Abstract

Jaminan Kesehatan Nasional (JKN) merupakan program pemerintah yang bertujuan memberikan perlindungan kesehatan bagi seluruh masyarakat Indonesia, termasuk peserta mandiri yang bukan penerima upah. Namun, tidak seluruh peserta mandiri secara konsisten membayar iuran setiap bulan. Penelitian ini bertujuan untuk menganalisis kemampuan dan kemauan membayar iuran JKN pada peserta mandiri di Kecamatan Tanjung Morawa. Desain penelitian yang digunakan adalah kuantitatif dengan pendekatan cross-sectional. Teknik pengambilan sampel dilakukan secara stratified random sampling dengan jumlah responden sebanyak 104 orang. Data diperoleh melalui kuesioner dan dianalisis menggunakan uji chi-square. Hasil analisis menunjukkan bahwa tidak terdapat hubungan yang signifikan antara pendapatan dengan kemampuan membayar iuran (p = 0,386), tetapi terdapat hubungan yang signifikan antara pengeluaran rumah tangga dengan kemampuan membayar (p = 0,034). Sementara itu, kemauan membayar iuran JKN memiliki hubungan yang signifikan dengan normatif tarif (p = 0,001), persepsi terhadap mutu pelayanan kesehatan (p = 0,003), dan riwayat penyakit katastropik (p = 0,022). Oleh karena itu, dibutuhkan pendekatan kebijakan yang menyeluruh, mencakup penyesuaian tarif yang proporsional, peningkatan kualitas pelayanan, dan edukasi berkelanjutan untuk memperkuat partisipasi peserta mandiri serta keberlangsungan program JKN

Abstract

Latar Belakang: Tingkat kesehatan anak di Indonesia masih tertinggal dibandingkan negara-negara Asia Tenggara. Berdasarkan laporan WHO 2020, indeks perkembangan dan kesehatan anak Indonesia berada di peringkat 117 dari 180 negara, jauh di bawah beberapa negara tetangga.Tujuan: Penelitian bertujuan mengidentifikasi faktor internal dan faktor eksternal yang mempengaruhi tingkat partisipasi ibu balita ke posyandu di wilayah kerja puskesmas belawan.Metode: Penelitian ini menggunakan pendekatan kualitatif dengan jenis penelitian deskriptif evaluatif. Penelitian dilaksanakan pada bulan Maret sampai April wilayah kerja Puskesmas Belawan Kota Medan Sumatera Utara. Pengambilan informan dilakukan dengan mengunakan menggunakan teknik purposive sampling. Pengumpulan data dilakukan melalui wawancara dan observasi, yang divalidasi menggunakan triangulasi sumber. Analisis data dilakukan dengan pendekatan SWOT.Hasil: Analisis ini menunjukkan bahwa strategi yang tepat untuk meningkatkan partisipasi ibu balita berada pada kuadran II SWOT (Weaknesses-Opportunities/WO). Skor IFAS menunjukkan kelemahan lebih dominan dibandingkan kekuatan (0,23), sementara skor EFAS menunjukkan peluang lebih besar dibandingkan ancaman (0,29).Kesimpulan: Menerapkan Strategi Turnaround (WO), yaitu memanfaatkan peluang eksternal untuk mengatasi kelemahan internal. Dengan starategi ini, diharapkan kehadiran ibu balita meningkat, sehingga Posyandu dapat berfungsi optimal dalam mendukung tumbuh kembang anak.

Abstract

The perception of primary service personnel towards the reward plan initiated by the Head of Section at the North Sumatra Provincial Health Office is important in understanding the effectiveness of work motivation policies in the government environment. This study aims to determine how primary service personnel view and assess the reward plan initiated by leaders as an effort to improve employee performance. This study uses a qualitative method with a descriptive approach. There were five informants, who were selected using purposive sampling based on certain criteria such as length of service, knowledge of the reward program, and willingness to provide information. Data were obtained through in-depth interviews, non-participatory observation, and documentation, then analyzed using the Miles and Huberman model, which includes data reduction, data presentation, and conclusion drawing and verification. The results of the study show that most employees have a positive perception of the reward plan because it is considered to increase work enthusiasm and productivity. However, some also expressed concerns that an overly competitive system could create pressure and reduce motivation. The success of the reward program is highly dependent on fairness, transparency of assessment, and empathetic communication between leaders and staff. In conclusion, the fair and transparent implementation of rewards can increase the motivation and loyalty of primary service personnel in supporting the achievement of organizational goals at the North Sumatra Provincial Health Office.

Abstract

The involvement of early-stage students (Level 1 PPDS) in emergency services often creates a dilemma between the need for learning and the demands of service efficiency. Therefore, a teaching hospital (RS X) implemented a policy limiting the involvement of Level 1 PPDS in the Emergency Department (ER) to maintain patient safety and service quality. This study aims to examine the implementation of this policy and its impact on the clinical learning process and the workload of medical personnel. The method used was qualitative descriptive through in-depth interviews and observations with the Head of the Emergency Department (UFP), Head of the ER, DPJP, and PPDS. Data were analyzed thematically to identify patterns in the experiences and perspectives of the informants. The results showed that this policy increased the effectiveness and safety of services, but also limited direct practical experience for Level 1 PPDS and increased the supervisory burden for senior PPDS and DPJP. Nevertheless, the implementation of the principle of staged responsibility and close supervision still supported safe and directed clinical learning. Overall, this policy is considered effective in maintaining a balance between service and education, although it needs to be balanced with strengthening the supervision system and simulation-based learning to maintain the quality of clinical education.

Abstract

Gaya kepemimpinan sendiri memiliki berbagai bentuk, terdapat tiga tipe gaya kepemimpinan, yaitu otoriter (authoritarian), demokratis (democratic) dan laissez faire. Ketiga gaya kepemimpinan ini masing-masing dapat memengaruhi motivasi kerja secara berbeda. Tujuan Penelitian ini untuk mengetahui hubungan gaya kepemimipinan dengan motivasi kerja pegawai di BKKBN Provinsi Sumatera Utara. Metode penelitian ini menggunakan kuantitatif dengan pendekatan cross sectional. Sampel dalam penelitian ini berjumlah 79 responden dengan menggunakan teknik pengambilan sampel. Analisa data menggunakan analisis univariat dan bivariat menggunakan uji Korelasi Spearman Rho. Terdapat hubungan yang signifikan antara gaya kepemimpinan dengan motivasi kerja pegawai di Perwakilan BKKBN Provinsi Sumatera Utara. Hasil penelitian juga menunjukkan bahwa hubungan tersebut bersifat negatif dengan kekuatan lemah, yang berarti semakin tinggi penerapan gaya kepemimpinan tertentu, maka motivasi kerja pegawai justru cenderung menurun. Hal ini mengindikasikan bahwa ada faktor pemicu seperti gaya kepemimpinan otokratis dan gaya laisses fair. Kepemimpinan yang diterapkan kemungkinan kurang sesuai dengan kebutuhan psikologis pegawai, sehingga meskipun dinilai tinggi secara formal, gaya tersebut tidak mampu memicu motivasi secara optimal. Oleh karena itu, disarankan agar pimpinan lebih menyesuaikan gaya kepemimpinan dengan karakteristik, kebutuhan, dan aspirasi pegawai untuk meningkatkan motivasi kerja secara lebih efektif.

Abstract

Arterial hypertension is highly prevalent in the elderly, adult andadolescent population. The results from a doctor's diagnosis in a population over 18 years of age in Indonesia found a hypertension prevalence rate of 8.4%. Sukamakmur is one of the villages in Binjai District, Langkat Regency, which has 59 people suffering from hypertension in 2019 and is one of the 3 biggest diseases in there. This study, therefore, aimed to determine the determinants of hypertension in Sukamakmur Village. This research was a quantitative analytic study with a cross sectional design with70 samples of 70 householders taken by purposive sampling. Univariate and bivariate (chi-square) statistical analyzes were performed by means of computerization. The findings of the study showed that the incidence of hypertension was 10%. However, there was no significant relationship between smoking habits (p=0.704, 95% CI=0.245-8.009), and age category (p=0.107, 95% CI=0.582-45.001) in the incidence of hypertension. People who experienced an increase in blood pressure were as many as 7 people (10%) and those who experienced an increase in blood sugar were as many as 3 people (4.3%). The results of this study were in accordance with village data which states that hypertension is higher than diabetes.

Abstract

The National Health Insurance (JKN) program, administered by BPJS Kesehatan (Social Security Agency for Health), is one of the government's efforts to ensure access to healthcare services for all. However, its implementation still faces various obstacles, one of which is pending claims. This study aims to analyze the causes of pending BPJS Kesehatan claims at the UPTD (Regional Government Hospital for Lung Disease Control) in North Sumatra Province and identify the dominant factors influencing the delay in claims settlement. This study used a descriptive quantitative method with secondary data obtained from BPJS Kesehatan claims reports and hospital financial data for the period January to July 2025. The data analyzed included the number of claims submitted, eligible claims, pending claims, and the categories of causes for pending claims. The results showed that the total number of claims submitted during this period reached 1,289 files, consisting of 1,224 outpatient claims and 65 inpatient claims. Of these, 77 claims (5.9%) were pending. The main cause of pending claims is the inconsistency of coding in class rates, 92.4% for outpatients and 82% for inpatients due to changes in hospital class rates, while the rest is caused by incomplete files and medical indications. Therefore, the right solution is to implement training activities to improve human resource competency. As a form of intervention, steps that can be taken are to develop Standard Operating Procedures (SOPs) for BPJS Kesehatan claims so that task implementation becomes more focused, uniform, and in accordance with applicable regulations to reduce the number of pending claims and improve hospital financial efficiency.

Abstract

Background: The condition of the management function (POAC) is one of the causes of declining employee performance in service organizations. Therefore, it is necessary to pay attention to the management function (POAC) in order to have a positive impact such as increasing employee performance at the Public health center Objectives: This study aims to determine the relationship between the management function (POAC) and employee performance at the Langga Payung Health Center.Research Metodes: The type of research used is quantitative with a cross sectional study design with a sample of 95 respondents. Data was collected by filling out a questionnaire. Data analysis was univariate and bivariate (chi square). Findings: The results showed that in general the management function (POAC) was categorized as good and the employee performance was classified as good. Meanwhile, the bivariate test proved the significance of the relationship only on the controlling function (p=0.008). Conclusion: There is no significant relationship between planning, organizing and actuating management functions with the performance of Langga Payung Health Center employees. And there is a significant relationship between the controlling management function and the performance of the Langga Payung Health Center employees. This means that respondents cannot prove that the Planning, Organizing and Actuating variables have a relationship with employee performance in this study and does not rule out other variables that can affect employee performance other than the management function at the Langga Payung Health Center.

Abstract

Background: Utilization of HIV testing during pregnancy is one of the policy efforts to prevent pregnant women from contracting the HIV virus or transmitting it to the unborn fetus. Deli Serdang has always been in second place in the district with the most HIV cases over the last 3 (three) years in North Sumatra. On the other hand, HIV testing by pregnant women in this district is relatively low, with a number of factors assumed to be the cause.Objective: This study aimed to investigate several factors with the use of HIV testing by pregnant women so that strategic efforts can be obtained to prevent HIV transmission by pregnant women.Methods: This research adopts a quantitative approach and employs a cross- sectional study design. The data collection period spans from June to July 2023, and the study was conducted within the operating area of in 5 (five) Primary healthcare areas with the most HIV cases in Deli Serdang regency. The sample was obtained using a cluster random sampling technique so that 285 pregnant women were obtained. Data were collected using a questionnaire that had been tested for validation and reliability, and then analyzed univariately, bivariately using the Chi-Square test, and multivariately using Binomial logistic regression.Results: This study found a significant relationship between the variables of knowledge, social support,  facilities and infrastructure, and the perspective of vulnerability with the use of HIV testing, respectively. Conclusion: The conclusion of this study found that there was a relationship between the utilization of HIV testing by pregnant women in the work area of the Puskesmas with knowledge, attitudes, facilities and infrastructure as well as a vulnerability perspective. Strategic and tactical efforts in the form of appropriate interventions in the provision of supporting facilities and infrastructure desired by pregnant women will enable an increase in HIV testing.

Abstract

Child sexual abuse is increasingly common in many countries and is a global concern. The Multi-disciplinary Teams (MDTs) reported 2,250 children recorded cases involving other forms of sexual violence such as molestation with a range of 561 boys and 1,689 girls. Child sexual abuse is still frequent and unresolved in Thailand. Sex education is essential for health promotion, and media plays an important role in promoting individual health status and prevention efforts. This study aims to educate the prevention of sexual violence through educational game media, namely snakes and ladders for adolescents to see the level of knowledge of adolescents regarding the prevention of sexual violence. This study is a quantitative study using a one group pretest posttest design, namely by conducting a pretest and posttest on 31 students at Thamavitya Mulniti School Thailand. Data on pre-test scores and post-test scores were analyzed using Paired Samples T-test with a 95% confidence level. Students' knowledge was obtained before education and snake and ladder games with an average score of 74.97 and students' knowledge after education and snake and ladder games with an average score of 83.16. The results of the study obtained P-Value 0.006≤0.05 means that at Alpha 5% there is a difference between the average knowledge of Pre-Test and Post-Test. There was an increase in student knowledge between before the intervention and after the intervention. The conclusion of this study is that there is an effect of sexual violence education with snakes and ladders on increasing adolescent knowledge.Keywords: Adolescent, sexual violence, snakes and ladders game
