Abstract

Background. Cancer treatment with hormone therapy such as Doxorubicin has been widely practised in breast cancer, bowel cancer, and others. However, sometimes the side effects of hormone therapy appear and worsen the patient's condition. The compounds (test compounds) Allicin, Capsaicin, Curcumin and Genistein have potential as anticancer compounds. The similarity of the medicinal properties of the test compounds with Doxorubicin, allows them to be used as cancer drug candidates. In addition, it is necessary to analyse ADMET to determine the efficacy and safety of the test compounds. Purpose. This study aims to analyse the ADMET of the compounds Allicin, Capsaicin, Curcumin and Genistein. Method. This research uses the in silico method. Test compounds; Allicin (PubCHem CID 65036), Capsaicin (PubChem CID 1548943), Curcumin (PubChem CID 969516), Genistein (PubChem CID 5280961) and Doxorubicin (PubChem CID 31703) as positive controls were obtained from PubChem. Analysis of test compounds using the PASS online programme to determine the binding activity of compounds with proteins. To determine absorption, distribution, metabolism, excretion, and toxicity (ADMET) using pkCSM online. DruLiTo was used to analyse the physico-chemical properties of the test compounds. Results. Allicin has good ADMET with Intestinal absorption 96.229 (%absorbed), BBB permeability 0.506 (log BB), Total Clearance 0.704 (log ml/min/kg), and does not cause hepatotoxicity. Conclusion. It can be concluded that all test compounds have their own advantages and disadvantages, but Allicin has better ADMET than other test compounds. Further research on Molecular Dynamics needs to be done to find out the interaction of test compounds with Caspase-3 Protein.

Abstract

Pelayanan kefarmasian telah mengalami transformasi fundamental dari pengelolaan sediaan obat menjadi pelayanan yang berfokus pada pasien (patient-oriented care). Sebagai garda terdepan sistem kesehatan primer, Puskesmas memegang peranan strategis dalam menyelenggarakan pelayanan kefarmasian yang bermutu dan berkelanjutan. Namun, implementasi standar pelayanan di lapangan sering kali menghadapi tantangan terkait kesenjangan regulasi dan praktik operasional. Salah satu upaya strategis untuk memperkuat implementasi ini adalah melalui integrasi kegiatan Praktik Kerja Profesi Apoteker (PKPA) sebagai bentuk pengabdian kepada masyarakat dari perguruan tinggi. Melalui kolaborasi antara mahasiswa, apoteker pembimbing, dan dosen, kegiatan PKPA memberikan nilai tambah nyata dalam bentuk edukasi dan pendampingan klinis. Pendekatan ini terbukti efektif dalam meningkatkan pemahaman serta kepatuhan tenaga kefarmasian terhadap standar yang berlaku. Hasil akhir dari sinergi ini tidak hanya memperbaiki kualitas pelayanan kefarmasian secara internal di Puskesmas, tetapi juga berkontribusi langsung pada peningkatan keselamatan pasien dan kualitas hidup masyarakat luas. Penguatan peran apoteker dalam tim kesehatan interprofesional melalui pendampingan berkelanjutan menjadi kunci keberhasilan transformasi pelayanan kesehatan di tingkat primer.

Abstract

Fenbufen Butanamine is a structural derivative of the non-steroidal anti-inflammatory drug (NSAID) Fenbufen, characterized by the addition of a butanamine group to its core structure. This modification is intended to improve the pharmacokinetic and pharmacodynamic properties of the parent compound, potentially enhancing its therapeutic efficacy. In this study, we conducted a detailed computational analysis of Fenbufen Butanamine’s chemical structure and molecular descriptors using data retrieved from the PubChem database. The chemical identity was confirmed by its IUPAC name, InChI, InChIKey, and SMILES notation, which provide precise molecular characterization essential for database referencing and further computational modeling. Key computational descriptors, such as molecular weight, logP, topological polar surface area (TPSA), and the count of hydrogen bond donors and acceptors, were analyzed to assess drug-likeness and predict pharmacokinetic behavior. Our findings indicate that Fenbufen Butanamine possesses a biphenyl core that contributes to hydrophobic interactions, while the butanamine side chain introduces hydrophilic properties. This amphiphilic nature is likely to influence the compound’s solubility, membrane permeability, and binding affinity to biological targets. The computed descriptors suggest favorable properties for oral bioavailability and potential interactions within biological systems. These results serve as a foundational step for in silico drug design and optimization of Fenbufen derivatives. Further experimental and pharmacological studies are warranted to validate these computational predictions and explore the therapeutic potential of Fenbufen Butanamine.

Abstract

Infeksi bakteri masih menjadi masalah kesehatan yang penting karena dapat meningkatkan angka kesakitan, memperpanjang masa perawatan, dan menambah biaya pengobatan, terutama dengan meningkatnya resistensi antimikroba. Salah satu bakteri yang sering menjadi penyebab infeksi adalah Escherichia coli, yang dapat menimbulkan gangguan pada saluran pencernaan maupun infeksi di luar usus. Pencarian sumber antibakteri alami dari tumbuhan menjadi alternatif yang perlu dikembangkan untuk mendukung pengendalian infeksi bakteri. Salah satu tanaman yang berpotensi adalah daun pulutan (Urena lobata L.) yang diketahui mengandung metabolit sekunder aktif. Penelitian ini bertujuan untuk mengetahui efek antibakteri fraksi etil asetat ekstrak etanol 96% daun pulutan terhadap bakteri Escherichia coli ATCC 25922. Penelitian dilakukan secara eksperimental in vitro menggunakan metode difusi cakram. Fraksi etil asetat diuji pada konsentrasi 5%, 10%, dan 15%, dengan streptomisin sebagai kontrol positif dan DMSO 10% sebagai kontrol negatif. Hasil ekstraksi menunjukkan bahwa dari 300 gram serbuk daun pulutan diperoleh 45 gram ekstrak kental dengan rendemen 15%, sedangkan hasil fraksinasi menghasilkan 18 gram fraksi etil asetat dengan persentase 40%. Skrining fitokimia fraksi etil asetat menunjukkan adanya flavonoid, alkaloid, saponin, dan tanin. Uji aktivitas antibakteri menunjukkan rerata diameter zona hambat pada konsentrasi 5%, 10%, dan 15% berturut-turut sebesar 23,50±2,15 mm; 23,17±2,02 mm; dan 27,93±3,86 mm. Konsentrasi 15% memberikan daya hambat tertinggi. Dengan demikian, fraksi etil asetat dari ekstrak etanol 96% daun pulutan terbukti menunjukkan aktivitas antibakteri terhadap Escherichia coli ATCC 25922.

Abstract

Penelitian ini bertujuan untuk menganalisis perencanaan dana anggaran pada pengelolaan pelayanan ibu hamil di Puskesmas Suruh tahun 2023. Kesehatan ibu hamil merupakan indikator penting dalam penilaian kesehatan masyarakat, mempengaruhi kesejahteraan ibu serta kesehatan dan perkembangan bayi yang dikandung. Penyediaan layanan kesehatan yang optimal bagi ibu hamil menjadi prioritas dalam program kesehatan di berbagai daerah, termasuk di Puskesmas Suruh. Berdasarkan data dari Peraturan Daerah Kabupaten Trenggalek Nomor 5 Tahun 2022, alokasi anggaran untuk layanan kesehatan ibu hamil adalah Rp 71.812.500, dengan persentase 11,20% dari total anggaran sebesar Rp 640.747.000 yang dialokasikan untuk penyediaan layanan tingkat daerah kabupaten. Layanan ini mencakup pemeriksaan rutin, penyuluhan kesehatan, pemberian suplemen, dan penanganan komplikasi kehamilan. Implementasi anggaran dilakukan dengan mempertimbangkan berbagai aspek, termasuk pengadaan sarana dan prasarana, pelatihan tenaga kesehatan, serta kegiatan monitoring dan evaluasi. Melalui analisis SWOT (Strengths, Weaknesses, Opportunities, Threats), penelitian ini mengidentifikasi kekuatan seperti dukungan dari pemerintah dan ketersediaan tenaga kesehatan yang kompeten, serta kelemahan seperti keterbatasan anggaran dan masalah administratif. Peluang yang dapat dimanfaatkan meliputi program pemerintah yang mendukung dan kerjasama dengan sektor swasta, sedangkan ancaman termasuk perubahan kebijakan anggaran dan bencana alam. Kesimpulan dari penelitian ini menekankan pentingnya perencanaan dana anggaran yang baik dan implementasi yang transparan untuk meningkatkan kualitas pelayanan kesehatan ibu hamil di Puskesmas Suruh, yang diharapkan dapat memberikan dampak positif bagi kesejahteraan ibu dan anak serta masyarakat secara keseluruhan.

Abstract

Ensuring the continuous availability of critical medicines in remote island regions remains a major challenge in decentralized health systems. This study examines the pharmaceutical supply chain in the remote islands of Maluku Province, Indonesia, through a supply chain mapping and vulnerability analysis using a System Dynamics (SD) approach. The findings indicate that fragmented governance structures, combined with extreme geographical dispersion, generate persistent vulnerabilities across procurement, distribution, and service delivery levels. Key reinforcing mechanisms driving medicine stockouts include delayed quantification and procurement processes, logistical bottlenecks in inter-island transportation, and shortages of skilled pharmaceutical personnel. The analysis further shows that interventions focused solely at the health facility level are insufficient to resolve systemic weaknesses. Instead, strengthening medicine security in Maluku requires integrated strategies at higher administrative levels, including investment in human capital, diversification of supply routes, strategic buffer management, and the use of transparent, data-driven logistics information systems. By adopting a systems-based analytical framework, this study provides evidence to support more resilient policy design for improving equitable access to essential medicines in Eastern Indonesia.

Abstract

Warfarin remains the primary oral anticoagulant for patients with mechanical heart valves, but considerable interindividual variability in stable dose requirements poses a clinical challenge. Although VKORC1 gene polymorphisms are well-established determinants of dose variability, the impact of objectively measured medication adherence is less clear. This study aimed to evaluate the independent and combined effects of VKORC1 polymorphisms and objectively measured medication adherence on stable warfarin dose requirements in patients with mechanical heart valves. Genotyping of VKORC1 −1639G>A (rs9923231) and 1173C>T (rs9934438) was performed using PCR–RFLP. Adherence over 90 days was expressed as the percentage of prescribed doses taken. The −1639A allele frequency was 81.7%, and patients with the AA genotype required significantly lower daily doses than GA or GG genotypes (p<0.001). Mean adherence was 89.4±11.2%. Multivariate regression showed that VKORC1 genotype, age, and adherence independently predicted stable dose,explaining 45.2% of variability. Stable warfarin dose was defined as an unchanged maintenance dose with a therapeutic INR (2.0–3.5) for ≥3 months.Adherence below 80% was associated with increased INR fluctuations and dose adjustments, highlighting adherence as a key clinical modifier alongside genetic factors.

Abstract

−This community service program was conducted in March 2025 in Gembleb Village, Pogalan District, Trenggalek Regency, East Java Province, an area with considerable natural resource potential in the form of red ginger (*Zingiber officinale* var. *rubrum*), which has not been optimally utilized. The problems faced by the community included limited knowledge regarding arthritis, dependence on chemical-based medications, and inadequate skills in processing herbal plants into beneficial products. In addition, the utilization of red ginger as a source of bioactive compounds, particularly gingerol, remained limited. This program aimed to improve community knowledge and skills in utilizing red ginger as a complementary therapy for arthritis through participatory education and training approaches. The implementation methods consisted of three stages: preparation, implementation, and evaluation. Educational sessions were conducted to enhance participants’ understanding of arthritis and inflammatory mechanisms, while training activities focused on the production of herbal products, including ginger beverages and instant ginger powder. Evaluation was carried out using pre-test and post-test assessments as well as direct observation of participants’ practical skills. The results showed significant improvements in both knowledge and skills. More than 80% of participants demonstrated increased understanding of arthritis and the benefits of red ginger and were able to independently process red ginger into herbal products. Furthermore, participants exhibited a more positive attitude toward the use of herbal therapy as a complementary treatment. This program contributed to enhancing community self-reliance and creating opportunities for the development of local potential-based herbal products.

Abstract

Green innovation has become a critical strategic mechanism through which manufacturing firms pursue competitive advantage amid increasing environmental and market pressures. This study aims to examine how green innovation contributes to competitive advantage in manufacturing industries by synthesizing empirical evidence from existing scholarly literature. Employing a qualitative, literature-based research approach, the study systematically reviews and analyzes peer-reviewed studies addressing green product innovation, green process innovation, organizational capabilities, and institutional drivers of competitiveness. The findings reveal that green innovation positively influences competitive advantage by enhancing differentiation, efficiency, adaptability, and market legitimacy. Moreover, the sustainability of these competitive gains is strongly shaped by complementary organizational factors such as green human resource practices, intellectual capital, dynamic capabilities, research and development investment, and supply chain integration. The study concludes that green innovation functions as a systemic strategic asset rather than an isolated environmental practice. These results contribute to the theoretical understanding of sustainability-driven competitiveness and offer strategic implications for manufacturing firms seeking long-term competitive resilience

Abstract

The increasing competitiveness of global tourism markets has intensified the strategic importance of understanding the behavioral mechanisms that encourage tourists to revisit destinations, particularly in emerging tourism contexts characterized by uneven service standards and dynamic visitor expectations. This study investigates the interrelationships between service quality, tourist satisfaction, and revisit intention, aiming to clarify their causal structure within emerging destinations. Employing an empirical, quantitative research design, data were collected from domestic and international tourists who had visited selected emerging destinations within the previous twelve months. Using a structured questionnaire and purposive sampling, the study analyzed responses through structural equation modeling to test both direct and mediated effects among the key constructs. The results demonstrate that service quality significantly influences tourist satisfaction, while tourist satisfaction exerts a strong and positive effect on revisit intention. Furthermore, tourist satisfaction partially mediates the relationship between service quality and revisit intention, with the indirect effect exceeding the direct effect. These findings indicate that revisit intention is primarily shaped through tourists’ post-consumption evaluative processes rather than service performance alone. The study contributes theoretically by refining the service quality–satisfaction–loyalty framework in emerging destination settings and offers practical insights for destination managers seeking sustainable competitiveness through satisfaction-oriented service strategies.
