreatic cancer and the factors that affect it. The aim of this study is knowing the 1-year survival of pancreatic cancer and its influencing factors in Cipto Mangunkusumo Hospital, Jakarta.Method: A retrospective cohort study was performed using data from the medical records of pancreatic cancer patients dr. Cipto Mangunkusumo Hospital between January 2012 - December 2016. Factors age, sex, metastasis, stage, comorbid and treatment were analyzed bivariate and multivariate using Cox Proportional Hazards Regression to obtain Hazard Ratio (HR) for each prognostic factor. The cumulative survival of 1 year after diagnosis is expressed by the Kaplan-Meier curve.Results: Of 83 subjects the proportion of males was 62.7%, age ≥ 50 years 68.7%, with age range 33-79 years, and 55 years on average. In bivariate analysis, there was a statistically significant relationship of survival with comorbid variables (HR = 2,116, 95% 953 1,335-3,513, p 0,002), metastasis (HR = 3,802, 95% CI: 1,995-7,249, p 0.001), palliative treatment (HR = 2,108 , 95% CI: 1,077-4,125, p = 0,029) and group without treatment (HR = 2,924, 95% CI: 1,496-5,716, p = 0,002). Multivariate analysis showed that metastasis provided the greatest risk of death with HR = 4.306 (95% CI: 2.125-8.724, p 0.001). Palliative group HR was 2.510 (95% CI: 1,245-5,061 p = 0.010) while the group without treatment gave HR 2,535 (95% CI: 1,277-5,032 p = 0,008). The 1-year survival rate is 14%, with a median survival of 6 months.Conclusion: The overall survival of one year of pancreatic cancer patients was 14%, with a median survival of 6 months. The presence of metastasis and not the curative therapy of Whipple surgery in patients with pancreatic cancer in dr. Cipto Mangunkusumo Hospital is the main factor that negatively affect the survival of 1 year

Abstract

The frequency of eosinophilic esophagitis (EE) cases is increasing along with increased understanding among doctors about EE. In the beginning, EE is mostly found in children population. It was first reported in adults by Landres on 1978. The prominent EE symptoms in adults are dysphagia and solid food impaction. Endoscopic examination reveals mucous ring of esophagus, white plaque on the mucosa. The diagnosis is supported by positive result of eosinophils, which forms infiltration on the mucosaof 20 eosinophils/ high power field in the distal or middle esophagus. Treatment by using diet and corticosteroid, either topical or systemic, shows adequate results. Immunomodulator treatment is promising, but it needs further investigation with larger sample. No consensus have been reached for EE, therefore it may affect the diagnosis, treatment and epidemiology data.   Keywords: esophagus, eosinophil, dysphagia, ring of esophagus

Abstract

Background: Endoscopy is the most accurate method for diagnosing the source of upper gastrointestinal bleeding. This study was aimed to evaluate the correlation between the timing of elective endoscopy and the length of hospital stay, the amount of transfusion given and incidence of recurrent bleeding or patient mortality. Method: A retrospective study was conducted in all patients with non-variceal upper gastrointestinal bleeding who had experienced elective endoscopy at Cipto Mangunkusumo Hospital between January 2007 and August 2008. Identification of clinical risk using clinical Rockall score was performed at the emergency room. Persistent bleeding, recurrent bleeding, surgical treatment and death were the outcome variables. Statistical analysis was performed using Chi-square/fisher exact test and linear regression. Results: There were 40 eligible cases with mean age of 53 ± 13 years; the greatest occurrence was at the age group of 50-59 years (12%), male (52.5%) and those who had clinical symptom of melena (52.5%). Twenty seven (67.5%) patients had Rockall score of 1-3 points and 13 (32.5%) had 4-6 points. There was only one patient who had adherent clots (Forrest grade II B). Endoscopy results revealed that the most common cause of bleeding was gastric ulcer, which occurred in 12 (30%) patients. There was no correlation between the timing of endoscopic procedures and outcome variable; however the length of hospital stay had a significant correlation with timing of endoscopic procedures. Conclusion: Elective endoscopy does not affect the variables of mortality and recurrent bleeding; however, it affects the length of hospital stay. Abstract

Background: Chronic functional constipation is a common problem that affects between 15-25% of the population and cause symptoms and disorders that creates discomfort, morbidity, and high costs for health care. Recently, the consumption of probiotics in treating chronic constipation in adults have been investigated. However, there are still limited and controversial evidences available from controlled trials. The aim of this study was to evaluate the effects of Lactobacillus reuteri (L. reuteri) in improving the Agachan constipation score, the number of L. reuteri in the feces and the fecal pH in the patients with chronic functional constipation.Method: A double-blind, placebo randomized controlled trial (RCT) was conducted in 40 adults (12 males/28 females with mean age 45.95 ± 16 years) affected by chronic functional constipation according to Rome III criteria. Patients were randomly assigned to receive a supplementation of L. reuteri or placebo for 4 weeks.Results: At week 4, the decrease in Agachan constipation score was from 17.00 to 8.00 with p 0.001, the increase number of L. reuteri was from 6.80 x 107 to 2.12 x 108 with p 0.001 and the decrease of pH feces was from 5.44 (SD 0.70) to 4.78 (SD 0.56) with p 0.001 in the L. reuteri group, otherwise in the placebo group there were no significant results in Agachan constipation score, the number of L. reuteri and fecal pH assessed.Conclusion: L. reuteri is more effective than the placebo group in improving the Agachan constipation score, increasing the number of L. reuteri in the feces and decreasing the fecal pH in adult with chronic functional constipation.

Abstract

Helicobacter pylori has been known as a cause of chronic gastritis, a predisposition to gastric and duocenal ulcers, and a class I gastric carcinogen. Throughout the world, H. pylori infection is very common, reaching 40% -50% of the population in developed nations and 80% – 90% of the population in developing nations. Several techniques have been used to detect H. pylori infection, such as the urea breath test, rapid urease test, serological test, as well as biopsies of gastric or duodenal tissues for culture and histopathology. In this review article, we will discuss a relatively new method to detect H. pylori antigen in stools with enzyme immunoassay, and comparisons with other standard techniques. However, the H. pylori stool antigen test is not yet commercially available in Indonesia.   Key words: Helicobacter pylori - stool antigen - enzyme immunoassay.
