Abstract

Introduction: There is an increase in caesarean sections trend globally. Caesarean section is generally performed to assist deliveries that cannot be carried out normally, but this also has a significant impact on both maternal and child health, as well as the economic burden so it is necessary to evaluate the factors that influence decision for caesarean section, to determine the relationship between parity, antenatal care Method: This study is an analytical observational study with a case control design conducted in July 2024 using medical record data of pregnant women who underwent caesarean section or vaginal delivery at the Pariaman City Regional General Hospital in the urban area and Sungai Dareh Regional Hospital at Dharmasraya in rural areas. The sample for this study were pregnant women who chose caesarean section and women after vaginal delivery who met the inclusion and exclusion criteria Results: In this study there were 354 pregnant women who underwent CS (n=177) and vaginal delivery (n=177). Bivariate analysis showed a relationship between parity (OR: 1.64 [95% CI: 1.07-2.52]; p=0.023), history of ANC (OR: 1.64 [95% CI: 1.06-2, 55]; p=0.026), pregnancy complications (OR:3.16 [95% CI: 1.98-5.03]; p=0.000), fetal presentation (OR: 2.02 [95% CI: 1.26-3.24]; p=0.003) and estimated fetal weight (OR: 1.98 [95% CI: 1.19-3.30]; p= 0.008) with the decision making of caesarean section. Multivariate analysis showed that the most influencing factors on the decision making of  caesarean section were pregnancy complications (OR 4.35 [95% CI: 2.58 – 7.34]; p= 0.000), fetal presentation (OR 3.21 [95% CI : 1.85 – 5.55]; p=0.000) and parity (OR 1.59 [95% CI: 1.01 – 2.52]; p=0.044). Conclusion: The factors that most influence the decision making of  caesarean section are pregnancy complications, fetal presentation, and parity.

Abstract

Introduction: Early menarche is associated with long-term health problems. Currently, adolescent girls are increasingly experience the change of menarche age. Short chain fatty acids (SCFA) are known to play a role in the early release of GnRH which induces an earlier onset of menarche. However, so far there have been no human studies assessing SCFA levels in individuals with early menarche. Method: This is a case-control study in elementary school students in Padang which conducted from July 2024 to November 2024. The sample was all elementary school students in Padang who met the inclusion and exclusion criteria. The data collected were demographic data and SCFA levels measured in faecal samples using the ELISA method. Bivariate analysis was performed using the unpaired T-test. Result: In this study, there were 25 students with early menarche and 25 normal students. Students with early menarche experienced the earliest age of menarche in 9 years old, 56% had excessive BMI, 20% had mother history of early menarche, 20% were born prematurely, 52% had  high-fat diet, 100% lived with their parents and had no history of pornography exposure. This study found the differences in fecal SCFA levels based on age of menarche with the mean SCFA levels found to be higher in the early menarche group compared to normal menarche (39.09 ±  6.64 vs 26.91±5.01 μmol/gram; p=0.000) Conclusion: This study found the differences in fecal SCFA levels based on age of menarche

Abstract

Background: Diabetes mellitus-induced hyperglycemia triggers oxidative stress, characterized by elevated malondialdehyde (MDA) and impaired catalase (CAT) activity. Stevia rebaudiana, rich in steviol glycosides and polyphenols, demonstrates promising antioxidant properties, yet systematic dose-response data on oxidative stress biomarkers remain limited. Objectives: To evaluate the dose-dependent effects of stevia leaf extract on serum MDA levels and CAT activity in alloxan-induced hyperglycemic rats. Methods: Twenty-five male Wistar rats were allocated into normal control, diabetic control (alloxan 120 mg/kg), and three treatment groups receiving alloxan plus stevia extract at 100, 200, or 400 mg/kg body weight orally for 14 days (n=5/group). Serum MDA and CAT were measured spectrophotometrically. Results: Diabetic control showed significantly elevated MDA (2.68±0.62 mg/dL) versus normal control (1.78±0.30 mg/dL). Stevia extract dose-dependently reduced MDA: 1.70±0.19, 1.54±0.20, and 1.38±0.09 mg/dL at 100, 200, and 400 mg/kg, respectively, representing 36.6%, 42.5%, and 48.5% reduction. The 400 mg/kg dose achieved MDA levels comparable to normal control. CAT activity showed dose-dependent restoration trend (7.92±0.76 to 8.58±0.52 mg/dL). Conclusion: Stevia leaf extract (400 mg/kg BW) effectively reduces oxidative stress in hyperglycemic rats through significant dose-dependent MDA reduction, with potential catalase benefits requiring further investigation.

Abstract

Introduction : Language development in children aged 1-3 years is a critical foundation for learning readiness and social interaction. Language disorders or delays are a significant public health problem, with risk factors such as inappropriate gadget use, lack of early stimulation, and parenting dynamics influenced by maternal employment status. Objective : To determine the relationship between gadget use, stimulation provision, and maternal employment status on the language development of children aged 1-3 years at the Koto Panjang Ikua Koto Public Health Center, Padang City. Method : This study employed a cross-sectional design. A sample of 75 children aged 1-3 years was selected using two-stage sampling (simple random sampling to select 10 posyandu, followed by consecutive sampling). Data were collected through observation of language development using the Denver II instrument and interviews using a questionnaire. Results : The results showed that the proportion of children with delayed language development was 37.3%. There was a significant relationship between gadget use and language development (p=0.000) with a 54.6 times greater risk, and between stimulation provision and language development (p=0.000) with a 184.5 times greater risk. However, there was no significant relationship between maternal employment status and children's language development (p=0.898). Recommendation : Health workers are expected to intensify education about the negative impacts of gadgets and the importance of quality language stimulation. Parents are advised to limit the duration of gadget use and improve the quality of interaction and verbal stimulation consistently according to the child's developmental stages.

Abstract

The Maternal Mortality Rate (MMR) in Padang City has increased during the 2018-2020 period. One of the efforts to reduce MMR is to provide integrated pregnancy services. The coverage of K1, K4, and K6 pregnancy visits is an important indicator in assessing the quality of these services, but the achievements of the three in 2019-2022 in Padang City have not reached the target. This study aims to find out the factors related to the visit of pregnant women to the Health Center. The method used is a mixed method with a cross sectional and sequential explanatory design. A total of 142 pregnant women in the third trimester became respondents in quantitative research, and 11 informants in qualitative research. The research was conducted in three health centers in Padang City. The results of the quantitative analysis showed that of the 10 variables studied, the level of knowledge had a significant relationship with pregnant women's visits (p = 0.009), and was the most dominant factor based on multivariate tests. The qualitative results showed that most of the respondents obtained information from digital platforms, but the Mother And Child Health book has not been used to the fullest, the classes of pregnant women are not evenly distributed, and there is still an assumption that pregnancy is a natural process that does not need to be studied. Efforts are needed to improve health promotion and education for pregnant women to increase knowledge and awareness about the importance of pregnancy visits, so that the target of pregnancy service coverage can be achieved.

Abstract

Latar Belakang: Karsinoma nasofaring (KNF) adalah keganasan yang berasal dari sel epitel nasofaring. Kelangsungan hidup dan prognosis pasien KNF sangat tergantung pada stadium tumor. Pemahaman yang baik tentang faktor-faktor yang mempengaruhi prognosis dan perkembangan penyakit sangat penting dalam pengelolaan KNF. Salah satu faktor yang berpotensi memainkan peran kunci dalam hal ini adalah enzim cyclooxygenase-2 (COX-2). Peningkatan aktivitas COX-2 dapat memfasilitasi sel-sel tumor untuk memperoleh berbagai kemampuan biologis yang berkontribusi pada pertumbuhan dan perkembangan tumor. Tujuan dari penelitian ini adalah untuk mengetahui perbedaan ekspresi enzim COX-2 berdasarkan stadium karsinoma nasofaring. Metode: Penelitian ini merupakan penelitian cross-sectional analitik komparatif numerik tidak berpasangan dua kelompok satu kali pengukuran pada jaringan tumor dalam bentuk blok parafin dari hasil biopsi pasien KNF dan dilakukan pemeriksaan imunohistokimia untuk menilai ekspresi COX-2 pada jaringan tumor KNF yang dibagi menjadi kelompok stadium dini dan stadium lanjut. Data dianalisis secara statistik dengan program komputer dan dinyatakan bermakna jika p<0,05. Hasil: Pada penelitian ini didapatkan rerata ekspresi COX-2 pada KNF stadium lanjut (69,59±11,75) lebih tinggi dibandingkan dengan KNF stadium dini (47,29±31,04), namun hasil uji statistik menunjukkan bahwa perbedaan tersebut tidak signifikan secara statistik (p=0,28). Kesimpulan: Terdapat kecenderungan peningkatan ekspresi COX-2 pada stadium lanjut dibandingkan dengan stadium dini, namun perbedaan ini tidak signifikan secara statistik.

Abstract

Latar Belakang: Cisplatin dapat digunakan sebagai terapi tunggal atau kombinasi untuk induksi atau terapi neoadjuvan pada tumor ganas kepala leher, memiliki berbagai toksisitas yang membatasi dosis dan bersifat kumulatif, bahwa stria vaskularis merupakan awal cedera yang diinduksi oleh Cisplatin. Cisplatin meningkatkan pembentukan reactive oxygen species (ROS) di koklea. Superoksida diubah menjadi hidrogen peroksida baik secara spontan maupun melalui superoxide dismutase (SOD). Tujuan penelitian untuk mengetahui korelasi antara perubahan konsentrasi enzim antioksidan SOD dan perubahan ambang dengar penderita kemoterapi dengan Cisplatin pada tumor ganas kepala leher. Metode: Penelitian ini menggunakan pretest-posttest one group design pada penderita tumor ganas kepala leher yang mendapat kemoterapi cisplatin dengan dosis 75 mg/m2. Sebelum dilakukan kemoterapi dilakukan pemeriksaan audiometri nada murni dan pengambilan sampel darah sebagai data awal. Dua minggu sesudah kemoterapi, dilakukan pemeriksaan audiometri nada murni dan pengambilan sampel darah kembali. Data dianalisis secara statistik dengan program komputer dan dinyatakan bermakna jika p<0,05. Hasil: Pada penelitian ini didapatkan peningkatan kadar SOD sesudah diberikan kemoterapi cisplatin dengan signifikansi p<0,05. Terdapat peningkatan nilai ambang dengar hantaran tulang pada frekuensi 250, 500, dan 8000 Hz sesudah kemoterapi dengan signifikansi p<0,05. Terdapat korelasi lemah antara kadar SOD dan ambang dengar hantaran tulang pada frekuensi 250, 2000, dan 4000 Hz, dan korelasi sedang pada frekuensi 500, 1000, dan 8000 Hz. Kesimpulan: Pemeriksaan kadar SOD berkorelasi lemah dan sedang terhadap ambang dengar penderita tumor ganas kepala leher yang mendapat kemoterapi cisplatin. Pemeriksaan kadar SOD dapat dijadikan prediktor untuk menilai kondisi stres oksidatif pada kejadian ototoksik sebagai pertimbangan pemberian antioksidan.

Abstract

Utilization of the Posyandu Lansia remains suboptimal in the Belimbing Health Center despite its essential role in preventive health services for older adults. Various demographic and behavioral factors have been examined, yet the determinants influencing service utilization remain inconsistent across studies. This study aimed to identify the factors associated with the utilization of the Posyandu Lansia, with a particular focus on the role of family support as a potential determinant. A quantitative analytic study with a cross sectional design was conducted among older adults registered in the Belimbing Health Center. Data were collected using a validated questionnaire measuring demographic characteristics, knowledge, attitudes, family support, health worker support, and accessibility. Statistical analysis included descriptive and bivariate testing using the Chi square method. The findings revealed that family support was the only factor significantly associated with the utilization of the Posyandu Lansia. Emotional encouragement was generally adequate, yet gaps were evident in instrumental support such as transportation and physical accompaniment, as well as informational support such as schedule reminders. Other variables including age, sex, education, occupation, knowledge, attitude, distance, and health worker support showed no significant association with service utilization. In conclusion, family support serves as the primary determinant of Posyandu Lansia utilization. Strengthening instrumental and informational support within the household is essential to improving attendance and ensuring consistent engagement in preventive health services among older adults.Keywords: Posyandu Lansia; service utilization; family support

Abstract

Hospitals are not systematically designed to detect service errors as in non-healthcare products, thus increasing risks to patient safety. RSUD Sadikin in Pariaman City, which has been operating for only seven years, has not yet established mechanisms or training to detect service errors. This community service activity aimed to enhance managerial competence in managing the risk of healthcare service errors through the application of Failure Mode and Effect Analysis (FMEA). The activity was conducted in the form of a workshop involving 30 participants from top, middle, and lower management levels in Oktober-November 2025 at Sadikin Hospital. FMEA competence was assessed before and after the workshop using a paired t-test. The results showed that the mean pre-test score was 70.67 (SD±17.604) and the post-test score was 84.67 (SD±14.794), with an improvement of 14.00 points (SD±14.004), which was statistically significant (p=0.000). These findings indicate that the FMEA workshop effectively improved participants’ managerial competence in early detection and mitigation of service risks. Follow-up mentoring is recommended to evaluate the sustainability of training effects at 3, 6, and 9 months after the activity.

Abstract

Background: Preeclampsia is a major pregnancy complication that significantly contributes to maternal and neonatal morbidity and mortality. Impaired placentation leads to an imbalance between angiogenic and antiangiogenic factors, particularly a reduction in placental growth factor (PlGF) levels, which is believed to play an important role in adverse neonatal outcomes. Objective: To determine the relationship between maternal serum PlGF levels and neonatal outcomes in normal pregnancies, early-onset preeclampsia, and late-onset preeclampsia at Dr. M. Djamil General Hospital, Padang. Methods: This study was an observational analytic study with a cross-sectional design. The study population consisted of pregnant women with normal pregnancies, early-onset preeclampsia, and late-onset preeclampsia who met the inclusion criteria. Maternal serum PlGF levels were measured using the enzyme-linked immunosorbent assay (ELISA) method. Neonatal outcomes assessed included birth weight, birth length, and Apgar scores. Data were analyzed using univariate and bivariate statistical analyses. Results: A total of 48 pregnant women were included in this study, comprising 16 normal pregnancies, 16 cases of early-onset preeclampsia, and 16 cases of late-onset preeclampsia. The mean age of the subjects was within the reproductive age range of 20–35 years. The highest mean PlGF levels were found in the normal pregnancy group, followed by the late-onset preeclampsia group, and the lowest in the early-onset preeclampsia group, with a significant p-value indicating a meaningful difference in PlGF levels among the three groups. Neonatal outcomes showed that the lowest mean birth weight, birth length, and Apgar scores were observed in the early-onset preeclampsia group. Bivariate analysis demonstrated a significant association between PlGF levels and birth weight in the early-onset preeclampsia group (p = 0.0001). Conclusion: There is a significant association between PlGF levels and preeclampsia, suggesting that PlGF has potential as a predictive biomarker for preeclampsia. Regarding neonatal outcomes, a significant association was found with birth weight in early-onset preeclampsia, while no significant association was observed with birth length and Apgar scores across the three groups.
