Abstract

BACKGROUND: Trans-radial access has become the new standard and is increasingly in demand in coronary angiography. However, this method can fail due to tortuosity of the right subclavian artery. OBJECTIVES: This study aims to identify the predictors of right subclavian artery tortuosity. METHODS: This retrospective case-control study was conducted at two of the largest hospitals in West Nusa Tenggara, Indonesia. All research data were obtained from secondary sources, including video recordings of coronary angiographies, angiography reports, and patient medical records. Predictors included age, gender, smoking, height, body mass index, hypertension, diabetes mellitus, systolic and diastolic blood pressure, and random blood sugar. RESULTS: The study involved 80 subjects, divided into right subclavian artery tortuosity and normal group, with 40 subjects each. The prevalence of tortuosity was 8.1%, while the success rate of trans-radial access was 70.0%. Four significant predictors were identified: smoking (adjusted OR 0.26; p = 0.019), hypertension (adjusted OR 4.83; p = 0.020), diastolic blood pressure (adjusted OR 1.05; p = 0.044), and body mass index (adjusted OR 1.16; p = 0.035). The optimal cutoff points were determined to be a body mass index of ≥ 24.4 (sensitivity 60.0%; specificity 60.0%; p = 0.006; AUC = 0.665) and diastolic blood pressure of ≥ 78.5 mmHg (sensitivity 67.5%; specificity 67.5%; p = 0.002; AUC = 0.685). CONCLUSION: This study has identified smoking, hypertension, diastolic blood pressure, and body mass index as significant predictors of right subclavian artery tortuosity in trans-radial coronary angiography access.

Abstract

Abstract

Hypothyroidism is a condition in which the thyroid gland is unable to produce enough hormones to meet the body's needs. This condition causes a slowed metabolism, which can lead to various symptoms such as fatigue, weight gain, dry skin, hair loss, constipation, cold intolerance, and facial puffiness. Given the widespread impact of hypothyroidism and the potential for serious complications, special attention is needed for early detection, diagnosis, and effective treatment of hypothyroidism. This literature review was conducted using journal review methods and data searches through sources such as PubMed, Google Scholar, and ScienceDirect. The search used the keywords "hypothyroidism," "hypothyroid," "thyroid gland," and "Hashimoto's thyroiditis." The results of the literature review found that the main cause of hypothyroidism is Hashimoto's thyroiditis, an autoimmune disease in which antibodies attack the thyroid gland. Hypothyroidism is diagnosed by examining elevated TSH and decreased free T4 levels, as well as anti-TPO antibody tests to detect Hashimoto's thyroiditis. Treatment for hypothyroidism generally involves administering levothyroxine, a synthetic thyroid hormone, to replace the deficient hormone and normalize metabolic function. Healthy lifestyle support, such as diet and stress management, is also important to help optimally manage this condition. In conclusion, hypothyroidism is characterized by a decrease in thyroid hormone production and preventive management is pharmacological in the form of levothyroxine, while non-pharmacological management is in the form of regulating food intake.

Abstract

The efficiency of emergency alert dissemination in highly populated and densely urban transport networks, such as Jakarta's integrated rail system, is undermined by sporadic connectivity and limited network resources. In this environment, an initial comparison of baseline Delay-Tolerant Network (DTN) routing protocols revealed that flooding-based routers, such as Epidemic, while achieving above-average delivery rates, suffered from high overhead and poor buffer utilization. This paper fills this gap by proposing the Combined Utility Router, a novel buffer management policy that overcomes the limitations of naive strategies, such as Drop-Oldest. Our approach holistically evaluates a message's value by assigning a weighted utility function based on its Time-To-Live (TTL), estimated total replicas, message size, and a user-defined priority. The router maintains high-value messages by discarding the message deemed the lowest utility score under the buffer constraint. Utility-based simulations in The ONE simulator demonstrate that applying our approach to Epidemic routing improves delivery probability, reduces average latency in high network congestion scenarios, while maintaining overhead rates. This work confirms that, in the context of developing reliable and efficient emergency communication systems for challenging urban topographies, optimizing buffer management extends beyond simply selecting the appropriate protocol.

Abstract

West Nusa Tenggara is an archipelago province with a high potential for marine resources and fisheries, allowing many inhabitants to make a living as fishermen. Some fishermen obtain their livelihood by diving with minimal or inadequate diving equipment. They are known as traditional diver fishermen. Various health risks can be experienced by them, including respiratory system health. This research aims to determine the lung function, including Forced Vital Capacity (FVC), Forced Expiratory Volume in 1 second (FEV1), and the FEV1/FVC ratio in traditional diver fishermen who dive using the breath-hold diving technique, as well as the characteristics of their diving activities. This research is a cross-sectional study with samples obtained through consecutive sampling techniques. The research subjects comprised 23 traditional diver fishermen in Impos Beach, Medana Sub-district, North Lombok District. A descriptive analysis was conducted on the data obtained. The research results showed that the average values of FVC and FEV1 were 3.27 L/minute and 3.004 L/minute, respectively, while the FEV1/FVC ratio obtained was 91.86%. On average, they dive 4.26 times per week, and all of them perform pressure equalization maneuvers to prevent barotrauma. Before diving, 12 subjects (52.17%) performed warm-up exercises, including stretching. After diving, none of the subjects take a hot shower, get a massage, or engage in strenuous activities. The lung function of the research subjects was normal, and they have taken preventive measures to avoid diving-related illnesses.

Abstract

The Internet-Based Temperature Control Lab (iTCLab) kit is an instructional tool designed to facilitate the understanding of fundamental concepts in dynamic systems and control, particularly Proportional-Integral-Derivative (PID) controllers. The background of this study lies in the gap between students’ theoretical knowledge and its application in real-world systems, which often poses a challenge in learning Dynamic Systems and Control courses. This research aims to evaluate the effectiveness of using iTCLab in improving students’ understanding of both PID control theory and its practical applications. The study was conducted in three stages: development of learning modules, implementation, and evaluation involving 30 Informatics students. Assessment was carried out through knowledge tests (pre-test and post-test) and perception surveys. The results indicated an increase in the average score from 52.30 to 76.48 (+46.26%, p < 0.001, Cohen’s d = 1.86), along with positive evaluations in terms of theory–practice integration (4.30), ease of use (4.10), content relevance (4.40), and learning satisfaction (4.20) on a 1–5 scale. These findings suggest that iTCLab is effective in strengthening students’ conceptual understanding and technical skills. From a practical standpoint, iTCLab is suitable for integration into Semester Learning Plans (RPS) and further development to support Internet of Things (IoT)-based distance learning.

Abstract

Background : Due to their higher nutritional needs, pregnant women are among the most vulnerable groups in society. The World Health Organization (WHO) estimates that 35–75% of pregnant women worldwide suffer from chronic energy deficiency (CED), which is one of the most common nutritional problems. Chronic Energy Deficiency is a condition in which the body does not receive adequate nutrients due to an imbalance in energy and protein intake, ultimately affecting fetal development. Objective: To determine the relationship between the prevalence of chronic energy deficiency among pregnant women at the Bara-Baraya Community Health Center (Puskesmas Bara-Baraya) in 2024. Methods: This study used age, knowledge, and economic status as independent variables, while the occurrence of chronic energy deficiency among the study subjects, observed and measured simultaneously, was the dependent variable. An analytical statistical test was used to explain the research findings, with a total sample of 32 pregnant women examined at the Bara-Baraya Community Health Center in Makassar City between March and April 2024. The sampling method used was accidental sampling. Results and Conclusion: The study showed that: There is a significant relationship between maternal age and the incidence of chronic energy deficiency among pregnant women at the Bara-Baraya Health Center in 2024 (significance value = 0.01). There is a significant relationship between maternal knowledge and the incidence of chronic energy deficiency (significance value = 0.02). There is also a significant relationship between economic status and the incidence of chronic energy deficiency (significance value = 0.04). Thus, the prevalence of CED among pregnant women at the Bara-Baraya Community Health Center in 2024 is associated with age, knowledge, and economic factors.
