Abstract

Pergantian pimpinan yang sering terjadi dalam organisasi, khususnya di institusi Kesehatan TNI AD seperti RS Baladhika Husada Jember, menimbulkan tantangan besar dalam menjaga konsistensi dan stabilitas operasional. Setiap pergantian biasanya membawa perubahan visi, kebijakan, dan gaya kepemimpinan yang dapat menimbulkan ketidakpastian dan gangguan dalam pelaksanaan tugas sehari-hari. Penelitian ini bertujuan untuk menganalisis peran Standar Prosedur Operasional (SPO) sebagai instrumen strategis dalam menjaga kesinambungan dan stabilitas institusi di tengah dinamika pergantian pimpinan. Metode yang digunakan adalah pendekatan kualitatif dengan studi kasus, melibatkan observasi, wawancara mendalam, dan dokumentasi terkait implementasi SPO di RS Baladhika Husada Jember. Hasil penelitian menunjukkan bahwa SPO berfungsi sebagai pedoman baku yang mengatur proses kerja secara sistematis dan konsisten, sehingga mengurangi ketergantungan pada individu tertentu dan meminimalisir risiko kesalahan. Implementasi SPO memudahkan adaptasi pimpinan baru, menjaga kelancaran operasional, serta meningkatkan motivasi dan kinerja karyawan. Kesimpulannya, SPO memiliki peran krusial dalam menjaga kualitas pelayanan dan stabilitas budaya organisasi, sekaligus menjadi alat kontrol yang efektif dalam menghadapi perubahan kepemimpinan secara berkala. Penelitian ini memberikan rekomendasi pengembangan model manajemen perubahan yang mengintegrasikan SPO dengan kepemimpinan adaptif dan teknologi informasi untuk masa depan.

Abstract

Perkembangan peradaban manusia yang terus melaju pesat, tidak menyangkal bahwa kegiatan organisasi era modern membutuhkan teori-teori manajemen dalam mencapai tujuan. Teori organisasi dan penerapannya dalam konteks rumah sakit akan dikaji melalui pendekatan literature review. Literatur yang dikaji mencakup jurnal nasional dan internasional yang relevan dengan tema organisasi rumah sakit dan manajemen layanan kesehatan. Hasil review menunjukkan bahwa teori birokrasi klasik, kontingensi, sistem terbuka, dan organisasi pembelajar merupakan kerangka teoritis yang umum digunakan. Setiap teori memiliki keunggulan dan keterbatasan tergantung pada konteks dan kompleksitas organisasi rumah sakit. Kajian ini memberikan wawasan tentang perlunya pendekatan teoritis yang adaptif dan integratif.

Abstract

The disparity in the distribution of specialist doctors in Indonesia is a structural challenge that creates disparities in access and quality of healthcare services between urban and rural/underdeveloped areas. The government addresses this issue through the Health System Transformation agenda, with the sixth pillar, Health Technology Transformation, as a key strategy. This study aims to analyze how the implementation of digital transformation strategies, specifically telemedicine supported by the SATUSEHAT ecosystem and Electronic Medical Records (RME), can be an effective solution to mitigate these challenges. Using systematic literature observation and SWOT analysis, this study examines Indonesia's digital ecosystem, compares it with best practices from Australia and Canada, and identifies critical success factors. The analysis shows that despite strong political commitment and an initial regulatory framework, implementation faces significant challenges related to digital infrastructure disruption, variability in healthcare human resource competencies, and data security issues. International case studies highlight the importance of a clear vision, stakeholder ownership, adaptable models, and operational efficiency. It concludes that digital transformation has significant potential to mitigate geographic challenges, but its success must rely on a holistic approach that integrates infrastructure strengthening, massive human resource capacity development, cybersecurity assurance, and the design of sustainable financing models. Strategic recommendations are formulated for macro-policy and managerial empowerment at the health facility level. Strengthening cross-sector collaboration, including public-private partnerships, is crucial for accelerating the adoption of digital technology in primary healthcare. Furthermore, adaptive monitoring and evaluation mechanisms are needed to ensure the transformation is aligned with the local context and population needs.

Abstract

Penyakit kronis seperti hipertensi dan diabetes melitus memerlukan penanganan jangka panjang yang berkelanjutan, terutama di fasilitas pelayanan kesehatan tingkat pertama (FKTP). Program Pengelolaan Penyakit Kronis (Prolanis) hadir sebagai strategi promotif dan preventif untuk meningkatkan kualitas hidup pasien melalui pendekatan terpadu, termasuk edukasi kelompok, aktivitas fisik, dan pemanfaatan media digital. Penelitian ini bertujuan untuk mengevaluasi pelaksanaan dan efektivitas Program Prolanis di Klinik Pratama Habibie Gorontalo. Penelitian ini menggunakan pendekatan kualitatif deskriptif dengan teknik pengumpulan data melalui wawancara mendalam dan kuesioner kepada peserta Prolanis, tenaga kesehatan, dan penanggung jawab program. Data dianalisis secara tematik untuk mengidentifikasi persepsi, tantangan, serta strategi pelaksanaan program. Hasil penelitian menunjukkan bahwa Prolanis di Klinik Pratama Habibie telah memberikan dampak positif terhadap peningkatan self-management pasien dan kualitas hidup mereka. Edukasi kelompok terbukti meningkatkan pemahaman pasien terhadap penyakit, kepatuhan berobat, serta rasa percaya diri dalam mengelola kondisi kronis. Aktivitas senam rutin membantu menstabilkan tekanan darah dan meningkatkan kebugaran. Media digital dan SMS reminder dinilai efektif dalam menyampaikan informasi dan meningkatkan kehadiran pasien, meskipun beberapa lansia masih mengalami hambatan akses teknologi. Pelaksanaan Prolanis di Klinik Pratama Habibie berjalan cukup efektif dan mampu meningkatkan kualitas hidup pasien penyakit kronis. Strategi yang mengintegrasikan edukasi, aktivitas fisik, komunikasi digital, dan dukungan emosional menjadi kunci keberhasilan program.

Abstract

This study aims to analyze the budgeting strategy of efficiency-based training at Patut Patuh Patju Hospital, West Lombok Regency. The approach used is qualitative descriptive through interviews, observations, and documentation of parties involved in planning, managing, and evaluating training budgets. Data analysis was carried out interactively using the Miles and Huberman model which included data reduction, data presentation, and conclusion drawn. The main focus of this research is to understand how the process of planning and preparing training budgets is carried out, the obstacles faced, the efforts taken to overcome them, and the hospital development strategy based on the current financial conditions. The results of the study show that the process of planning and preparing training budgets at Patut Patuh Patju Hospital is carried out systematically through the annual Training Need Assessment (TNA) mechanism, although it is not completely based on the evaluation of training results. The main obstacle faced is budget limitations due to financial rationalization and suboptimal coordination between departments. Efforts to overcome obstacles are carried out through budget efficiency, postponement of non-priority training, utilization of internal resources, and collaboration with external institutions through webinar-based online training. The hospital development strategy is directed at optimizing human resources and strengthening the training system based on actual needs, accompanied by continuous performance evaluation and the use of technology to support the effectiveness of training.

Abstract

Process efficiency is a key factor in improving the quality of healthcare services, especially in outpatient facilities such as clinics. This study presents a case study at Promec Clinic in Jakarta, aiming to identify bottlenecks and design process improvement recommendations to reduce end-to-end lead time. A descriptive and explanatory quantitative approach was employed, integrating Time and Motion Study (TMS) and Value Stream Mapping (VSM) as primary analysis tools. Data were collected through direct observation of 100 patients, interviews with staff, and operational process satisfaction questionnaires. The results indicate that the "during examination" stage contributes 73.6% of the total service time, while the "before examination" stage is the primary source of operational errors, particularly in form filling (45.59%) and data entry (39.71%). Pareto and Fishbone analyses reveal that manual procedures, lack of standardization, and reliance on informal communication are root causes of inefficiency. Improvement recommendations include digitizing patient forms using a dedicated registration system, implementing self-service kiosks, conducting periodic staff training, and developing Lean-based Standard Operating Procedures (SOPs). These findings offer practical contributions for clinic management in designing more efficient processes and serve as a reference for applying Lean methodologies in primary healthcare settings in Indonesia.

Abstract

This study aims to explore in depth the patients’ perceptions of the role of rapid response from specialist doctors in improving hospital service quality, focusing on Advent Hospital Manado. The research method used is a qualitative approach with a descriptive phenomenological design, where data were collected through in-depth interviews with patients and their families who had direct interactions with specialist doctors. The results show that the speed of the doctors’ response plays a central role in shaping patients’ sense of safety, trust, and satisfaction. Slow responses often lead to emotional anxiety and negative perceptions of hospital professionalism. Determining factors such as internal communication systems, organizational flexibility, patient-centered service culture, and individual professionalism significantly influence patients’ perceptions of service quality. This research emphasizes that response speed is not merely a technical matter, but also reflects the values of empathy and moral responsibility among healthcare professionals. Therefore, an integration of adaptive hospital management systems, the utilization of information technology, and the cultivation of a responsive culture among specialist doctors are needed to enhance holistic and humanistic service quality.

Abstract

Hope and Prayer hospital (RSHD) is a health service facility owned by the local government that is responsible for providing medical services while managing various resources, including assets such as buildings, medical equipment, operational vehicles, and other supporting facilities. In practice, asset management at RSHD Bengkulu still faces various obstacles even though it has a large total fixed asset value. The purpose of this study is to find out the management strategy in identifying the assets owned, asset management and the implementation of management in relation to increasing income at the Bengkulu City Hospital. The results of the research were obtained through proper interviews which were then analyzed so that a comprehensive assessment related to process planning, organizing, commanding, coordinating, and controlling was obtained. The strategy carried out by the management by systematically carrying out planning is supported by a structured organizing process even though with limitations, it can still collect data and plan carefully. RSHD also persistently implements SOPs that are evaluated and supervised through daily reporting to periodic meetings, this also illustrates the role of management in controlling. Behind that, all the Bengkulu City RSHD is still a new hospital, where most of the assets are grants from the city government, so the implementation of better asset management is needed in the future in facing global challenges.

Abstract

Penelitian ini bertujuan untuk menghitung unit cost layanan ekokardiografi di RSAL dr. Oetojo Sorong, Papua Barat Daya, dengan menggunakan metode Activity-Based Costing (ABC) dan membandingkannya dengan tarif INA-CBG’s yang diterapkan. Metode penelitian yang digunakan adalah pendekatan kuantitatif deskriptif dengan fokus pada analisis biaya langsung, biaya overhead, dan alokasi biaya berdasarkan aktivitas. Data yang digunakan meliputi data primer berupa observasi langsung dan wawancara, serta data sekunder seperti laporan keuangan dan rekam medis rumah sakit. Hasil penelitian menunjukkan bahwa unit cost layanan ekokardiografi yang dihitung menggunakan metode ABC lebih tinggi dibandingkan dengan tarif INA-CBG’s, yang mengindikasikan adanya disparitas antara tarif standar dan biaya aktual. Penerapan metode ABC memberikan manfaat signifikan bagi RSAL dr. Oetojo Sorong dalam mengidentifikasi aktivitas yang memakan biaya besar, meningkatkan efisiensi operasional, serta memberikan transparansi dalam pengelolaan biaya. Penelitian ini juga memberikan rekomendasi kepada manajemen rumah sakit untuk mengevaluasi tarif layanan dan BPJS Kesehatan untuk meninjau kembali kesesuaian tarif INA-CBG’s dengan biaya aktual layanan kesehatan.

Abstract

This research is motivated by the fact that problems are still found in the control of the supply of non-communicable disease drugs, especially Diabetes Mellitus (DM) and Hypertension (HT), at the North Cimahi Health Center, such as stock vacancies, excess of certain drugs, and limited use of information systems. The purpose of this study is to analyze the management strategy of drug inventory control, including the planning, organization, implementation, and control stages, as well as to evaluate the effectiveness of these strategies in supporting the sustainability of health services. This study uses a descriptive qualitative method with data collection techniques through interviews, observations, and documentation, and is analyzed using the Miles and Huberman interactive model. The results of the study show that drug needs planning is carried out based on monthly usage data and patient visit trends, but is still constrained by distribution quotas and budget allocation from the Health Office. The organizing strategy is carried out with a clear division of tasks between the heads of health centers, pharmacists, pharmacists, and administrative staff, and is supported by the use of e-catalogs. The implementation of the strategy involves manual and digital recording, the use of Economic Order Quantity (EOQ), Reorder Point (ROP), and Safety Stock methods  to maintain inventory stability, although there are still obstacles in the form of demand fluctuations and distribution delays. The control process is carried out through daily monitoring, internal audits, and periodic reporting mechanisms to the Health Office, with contingency strategies in the form of restrictions on the administration of drugs or referring patients to external pharmacies. Overall, control strategies have proven effective for priority drugs such as Amlodipin and Metformin, but they still need to be strengthened for alternative drugs such as Glibenklamidi which are prone to vacuum. The conclusion of this study is that the implementation of strategic management in controlling drug inventory at the North Cimahi Health Center has supported the availability of the main drug, the sustainability of therapy, and patient satisfaction, although it still faces limited resources, external supply, and data accuracy. The implications of this study emphasize the need to optimize information systems, strengthen internal coordination, and increase strategic flexibility so that drug inventory control can be more adaptive, efficient, and sustainable.
