Abstract

ABSTRAK Latar Belakang : Diabetes melitus merupakan penyakit metabolik dengan karakteristik hiperglikemia sebagai akibat kelainan sekresi insulin maupun kerja insulin. Diabetes sebagai salah satu penyebab dislipidemia sekunder, sehingga pengelolaan glukosa darah merupakan pencegahan primer timbulnya komplikasi penyakit kardiovaskular. Hasil Riskesdas tahun 2018, prevalensi diabetes melitus yang didiagnosis dokter pada penduduk di semua umur sebesar 3,4% di Kota Surabaya.Tujuan : Penelitian ini untuk mengetahui perbedaan profil lipid pada level HbA1C normal, prediabetes dan diabetes melitus.Metode : Penelitian cross sectional, teknik pengambilan sampel dengan simple random sampling dari data rekam medis pasien rawat jalan di Rumah Sakit Islam Surabaya  dari 1 Januari tahun 2018 sampai dengan 31 Desember 2019 berusia 35-80 tahun dan mendapat pemeriksaan HbA1c, kolesterol, trigliserida dan LDL-kolesterol pada waktu yang sama dan terdokumentasi lengkap pertama sekali sehingga diperoleh besar sampel 73 data pasien. Uji Anova one way digunakan untuk mengetahui perbedaan rata-rata kolesterol, trigliserida dan LDL-kolesterol berdasarkan HbA1C.Hasil : Hasil penelitian menunjukkantidakada perbedaan yang signifikan rata-rata kolesterol, dan LDL-kolesterol dengan tingkatan HbA1C (p>0,05), ada perbedaan yang signifikan rata-rata trigliserid dengan HbA1C normal, prediabetes, dan diabetes (p=0,01). Hasil multiple comparison dengan metode Tukey HSD menunjukkan perbedaan signifikan rata-rata trigliserid  pada HbA1C normal dengan diabetes (p=0,039) dan prediabetes dengan diabetes (p=0,044).Kesimpulan :Perbedaan rata-rata trigliserida signifikanpada HbA1Ckategorinormal dan prediabetes dengan diabetes, pentingnya mengendalikanglukosa darah untuk mencegahkomplikasi kardiovaskuler pada penderita diabetes melitus yang dapat dilakukan melalui pemantauan mandiri glukosa darah, pola hidup sehat, aktivitas fisik secara teratur, terapi nutrisi medis sesuai kebutuhan, menurunkan berat badan bagi yang mengalami obesitas, tidak merokokdan intervensi obat anti hiperglikemia jika dibutuhkan.Kata Kunci : diabetes, HbA1C, kolesterol, trigliserid, LDL-kolesterol. ABSTRACT  Background :Diabetes melitus is a metabolic disease characterized by hyperglicemia as a result of abnormal insulin secretion and insulin action. Riskesdas 2018 said that the prevalence of doctors' diagnosed diabetes in the population at all age 3.4% in Surabaya.Objective : This study is to find out the difference in lipid profiles on normal HbA1Clevels, pre-diabetes and diabetes mellitusMethod: Cross-sectional study, the sampling technique used was simple random sampling fromoutpatient medical recordsthe Surabaya Islamic hospital's from 1st of January 2018 to 31st December 2019 aged 35-80 years and checked for HbA1C, cholesterol, triglyceride and LDL-cholesterol at the same and firsttime documented. Sample sizes of 73 data analized with One Way Anova test was used to identify differences in mean cholesterol, triglyceride and LDL-cholesterol based Hba1C.Results :The results showed that there was no significant difference mean cholesterol and mean LDL-cholesterol with HbA1C levels (p> 0.05), there were significant differences mean the triglyceride with normal HbA1C levels, pre-diabetes, and diabetes (p= 0.01). Multiple comparason results using Tukey HSD methods showed that there was significant differences mean the triglycerid on normal HbA1C levels with diabetes (p= 0.039) and the mean triglyceride ebetween hba1c prediabetesand diabetes (p= 0.044).Conclusions: The mean difference trigliseride signifnificant in normal HbA1C levels and pre-diabetes with diabetes.The importantce of controlling blood glucose to prevent cardiovasculer complication in people with diebetes mellitus can be done through education on independent monitoring of blood glucose, healthy lifestyle, reguler physical activity, medical nutrition therapy according to the needs, lost weight for those who are obese, do not smoke and  anti-hyperglicemia drug intervention if needed.

Abstract

Cases of sexual abuse of children is one of the cases that have increased quantitatively also qualitative. The parental care in providing reproductive health education influence children’s growth and development, especially in reproductive health problems.This study aimed to analyze the relationship patterns of parenting with reproductive health behavior of children in order to prevent sexual abuse of children in Mojokerto. The research design was crosssectional. This research was conducted in Mojokerto in June 2016. The population were all students in 4th and 5th grade and their parents. The research sample of 103 people that selected using simple random sampling. Data collection was carried distributing questionnaires to the respondents. Data analyzed by using Chi Square and Fisher's Exact test. The results showed that there is a relationship between the parenting with reproductive health action in order to prevent child sexual abuse in children.Reproductive health education by parents to children can increase children's awareness and confidence that will eventually affect attitudes. If the child is already getting reproductive health education from parent, then the child will try to keep her reproductive health and trying to keep himself in order to avoid sexual violence.

Abstract

The Covid 19 pandemic affected all aspects of life and almost all groups of society, including pregnant, childbirth and postpartum woman. This study aimed to analyze the effect of spatial determinants that consist of antenatal care, childbirth services, post-partum services and demographic factors on maternal mortality in East Java Province before and during the Pandemic Era. This study was an unobstrusive measure research with the unit of analysis in this study were all pregnant, childbirth and postpartum women that spread accros 38 districts in East Java Province during 2020. Data were analyzed using Geographically Weighted Regression. The results showed that maternal mortality in the province of East Java had a spreading pattern, thus indicating a spatial influence. The variables of childbirth services by health personnel and antenatal care had a significant effect on maternal mortality. There are 5 groups of districts that show a similarity of significant factors. This result showed that the diversity of health service accessibility in each region affects maternal mortality during the Covid 19 pandemic in East Java province. Childbirth assistance by health personnel and accessibility of antenatal services during the Covid 19 Pandemic affected maternal mortality.The diversity of accessibility influenced by local government policies and community perceptions in each region about Covid 19 pandemic will affect the accessibility of antenatal services and childbirth so it can be dangerous for the safety of pregnant , childbirth and postpartum woman.

Abstract

Abstract. Dengue fever (DHF) is a significant public health problem in Surabaya City, posing challenges in prevention efforts. This study aims to explore the potential of the Sayang Warga application in increasing the role of community cadres in DHF prevention efforts. The purpose of this Community Service is to educate about Dengue Fever Prevention and the Sayang Warga application Kader Surabaya Hebat mothers to increase the knowledge of Kader Surabaya Hebat mothers to prevent Dengue Fever.. The results of this study The results of the questionnaire showed 70.2% of participants said that the theme taken was very good, 26.67% stated that it was good and 2.98% stated that it was sufficient. This shows that the theme taken is exciting and liked by the participants. Evaluation is also carried out on the participants of the service activities by doing the preetest and postest carried out on the theme taken. The pretest results showed an average score of 75 and the posttest results with an average score of 100. Community service at the Sawahan Health Center, Sawahan Subdistrict, Surabaya City is very beneficial for Surabaya Hebat Cadres because it can increase their knowledge about dengue hemorrhagic fever and the Dear Citizen Application.

Abstract

Background: Pulmonary tuberculosis (TB) is a chronic infectious disease caused by the bacterium Mycobacterium tuberculosis. Diagnosis of TB can be confirmed in two ways, namely bacteriological diagnosis (if AFB sputum is found (+) and clinical diagnosis is (if BTA sputum is found (-), but chest X-ray is (+) TB). Objective: to determine the alignment of sputum conversion and extensive resolution of intensive phase lesions on chest radiographs which determine the prognosis of pulmonary TB therapy. Methods: The study design was a retrospective cohort analytic with a retrospective longitudinal study design. Data from medical records of pulmonary TB patients who have undergone therapy for six months or more at the Pulmonary Polyclinic RSI Jemursari Surabaya. The number of samples was 48 patients aged 41-60 years. All of these pulmonary TB patients were smear positive (BTA+). X-ray examination was done before and after therapy. Results: analysis using the Wilcoxon Signed Rank test to assess differences in the grade of lung lesions before and after therapy, obtained p = 0.003 (p <0.05) meaning there is a significant difference. Sputum conversion was also carried out after therapy, 89.6% of TB patients in this study experienced sputum conversion (BTA negative). To determine the alignment of sputum conversion with the resolution of lesion area, Kappa coefficient analysis K=0.033 (p>0.05) was performed with the results of 50% of patients, 47.9% showed improvement in lung lesions and sputum conversion, while 2.1% showed no improvement of lung lesions and no sputum conversion. The rest, 50% showed no congruence in the results of lung lesion repair and sputum conversion. Conclusion: The results of Kappa coefficient analysis showed that K=-0.110 (p>0.05) showed that there was no congruence between the results of chest x-ray examination of lung lesions before and after therapy (improved or not) with sputum conversion

Abstract

Kabupaten Jember merupakan salah satu kabupaten  yang memiliki jumlah kasus TBC (TB) termasuk kasus TB anak tertinggi di Jawa Timur. Skrining TB merupakan salah satu strategi penanggulangan TB anak yang dapat dilakukan secara aktif, intensif dan massif. Skrining TB anak secara aktif masif dapat dilaksanakan oleh petugas kesehatan dan juga masyarakat dalam hal ini kader. Belum memadainya tata laksana TB yang sesuai standar dan belum optimalnya peran kader kesehatan manjadikan penemuan kasus TB di Kabupaten Jember masih rendah. Selain itu, masih terdapat kelemahan dan keterbatasan dalam pelaksanaan skrining karena masih dilakukan secara manual sehingga peru digitalisasi dalam pelaksanaan skrining TB. Metode yang digunakan dalam pengabdian kepada masyarakat ini adalah ceramah, diskusi, tanya jawab, dan pelatihan kasus kepada kader dan tenaga kesehatan di Kabupaten Jember sebanyak 43 orang. Kegiatan capacity building dilaksanakan pada tanggal 15 Juli 2023 di Aula Dinas Kesehatan Kabupaten Jember. Kegiatan ini diawali dengan pre-test, pemberian materi, pelatihan, serta post-test. Didapatkan hasil bahwa terdapat peningkatan kemampuan kader dan tenaga kesehatan dari nilai rata-rata pre-test 85,5 meningkat menjadi 91,6 pada post-test. Diharapkan kegiatan skrining TB anak di Kabupaten Jember dapat dilakukan secara lebih aktif dan masif sehingga cakupan penemuan kasus meningkat yang dilanjutkan dengan treatment yang juga meningkat.

Abstract

Background: Toddler diarrhea is still a problem both globally and in Indonesia. Diarrhea remains one of the main causes of death among children under five. Environmental factors are one of the factors that can influence the incidence of toddler diarrhea. Objectives: To determine the relationship between environmental factors including sources of drinking water, toilet facilities and hand washing facilities in affecting the incidence of diarrhea in children under five in Java Island. Methods: This study uses a cross-sectional research design. The data used comes from the 2017 Indonesian Demographic and Health Survey (IDHS).. The study population was children under five in all provinces on the island of Java. The analytical method used is multivariate analysis with logistic regression. Result: There is a significant effect between the variable sources of drinking water (OR=2.188), toilet facilities (OR=1.848), and hand washing facilities (OR=2.040) with toddler diarrhea. Conclusion: Environmental factors, namely sources of drinking water, toilet facilities, and hand washing facilities, affect the incidence of toddler diarrhea on the island of Java.

Abstract

Analysis and evaluation of the measles surveillance system is continuous monitoring in efforts to prevent and control measles cases through recording reporting. The aim was to describe, identify and develop a model for a measles surveillance reporting recording system in North Maluku Province based on a systems approach (input, process and output). This research is a system development that includes actions to create a new system or improve an existing one, carried out at the North Maluku Provincial Health Service from January to February 2024. The research involved the measles surveillance information system unit and informants. Information system development stages include data normalization, grouping data into tables or relationships, and creating Entity Relationship Diagrams. The findings show that in the analysis of problems with the information system for recording measles surveillance reporting, system problems were found in the input, process and output components. This reporting note provides information regarding the measles disease surveillance system, namely in health facilities, laboratories and surveillance units. The measles surveillance program has improved its information system, but faces challenges such as inconsistent patient IDs, duplicative data recording, manual storage, and 70% completeness and accuracy. In conclusion, although the measles surveillance program has experienced improvements in its information system, further efforts are still needed to increase accuracy and efficiency through data automation and standardization of variable recording.

Abstract

Background Speech and language problems are developmental disorders often observed in children. Risk factors for speech and language disorders can occur prenatally, perinatally, or postnatally. Recognizing the risk factors for speech and language problems is essential for early diagnosis and intervention. Objective To identify risk and protective factors of speech and language disorders in children. Methods This case-control study was conducted in the Growth and Development Clinic at Dr. Soetomo Hospital, Siloam Hospital, Kendangsari Merr Hospital, and Darmo Hospital, Surabaya, East Java. Subjects with normal development were included in the control group, subjects with speech-language development disorders with no additional problems were included in the primary case group, and subjects with speech-language disorders and additional problems were included in the secondary case group. Subjects underwent history-taking and developmental examinations to assess for risk factors and types of speech-language disorders. Results During March – July 2023, 162 subjects aged 2-6 years met the inclusion criteria. The significant risk factors in the primary group were male sex (P=0.000), age 2-3 years (P=0.01), and maternal age 19-35 years (P=0.018). The protective factor identified was not bilingual (P=0.046). In the secondary group, the significant risk factor was male gender (P=0.002). The protective factors obtained were absence of seizures (P=0.028) and not being bilingual (P=0.045). Conclusion Male gender is a risk factor while non-bilingual is protective for speech-language disorders in children.

Abstract

Low birth weight (LBW) is still a magnitude problem in Indonesia with a multifactorial causes. Studies revealed that one of cause of LBW-infants birth is mother's behavior in treating her pregnancy, including traditional pregnancy care practices. The purpose was to develop the structural model on community setting that might describe the LBW infants-birth. A case-control study on 50 post-partum mothers with LBW and 50 with normal birthweight infants was held in Kupang, East Nusa Tenggara, Indonesia, November 2016 - May 2017. Mother who have spontaneous birth, singleton, and received integrated ANC were inlcuded. Several data such as maternal characteristics, pregnancy knowledge, perceived behavioral control (PBC), maternal health status, obedience to traditional pregnancy care, were collected using questionnaire and medical records tracing. Statistical analysis was performed using path analysis from SmartPLS 3.2.7 and considered significant path when the coefficients were not zero with P value <0.05. Outer and inner model analysis showed two significant paths, both are come from maternal characteristic that influence the LBW infants-birth through pregnancy knowledge (0.489, P=0.000) and PBC (0,425, P=0,000). In the first path, PBC affect maternal health status (0.217, P=0.021) which in turn will cause LBW (0.201, P=0.001). The second path, PBC affect the obedience to traditional pregnancy care (0.474, P=0.000) and then influence to LBW (0.316, P=0.000). As conclusion, maternal characteristics, pregnancy knowledge, PBC, maternal health status, and obedience to traditional pregnancy care are key points that might be used as a significant variables in preventing LBW-infants birth.
