Abstract

The prevalence of smoking among children and adolescents (school age) in Indonesia continues to increase, even though there have been various regulations such as Permendikbud No. 64 of 2015 and PP 109 of 2012. This fact indicates that schools are not yet able to become a safe and free environment from cigarette exposure. This study aims to analyze the dynamics of school-age smoking prevention policies using the Kingdon's Multiple Streams Framework (MSF), which identifies three major currents in policy: problem, policy, and politics. Through an integrative literature review approach to various scientific sources and policies for the 2021–2025 period, this study found that problem and policy streams have been relatively strong, but political streams are still weak. The lack of political commitment, the strong influence of the cigarette industry, and the lack of ratification of the FCTC are the main obstacles to the opening of the policy window. The proposed strategy includes a softening up approach with a focus on preventing smoking in schools as a form of protecting the future generation of the nation, as well as coupling focusing events by utilizing viral events to suppress public opinion and policymakers. The role of policy entrepreneurs from academics, teachers, NGOs, and educational and health institutions is very crucial in uniting the three policy streams. This study encourages the birth of new policies that are more specific, evidence-based, and sustainable in protecting school children from the dangers of smoking.

Abstract

BACKGROUND: Maternal obesity has been associated with altered fetal development involving metabolic, hormonal, vascular, and epigenetic processes. The leptin (LEP) gene is crucial for placental angiogenesis and fetal growth. However, evidence linking placental LEP promoter methylation status with both leptin levels and umbilical artery vascular resistance in the context of maternal obesity remains limited. Therefore, this study was conducted to investigate the association of placental LEP promoter hypomethylation with increased leptin levels and umbilical artery vascular resistance in maternal obesity.METHODS: A cross-sectional study was conducted in 35 obese and 35 normal-body mass index (BMI) pregnant women delivering at term. Genomic DNA was extracted from placental tissue for the placental LEP promoter methylation examination using bisulfite conversion and CpG pyrosequencing. Umbilical artery systolic/diastolic (S/D) ratio was measured by Doppler ultrasonography, and umbilical cord leptin levels were analyzed from umbilical cord blood using enzyme-linked immunosorbent assay (ELISA).RESULTS: Total LEP promoter methylation did not differ significantly between groups (p=0.252), but five CpG sites (CpG 5, 11, 13, 16, and 17) showed significant hypomethylation in the obesity group. Umbilical cord leptin levels were significantly higher in infants of obese mothers (p=0.002). The S/D ratio was also significantly higher in the obesity group (p<0.001), indicating increased placental vascular resistance. Maternal age, parity, and gestational age were comparable between groups.CONCLUSION: Placental LEP promoter hypomethylation at specific CpG sites (CpG 5, 11, 13, 16, and 17) in maternal obesity is associated with increased leptin levels and elevated umbilical artery vascular resistance, suggesting a potential epigenetic mechanism linking maternal obesity to placental vascular dysfunction and altered fetal development.KEYWORDS: maternal obesity, leptin, DNA methylation, placenta, umbilical artery Doppler, fetal programming

Abstract

Anemia among adolescent girls remains a major public health problem, largely due to poor adherence to iron supplementation programs. According to the 2024 Nanggalo Public Health Center report, SMP IT Dar El Iman Padang had the highest rate of anemia among adolescent girls in the area. This community engagement project aimed to improve knowledge and adherence to iron tablet (TTD) consumption through the TERATAI (Teratur Minum Tablet Tambah Darah, Tolak Anemia) program. The activity was conducted on August 25, 2025, at SMP IT Dar El Iman, Padang, involving 114 seventh-grade female students. Interventions included interactive health education, educational video screening, poster distribution, appointment of anemia ambassadors, and training on the use of Google Forms for monitoring TTD consumption. Evaluation was carried out through pre- and post-tests to assess knowledge improvement and through a four-week monitoring of adherence. Data were analyzed using the Wilcoxon Signed Rank Test with a significance level of p<0.05. The mean knowledge score increased from 65.4 ± 9.2 to 82.1 ± 8.5 after the intervention (p<0.001), representing an average improvement of 25.5%. TTD adherence rose from 58.7% to 86.4% by week four. Furthermore, 91% of participants reported that the activity was engaging and beneficial, and teachers observed greater student motivation to take TTD regularly. The TERATAI program effectively improved students’ knowledge and adherence to TTD consumption. This school-based educational approach, supported by healthcare professionals and teachers, demonstrated positive impacts and is highly replicable as a sustainable model for adolescent anemia prevention programs.

Abstract

Gerakan Pekerja Perempuan Sehat Produktif (GP2SP) merupakan program nasional yang bertujuan meningkatkan derajat kesehatan dan produktivitas pekerja perempuan melalui berbagai intervensi kesehatan di tempat kerja, meliputi aspek gizi, kesehatan reproduksi, pencegahan penyakit, serta dukungan bagi ibu hamil dan menyusui. Meskipun memiliki peran penting, implementasi GP2SP di tingkat perusahaan belum terintegrasi secara optimal ke dalam sistem organisasi, sehingga belum menjadi bagian dari budaya dan mekanisme kerja yang berkelanjutan. Penelitian ini bertujuan untuk mengeksplorasi faktor-faktor yang menyebabkan belum terintegrasinya GP2SP dengan sistem perusahaan. Penelitian menggunakan desain mixed- methods dengan pendekatan kuantitatif dan kualitatif. Studi dilakukan di dua perusahaan di Provinsi Riau pada periode Agustus–Oktober 2025. Data kuantitatif diperoleh melalui survei terhadap 104 pekerja perempuan, sedangkan data kualitatif dikumpulkan melalui wawancara mendalam dengan empat informan kunci. Analisis data kuantitatif dilakukan secara univariat dan bivariat menggunakan uji chi-square, sedangkan analisis kualitatif dilakukan melalui proses coding, kategorisasi, dan penarikan tema. Hasil penelitian menunjukkan bahwa implementasi GP2SP belum terintegrasi secara sistemik dalam manajemen perusahaan. Meskipun telah ditetapkan melalui kebijakan internal, program belum terinstitusionalisasi pada tingkat struktural, operasional, maupun kultural. Penugasan personel yang bersifat non-struktural, rendahnya pemahaman karyawan, serta minimnya partisipasi berpotensi menjadikan GP2SP sebatas pemenuhan administratif tanpa memberikan dampak nyata terhadap kesehatan dan produktivitas pekerja perempuan.
