Abstract

Background: Patients with Chronic Kidney Disease (CKD) undergoing maintenance Hemodialysis (HD) face physical, psychological, social, and spiritual challenges that reduce quality of life. This scoping review mapped and synthesised evidence on supportive care needs and patient preferences to inform the development of holistic, person-centred, and culturally responsive Kidney Supportive Care (KSC) models for adults receiving maintenance HD. Methods: This scoping review followed the PRISMA-ScR guidelines and the Arksey and O'Malley methodological framework, guided by the Population–Concept–Context (PCC) approach. Literature searches were conducted on May 25, 2026, across PubMed, Scopus, ScienceDirect, and EBSCOhost. Eligible studies included English- and Indonesian-language publications from 2021–2026 involving adults with CKD receiving maintenance HD. Qualitative, quantitative, and mixed-methods studies, as well as systematic reviews, were included. Study selection, data charting, thematic synthesis, and methodological quality appraisal were conducted using predefined criteria and Joanna Briggs Institute (JBI) critical appraisal tools. Results: Twelve studies met the inclusion criteria. Four overarching themes were identified: (1) multidimensional supportive care needs, including physical symptom management, psychological support, social and financial assistance, spiritual care, and informational needs; (2) patient preferences for patient-centered care, emphasizing effective communication, shared decision-making, family involvement, and respect for spiritual values; (3) barriers to KSC implementation, including workforce limitations, inadequate psychosocial resources, financial burdens, insufficient professional training, and organizational constraints; and (4) strategies to strengthen KSC through interdisciplinary collaboration, Patient-Reported Outcome Measures (PROMs), family engagement, and integrated palliative care approaches. The review also identified evidence gaps regarding culturally adapted supportive care models, caregiver-support interventions, and implementation strategies in resource-limited settings. Conclusion: Cultural beliefs are crucial in shaping treatment choices for ENT conditions. Public awareness campaigns by the Ministry of Health and local health authorities are recommended to educate communities on the benefits and risks of traditional medicine while promoting evidence-based healthcare practices.

Abstract

Currently, there are no suggested psychological interventions or outcomes in the diabetic management guidelines. This case report presents the management of a 66-year-old female patient, Mrs. R, with diabetes mellitus, hypertension, and gastroesophageal reflux disease (GERD) who underwent exploratory laparotomy for retained stone exploration in the common bile duct (CBD). This Case report aimed to explore the implementation of mindfulness in patients post-operative. The patient experienced significant hyperglycemia, with blood glucose levels peaking at 436 mm Hg. Psychological assessment revealed a severe anxiety score of 31, which likely contributed to her unstable glycemic control. Throughout her unstable glycemic control. Throughout her severe condition, the patient was managed with total parenteral nutrition (TPN) due to fasting and green gastric contents, as part of the nursing intervention. Comprehensive nursing interventions, including regular blood glucose monitoring, insulin administration, and psychological support were employed. Mindfulness techniques such as guided meditation and breathing exercises have been introduced to alleviate hospital-induced distress and manage blood glucose levels. Following these interventions, her blood glucose level decreased to 167 mmHg, and the patient reported feeling calmer and more spiritually connected (Hamilton Rating Scale/ HARS 18). This case describes the management of post-surgical diabetic patients, highlighting the benefits of integrating psychological interventions to improve glycemic control and overall patient outcomes. Therefore, psychological interventions, particularly mindfulness, are needed for integration in diabetes management, especially in hospitals.

Abstract

Background: Type 2 diabetes mellitus (DM) is a major global health challenge. Effective management requires non pharmacological interventions in addition to medication. Hydrotherapy, a non invasive and affordable water-based therapy, shows promise in improving blood glucose, insulin sensitivity, and vascular function. This review identifies hydrotherapy interventions for type 2 DM and evaluates their clinical impact. Purpose: To analyze how to improve adherence in type 2 diabetes mellitus patients in implementing the five pillars of diabetes mellitus management. Method: This integrative reviews the size devidence on hydrotherapy for type 2 DM, guided by two questions: "what forms of hydrotherapy intervention are used in the management of type 2 diabetes mellitus?" and "What is the impact of hydrotherapy on clinical out comes for patients with type 2 diabetes mellitus?". The population, intervention, outcome (PIO) frame work was used. A systematic literature search was conducted across major electronic data bases (PubMed, Scopus, Web of Science, EBSCO host: Medline). Seventeen relevant studies were included after screening. Results: Hydrotherapy interventions include hot water baths/immersions, aquaticexercise, hot/cold compression, and oral water consumption. Aquatic exercise can lower HbA1c, fasting glucose, and insulin levels. Hot baths may reduce poor glycemic control and decrease fasting plasma glucose. The tests revealed animmediate decrease in blood glucose. Oral hydrotherapy improved the ankle brachial index. Hydrotherapy also improved quality of life, physical health and reduced stress. The benefits extend to lipid profiles and obesity , as well as blood flow, vascular function, pain, and mobility. Conclusion: Hydrotherapy is a potential complementary intervention for type 2 DM, offering benefits interms of glycemic control, vascular function, quality of life, and physical symptoms. Its non pharmacological, non invasive, afford able, and safe nature make sit a viable nursing option, especially in resource-limited are as. While nurses are crucial in its implementation, current studies have limitations interms of methodology, standardization, and long-term data. Further robust research is needed for widerad option.

Abstract

Key word yang digunakan adalah fatigue, nursing intervention, end stage renal deseases, chronic kidney deseases, hemodialysis, self management, intervensi keperawatan, penyakit ginjal, gagal ginjal kronis, hemodialisis dan manajemen diri.
