Abstract

Patient adherence is a key factor that determines the outcome of diabetes mellitus therapy. This study was conducted to determine the effect of a medication reminder chart on level adherence and clinical outcome in patients with Type 2 diabetes mellitus. The experimental study used pretest-posttest design with control group was conducted during February until April of 2016 at Sleman Hospital in Yogyakarta. Patients were categorized into two groups of different subjects, who received of medication reminder chart tools were 40 patients (Code A) and the other 40 patients were assigned to control group (Code B). Morisky Medication Adherence Scale of 8-item (MMAS-8) was used to measure adherence. Clinical outcome were measured based on FBG decrease (mg/dl). Two independent sample t-test was using to determine the mean differences of the average score of the adherence level before and after using the medication reminder chart.  The results of this study showed that Medication Reminder Chart compared control groups improved adherence (p=0.000) but did not improve clinical outcome (p=0.411). The pharmacist might use The Medication Reminder Chart to improve medication adherence for chronic disease.

Abstract

In the developed countries, the incidence of cesarean section year by year increases by 7 –10 %. There have been various kinds of cesarean section in several hospitals. In PKU Muhammadiyah Yogyakarta hospital, the incidence of cesarean section in 2001 increased up to 21 %. There are also various kinds of antibiotics used to deal with cesarean section cases which caused various kinds of medical cost. Therefore, identification of antibiotics cost, which used to deal with cesarean section in PKU Muhammadiyah Yogyakarta hospital was studied.The study was performed by epidemiological survey of cesarean section via patient’s medical record, medicinal cost for treatment and surgery, and patient’s invoice in the year 2000. The study involved recording identification of patients and physicians identities, antibiotics and other medicines used, surgical equipment, cesarean section indications, pharmacy and other cost, duration of staying related to cesarean section.Results showed that the majority of cesarean section has prolonged labour (26%). The majority of antibiotics used in cesarean section is Sulbenicillin injection with Klindamicin oral (31%) and Amoxicillin/potassium clavulanate injection with Amoxicillin oral (23%). Cost of antibiotics were 12.5% - 44.4% of pharmacy expenses and 6.7% - 14.8% of total expenses. No correlation between antibiotics cost and hospital stays.Key words : cost, antibiotic, cesarean section

Abstract

Diabetes mellitus is a complex disorder that involves multiple pathophysiological defects. As the disease progresses, further functional decline in β-cell is apparent. In most cases, patients on oral antidiabetic therapy will require not only an increase in dose, but also the addition of a second or third oral agent. The aim of this study was to evaluate the effectiveness and safety of triple therapy with sulfonylurea, metformin, and acarbose in patients with poorly controlled glycemia. The study design was a prospective observational study in 49 type 2 diabetic patients followed in Department of Endocrinology RSUP Dr Sardjito Indonesia from May 2007 to September 2008. Patients with hypertension were included if their blood pressure was well controlled with antihypertensive medication. All patients with documented gastrointestinal disease were excluded. At baseline and at 3-month intervals, levels of HbA1C, fasting and postprandial plasma glucose, hypoglycemic episodes, and edverse event were evaluated. Fourty nine patients, 22 men and 27 women, aged 62.84 +7.85 years, diabetes duration of 11.92 +6.09 years were studied. The initial HbA1C level was 8.08 +1.89 % which significantly increased to 8.73 +2.37 % (p<0.05). Only 32.98 % of subjects achieved target value of HbA1C (≤ 7 %). Fasting and post-prandrial plasma glucose values were increased from 160.39 +60.25 mg/dL to 170.71+56.60 mg/dL and 210.31+ 75.88 mg/dL to 218.67 +75.03 mg/dL respectively, but not significantly different. Acarbose was more frequently associated with flatulence (46 %) and abdominal bloating (12 %), metformin with flatulence (12 %), nausea (14 %), diarrhea (2 %) and abdominal discomfort (6 %). In conclusion, combination therapy of sulfonylurea, metformin, and acarbose in type 2 diabetic patients with poorly controlled glycemia can cause the additive risk of adverse events.Key words : triple therapy, glycemic control, type 2 diabetes mellitus

Abstract

The cost of antibiotic prophylaxis sulbenicillin versus coamoxiclav (in term of medication use and treatment of complications) to treat cesarean section was compared.The medical records of patient receiving intravenous sulbenicillin orcoamoxiclav for the treatment of cesarean section in PKU Muhammadiyah Hospital were retrospectively reviewed. Data were collected for patients treated from January 1, 2000 to December 31, 2000. Patient data collected included patient data based, indication of cesarean section, abdominal incision and oral antibiotics. Cost data collected included drug acquisition cost, cost of drug supplies, nursing time to administer the agents and cost of managing complication.The medical records of 98 patients were identified and reviewed. The average length of stay was 4.85 days for the sulbenicillin group and 4.90days for the coamoxiclav group. The average total including the cost of complication was Rp. 140.509 in sulbenicillin group and Rp. 376.310 in coamoxiclav group.In conclusion, the study demonstrates that intravenous coamoxiclav has a higher cost than that of intravenous sulbenicillin. This result can be used to assist institution, clinicians and pharmacists in determining the most appropriate and efficient use of drugs. These data can be a powerful tool to support various clinical and policy drug use decisions, for example included formulary management and drug use policy or guidelines.Key words: antibiotics, cesarean section, cost.
