Abstract

Background: Diabetes Mellitus (DM) is a chronic disease that has the potential to cause serious complications, one of which is peripheral circulation disorders that can lead to diabetic ulcers and even amputation. Early detection of circulatory disorders can be done through the Ankle Brachial Index (ABI) examination. One of the non-pharmacological interventions that is useful in improving blood circulation in the lower extremities is diabetic foot exercises. Objective: to determine the effect of diabetic foot exercises on changes in ABI values in DM patients. Method: The study used a pre-experimental design with a one-group pretest-posttest approach. The sample consisted of 76 respondents with DM in the working area of the Karanganyar Community Health Center. The intervention in the form of foot exercises was given three times in two weeks, while the ABI examination was carried out using a manual sphygmomanometer according to standard procedures. Results: The results of univariate data analysis used frequency distribution, while bivariate analysis was carried out using the Paired t-test and Wilcoxon test. The results of the Wilcoxon test showed a p-value of 0.000 (p <0.05), so Ha was accepted and Ho was rejected. This indicates a significant effect of diabetic foot exercises on improving ABI values in DM patients. Conclusion: Diabetic foot exercises have been shown to be effective in improving peripheral blood circulation and increasing ABI values in DM patients. This intervention can be recommended as a simple, inexpensive, and easy-to-implement non-pharmacological method to prevent foot complications, including diabetic ulcers and amputations.

Abstract

Abstract

Background : The regional government of Nusa Tenggara Timur (NTT) province has made many efforts to accelerate the reduction of stunting prevalence. However, until now, NTT Province still contributes to a high prevalence of stunting. Objective : The study aims to determine the effect of religious figure support and parenting patterns on stunting incidents in the NTT Islands Dry Land Region. Methods : The research was conducted in May - August 2024 in Batakte Village, West Kupang District, Kupang Regency with a sample of 60 mothers of toddlers and 10 informants who were religious leaders. This type of research is quantitative descriptive using case control study design. The collected data were analyzed univariately and bivariately using che square analysis. Results : The results of the study showed that 96.67% of mothers of toddlers were directly involved in the religious figure innovation program and 91.67% of mothers of toddlers had good food and nutrition parenting patterns. The results of the analysis show that support from religious figures and maternal parenting patterns have an influence (pValue = 0.000 < 0.005) on the incidence of stunting. Further analysis showed that toddlers would be at 36 times greater risk (OR = 36; 95%CL = 8.108-59.894) of experiencing stunting if they did not receive support from religious figures and poor maternal parenting factors gave toddlers a 23 times greater risk (OR = 23; 95%CL = 8.108-39.850) of experiencing stunting. Conclusion: support from religious figures and maternal parenting patterns related to food consumption are important factors in preventing stunting. Suggestion: religious figures together with local governments can carry out nutritional advocacy activities, nutritional counseling and improving nutritional institutional governance to support accelerated steps to overcome stunting problems in dry land and island areas of NTT

Abstract

Toxic parenting is a detrimental parenting pattern that can hinder adolescents’ psychological development, particularly in shaping their self-esteem. This parenting style includes excessive demands, emotional neglect, and demeaning criticism directed toward the child. This study aims to examine the relationship between toxic parenting and self esteem among students at SMA Kristen 1 Kalabahi. Employing a quantitative approach with a correlational design, the study involved 215 students aged 14-19 years selected using a quota sampling technique. The instruments used were the Toxic Parenting Scale based on the aspects proposed by Dunham, Dermer, and Carlson and the Self-Esteem Inventory developed by Coopersmith. The results indicated a significant negative relationship between toxic parenting and self-esteem (r = -0.306; p = 0.000), meaning that higher levels of toxic parenting are associated with lower levels of self-esteem. Most students were categorized as having moderate self-esteem (82.8%) and moderate levels of toxic parenting (76.3%). Regression analysis showed that toxic parenting contributed 9.36% to the variance in self-esteem, with the dismissive parents aspect being the most dominant factor in lowering students’ self-esteem. These findings highlight the crucial role of emotional bonding between parents and children in fostering healthy self-esteem, and suggest the need for parenting education interventions to promote healthier parenting practices that support adolescents’ psychological development..

Abstract

Background: Hospitals are institutions that provide comprehensive individual health services. To ensure quality and continuous improvement of these services, hospitals are guided by regulations issued by the Ministry of Health. One key indicator of service quality is patient waiting time at outpatient clinics. Objectives: This study aims to explore several factors affecting service quality at the outpatient polyclinic of RSUD S.K. Lerik, Kupang. These factors include response time of health and non-health workers, hospital budget allocation, worker competence, availability of essential documents, and the overall service flow mechanism. Methods: This qualitative study applied a management theory approach and was conducted at RSUD S.K. Lerik, Kupang. Data were collected from eight informants selected through purposive sampling. Results: The Results of the study showed that that although the response time of health and non-health workers and the availability of service flow mechanisms are relatively good, there are still several challenges. The number of personnel remains limited, and there is no specific budget allocated to the outpatient clinic; instead, needs are recorded and submitted to hospital management. Equipment and materials such as computers and network systems are available, but often insufficient, especially in urgent situations. Despite these limitations, the flow of services generally proceeds without major delays. Conclusions: It is recommended that hospital officers ensure timely service delivery according to established standards to minimize patient waiting times. Improving the management system and increasing attention to service response time in outpatient clinics can help enhance the quality, safety, and comfort of healthcare services provided.
