Abstract

Antibiotic resistance is a global health problem in the world. Antibiotic resistance is defined as the absence of inhibition of bacterial growth by systemic administration of antibiotics with normal doses or minimal inhibitory levels. The World Health Organization (WHO) seeks to control antibiotic resistance globally by making global action plans to combat bacterial resistance to antibiotics, namely increasing the wise use of antibiotics and evaluating the use of antibiotics. The qualitative evaluation that can be used is the Gyssens method. The Gyssens method was chosen because this method has more specific aspects to evaluate each of the important parameters included in the use of antibiotics such as indication, effectiveness, safety, price and spectrum, and can also evaluate treatment duration, dose, interval, and route of administration as well as time of giving. Therefore, this method is the main choice for evaluation of antibiotics in Indonesia.

Abstract

Cervical cancer is a malignancy of the uterine cervix. Cervical cancer is one of the cancers that most often causes death in women in the world, especially in developing countries where one of the main causes is infection with the Human Papilloma Virus (HPV). The incidence of cervical cancer both in Indonesia and in the world tends to increase from year to year. The latest data shows that in 2018 there were 570,000 cases of cervical cancer in the world and in 2013 as many as 98,692 cases of cervical cancer occurred in Indonesia. According to the American Cancer Society, factors that can increase the risk of developing cervical cancer are HPV infection, family history of cervical cancer, use of hormonal birth control types, sexual behavior, age, parity, inappropriate diets that tend to be unhealthy diets and smoking habits. The number of parity of a woman associated with the incidence of cervical cancer. This is because every delivery, the cervix will experience trauma during the remodeling process. Deviations during the remodeling process cause changes in the extracellular components which impact important molecules that act as anticancer, namely E-Cadherin where the content will decrease in the cervical membrane so that the risk of cervical cancer will increase.

Abstract

Hypertension is a systolic blood pressure 140 mmHg or higher and a diastolic blood pressure 90 mmHg or higher.22 In 2020 WHO (Word Health Organization) reported that there were 1.13 billion cases of hypertension and has caused 9.4 million dead.22 At the United States, in 2017-2018 it was reported that 63% people over 60 years old suffered from hypertension with a higher percentage of women (66.8%) than men (58.5%).2 According to the 2018 RISKESDAS results patients with hypertension in the elderly amounted to 63.5%. Due to its high prevalence, hypertension is a major global health problem and the main cause of death and disability in the world every year.12 Treatment of hypertension is carried out for a long time so that the success of hypertension treatment is strongly influenced by self management of patients to adhere to the treatment, routinely checking blood pressure, increasing physical activity, reducing cunsumption of foods containing sodium.11 Self management of hypertension has an important role in effective control off blood pressure in patient with hypertension.3 One of the ways to increase knowledge related to the prevention and control of hypertension is through education.10 The education provided in the form of theoretical materials such as lectures, discussions, counseling, e-learning, and also practices such as workshops or training with various related topics. The materials provided is in the form of facts about hypertension such as definitions, signs and symptoms, risk factors, how to diagnose and treat them, normal blood pressure, hypertension prevention, complications, drug use, drug side effects, the importance of taking medication regulary, the importance of blood pressure checks periodically, diet, physical activity, relaxation techniques such as deep breathing, muscle relaxation, guided imagery.18

Abstract

Potentially inappropriate prescribing (PIP) is a drug prescription without a proper clinical indication. The tools used to identify PIP in the geriatric patient is the STOPP criteria. The STOPP criteria evidently to improve treatment suitability, reduce medical costs, and reduce the occurrence of adverse drug reaction. The aim of this study is to evaluate the potentially inappropriate prescribing in geriatric patients using the STOPP criteria. This study is observational study used a cross-sectional approach. Geriatrics is an age group that is vulnerable to the side effects of drug use. Drug administration in geriatric patients is complex and requires a lot of consideration due changes in body composition and function, comorbidities, sensory and cognitive disorders, and polypharmacy, so that assistance with Potentially Inappropriate Medications (PIMs) and PPOs (Potentially Prescription Omissions) is important to improve the quality of treatment.

Abstract

Tuberculosis (TB) is an infectious disease caused by Mycobacterium tuberculosis, which can affect various organs, especially the lungs. It is transmitted through the air, making it a global health problem. In Indonesia, the number of TB cases is quite significant, indicating serious challenges in handling this disease. In an effort to improve the effectiveness of health services, service innovations need to be developed with a focus on providing comfort for Tuberculosis (TB) patients during the treatment process. This study examines innovations in TB treatment therapy in Indonesia with a focus on understanding, identifying and addressing critical barriers to TB treatment. This research highlights the importance of innovations in TB treatment as a solution to overcome healthcare access barriers, improve efficiency, and reduce geographical barriers. Forms of innovation in TB treatment include diverse approaches to improve treatment effectiveness. Mobile Health (mHealth) provides health access solutions for individuals in remote areas through mobile technology. Digital Health, such as SMS and Voice over Text (VOT), supports patient monitoring and treatment success, requiring government technology infrastructure support. Telemedisin facilitates real-time communication between patients and health workers, minimizing the risk of treatment discontinuation. Innovative programs, such as TOSS-TB and BATAS PETIR, demonstrate success through political, economic, social support, and technology implementation. The use of technology for MDR tuberculosis involves a holistic approach, integrating technology, economic support, and social aspects. Continuous evaluation and adaptation of innovations are key to improving TB control outcomes globally.

Abstract

Hypertension is called the "silent killer" because there are no specific symptoms for sufferers and it is the main cause of death throughout the world. Hypertension or high blood pressure is a condition where systolic blood pressure increases ≥ 140 mmHg and diastolic blood pressure ≥ 90 mmHg. According to WHO (World Health Organization), an estimated 1.28 billion adults aged 30-79 years suffer from hypertension worldwide. Riskesdas data for 2018 states that the prevalence of hypertension in Indonesia is 658,201. The pathophysiology of hypertension occurs through the process of forming angiotensin I into angiotensin II by angiotensin I converting enzyme (ACE). Management of hypertension includes non-pharmacological management and pharmacological management. Some non-pharmacological management measures include weight loss, implementing Dietary Approaches to Stop Hypertension (DASH), adopting a low salt diet, consuming potassium supplements, increasing physical activity, and reducing alcohol consumption. Recommended pharmacological therapy in hypertensive patients such as ACEi or ARB with CCB or thiazide/thiazide-like diuretics.

Abstract

Hyperglycemia is a condition that often occurs in some patients on parenteral nutrition therapy, both with and without a diagnosis of diabetes mellitus. The incidence of hyperglycemia is closely related to an increase in length of stay and mortality in hospital. This literature review aims to provide an overview of the management of hyperglycemia related to parenteral nutrition therapy. The source search method was carried out using the Google Scholar, Pubmed and Elsevier search engines. The keywords used are management of hyperglycemia, risk factors, parenteral nutrition. Based on the results of the literature review, it was found that several factors trigger the incidence of hyperglycemia, including BMI > 25 kg/m2, advanced age, higher HbA1c, history of type 2 DM, use of glucocorticoid drugs and administration of excess carbohydrates and reduced insulin sensitivity. Several strategies to prevent hyperglycemia are carried out by providing low dextrose, hypocaloric parenteral nutrition, or combining parenteral and enteral nutrition. Providing combined nutrition can improve glycemic control in patients with sufficient endogenous insulin stores. Meanwhile, therapeutic measures need to be given if hyperglycemia persists after preventative strategies are implemented. Therapeutic measures that can be taken include administering insulin intravenously or subcutaneously to patients in stable condition. Alternatively, insulin can be added to the parenteral nutrition mixture. It is important to closely monitor blood glucose levels to reduce the risk of hyperglycemia or hypoglycemia.

Abstract

Geriatric patients an elderly patients suffering from a variety diseases (multimorbidity) caused by a decrease in body organ function. Multimorbidity in geriatric patients tends to lead to increased prescriptions and risks of polypharmacy. Polypharmacy can be defined as the use of five or more types of medications by a patient simultaneously. Polypharmacy is a major concern because of negative health impacts on patients, such as increased incidence of adverse drug reactions, harmful drug interactions, decreased medication adherence, and potentially inappropriate prescribing. Collaboration among doctors, pharmacists, other healthcare professionals, and families plays an important role in ensuring the effectiveness of treatment for geriatric patients, thus making the medications used safe and appropriate and reducing the negative impact of polypharmacy.

Abstract

Gejala-gejala gangguan depresi adalah kesedihan, kehilangan minat atau kesenangan, perasaan bersalah atau harga diri yang rendah, gangguan tidur atau nafsu makan, perasaan lelah, dan konsentrasi yang buruk, yang dapat berujung pada bunuh diri. Sebagian besar pasien depresi tidak merespons dengan baik terhadap antidepresan yang saat ini tersedia. Hampir 30% pasien depresi tidak mengalami remisi. Terapi non-farmakologi menjadi alternatif pilihan. Metode yang digunakan adalah literature review dan mengambil data yang bersumber dari Pubmed. Kata kunci yang digunakan, yaitu “Nonpharmalogical treatment of depression” dan didapatkan 191 artikel dan jurnal ilmiah. Sebanyak 7 artikel dan jurnal ilmiah terpilih menjadi sumber acuan. Hasilnya adalah terapi non-farmakologi yang dapat dilakukan, seperti akupuntur, latihan fisik (aerobik, yoga, dan meditasi), pengurangan tidur dan terapi cahaya, terapi kejang magnet, stimulasi intermiten theta-burst, dan stimulasi dalam magnetic transcranial, serta terapi ekspresi.

Abstract

Obesity is a global epidemic health problem and the cases continue to increase every year. Obesity is a predisposing factor for an increase in blood sugar levels. A relatively simple and easy way to prevent obesity and control blood sugar levels is to exercise regularly. One of them is the zumba exercise. During exercise, muscle cells use more glucose and other nutrient fuels than usual for muscle contraction activities. During physical activity, insulin sensitivity increases and causes a decrease in plasma blood sugar levels. This study aims to determine differences in fasting blood sugar levels for those who have exercised for the first time and who have done exercise twelve times in obese women. This research is an observational analytic study with a cross-sectional approach. In this study, fasting blood sugar levels were measured in 52 respondents who took part in zumba exercises at Fitness Center Bandar Lampung. Sampling was carried out on respondents who did zumba exercise for the first time and who had done zumba exercise twelve times. The results showed that the average fasting blood sugar levels for those who had exercised for the first time and who had done exercise twelve times were 98.65 and 91.38 mg/dl, respectively. Based on statistical tests, zumba exercise has a significant difference in changes in the value of fasting blood sugar levels in obese women who have done exercise.
