Abstract

Prevalensi asma yang terus meningkat menjadi perhatian bagi tenaga kesehatan. Kepatuhan pasien asma dalam penggunaan obat dan terwujudnya kualitas hidup pasien yang baik menjadi tujuan utama dalam penatalaksanaan asma. Penelitian ini bertujuan untuk mengetahui hubungan antara kepatuhan penggunaan antiasma dengan kualitas hidup pasien asma. Populasi target dalam penelitian ini adalah semua pasien rawat jalan yang  menggunakan terapi anti asma pada tahun 2012. Sampel penelitian ini adalah pasien asma dewasa dengan kategori asma menetap. Sebanyak 72 pasien diambil secara purposive dari populasi target. Penelitian ini menggunakan metode penelitian non eksperimental dengan rancangan cross sectional. Pengambilan data dilakukan secara concurrent dan retrospektif. Tingkat kepatuhan pasien diukur menggunakan kuesioner MMAS (Morisky Medication Adherence Scale) dan kualitas hidup pasien diukur menggunakan AQLQ (Asthma Quality of LifeQuestionnaire). Hasil penelitian menunjukkan 52,78% pasien memiliki kepatuhan rendah dan sebesar 47,22% pasien memiliki kepatuhan sedang. Faktor yang berhubungan secara signifikan dengan kepatuhan adalah pola kunjungan pasien. Jumlah pasien asma dengan kualitas hidup kurang baik sebesar 30,56% dan pasien dengan kualitas hidup sedang sebesar 69,44%. Penelitian menyimpulkan bahwa tidak terdapat hubungan antara kepatuhan penggunaan antiasma dengan kualitas hidup pasien asma (p = 0,842).

Abstract

Exacerbation of asthma is a progressive step in progressive breathing, coughing, wheezing, breathing. In Indonesia salbutamol nebulation is used for the therapy of the first exacerbation attack. In addition, intravenous aminophylline is also recommended as a therapy, even if the use of aminophylline abroad is reduced due to its smaller effectiveness and often leads to greater unwanted reaction events. The effectiveness of asthma exacerbation therapy needs to be monitored and one of the monitoring parameters that is easily seen is the rate of breath.The aim of this study was to investigate the difference in respiratory rate values using salbutamol nebulisation and intravenous aminophylline in patients with asthma exacerbations in hospitals in Surabaya. The research was conducted by using quasi experimental method and data collection by purposive sampling. Analysis of the data to be used is a independent t-test. The results showed that the salbutamol group decreased respiratory rate by 50.00%, there were also patients who experienced an increase of breath rate of 13.64%. In the aminophylline group experienced a decrease in respiratory rate of 43.75% with an increase of breath rate of 18.75%. From the results of the study, it is necessary to monitor the effectiveness and the need to monitor the unwanted reactions of both treatments so that the therapies produced in the treatment of asthma exacerbations are more effective.Key words: Aminophylline, exacerbation asthma, respiratory rate, salbutamol

Abstract

Kualitas hidup merupakan salah satu acuan keberhasilan dari upaya pengobatan termasuk kemoterapi pada pasien kanker. Pasien kanker yang menjalani kemoterapi dapat mengalami perubahan pada kualitas hidupnya. Tujuan penelitian ini adalah untuk menilai kualitas hidup pasien kanker serviks dengan menggunakan kuesioner EQ-5D sebelum dan setelah mendapatkan kemoterapi. Penelitian ini merupakan penelitian non-eksperimental bersifat analitik menurut perspektif pasien dengan teknik pengumpulan data secara prospektif. Subjek penelitian adalah pasien kanker serviks rawat inap kelas 3 yang memenuhi kriteria inklusi. Penelitian dilakukan di RSUP Dr. Hasan Sadikin Bandung. Data yang digunakan adalah data pasien dari bulan Juni sampai Desember 2015. Penelitian dilakukan dengan menggunakan kuesioner EQ-5D. Selanjutnya dilakukan perhitungan persentase permasalahan pasien, nilai indeks EQ-5D (utility) dan nilai EQ-5D VAS. Nilai utility dan EQ-5D VAS dianalisis secara statistik dengan uji Wilcoxon (α=5%) untuk melihat terjadinya perubahan kondisi yang bermakna sebelum dan setelah kemoterapi. Hasil menunjukkan bahwa setelah kemoterapi siklus pertama, persentase masalah kemampuan berjalan/bergerak dan kegiatan yang biasa dilakukan mengalami peningkatan, rasa sakit/tidak nyaman dan rasa cemas/depresi mengalami penurunan, sedangkan perawatan diri tidak mengalami perubahan. Berdasarkan hasil uji statistik, setelah kemoterapi siklus pertama, nilai utility dan EQ-5D VAS mengalami peningkatan dan menunjukkan peningkatan bermakna (nilai T hitung masing-masing 2,0 dan 4,5) pada pasien stadium I.Kata kunci: Kanker serviks, kemoterapi, kualitas hidup, kuesioner EQ-5D Quality of Life Evaluation of Cervical Cancer Inpatients Before and After Chemotherapy by Using the EQ-5D QuestionnaireAbstractThe quality of life is one of success parameters of the treatment effort including chemotherapy in cancer patients. Cancer patients undergoing chemotherapy may experience changes in their quality of life. This study aimed to assess the quality of life of patients with cervical cancer in Indonesia using the EQ‑5D questionnaire pre and post-chemotherapy. The research was an analitycal non-experimental study according to the patient’s perspective using prospective data collection techniques. The research subjects were the patients who suffered from cervical cancer who were hospitalized in class 3 and fullfilled the inclusive requirement. The research was conducted in Dr. Hasan Sadikin Bandung general hospital, from June until December 2015. The study was conducted by the EQ-5D questionnaire. Futhermore, the percentage of the patients’ problems, EQ-5D index (utility) and EQ-5D VAS were calculated. Utility and EQ-5D values was analysed statistically using Wilcoxon test (α=5%) to determine if there were significant condition changes before and after chemotherapy. The results showed that after the first cycle of chemotherapy, the percentage of patients reporting problem is related to ability to walk/move and the usual activities problem increased, pain/discomfort and anxiety/depression problem decreased, while the self‑care problem did not change. After the first cycle of chemotherapy, the utility and EQ-5D values increased and showed significant improvement (T count value=2.0 and 4.5 respectively) in patients with stage I. Abstract

Background: Diabetes Mellitus is a chronic disease whose prevalence is high enough at RS PKU Muhammadiyah Yogyakarta, particularly since National Health Insurance was launched on 2014. Prospective payment system with INA CBGs rates has to be calculated thoroughly in order to enable the hospital to provide a quality health care. This research aims to determine the cost component that has the greatest impact to the cost of illness Diabetes Mellitus, and to determine whether there are significant differences Alt the direct medical costs of DM with and without complication, and to calculate the difference between the real cost of illness and INA CBGs rates. METHOD This research was conducted at PKU Muhamadiyah Hospital with non experimental analytic and cross-sectional study. Data retrospectively retrieved from secondary data of all National Health Insurance patients who was diagnosed Diabetes Mellitus based on ICD 10 code. The cost calculated was the direct medical costs (hospital perspective). The bivariate correlation test was used to determine the relationship of component costs and the cost of illness Diabetes Mellitus disease. The independent sample t-test was used to test differences in the average cost of DM with and without complications. RESULTS Cost of illness DM type 1 for outpatient was Rp 563.817, higher than INA CBGs rates which is Rp 165.400 (Q-5-44-O, Other Chronic Disease). Cost of illness DM type 2 for outpatient was Rp 374.197, higher than INA CBGs rates of Rp 165.400 (Q-5-44-O, Other Chronic Disease). Cost difference between cost of illness DM for inpatient and INA CBGs (code E-4-10-I Diabetes Mellitus and Nutrition/ Metabolic light disorder) rates was Rp 445.748 lower than INA CBGs rates (Class III ward), Rp 2.250.000 lower than INA CBGs rates (Class II ward), and Rp 1.125.000,- is higher than INA CBGs rates (Class I ward). There is no significant difference between the average cost of illness Diabetes Mellitus with and without complication. CONCLUSIONThere is a difference between the cost of illness Diabetes Mellitus (hospital perspective) and INA CBGs rates which could decrease quality of care for outpatient with type 1 and type 2 Diabetes Mellitus. PENDAHULUAN Diabetes Melitus merupakan salah satu penyakit kronis yang prevalensinya cukup tinggi di RS PKU Muhammadiyah Yogyakarta apalagi sejak diluncurkannya program Jaminan Kesehatan Nasional pada th 2014 yang lalu sebagai implementasi Undang-Undang No 40 tahun 2004 tentang Sistem Jaminan Sosial Nasional. Sistem pembayaran prospective payment system dengan tarif INA CBGs yang diberlakukan memerlukan perhitungan yang valid agar tidak merugikan pemberi pelayanan dan menurunkan kualitas pelayanan itu sendiri. METODE Penelitian ini dilakukan di RS PKU Muhammadiyah Yogyakarta menggunakan metode analitik non eksperimental dengan rancangan cross sectional study. Data yang digunakan untuk penelitian ini adalah data biaya terapi semua pasien JKN yang didiagnosa Diabetes Mellitus berdasarkan kode ICD 10. Biaya yang diperhitungkan adalah biaya medik langsung (direct medical cost) meliputi biaya administrasi, biaya obat dan alat medik habis pakai, jasa pelayanan medik, biaya tindakan medik, biaya penunjang medik serta biaya akomodasi, khusus untuk pasien rawat inap. Data diambil secara retrospektif, kemudian dianalisis secara deskriptif dan perbedaan biaya terapi per episode antara DM dengan komplikasi dan DM tanpa komplikasi diuji secara statistik. Biaya terapi yang diperoleh kemudian dibandingkan dengan tarif INA CBGs untuk melihat selisih yang terjadi. HASIL Biaya terapi penyakit DM tipe 1 per episode rawat jalan sebesar Rp 563.817 + 255.080, dan yang masuk dalam paket sebesar Rp 247.200,- , lebih tinggi Rp 81.400,- dari tarif INA CBGs yang telah ditetapkan dalam PMK 59 th 2104 yaitu sebesar Rp 165.400,- (kode Q-5-44-O, Penyakit Kronis kecil lain-lain). Untuk penyakit DM tipe 2, biaya terapi per episode rawat jalan sebesar Rp 374.197 + 323.237, dan yang masuk dalam paket sebesar Rp 185.376,- , selisih Rp 19.976,- lebih tinggi dari tarif INA CBGs yang telah ditetapkan dalam PMK 59 th 2104 yaitu sebesar Rp 165.400,- (kode Q-5-44-O, Penyakit Kronis kecil lain-lain). Selisih biaya terapi pasien DM per episode rawat inap dengan Tarif INA CBGs (kode E-4-10-I Penyakit Kencing Manis dan Gangguan Nutrisi/Metabolik Ringan), untuk kelas III selisih positif Rp 445.748,-. untuk kelas II selisih positif Rp 2.250.000,- dan untuk kelas I selisih negatif Rp 1.125.000,-. Tidak terdapat perbedaan yang signifikan secara statistik antara biaya terapi DM tanpa komplikasi dengan biaya terapi DM dengan komplikasi. Hampir seluruh obat yang digunakan adalah obat DM yang masuk dalam FORNAS. KESIMPULAN Terdapat selisih biaya terapi penyakit DM pasien JKN antara tarif Rumah Sakit PKU Muhammadiyah Yogyakarta dengan tarif INA CBGs yang berpotensi menimbulkan kerugian bagi RS PKU Muhammadiyah Yogyakarta serta berpotensi menurunkan kualitas pelayanan untuk pasien rawat jalan baik DM tipe 1 maupun DM tipe 2.

Abstract

Surgical Site Infection/SSI is the result of the contamination of bacteria whichenter the body during surgery. High risk of SSI is occured in gastrointestinal surgeryand influenced by the characteristics of the patients such as age and comorbidity sincethey deal with immunity system of the body. The research was aimed to identity theinfluence of age and comorbidity to the high risk of SSI in gastrointestinal surgerypatients. The research was a cross sectional research conducted prospectively at PKUMuhammadiyah Hospital Yogyakarta from January to March 2012. The researchsubjects were all inpatients undergoing gastrointestinal surgery, having visiblesurgical site and using antibiotic to prevent surgical site infection. There are 68patients in the research. Data that were analyzed comprised the characteristics ofpatients such as age and congenital disease to the risk of surgical site infection. Theresult of this research indicated that 2 patients (2,94%) suffering SSI. Based on the age,SSI happened to 2 patients in the age range of 41-60 years old. However, based on thestatistical result, there was no correlation between age and the existence of SSI(p>0.05). SSI also occured in 2 patients with comorbidity. However, there wascorrelation between the existence of comorbidity and SSI (p<0.05).

Abstract

The incidence of bleeding is increased in high risk patients with critical conditions in Intensive Care Unit (ICU). Appropriate prophylaxis medicine is necessary to reduce the incidence of stress ulcer bleeding during hospitalization in ICU which further can minimize the cost of patient care in the ICU. Currently, lansoprazole and pantoprazole are used as a stress ulcers prophylaxis. Nevertheless, there has not been sufficient evidence proving their effectiveness. This study was aimed to compare the effectiveness and cost between iv lansoprazole and iv pantoprazole as a stress ulcer prophylaxis in ICU. In this retrospective observational study, the data were obtained from medical records of all patients admitted to ICU in a District Hospital in Yogyakarta from January 2014 until December 2016. Effectiveness of therapy were obtained from objective data in the medical record by looking at the incidence of major (clinically significant) and minor (overt gastrointestinal) bleeding. Chi-square analysis was performed to analyze the difference of bleeding incidence. The average cost was presented as Expected Monetary Value (EMV), which consisted of the cost of prophylaxis and treatment of bleeding. The difference in average cost was analyzed using independent t-test. A total of 119 patients were included in this study. There were 62 patients in the lansoprazole group and 47 patients in the pantoprazole group. Forty eight patients (77,4%) from lansoprazole group and 35 patients (61,4%) from pantoprazole group did not experience any GI bleeding. There was no statistical difference in the incidence of GI bleeding between the two groups (CI 95%, P-value = 0.057). The EMV of lansoprazole group was higher than pantoprazole group IDR 645.122,57 and IDR 511.629,39 respectively. In conclusion, there was no significant difference regarding the effectiveness between iv lansoprazole and iv pantoprazole as stress ulcer prophylaxis. Costs for the prophylaxis of stress ulcers is lower on the use of pantoprazole compared to lansoprazole.

Abstract

Counseling is one part of the clinical pharmacy service at the pharmacy that is the responsibility of the pharmacist to achieve therapeutic goal. Community needs for counseling have increased so that the active participation of pharmacists is needed through the provision of professional services. This research needs to be done to find out the amount of counseling services by pharmacists who are willing to be paid by patients with a WTP (Willingness to Pay) value parameter. In addition, the value of WTP can also describe the indirect benefits felt by the community towards counseling services by pharmacists at the pharmacy. This research is a descriptive observational research through a survey. The study was conducted at the pharmacy in the Polokarto sub-district of Sukoharjo regency in January – February 2016 with pharmacy visitor respondents who received counseling services by pharmacists. A total of 82 patients participated in this study. The amount of WTP is determined based on the average WTP value chosen by the patients with the payment card method. The result showed that the average WTP value of pharmacist counseling services at pharmacies in Sukoharjo regency was Rp. 15,892.

Abstract

Inadequate antibiotic therapy for meningitis and bacterial encephalitis is one of the factors that can jeopardize patient safety and turn into a public health issue in a number of nations, including Indonesia. This phenomena has the potential to raise health-care expenses, which should be avoided in the era of the JKN program's implementation. The goal of this research was to describe the profile of antibiotic use and cost in hospitalized meningitis and bacterial encephalitis patients. This cross-sectional observational study took place from January to December 2019. As study materials, patient medical records and billing data were employed. To enhance the data obtained, confirmation with medical and pharmaceutical personnel is required. Antibiotic use and expenditures were described using descriptive analysis. A total of 71 patients, both children and adults, met the study's inclusion criteria. Antibiotics were given to all of the patients, and the results revealed that third generation cephalosporins (49.375%) were the most commonly recommended antibiotic class, either alone or in combination. Antibiotic use receives 46.94% of total medication funding. According to the length of stay category, the majority of patients (45.99 %) were hospitalized for 8-14 days consecutively with antibiotic medication. Antibiotics, which are typically administered to patients with meningitis and bacterial encephalitis, had no effect on the length of stay in the hospital. Antibiotic prescriptions must be carefully reviewed at top referrel hospital, taking into account the local germ map.

Abstract

Resiko kejadian perdarahan meningkat pada pasien dengan kondisi kritis di intensive care unit (ICU) sehingga kebutuhan penggunaan profilaksis cukup tepat untuk mengurangi kejadian perdarahan selama di ICU. Kajian penggunaan profilaksis yang efektif sangat diperlukan untuk mencegah komplikasi tersebut dengan harapan dapat meminimalkan kejadian perdarahan saluran cerna pada pasien di ICU. Penelitian ini bertujuan untuk mengetahui efektivitas dan biaya penggunaan lansoprazol dan pantoprazol sebagai profilaksis stress ulcers di ICU rumah sakit di Yogyakarta. Penelitian ini merupakan penelitian non eksperimental dengan rancangan penelitian kohort retrospektif. Subjek penelitian adalah seluruh pasien rawat inap di ICU dengan usia ≥ 18 tahun sampai 65 tahun yang mendapatkan terapi obat lansoprazol dan pantoprazol sebagai profilaksis stress ulcers selama bulan Januari 2014 – Desember 2016 yang memenuhi kriteria inklusi dan eksklusi. Penelitian dilakukan dengan membandingkan efektivitas terapi obat lansoprazol dan pantoprazol dengan melihat kejadian perdarahan minor dan mayor. Data penelitian dianalisis menggunakan Chi-Square untuk melihat perbedaan efektivitas antara 2 kelompok terapi. Dari total 119 pasien yang memenuhi kriteria inklusi, terdapat 48 pasien (77,4%) dari kelompok terapi lansoprazol yang tidak mengalami kejadian perdarahan, sedangkan pada kelompok pantoprazol pasien yang tidak mengalami kejadian perdarahan sebanyak 35 pasien (61,4%). Secara statistik efektivitas lansoprazol dan pantoprazol sebagai profilaksis untuk mengatasi kejadian perdarahan tidak berbeda signifikan (P = 0,057). Dari penelitian ini dapat disimpulkan bahwa tidak terdapat perbedaan efektivitas antara lansoprazol dan pantoprazol sebagai profilaksis stress ulcers pada pasien dengan resiko tinggi perdarahan saluran cerna di ICU.
Economic evaluation of antibiotic usage for cesarean section in PKU Muhammadiyah Yogyakarta Hospital Tri Murti Andayani; Riswaka Sudjaswadi

Abstract

In the developed countries, the incidence of cesarean section year by year increases by 7 –10 %. There have been various kinds of cesarean section in several hospitals. In PKU Muhammadiyah Yogyakarta hospital, the incidence of cesarean section in 2001 increased up to 21 %. There are also various kinds of antibiotics used to deal with cesarean section cases which caused various kinds of medical cost. Therefore, identification of antibiotics cost, which used to deal with cesarean section in PKU Muhammadiyah Yogyakarta hospital was studied.The study was performed by epidemiological survey of cesarean section via patient’s medical record, medicinal cost for treatment and surgery, and patient’s invoice in the year 2000. The study involved recording identification of patients and physicians identities, antibiotics and other medicines used, surgical equipment, cesarean section indications, pharmacy and other cost, duration of staying related to cesarean section.Results showed that the majority of cesarean section has prolonged labour (26%). The majority of antibiotics used in cesarean section is Sulbenicillin injection with Klindamicin oral (31%) and Amoxicillin/potassium clavulanate injection with Amoxicillin oral (23%). Cost of antibiotics were 12.5% - 44.4% of pharmacy expenses and 6.7% - 14.8% of total expenses. No correlation between antibiotics cost and hospital stays.Key words : cost, antibiotic, cesarean section
