Abstract

Aims: This study evaluated palliative care services in Indonesia, focusing on proposing a community-based model to enhance cancer care. Method: A mixed-method approach was employed, involving quantitative analysis of 200 patients’ personal and treatment factors and qualitative insights from 21 stakeholders to FGDs and in-depth interviews. Results: The qualitative results highlight five themes: 1) illness as a subjective and culturally influenced concept, 2) family’s role in treatment decision-making, 3) perceptions of healing through multiple treatment modalities, 4) finding positive meaning in illness, and 5) hopes and aspirations for recovery. The quantitative results show that 55.5% of patients had inadequate knowledge, with 69.5% preferring modern therapy. A significant relationship was found between knowledge, perception, and treatment options, while no significant correlation was observed with self-confidence or diagnosis date. Conclusion: An integrated community-based palliative care model tailored to Indonesia’s healthcare system is proposed to improve the quality of life for cancer patients.

Abstract

Diabetes mellitus (DM) is a significant global health issue requiring effective patient self-management to prevent severe complications like cardiovascular disease and organ damage. Health education is crucial for empowering patients, and booklets offer a cost-effective, accessible, and structured medium for delivering essential information. This scoping review investigated the use and impact of booklet-based education for DM patients. Following the Arksey and O'Malley framework, researchers searched CINAHL, PubMed, and Scopus databases for original English-language studies published between 2015 and 2024, using keywords related to diabetes, education, and booklets. From the search, 11 relevant articles were selected for analysis. The findings consistently showed that using booklets as an educational intervention effectively improved DM patients' knowledge, self-efficacy, and self-care behaviours. Key themes identified included the direct impact of booklets on knowledge enhancement, the benefit of combining theoretical information with practical application advice within the booklets, and the important role of family support in successful diabetes management, which can be facilitated by shared educational materials. The success of booklets is attributed to their structured format, ease of understanding, and continuous accessibility for patients. Factors influencing the effectiveness of this educational approach include patient literacy levels, the degree of family involvement, and the frequency of educational reinforcement. In conclusion, booklet-based education is a proven, effective strategy for improving self-management components in DM patients. The review highlights booklets as affordable and flexible tools, while also suggesting areas for future research, particularly concerning long-term clinical outcomes and the potential for integrating booklet content with modern digital health.

Abstract

Vascular complications in patients with diabetes mellitus (DM) are a major cause of morbidity and mortality, presenting a considerable impact on quality of life and global health burden. Although diabetes management continues to improve, vascular complications remain a major challenge. It is important to understand the factors that influence these complications to support more effective prevention and management. To explore the factors influencing vascular complications in patients with DM. This study used a literature review method. Articles were searched through three major databases, namely CINAHL, PubMed, and Scopus, using keywords such as "vascular complications," "DM," and "factors affecting." Inclusion criteria included English-language articles published within the last ten years (2016–2025), in the form of original research relevant to vascular complications in patients with DM. Data were extracted manually using a table that included authors, objectives, design, samples, variables, and outcomes. Analysis was conducted using a qualitative descriptive approach to identify key themes. A total of 14 articles met the inclusion criteria. From the analysis, six main themes were obtained: biochemical factors and biomarkers, clinical factors, cardiometabolic risk factors, vascular structural and anatomical changes, molecular mechanisms, and prevalence of vascular complications. The results showed that high HbA1c levels, hypertension, diabetes duration of more than 10 years, and increased biomarkers such as Bisphenol A (BPA) significantly increased the risk of vascular complications. Vascular complications in patients with DM are multifactorial, involving complex interactions between biochemical, clinical, and molecular factors.

Abstract

Background: The risk of osteoporotic fracture among persons with HIV was higher than the persons without HIV. Traditional factors are also found as the risk factor affecting fracture risk among persons with HIV and general population. Predicting the fracture risk among the high-risk group is important to develop a comprehensive fracture prevention program. Purpose: This study aimed to compare the estimation of the 10-year fracture risk between persons with HIV and persons without HIV using the FRAX™ algorithm. Methods: This study recruited 245 participants from August to November 2023, while 221 participants agreed to participate. The participants consist of 107 persons with HIV and 114 persons without HIV. The estimation of the ten-year probability of major osteoporotic and hip fractures was calculated using the FRAX™ algorithm. The participant's characteristics related to osteoporotic fracture risk was analyzed using a Chi-Square analysis. Results: The overall mean score of 10-year probability of major osteoporotic fracture (MOF) was 3.1% (SD 1.9) for the HIV group and 2.7% (SD 2.3) for non-HIV. For the 10-year probability, hip fracture (HF) risk was 0.5% (SD 0.5) for the HIV group and 0.6% (SD 0.9) for non-HIV. For MOF, HIV persons with fracture history showed a lower score (3.5%) compared to persons without HIV (5.3%). Smoker HIV persons showed the same MOF score (4.6% vs. 4.6%) but lower HF score (0.8% vs. 1.6%) when comparing to persons without HIV, respectively. HIV persons with glucocorticoid use showed a higher MOF probability score than persons without HIV (2.8% vs 2.7%). Conclusions: The 10-year fracture risk was higher among persons with HIV compared to persons without HIV. Fracture history, smoking behavior, and glucocorticoid use were identified as the potential factors associated with the risk. Further analysis using multivariate regression analysis may require to confirm the factors associated with high fracture risk.

Abstract

Background: Healthcare-associated infections (HAIs) are complications of healthcare that can increase morbidity, mortality, length of hospitalization, and additional costs, including surgical site infections (SSIs) caused by various factors such as microorganisms from the surgical environment. One important step in preventing SSIs is surgical hand preparation using antimicrobial soap or alcohol-based fluids which aims to reduce microorganisms, prevent infection, and increase the success of surgical procedures according to WHO and CDC guidelines. Purpose: To determine the effectiveness of implementing surgical hand preparation using antiseptic-based hand washing and hand rubbing methods. Method: Scoping review with article sources from PubMed, Scopus, and ScienceDirect published in the last 6 years in the period 2018-2024. The keywords used are "surgical team" or surgical staff or surgical procedures, "surgical hand preparation" or surgical cleaning or surgical rubbing, "bacteria" or microorganisms. Article assessment uses guidelines from The PRISMA Extension for Scooping Review (PRISMA-ScR). Results: The most widely used type of antiseptic fluid in the operating room area is chlorhexidine gluconate with concentrations of 1%, 3%, and 5% for the handrub method, and a concentration of 4% for the handwash-scrub method. The amount of antiseptic needed for handrub is 6-10 ml and for handwash-scrub 20 ml; and the surgical handwash and surgical handrub methods are equally effective in reducing the number of germs on the hands. Conclusion: Surgical handwash-scrub and surgical handrub are equally effective in reducing the number of bacteria on the hands, but surgical handrub with alcohol-based antiseptic and chlorhexidine is the main choice recommended by WHO and CDC because it is more practical, efficient, and effective than surgical handwash.   Keywords: Antiseptic; Handscrub; Handrub; Surgical Hand Preparation.   Pendahuluan: Health care–associated infections (HAIs) adalah komplikasi pelayanan kesehatan yang dapat meningkatkan morbiditas, mortalitas, durasi rawat inap, dan biaya tambahan, termasuk infeksi daerah operasi (IDO) yang disebabkan oleh berbagai faktor seperti mikroorganisme dari lingkungan operasi. Salah satu langkah penting dalam pencegahan IDO adalah persiapan tangan bedah menggunakan sabun antimikroba atau cairan berbasis alkohol yang bertujuan mengurangi mikroorganisme, mencegah infeksi, dan meningkatkan keberhasilan prosedur bedah sesuai pedoman WHO dan CDC. Tujuan: Untuk meyelidiki tentang efektivitas dari pelaksanaan surgical hand preparation metode handscrub dan handrub berbasis antiseptik. Metode: Scoping review dengan sumber artikel berasal dari pubMed, Scopus, dan ScienceDirect yang diterbitkan 6 tahun terakhir pada rentang 2018-2024. Kata kunci yang digunakan yaitu “surgical team” or surgical staff or surgical procedures, “surgical hand preparation” or surgical scrubbing or surgical rubbing, “bacteria” or microorganism. Penilaian artikel menggunakan panduan dari The PRISMA Extension for Scooping Review (PRISMA-ScR). Hasil: Jenis cairan antiseptik area kamar operasi yang paling banyak digunakan adalah chlorhexidine gluconate konsentrasi 1%, 3%, dan 5% untuk metode handrub, konsentrasi 4% untuk metode handwash-scrub. Jumlah antiseptik yang diperlukan untuk handrub 6-10 ml dan untuk handwash-brush 20 ml; dan metode surgical handwash dan surgical handrub sama-sama efektif dalam menurunkan jumlah kuman ditangan. Simpulan: Surgical handwash-scrub dan surgical handrub sama-sama efektif dalam menurunkan jumlah bakteri di tangan, tetapi surgical handrub dengan antiseptik berbasis alkohol dan chlorhexidine, menjadi pilihan utama yang direkomendasikan WHO dan CDC karena lebih praktis, efisien, dan efektif dibandingkan surgical handwash.   Kata Kunci: Antiseptik; Handscrub; Handrub; Surgical Hand Preparation.

Abstract

Background: Despite the effectiveness of antiretroviral therapy (ART) in reducing HIV-related morbidity and mortality, the retention of HIV care remains suboptimal in Southeast Asia. Objective: This systematic review and meta-analysis aimed to investigate the coverage of retention in care and the likelihood factors for retention in HIV care among Southeast Asian people living with HIV, to inform targeted interventions and policy improvements. Methods: Following the Preferred Reporting Item for Systematic Review and Meta-analysis (PRISMA) guidelines, this study included observational studies reporting factors associated with retention in HIV care among Southeast Asian adults, with searches conducted in PubMed, Scopus, Cochrane Library, and CINAHL up to July 15, 2024. Demographic and clinical factors were analyzed using a random-effects model with the generalized linear mixed-effect model (GLLM) to estimate proportion and the DerSimonian-Laird method to estimate odds ratios (OR) with 95% confidence intervals (CI), assessing heterogeneity using the I² statistic. Results: Among the eleven studies with 46,480 pooled participants analyzed, the coverage of retention in care revealed a pooled proportion of 75.2% (95% CI: 66.7-82.1). Significant clinical factors associated with a higher likelihood of retention included high CD4 count (≥200 cells/mm³) (OR 2.17 (95%CI: 1.19-3.97, p = 0.01), WHO stage 3-4 (OR 2.06, 95%CI: 1.09-3.87, p = 0.02), not being on ART (OR 6.88, 95%CI: 1.89-25.06, p = 0.001), hemoglobin levels ≥10 g/dL (OR 0.50, 95% CI: 0.25-0.99, p = 0.04), and demographic factors of employment (OR 1.18, 95% CI: 1.02-1.38; p = 0.03). Other clinical factors, such as HIV stage, TB co-infection, drug abuse/substance use, and hemoglobin levels, did not significantly affect the likelihood of retention. Similarly, demographic factors such as age, gender, education, marital status, and geographic setting also showed no significant impact on likelihood retention. Conclusion: Retention in care among Southeast Asian people living with HIV was still below 95%. Clinical factors, particularly high CD4 counts, WHO stage, and the absence of ART, were likelihood factors for retention in HIV care, whereas other clinical and demographic factors studied did not show a significant impact. A universal test and treatment strategy is required to improve retention in care.

Abstract

Hemodialysis (HD) is part of the therapy for chronic kidney disease (CKD) patients to partially replace kidney function. However, the routine process of hemodialysis, which is performed two to three times a week, has an impact to patient’s quality of life. Spiritual-based interventions, such as listening to Quran recitation, are emerging as a complementary-therapies to address the symtomps experienced by patients undergoing hemodialysis. However, the potential use of listening to the quran in patients undergoing hemodialysis remains limited. Therefore, this study aims to explore and assess the effects of listening to Quran recitation in CKD patients undergoing hemodialysis. The scoping review was used in this study. The eligibility criteria of this study, include all full-text primary studies written in English and published between 2015 and 2024. The literature systematically searched using 3 databases, such as PubMed, Scopus, and EbscoHost with keywords "chronic kidney disease AND listening quran OR listening koran AND hemodialysis.” Additionally, search engine, Google Scholar, also used to identify additional literature. A total of 9 articles were included consisting of 3 RCTs and 6 quasi experimental studies. The effects of listening to Quran recitation were divided into two benefit cluster. 1) Psychological benefits, such as reduced anxiety, lower depression, and improved quality of life. 2) Physical benefits, such as improved physical condition and reduced fatigue, lower blood pressure, and reduced systemic inflammation. The findings suggest that Quran recitation can be a valuable, non-invasive adjunctive therapy to support holistic care models for CKD patients undergoing hemodialysis.

Abstract

Ischemic stroke is a cause of physical disability. These defects result in impaired biopsychosociospiritual adaptation in stroke patients. The inability to adapt will cause an anxiety reaction which triggers an increase in cortisol levels. The spiritual care model helps patients find meaning and purpose in life as well as an optimistic attitude so that they have better adaptability. This type of research is Quasy Experimental with a pretest-posttest control group design. The study population was ischemic stroke patients in the Outpatient Department of Dr. Regional General Hospital. Moewardi Surakarta. The total sample consisted of 60 respondents, divided into 30 control group respondents and 30 intervention group respondents, who received a spiritual care model intervention in the outpatient room of Dr RSUD. Moewardi Surakarta. The research results show the influence of the Spiritual Care Model Using Callista Roy's Adaptation Theory Approach on Cortisol Levels in Ischemic Stroke Sufferers with a significance value of 0.043 < 0.05. The Spiritual care model influences the amygdala, which stimulates the hypothalamus through the hypothalamic-pituitary-adrenal (HPA) axis, thereby inhibiting the excretion of corticotropin-releasing factor (CRF). The pituitary gland then stimulates adrenocorticotropic hormone (ACTH), which reduces cortisol. This research has a contribution that spiritual care interventions can improve the adaptability of biopsychosocial rehabilitation of stroke patients. Abstract

Currently, there are no suggested psychological interventions or outcomes in the diabetic management guidelines. This case report presents the management of a 66-year-old female patient, Mrs. R, with diabetes mellitus, hypertension, and gastroesophageal reflux disease (GERD) who underwent exploratory laparotomy for retained stone exploration in the common bile duct (CBD). This Case report aimed to explore the implementation of mindfulness in patients post-operative. The patient experienced significant hyperglycemia, with blood glucose levels peaking at 436 mm Hg. Psychological assessment revealed a severe anxiety score of 31, which likely contributed to her unstable glycemic control. Throughout her unstable glycemic control. Throughout her severe condition, the patient was managed with total parenteral nutrition (TPN) due to fasting and green gastric contents, as part of the nursing intervention. Comprehensive nursing interventions, including regular blood glucose monitoring, insulin administration, and psychological support were employed. Mindfulness techniques such as guided meditation and breathing exercises have been introduced to alleviate hospital-induced distress and manage blood glucose levels. Following these interventions, her blood glucose level decreased to 167 mmHg, and the patient reported feeling calmer and more spiritually connected (Hamilton Rating Scale/ HARS 18). This case describes the management of post-surgical diabetic patients, highlighting the benefits of integrating psychological interventions to improve glycemic control and overall patient outcomes. Therefore, psychological interventions, particularly mindfulness, are needed for integration in diabetes management, especially in hospitals.

Abstract

Background: Human Immunodeficiency Virus (HIV) is a virus that attacks lymphocytes, leading to a gradual decline in the body's immune system and increasing susceptibility to various diseases, including Acquired Immunodeficiency Syndrome (AIDS). Men who have sex with men (MSM) are at a higher risk of contracting HIV/AIDS due to unsafe sexual behavior. Objective: This scoping review aims to explore the factors associated with safe sexual behavior in MSM Living with HIV. Method: The scoping review is based on the framework by Arksey and O'Malley. A literature search involved various databases, such as EBSCO, PubMed, Science Direct, and CINAHL I, with the keywords: "Factors Associated" AND "Safe Sex Behavior" AND "Men Who Have Sex With Men OR MSM OR LSL". The selection process for research articles involved the use of inclusion criteria. The results were analyzed by extracting data into a data mapping table (PRISMA Flow Diagram). Results: 20 articles discussed factors related to safe sexual behavior among men with HIV/AIDS. These factors include individual perception, knowledge, and values. Discussion: It can be concluded that individual perception strongly influences safe sexual behavior among men, as individual perceptions can guide and motivate behavior.
