Abstract

Mangrove endophytic fungi can produce bioactive substances with diverse  biological functions. This study aims to evaluate the chemical profile of mangrove fungal endophytic extracts that inhibit clinical pathogenic bacteria resistant to various antibiotics. The fungi were collected from Petengoran mangrove forest, Lampung Province. Fungal isolates were grown on shrimp shell media using solid-state fermentation for 14 days. The fungal biomass was extracted using ethyl acetate, and the active components were evaluated using thin layer chromatography. The extract was partitioned with dichloromethane/water and its bioactivity was tested using TLC-bioautography and agar diffusion methods. The active fraction was identified using LC-MS/MS. The LC-MS/MS data was interpreted with SIRIUS 5.8.6, and the drug-likeness and toxicological characteristics were assessed using ADME/Tox and STopTox machine learning tools. Morphological analysis showed that isolate 22PLP1F1 was an Aspergillus sp., with spherical conidia at the hyphae tips. Through phylogenetic analysis it was confirmed that isolate 22PLP1F1 is Aspergillus sydowii with similarity 98.9%. Initial TLC examination indicated the production of alkaloids, polypeptides, and steroids. Antibacterial assays showed that the polar portion inhibited multi-drug resistance (MDR) Staphylococcus aureus, while the active fraction at 2 mg/mL inhibited MDR Pseudomonas aeruginosa. LC-MS/MS analysis revealed a major chromatogram peak at a retention time of 8.67; m/z 488.2196, suggesting a novel derivative of a compound at a retention time of 7.82; m/z 446.208. ADME/Tox analysis indicated that the compounds do not penetrate the BBB but remain in the GI absorption region. Further research is needed to elucidate the active compounds’ mechanism of action and conduct bioengineering studies.

Abstract

Abstract Diabetes Mellitus causes complications in the form of sexual libido dysfunction. Some of the treatment for libido dysfunction was aphrodisiac (piperin) which consist in black pepper. This study aimed to determine the effect of black pepper (piper nigrum l) extract on male white rat (Rattus norvegicus) libido (sexual behavior) with diabetes mellitus. The study was an experimental laboratory with a post-test only control group design using 30 samples of male white rats (Rattus norvegicus) which divided into 5 groups. Each divided into, control group 1 (K1) consist of normal rats; control group 2 (K2), diabetic rats induced by alloxan 150 mg/kg; treatment group 1 (P1), diabetic rats induced by alloxan 150 mg/kg and given black pepper extract 122.5 mg/kg; treatment group 2 (P2), diabetic rats induced by alloxan 150 mg/kg and given black pepper extract 245 mg/kg; treatment group 3 (P3), diabetic rats induced by alloxan 150mg/kg and given sildenafil 1mg/kg. Libido’s measurement was done by observing intromission latency in second, mount latency in second, mount frequency by its quantity. Kruskal-Wallis test was used to analyzed introducing  latency, whereas mounting latency and mounting frequency was analyzed with One Way ANOVA.  The results showed that introducing latency of group P1 and P2 significantly different compared to group K2 (p=0,003, p=0,003), Mounting latency of group P1 significantly different compared to group K2 (p=0,000) and mounting frequency of group P1 and P2 significantly different compared to group K2 (p=0,000,p=0,006). Black pepper extract enhances male white rat (Rattus norvegicus) libido with diabetes mellitus.

Abstract

Water apple (Syzygium aqueum) is a plant widely utilized by publics for both consumption and medicinal purposes. Traditionally, it is used as an alternative treatment for diarrhea, headaches, and cough. The method to maximize the potential of water apple leaves is by extracting the active compounds in it. These extracts have potential as antioxidants, anti-inflammatory, anti-cholesterol, antidiabetics, and antidiarrheal agents. Previous studies have shown that water apple leaves contain various active compounds such as alkaloids, flavonoids, terpenoids, saponins, and phenolics. This research aimed to analyze the active compounds and determine the density of the water apple leaf extract. The study was conducted as a laboratory-based experimental research at the Botany Laboratory, FMIPA University of Lampung. The method employed was maceration of water apple leaves using 96% ethanol as a solvent, followed by qualitative phytochemical tests and density determination. Qualitative tests conducted included assessments for flavonoids, terpenoids, tannins, saponins, phenolics, alkaloids, and steroids. The density of the extract was determined by measuring the mass of the extract using an analytical balance and dividing it by its volume. The results showed that maceration using 96% ethanol yielded 110.62 grams of extract with a yield percentage of 11.062%. Phytochemical tests revealed the ethanol extract contained flavonoids, phenolics, tannins, saponins, alkaloids, and steroids. The density of the ethanol extract was calculated to be 0.9209 g/mL.

Abstract

Hemoglobin is an important protein in the blood whose job is to transport oxygen to body tissues. Low hemoglobin levels (anemia) in laparotomy patients can affect postoperative recovery time, including the duration of hospitalization. This study aims to evaluate the relationship between pre-operative hemoglobin levels and length of stay in laparotomy patients. Data were obtained from medical records of patients who underwent laparotomy at the hospital, including hemoglobin levels before surgery, length of stay, and additional variables such as Body Mass Index (BMI) and Surgical Apgar Score (SAS). The analysis showed that patients with low hemoglobin levels had a greater risk of experiencing longer hospital stays. This is due to suboptimal oxygen supply to the tissue, thereby slowing wound healing and increasing the risk of complications, such as infection and organ failure. Treating anemia with blood transfusions or iron supplementation has proven effective in increasing hemoglobin levels and speeding up patient recovery, with the average length of stay reduced to less than four days. Apart from hemoglobin levels, factors such as BMI, SAS, level of complexity of surgery, and the patient's health condition also influence the length of treatment. These findings emphasize the importance of monitoring and correcting hemoglobin levels before surgery as a preventive measure to speed recovery, minimize complications, and reduce hospital operational burden, especially in health facilities with limited resources. Further research is needed to determine the most effective clinical interventions in managing anemia in preoperative patients.

Abstract

Lindur mangrove plants (Bruguiera gymnorrhiza) are known to contain secondary metabolite compounds which function as antibacterials. These secondary metabolite compounds include alkaloids, flavonoids, tannins, phenols, saponins, terpenoids and steroids. The design of this research was experimental, carried out to determine and compare the phytochemical components contained in 96% ethanol extract and n-hexane of Bruguiera gymnorrhiza mangrove bark. Bruguiera gymnorrhiza mangrove bark was taken at KPH Gunung Balak, East Lampung, then 4.5 kg of wet mangrove bark was obtained and dried for 7 days, then the extract was made using the maceration method for 3x24 hours using 2 types of solvents, namely 96% ethanol and n-hexane with a ratio of 1:10 between simplicia and solvent. Thick extracts were obtained from the two types of solvents which were then carried out qualitative phytochemical tests.  The results of this study showed that 96% ethanol extract and n-hexane of Bruguiera gymnorrhiza mangrove bark positively contained secondary metabolite compounds. The 96% ethanol extract of Bruguiera gymnorrhiza mangrove bark contains all types of secondary metabolite compounds, namely alkaloids, flavonoids, tannins, phenols, saponins, terpenoids and steroids, while the n-hexane extract contains secondary metabolite compounds, namely alkaloids, flavonoids, tannins, phenols, terpenoids, steroid but does not contain saponins. The conclusion of this research is that the 96% ethanol extract of Bruguiera gymnorrhiza mangrove bark contains more secondary metabolite compounds than the n-hexane extract of Bruguiera gymnorrhiza mangrove bark.

Abstract

Background: Hypertension is one of the non-communicable diseases (NCDs) that is the leading cause of death globally. Lampung Province recorded a prevalence of hypertension of 29.9% with a significant burden of cases in the elderly group. Purpose: To analyze the factors that influence the incidence of hypertension in the elderly. Method: This cross-sectional analytical survey study was conducted at the Tresna Werdha home for the elderly and Tanjung Sari Health Center, Natar District, South Lampung in May-June 2024. The study population was 176 elderly people, consisting of 80 elderly from Panti Tresna Werdha and 96 elderly from Tanjung Sari Health Center. Data analysis was carried out using a logistic regression test with a significance level of α <0.05. Results: Showed that in the health center group, significant variables associated with hypertension included a history of hypertension (OR: 217), vegetable and fruit consumption (OR: 3,735), oil consumption (OR: 93,444), and salt consumption (OR: 3,182). Meanwhile, in the home for the elderly group, significant variables included a history of hypertension (OR: 3.567), physical activity (OR: 4.660), social activity (OR: 14.538), social interaction (OR: 6.152), and mental status (OR: 3.462). Conclusion: Hereditary factors, consumption patterns, and social and mental aspects contribute greatly to the incidence of hypertension in the elderly. Community-based interventions and strengthening healthy lifestyles are needed to reduce the prevalence of hypertension in the elderly.   Keywords: Hypertension; Elderly; Non-Communicable Diseases (NCDs).   Pendahuluan: Hipertensi merupakan salah satu penyakit tidak menular (PTM) yang menjadi penyebab kematian tertinggi secara global. Provinsi Lampung mencatat prevalensi hipertensi sebesar 29.9% dengan beban kasus yang signifikan pada kelompok lansia. Tujuan: Untuk menganalisis faktor-faktor yang memengaruhi kejadian hipertensi pada lansia. Metode: Penelitian survei analitik cross sectional, dilakukan di Panti Tresna Werdha dan Puskesmas Tanjung Sari Kecamatan Natar Lampung Selatan pada bulan Mei-Juni 2024. Populasi adalah orang lansia berjumlah 176 orang yang berasal dari Panti Tresna Werdha berjumlah 80 lansia dan Puskesmas Tanjung Sari berjumlah 96 lansia. Analisis data dilakukan menggunakan uji regresi logistik dengan tingkat signifikansi α<0.05. Hasil: Menunjukkan bahwa pada kelompok puskesmas, variabel signifikan yang berhubungan dengan hipertensi meliputi riwayat hipertensi (OR: 217), konsumsi sayur dan buah (OR: 3.735), konsumsi minyak (OR: 93.444), dan konsumsi garam (OR: 3.182). Sementara pada kelompok panti, variabel yang signifikan mencakup riwayat hipertensi (OR: 3.567), aktivitas fisik (OR: 4.660), aktivitas sosial (OR: 14.538), interaksi sosial (OR: 6.152), dan status mental (OR: 3.462). Simpulan: Faktor keturunan, pola konsumsi, serta aspek sosial dan mental berkontribusi besar terhadap kejadian hipertensi pada lansia. Intervensi berbasis komunitas dan penguatan pola hidup sehat sangat diperlukan untuk menekan prevalensi hipertensi di kalangan lanjut usia.   Kata Kunci: Hipertensi; Lansia; Penyakit Tidak Menular (PTM).

Abstract

Demam Berdarah Dengue (DBD) merupakan masalah kesehatan masyarakat yang serius di Indonesia. Wilayah kerja Puskesmas Banjarsari Kota Metro mengalami peningkatan signifikan kasus DBD, dari 5 kasus pada tahun 2022 menjadi 87 kasus pada tahun 2024. Faktor lingkungan, khususnya ketersediaan tutup penampung air dan frekuensi pengurasan, diduga menjadi faktor risiko utama kejadian DBD. Tujuan penelitian ini untuk menganalisis hubungan antara ketersediaan tutup penampung air dan frekuensi pengurasan penampung air dengan kejadian Demam Berdarah Dengue di wilayah kerja Puskesmas Banjarsari Kota Metro tahun 2025. Penelitian ini menggunakan desain cross-sectional dengan pendekatan kuantitatif. Sampel penelitian berjumlah 389 responden yang dipilih menggunakan teknik proportionate stratified random sampling, terdiri dari 57 kasus positif DBD dan 332 kasus negatif DBD. Data dikumpulkan melalui kuesioner yang teruji validitas (nilai r berada pada rentang 0,467-0,786) dan reliabilitas nya (alpha Cronbach 0,97) dan observasi lapangan. Analisis data menggunakan uji chi-square. Hasil penelitian menunjukkan bahwa sebagian besar responden tidak memiliki tutup penampung air (47,8%) dan tidak melakukan pengurasan secara rutin (49,9%). Analisis bivariat menunjukkan terdapat hubungan signifikan antara ketersediaan tutup penampung air dengan kejadian DBD (p-value = 0,018; OR = 2,080; 95% CI = 1,165-3,715). Responden yang tidak memiliki tutup penampung air berisiko 2,080 kali lebih tinggi mengalami DBD. Terdapat hubungan signifikan antara frekuensi pengurasan penampung air dengan kejadian DBD (p-value = 0,043; OR = 1,888; 95% CI = 1,057-3,371). Responden yang tidak menguras penampung air minimal satu kali seminggu berisiko 1,888 kali lebih tinggi mengalami DBD. Ketersediaan tutup penampung air dan frekuensi pengurasan penampung air merupakan faktor risiko yang berhubungan signifikan dengan kejadian DBD di wilayah kerja Puskesmas Banjarsari Kota Metro. Diperlukan peningkatan kesadaran masyarakat melalui program promosi kesehatan dan intensifikasi pelaksanaan Pemberantasan Sarang Nyamuk (PSN) 4M Plus.

Abstract

Introduction: The prevalence of anemia in teenage girls in Lampung Province is still quite high. In preventing and overcoming anemia in teenage girls, it is possible to increase the intake of iron-rich foods and supplementation with blood-added tablets (TTD). It is necessary to develop innovative multifaceted interventions in improving dietary intake behavior, especially iron. This study was conducted to analyze the effect of multifaceted intervention on iron intake and hemoglobin (Hb) levels in teenage girls with mild moderate anemia at the Vocational High School in Tanjung Sari, South Lampung Regency. Methodology: Quasy experimental research with one group pretest and posttest design. The study was conducted from January to May 2022 with a sampling technique that is consecutive sampling on anemic teenage girls at the vocational high school state in Tanjung Sari and the number of research samples was 41 respondents. The respondents carried out a multifaceted intervention in the form of nutrition education, food supervision for parents and teachers and the provision of iron supplementation for six weeks. The analysis test used paired t-test with the SPSS program and it was considered to have a significant effect if p0.05. Result: There was an increase in the average iron intake (increase 7.60 mg) and an increase in the average hemoglobin level (increase 1.06 g/dL). The results is showed a significant effect of multifaceted intervention on iron intake (p = 0.000) and a significant effect on hemoglobin (Hb) levels (p = 0.000). Discussion: Based on the results of the analysis showed that there was a significant effect of multifaceted intervention on iron intake and hemoglobin (Hb) levels in anemic teenage girls at the vocational high school in Tanjung Sari.

Abstract

Infeksi parasit usus merupakan salah satu masalah kesehatan yang signifikan di Indonesia, terutama di kalangan anak-anak sekolah dasar yang hidup di lingkungan dengan sanitasi dan kebersihan yang buruk. Studi ini bertujuan untuk mengevaluasi hubungan antara infeksi parasit usus dan status gizi anak sekolah dasar di Indonesia melalui tinjauan sistematis. Penelitian ini menggunakan pendekatan systematic review dengan panduan PRISMA. Proses penyaringan dilakukan dengan mengidentifikasi dan menganalisis artikel dari basis data elektronik yakni Google Scholar. Kata kunci yang digunakan mencakup kombinasi istilah-istilah berikut: "infeksi parasit usus," "status gizi," "anak sekolah dasar," dan "Indonesia", baik dalam bahasa Inggris maupun Indonesia, dengan penggunaan operator Boolean untuk meningkatkan sensitivitas pencarian Setelah proses penyaringan, artikel yang memenuhi kriteria inklusi dan eksklusi dilakukan penilaian bias menggunakan NOS adapted for cross-sectional studies. Seluruh artikel terpilih akan dilakukan analisis terhadap masing-masing hasil penelitian. Terdapat hubungan yang bervariasi antara infeksi parasit usus dan status gizi berdasarkan indikator Indeks Massa Tubuh (IMT), Berat Badan per Umur (BB/U), Tinggi Badan per Umur (TB/U), dan Berat Badan per Tinggi Badan (BB/TB). Beberapa studi menemukan hubungan signifikan, khususnya pada indikator TB/U dan IMT, sementara penelitian lain menunjukkan hasil yang tidak signifikan. Hubungan antara infeksi parasit usus dan status gizi anak sekolah dasar di Indonesia bersifat kompleks dan menunjukan hasil yang tidak konsisten pada setiap penelitian. Diperlukan penelitian lebih lanjut, terutama studi longitudinal, yang dapat mengevaluasi dampak jangka panjang infeksi parasit usus terhadap status gizi anak dengan mempertimbangkan faktor sosial, ekonomi, dan lingkungan.

Abstract

Diabetic nephropathy is one of the main causes of end-stage renal failure or End Stage Renal Disease (ESRD) in patients with type 2 diabetes. Diabetic nephropathy is defined as the presence of albumin or protein of more than 30 mg/day in urine on two examinations within a period of 3-6 months, as well as a decrease in the glomerular filtration rate. Urinalysis examination using the dipstick method is carried out for certain indications or suspicions of complications of diabetes mellitus, one of which is diabetic nephropathy. Dipstick examination can detect protein, pH, glucose, specific gravity, ketones, urobilinogen, nitrite, blood, bilirubin, and leukocyte esterase. The specific gravity of urine depends on the concentration of glucose, urea, and protein particles in the solution. Therefore, the purpose of this study was to determine the relationship between protein in urine and specific gravity in patients with diabetic nephropathy. Observational analytical research was conducted with a cross-sectional approach. This study was conducted at the Medical Records Installation of Dr. H. Abdul Moeloek Hospital. The population of this study was diabetic nephropathy patients treated at Dr. H. Abdul Moeloek Hospital in January-December 2023. The sampling technique used the total sampling technique so that 49 samples were obtained that met the inclusion and exclusion criteria. The data obtained were analyzed using the Spearman correlation test. The results showed a correlation coefficient (r) of 0.589 and a significance p-value = 0.000. There is a relationship between urine protein and urine specific gravity in diabetic nephropathy patients.
