Abstract

One of the problems for human resource management is the desire to leave (turnover intentions) which can lead to the nurse's decision to leave her job. One way for nurses to stay in the hospital is to increase employee retention. The purpose of this study was to develop recommendations to increase employee retention based on the Internal service quality approach at Batu Bara Hospital. This research is descriptive research with a quantitative approach. This research was conducted at the Batu Bara Hospital from September 2020 to March 2022. The research population used was nurses who were actively working at the Batu Bara Hospital, totaling 105 people with a total sample of 105 respondents. Linear regression tests and multiple linear regression tests were used to determine the effect between the dependent variable and the independent variable. The results showed that the cooperation variable and role conflict variable had no effect on increasing nurse retention, and the variables of suitability to work, suitability to technology, self-control ability, supervisory system, and role ambiguity had an effect on increasing nurse retention at Batu Bara Hospital. In this study together the independent variables have an influence on the dependent variable with a large effect of 95.9%. Suggestions for the Batu Bara Hospital should further strengthen the relationship between nurses by holding regular meetings, providing knowledge to nurses about the organizational components of Batu Bara Hospital through guidance from managers/supervisors and regular training to nurses, improving the quality of existing job designs that can This is done by completing the equipment and technology needed by nurses and reviewing the existing reward system, where the reward system must implement a justice system based on the workload, education, work experience and achievements of nurses.

Abstract

Cases of Occupational Accidents (KAK) and Occupational Diseases (PAK) at Royal Prima Hospital Medan in 2017-2020 showed a significant increase, especially in 2020. This condition was due to Royal Prima Hospital receiving/serving Covid-19 patients so that they were overwhelmed in handling The surge in patients impacting the workforce is experiencing fatigue and illness. The purpose of this study is to analyze the factors that influence the implementation of Occupational Safety and Health (K3). This research is a quantitative analytic study with a cross-sectional approach. The research was conducted at Royal Prima Hospital Medan. The research population was 915 people and the sample was 100 people. Sampling by the Hsystematic way (systematic sampling). Data analysis used univariate, bivariate with chi-square test, and multivariate with multiple logistic regression at 95% confidence level (α=0.05). The results showed that the factors that influenced the implementation of Occupational Safety and Health (K3) at Royal Prima Hospital Medan were action (p=0.045), policy (p=0.010), standard operating procedures (p=0.011), and leadership (p=0.011). = 0.020). Variables that had no effect were education (p=0.461), belief (p=0.538), perception (p=0.235), knowledge (p=0.578) and attitudes (p=0.730). The most dominant variable influencing the implementation of K3 at Royal Prima Hospital Medan is policy. Implementing officers who declare hospital policies are good, have a 5.5 times higher chance of implementing K3 well than those who state policies are not good.

Abstract

This study aims to find out how to develop an online learning module to increase the capacity of nurses and midwives in promoting preconception health in primary health care services at Tiara Kasih Sejati Hospital Pematangsiantar. This type of research is classified into descriptive research with a quantitative approach. The sample of this study amounted to 93 samples consisting of midwives and nurses. Data was collected by using counseling methods and giving questionnaires. Data analysis using univariate. The results showed the difference in the increase in respondents' knowledge of 4.34 so that the average score of respondents' knowledge before and before being given the online learning module increased from 9.52 to 13.86. The benefit of the module is to increase the perception of midwives and nurses in improving health preconceptions in primary health care settings by 75.3% of midwives and pediatric nurses increasing the promotion of health preconceptions in primary health care settings. The ability level of midwives and nurses in accessing online learning modules regarding preconception is 55.9% the best and 44.1% not good. The assessment of the quality of the online learning module given to midwives and nurses, midwives and nurses who rated it good was 77.4% and those who rated it bad were 22.6%. The nutritional needs needed during the preconception period are macronutrients and micronutrients.

Abstract

Health nursing services in hospitals are one of the most important and goal-oriented services that focus on implementing professional nursing care according to nursing standards, depending on how nurses perform in hospitals in applying nursing care standards. Job stress is a phenomenon that affects employees differently, in different work contexts. A clean work environment will make nurses work more enthusiastically and vice versa. Job stress causes nurses to lose the ability to control it. The purpose of this study was to determine the description of individual factors, work, outside work, and work stress on female nurses in the inpatient room of Sibolga Public Hospital. The type of research used is quantitative research with analytical design through a cross-sectional approach. The population and samples used were 95 female nurses who served in the inpatient ward of the Sibolga Hospital. The results showed that individual factors, work, and outside work had a significant influence on work stress on female nurses in the inpatient room at Sibolga Hospital.

Abstract

A good work program is needed by using the WISN (Workload Indicators of Staff Needing) method, which helps see the workload given to and received by medical workers in health facilities. This study aims to determine the appropriate number of employees using the WISN (Workload Indicators of Staff Needing) method to reduce the workload in the hospital's medical rehabilitation polyclinic. This research is a descriptive study with quantitative data analysis conducted at the H. Abdul Manan Simatupang Hospital in 2021. The data taken is medical record data on the total number of patient visits at the medical rehabilitation polyclinic and staffing data for the rehabilitation polyclinic. H. Abdul Manan Simatupang Kisaran Hospital. We found a relatively high number of visits at the Medical Rehabilitation Polyclinic; the workforce data obtained totaled four employees who handled patients directly. The study's results found that the available work time was not directly proportional to the existing standard workload, where the available work time is 135,360 minutes per year while the Standard Workload is 347,424 minutes per year. The workload in the Medical Rehabilitation Polyclinic is too high, and we can obtain the cause from the number of operational staff on duty.

Abstract

Tuberculosis is an infectious disease caused by the bacterium Mycobacterium tuberculosis, this bacterium usually attacks the lungs, but TB bacteria can attack any part of the body such as the kidneys, spine and brain. TB is one of the top 10 causes of death and the main killer of HIV sufferers worldwide. Based on WHO's 2018 Global Truberculosis Report, the incidence of tuberculosis in Indonesia in 2018 increased by 420,994 cases compared to the number of cases in 2017 of 360,565 cases and 2016 of 330,910 cases. According to gender, the number of cases in men is higher than in women, namely 1.4 times compared to women in each province throughout Indonesia.Methods: The study used a quantitative approach with an analytic observational type with a cross-sectional research design. Research results: motivation variables with significant values (0.000), medical history (0.005), knowledge (0.04), socioeconomic conditions (0.035) and education (0.022) have a relationship to the recovery of TB patients. While other variables are not related to the recovery of TB patients.

Abstract

BACKGROUND: Diabetic kidney disease (DKD) is driven by chronic inflammation and endothelial dysfunction, which often persist despite standard pharmacological treatments. Autologous dendritic cell (DC) immunotherapy offers a novel approach to restore immune homeostasis and improve renal vascular function. While the use of autologous DC for immune homeostasis has been previously discussed, not many studies have explicitly reported on the modulation of renal perfusion parameters, such as peak systolic velocity (PSV) and resistive index (RI), following DC immunotherapy. Therefore, this study was conducted to evaluate the effect of autologous DC administration on renal hemodynamics, including PSV and RI, as well as inflammatory biomarkers, including tumor necrosis factor (TNF)-α and vascular cell adhesion molecule (VCAM-1) in DKD patients.METHODS: Thirty-one DKD patients were selected via simple random sampling. All subjects underwent autologous DC therapy, which was administered via intravenous infusion at a concentration of approximately 1×107 cells suspended in 100 mL of normal saline. PSV and RI were measured using Renal Doppler Ultrasonography, while TNF-α and VCAM-1 were quantified using the Enzyme-Linked Immunosorbent Assay (ELISA) method. All measurements were conducted before intervention and 60 days after intervention to evaluate the therapeutic efficacy.RESULTS: DC therapy led to significant alterations in renal hemodynamic parameters. The mean PSV decreased from 52.74 to 38.21 cm/s (p=0.016), while RI showed a modest increase from 0.7350 to 0.7550 (p=0.028). Greater hemodynamic effects were observed in patients with well-controlled glycemia, lower serum urea, and microalbuminuria. In contrast, no significant changes were detected in TNF-α and VCAM-1 levels.CONCLUSION: Autologous DC therapy delivers measurable, statistically significant benefits in renal vascular parameters for DKD, particularly in early-stage disease and metabolically stable patients. These findings may support DC therapy as a promising adjunctive strategy to improve renal microcirculation in DKD. KEYWORDS: diabetic kidney disease, dendritic cell therapy, TNF-α, VCAM-1, PSV, RI

Abstract

Abstract The Emergency Department (ED) requires effective communication between nurses and patients to ensure patient safety and quality of care; however, various barriers to communication remain prevalent. This study aimed to analyze differences in communication barrier factors between nurses and patients in the ED of Andini Hospital, Pekanbaru. This observational analytic study employed a retrospective cohort approach and involved 80 respondents (40 nurses and 40 patients) selected through consecutive sampling using a questionnaire. Data analysis was performed using an independent t-test or the Mann–Whitney test with a significance level of p < 0.05. The results demonstrated no statistically significant difference in nurse-related factors between the nurse and patient groups (p = 0.134). In contrast, health system–related factors showed a significant difference (p < 0.001), with nurses perceiving system-related communication barriers to be higher than patients. These findings indicate that communication barriers in the ED are more predominantly influenced by health system factors rather than individual nurse-related factors. Therefore, strengthening service systems and institutional support is necessary to improve communication effectiveness and enhance patient safety in the ED. This study investigates the relationship of organizational citizenship behavior, organizational commitment, and job satisfaction, with the performance of inpatient nurses at RSU Royal Prima Medan (RPMH). Aquantitative correlational (explanatory) design was employed, involvinga total sample of 151 nurses in 2024. Data analysis was conducted using univariate, bivariate, and multivariate techniques. The findings reveal that organizational citizenship behavior, rganizational commitment, and job satisfaction are significantly associated with nurse performance, with job satisfaction demonstrating the strongest correlation. Theseresults underscore the importance of strengthening job satisfaction and organizational factors to enhance nurse performance and improve overall hospital service quality.
