Abstract

The use of metformin in insulin therapy hopefully could increase effectiveness of type 2 diabetes treatment equal with the expenditure. Non experimental research was conducted to investigate cost-effectiveness of insulin use and insulin-metormin combination. Data was taken prospectively from baseline until month 3. The measurable cost was direct medical cost which covered cost of antidiabetic drugs, cost of complication drugs, doctor’s fee, and cost of laboratory checkup. The measurable effectiveness of therapy was reduction of FPG and HbA1c level. ACER (average cost-effectiveness ratio) was calculated based on cost and effectiveness ratio in both groups. ICER (incremental cost-effectiveness ratio) was calculated based on difference of cost and effectiveness ratio in both groups. Results showed FPG level reduction in insulin group and insulin-metformin combination consecutively was 25,05% and 1,03%. Increase of HbA1c level in insulin group and insulin-metformin combination consecutively was 6,21% and 1,87%. Based on HbA1c level reduction, ACER value in insulin group and insulin-metformin combination consecutively was Rp -37.617,00 and Rp -161.729,00, ICER value was Rp 15.860,00. From the results it can be concluded that insulin-metformin combination was more cost-effective than insulin in maintaining HbA1c level. ABSTRAK Penelitian ini dirancang untuk mengetahui efektivitas biaya pada pasien DM tipe 2 yang menggunakan insulin dan kombinasi insulin-metformin. Data diambil secara prospektif dari baseline hingga bulan ke-3. Analisis efektivitas biaya dilakukan dengan perspektif ASKES. Komponen biaya yang diukur adalah biaya medik langsung, mencakup biaya antidiabetik, biaya komplikasi, biaya kunjungan ke klinik dan biaya pemeriksaan laboratorium. Efektivitas terapi yang diukur adalah penurunan kadar GDP dan HbA1c. ACER (average cost-effectiveness ratio) dihitung berdasarkan rasio biaya dan efektivitas terapi pada kedua kelompok terapi. ICER (incremental cost-effectiveness ratio) dihitung berdasarkan rasio antara selisih biaya dan efektivitas terapi pada kedua kelompok terapi. Hasil penelitian menunjukkan penurunan kadar GDP pada kelompok insulin dan kombinasi insulin-metformin berurutan adalah 25,05% dan 1,03%. Peningkatan kadar HbA1c pada kelompok insulin dan kombinasi insulin-metformin berurutan adalah 6,21% dan 1,87%. Berdasarkan penurunan kadar HbA1c, nilai ACER pada kelompok insulin dan kombinasi insulin-metformin berurutan adalah Rp -37.617,00 dan Rp -161.729,00, nilai ICER adalah Rp15.860,00. Berdasarkan hasil penelitian dapat disimpulkan bahwa kombinasi insulin-metformin lebih cost-effective dalam mempertahankan kadar HbA1c.

Abstract

Counseling is one part of the clinical pharmacy service at the pharmacy that is the responsibility of the pharmacist to achieve therapeutic goal. Community needs for counseling have increased so that the active participation of pharmacists is needed through the provision of professional services. This research needs to be done to find out the amount of counseling services by pharmacists who are willing to be paid by patients with a WTP (Willingness to Pay) value parameter. In addition, the value of WTP can also describe the indirect benefits felt by the community towards counseling services by pharmacists at the pharmacy. This research is a descriptive observational research through a survey. The study was conducted at the pharmacy in the Polokarto sub-district of Sukoharjo regency in January – February 2016 with pharmacy visitor respondents who received counseling services by pharmacists. A total of 82 patients participated in this study. The amount of WTP is determined based on the average WTP value chosen by the patients with the payment card method. The result showed that the average WTP value of pharmacist counseling services at pharmacies in Sukoharjo regency was Rp. 15,892.

Abstract

Latar belakang: Penggunaan antibiotik profilaksis ditujukan untuk mengurangi ILO. Penggunaan antibiotik profilaksis terbukti dapat mengurangi kejadian ILO dan dianjurkan untuk diberikan pada tindakan yang memiliki tingkat risiko infeksi yang tinggi. Antibiotik yang digunakan dalam seksio sesarea berdasarkan laporan ASHP  adalah golongan penisilin dan sefalosporin generasi pertama (cefazolin). Tujuan: Mengetahui efektivitas biaya pada penggunaan antibiotik profilaksis amoxicillin dan cefazolin pada kasus bedah sesar di RSUD Jombang tahun2017. Metode: Desain penelitian cross-sectional yang dilakukan secara retrospektif dengan perspektif Rumah Sakit. Hasil: Cefazolin memiliki nilai efektifitas yang lebih besar dan nilai ACER yang lebih kecil dari pada amoxicillin yaitu nilai efektivitas terapi 0,907 dengan nilai ACER Rp6.696.388.81 sedangkan amoxicillin 0,861 dengan nilai ACER Rp7.231.116.61. Simpulan: Antibiotik cefazolin lebih cost-effective dibandingkan dengan amoxicillin.

Abstract

Pharmacy Department A.M. Parikesit Regional General Hospital always working to improve the quality of service, analyses is need to find the performance in Pharmacy Department A.M. Parikesit Regional Public Hospital. The concept of the balanced scorecard is a performance evaluation tool that is comprehensive, coherent and relevant to be applied in hospitals for improving the performance of every aspect of total. The study to evaluate the performance Pharmacy Department A.M. Parikesit Regional General Hospital through learning and growth perspective. This research used non experimental case study arrangement research the explorative descriptive approach. Data collection was retrospective and prospectively in the form of quantitive data. Qualitative data was obtained by questionnaires, depth interview with the head of Pharmacy Departmen A.M. Parikesit. The quantitative data obtained through direct observation and personnel file. The results of research with the used balanced scorecard at a performance perspective learning and growth shown that: a. human capital: Good category productivity of employess, the level of employee productivity is 86.73%, the percentage of employees who received training in 2013, 2014 and from January to July 2015 is respectively for 15.38%, 32.25% and 97, 72%, the average value of the high category in morale, job satisfaction, knowledge, empskill and talent; b. information capital showed the average category of high on the aspect of information technology, a database and network, but need the information management system updates for support the management and drug reporting and need to improve communication with the customer network.; c. organization capital showed the value of the average category of high on the aspect of cultural organization ,leadership and alignment.

Abstract

ABSTRACTBreast cancer still became priority concern in developed country, but 69% the dead incident came from developing country. Many of them been diagnose at advance stage, that make treatment and cost more higly. The objectives of this study were to know the burden cost of breast cancer patient in Prof Dr Kandou Medical Center Manado based on hospital perspective, the difference between the real cost and the INA-CBG’s cost, and the key factors that might drive the burden of the real cost of breast cancer. The study was a pharmacoeconomics analys based on hospital perspective to direct medical cost, using non-experimental analysis with cross-sectional design. Data restropectively collect from breast cancer inpatient registered as member of National Health Insurance (JKN) during September 2017-August 2018 period and from financial department of Prof Dr Kandou Medical Center Manado. The sample was used 329 episode of treatment. the data being analyzed using one sample t-test to compare the congruence between the real cost and INA-CBG’s cost. Multivariat linear regression Analysis to find the various predictor factors that might affect on the real cost. The result of the study showed that the average of the real cost per episode of treatment in chemotherapy patient with C-14-3 code was Rp. 13.806.325, treatment in breast procedure with L-1-50 code was Rp. 24.306.626, and treatment for breast tumor with L-4-11 code was Rp. 11.989.772. The difference value between INA-CBG’s and the real cost was Rp. -668.973.281 for 183 breast cancer patient. The congruence value between the real cost and the INA-CBG’s cost generally significantly different except for C-4-13-II, L-1-50-III and L-4-11-I. Based on multivariat linier regression analysis, severity level, :LOS, and treatment care was the factors that affect the real cost. Keywords : Cost of illness, INA-CBG’s, Breast Cancer, Prof. Dr. R.D Kandou Medical Center Hospital Manado ABSTRAKKasus kanker payudara memang menjadi permasalahan penting di negara maju, namun 69% angka kematian terjadi di negara berkembang. Kebanyakan kasus kanker payudara ditemukan berada pada stadium lanjut, dimana menyebabkan upaya dan nilai biaya pengobatan semakin besar. Tujuan penelitian ini adalah untuk mengetahui biaya terapi penyakit kanker payudara di RSUP Prof Dr Kandou Manado berdasarkan perspektif rumah sakit, mengetahui perbedaan antara tarif rumah sakit dengan tarif paket INA-CBGs pada perawatan pasien kanker payudara, dan mengetahui faktor apa saja yang berpengaruh terhadap besarnya biaya terapi penyakit kanker payudara. Penelitian ini merupakan analisis farmakoekonomi berdasarkan perspektif rumah sakit terhadap biaya medis langsung, berupa penelitian analitik noneksperimental dengan rancangan penelitian cross sectional. Data diambil secara retrospektif dari pasien rawat inap kanker payudara peserta Jaminan Kesehatan Nasional (JKN) selama periode September 2017-Agustus 2018 dan dari bagian keuangan di RSUP Prof Kandou Manado. Sampel yang digunakan adalah sebanyak 329 episode perawatan. Analisis data dilakukan dengan one sample t-test untuk membandingkan total biaya riil dengan tarif INA-CBGs. Analisis korelasi multivariat regresi linier untuk mengetahui faktor yang mempengaruhi total biaya riil. Hasil penelitian pasien rawat inap peserta JKN di RSUP Prof Dr Kandou Manado periode September 2017 – Agustus 2018 menunjukkan rata-rata biaya riil pasien kemoterapi per episode perawatan dengan kode C-4-13 sebesar Rp. 13.806.325, tindakan pada payudara dengan kode L-1-50 sebesar Rp. 24.306.626 dan tumor payudara dengan kode L-4-11 sebesar Rp. 11.989.772. Selisih tarif INA-CBG’s dengan total biaya riil adalah sebesar Rp.-668.973.281 untuk total 183 pasien. Kesesuaian total biaya riil dengan tarif INA-CBG’s menggunakan one sample t-test secara umum berbeda bermakna, kecuali pada kode C-4-13-II, L-1-50-III dan L-4-11-I. Abstract

Health Chronic Disease Management Program (Prolanis) BPJS Health as a system of health services and proactive approach implemented in an integrated manner involving participants, health facilities and BPJS Health becomes an important factor because it becomes one of the indicators of the fulfillment of the commitment of service at puskesmas. This study aims to determine the management of Prolanis which includes membership, activities, availability of drugs and availability of funds Prolanis, knowing the barriers to the implementation of Prolanis and know the ratio of visits and quality of life of participants Prolanis. The research was conducted by descriptive method through interview technique, check list and WHOQoL questionnaire for quality of life measurement. Data retrieval used primary data by interviewing 40 informants and filling questionnaire on 262 participants of Prolanis. The secondary data was obtained from P-Care Puskesmas application. Analysis of the results of research was using descriptive analysis with narrative exposure accompanied by data tables. The results showed that from 20 Puskesmas 1 Puskesmas had not fulfilled the minimum requirement of Prolanis formation, 4 Puskesmas had not conducted gymnastic and educational activities, 2 Puskesmas did not implement pharmacy service standard for drug distribution Prolanis and 4 Puskesmas had not utilized Prolanis fund provided by BPJS Kesehatan. 50% in some puskesmas cause puskesmas to be in unsafe zone for assessment of capitation indicator. For measurement of quality of life of participants Prolanis obtained 95, 04% of respondents have good quality of life.

Abstract

Rumah Sakit Umum Daerah dr. Abdul Aziz Singkawang sebagai Badan Layanan Umum Daerah (BLUD) perlu melakukan pengukuran tentang pencapaian indikator-indikator Standar Pelayanan Minimal Rumah Sakit yang telah ditetapkan. Penelitian ini bertujuan untuk mengetahui gambaran pelaksanaan Standar Pelayanan Minimal Rumah Sakit bidang farmasi di Instalasi Farmasi RSUD dr. Abdul Aziz Singkawang yaitu meliputi waktu tunggu pelayanan obat, tidak adanya kejadian kesalahan pemberian obat, kepuasan pelanggan dan penulisan resep sesuai formularium. Penelitian ini merupakan penelitian non eksperimental deskriptif dengan menggunakan metode pendekatan secara kualitatif dan kuantitatif. Penelitian dilakukan selama 1 bulan pada bulan Desember 2016. Data penelitian dikelompokkan menjadi data kualitatif dan kuantitatif. Data Kualitatif berupa wawancara mendalam dengan Kepala Instalasi Farmasi, karyawan, dokter sedangkan data kuantitatif berupa observasi dan kuesioner kepada pasien rawat jalan. Data hasil penelitian dianalisis secara deskriptif dengan rumus perhitungan yang sesuai. Hasil penelitian didapatkan rata-rata waktu tunggu pelayanan resep jadi 18, 42 menit, resep racikan 40,37 menit, tidak adanya kesalahan pemberian obat 100%, kepuasan pelanggan 96,47%, penulisan resep sesuai formularium 95,95%. Dapat disimpulkan untuk waktu tunggu pelayanan obat, tidak adanya kejadian kesalahan pemberian obat dan tingkat kepuasan pelanggan terhadap pelayanan farmasi telah memenuhi standar namun untuk kesesuaian penulisan resep dengan formularium belum memenuhi standar. Faktor yang mempengaruhi pelaksanaan Standar Pelayanan Minimal yaitu keterbatasan SDM, peresepan dokter terhadap obat-obat diluar formularium, sarana dan prasarana yang belum optimal, sosialisasi formularium belum optimal dan Kebijakan dalam pelayanan resep untuk menghindari kejadian kesalahan pemberian obat.

Abstract

Background: The growth rate of pharmacy in the city of Yogyakarta is very rapid so that it can cause competition between pharmacy. Objective: Pharmacy service performance is very important to be reviewed because it can be used as a parameter to determine management strategies. One method that is recommended in assessing pharmacy performance is the balanced scorecard method. Methods: This research is non-experimental explorative descriptive design with the aim to find out the performance of the apotek XX Yogyakarta based on four perspectives of the balanced scorecard method, financial side, customer, internal business processes, learning and growth of pharmacy and provide recommendations for improvement based on the results of the performance evaluation. Data collection was conducted retrospectively and prospectively in the form of qualitative and quantitative data. Results: The results of the performance apotek XX Yogyakarta using the balanced scorecard method research from a financial perspective shows the value of ITOR has decreased every year, Internal business process perspective is still found the existence of DRP and pharmacies have not met 100% of drug needs and in the perspective of learning and employee productivity growth is valued less. Conclusion: However, over all the results are good because it meets the standard values.

Abstract

Indonesian Case Based Groups (INA-CBGs) implementation on hospital financing for stroke patient using Jaminan Kesehatan Masyarakat (Jamkesmas) at 3rd class hospitalization requires therapy plan management and cost analysis because stroke is a leading cause of death worldwide and need high treatment cost. It is purposed to skimp the hospital expenditure and avoid deficit suffering. The study objectives was to determine the suitability cost between real stroke treatment cost and health financing based on INA-CBGs, and to determine the highest cost component on stroke treatment at Jogja hospital. This cross sectional observational study used retrospective sampling method. The subjects were taken from all stroke patient population using Jamkesmas insurance at Jogja Hospital, were hospitalized between January 2011-April 2012 and met the inclusion criteria. The subject was then classified based on INA-CBGs. Patient’s length of stay and costs (direct medical and direct non-medical costs) were defined as study variable. Data was analysed using descriptive analysis and Mann Whitney test. Throuh this study we conclude the average rate of real stroke treatment cost at Jogja Hospital was higher insignificantly than INA-CBGs based cost and the highest treatment cost was on the medicine and medical equipment.
