Abstract

The purpose of this study was to determine the level of implementation of the flexibility of PPK-BLUD in Sukamara Health Center and the obstacles faced in the implementation process. This study adopts the concept of New Public Management which is translated by the government through the establishment of a Regional Public Service Agency system that aims to improve the quality of services to the community through financial management flexibility. The research method used in this research is descriptive qualitative with a case study approach. Data collection techniques used are Observation, Interview and Documentation. The results show that the flexibility that has been applied is the flexibility of income, expenditure, cash management, accounts receivable, goods management, human resource management (hr), remuneration, cooperation, reporting and accountability. While the flexibility that has not been implemented is the flexibility of debt, investment, SiLPA, service tariff. This is due to limited human resources in managing finances, the lack of local government regulations regarding technical guidelines for implementing BLUDs and the lack of synergy between stakeholders in optimizing the implementation of BLUD flexibility

Abstract

The Hospital Management Information System (SIMRS) is a vital tool in supporting efficient, integrated, and data-driven healthcare services. This study aims to evaluate the implementation of SIMRS at RSUD Kabupaten Mimika using the Human, Organization, and Technology – Fit (HOT-Fit) Model. The model assesses the alignment and readiness across the dimensions of technology (input), human and organizational processes, and the resulting system benefits (output). This research employed a qualitative approach using in-depth interviews, observations, and document analysis. Data were collected from seven key informants across primary service units and three triangulated informants from the quality management, human resources, and training divisions. Thematic analysis was conducted based on the HOT-Fit framework. The findings indicate that in terms of technology, the system still faces technical issues such as slow performance, limited data correction features, and the absence of a fully realized paperless culture. In the human dimension, user competence remains uneven, with limited ongoing training and no standardized system for assigning SIMRS responsibilities. From the organizational perspective, there is a lack of standardized SOPs, weak inter-unit coordination, and the absence of a dedicated SIMRS management team. Nevertheless, from the benefit/output dimension, SIMRS has contributed positively to increasing hospital revenue and service efficiency, although data utilization for decision-making remains limited. This study recommends strengthening internal policies, implementing continuous training, upgrading technology infrastructure, and conducting regular evaluations to ensure the sustainability and optimization of the system. The findings may serve as a policy reference and practical insight for other hospitals implementing or planning to implement SIMRS

Abstract

This study investigates the impact of service separation, medical service quality, and polyclinic facilities on patient satisfaction in a private hospital in Tangerang. Based on a survey of 102 respondents, all three variables significantly affect satisfaction. The t-test results show significant effects for each variable (t = 4.127 to 7.695, p < 0.001), and the F-test confirms the model's overall significance (F = 33.900 for BPJS; F = 16.737 for non-BPJS, p < 0.001). Medical service quality and facilities are the most influential factors. Non-BPJS patients report higher satisfaction levels. Abstract

Electronic Medical Records (EMR) allow for the digital management of patient information, replacing traditional paper-based methods. “X” Primary Clinic faces challenges in medical and administrative efficiency, particularly regarding service time, which is a common patient complaint. This study aims to optimize EMR utilization as a solution to enhance work effectiveness. A qualitative descriptive research approach with focused interviews was employed. The study population consists of employees at “X” Primary Clinic involved in EMR implementation. The purposive sampling technique was used, with 10 informants, including the branch manager, human resources and administration supervisor, general practitioners, nurses, administrative staff, radiographers, laboratory analysts, IT personnel, and medical record officers. Data were collected through observations, interviews, and document analysis. The findings reveal adequate technological infrastructure, although general practitioners still rely on paper-based records. Moreover, EMR training for staff is limited and lacks regular implementation, affecting their ability to fully utilize the system. While management has provided supportive facilities for EMR adoption, routine evaluations and monitoring need improvement. Government support in EMR regulations and oversight is significant; however, network access and training for healthcare facilities require further enhancement. This study concludes that optimizing EMR implementation can improve work effectiveness and healthcare service quality, necessitating regular evaluations, enhanced training programs, and stronger policy support from management and the government to maximize EMR's benefits for medical staff and patients at “X” Primary Clinic

Abstract

Indonesia is one of the countries prone to disasters. Most areas in Indonesia are at high risk of natural disasters. This is due to the geographical, geological, hydrological, social, and demographic conditions of Indonesia. Fast and appropriate first aid for disaster victims is very important to anticipate further severity of injuries. MFR (Medical First Responder) is a trained first responder who first arrives at the scene to provide medical treatment in emergency situations before the victim is taken to a health facility. MFR is part of the public service program in terms of disaster preparedness in each region. The factor that supports success in disaster management is disaster management activities. Emergency training is a training program that aims to provide knowledge, skills, and understanding to SPGDT personnel in responding to emergency situations, so that they can take appropriate steps to protect lives, minimize losses, and restore security and normal conditions after an emergency occurs. This study aims to determine the effect of emergency training on the knowledge, attitudes, and practices of medical first responders in disaster preparedness in Denpasar City. This type of research is quantitative research with 63 respondents. This study used an instrument in the form of a Knowledge, Attitude, Practice of Disaster Management (KAP-DM) questionnaire with 42 questions. The data collection technique used purposive sampling. The data analysis method in this study used the Wilcoxon test using SPSS tools. The results of the bivariate analysis obtained p = 0.01 in knowledge, p = 0.03 in attitude, and p = 0.03 in medical first responder practices after being given emergency training. The results of the study concluded that emergency training has a significant influence on the knowledge, attitudes, and practices of medical first responders in disaster preparedness in Denpasar City.

Abstract

A common issue faced by Puskesmas is the low rate of patient revisit intention, which can impact the quality of healthcare services. One of the factors that may influence this issue is patient trust in the services provided, as well as the physical environment of the Puskesmas, which can create comfort for patients. However, few studies have examined the relationship between patient trust and the physical environment of Puskesmas with revisit intention, especially with patient engagement as a mediating factor. This study aims to examine the influence of patient trust and the physical environment of Puskesmas on patient revisit intention, with patient engagement as a mediator, at Puskesmas “X” , Jambi City. This quantitative study uses a survey design and purposive sampling of 100 patients who have received services at the Puskesmas. Data were collected using questionnaires measured with a Likert scale and analyzed using Structural Equation Modeling (SEM). The results of the study show that patient trust has a positive and significant effect on patient engagement, which in turn influences patient revisit intention. Furthermore, the physical environment of the Puskesmas also has a direct influence on patient engagement and revisit intention. Patient engagement serves as a mediator that links the influence of patient trust and the physical environment of Puskesmas on patient revisit intention. These findings indicate the importance of managing patient trust and the physical environment to enhance patient involvement and their intention to revisit the Puskesmas. This study contributes to policies for improving the quality of healthcare services at Puskesmas to increase patient satisfaction and revisit intention

Abstract

Telemedicine is a breakthrough in health services that allows interaction between medical personnel and doctors without meeting in person. The presence of telemedicine in health services is very beneficial during the COVID-19 pandemic when the physical distancing protocol is implemented. Telemedicine in the regulations set by the Indonesian government allows the implementation of telemedicine services that include consultation, diagnosis, therapy, and even monitoring. Telemedicine has the potential to be a solution to problems in Indonesia as a country with an uneven distribution of medical personnel and a doctor-to-community ratio that is still insufficient, so it is considered a good strategy for health services. The purpose of this study was to determine the benefits of telemedicine as a health service strategy during the pandemic and post-pandemic through a literature review. This study is a literature review obtained from the digital databases PubMed, Google Scholar, and Neliti with the keywords "telemedicine", "telemedicine", "e-health", "teleconsultation", "teleconsultation", "telehealth", and "Indonesia" which are adjusted to the inclusion and exclusion criteria. Based on the results of the study, 10 research articles from 2014-2024 were included. Telemedicine is a health service solution during the COVID-19 pandemic. In the post-pandemic era, telemedicine can still be an option for health services because it is still considered to have meaningful benefits

Abstract

Kesehatan merupakan aspek yang sangat penting dalam kehidupan manusia dan merupakan hak mendasar manusia. Hak tersebut diwujudkan dalam bentuk penyediaan fasilitas kesehatan. Klinik dapat dikembangkan menjadi rumah sakit dengan menggunakan beberapa rancangan strategis yang mencakup beberapa hal, seperti : kerjasama, pembangunan, modal, sumber daya manusia, organisasi, promosi, program, inovasi, dan birokrasi. Rancangan tersebut dapat menjadi panduan bagi klinik dalam menentukan apakah dapat dikembangkan menjadi rumah sakit. Literature review ini bertujuan untuk mengetahui dasar - dasar pertimbangan dalam penentuan pengembangan klinik menjadi rumah sakit. Studi ini dilakukan dengan metode literature review dan mengikutsertakan publikasi dari tahun 2020 - 2024. Pencarian literatur dilakukan menggunakan Google Scolar. Artikel ini menganalisa 5 studi yang memenuhi kriteria inklusi yang telah ditetapkan. Hasil Penelitian ini adalah terdapat beberapa strategi yang dapat dilakukan dalam pengembangan klinik menjadi rumah sakit, salah satunya yaitu product development, yaitu melakukan pengembangan produk dengan cara melakukan penambahan ruang perawatan, seperti kamar operasi, ruang icu dan ruang picu/nicu, melakukan penambahan peralatan medis, seperti inkubator untuk bayi dan ventilator, dan melakukan pengembangan pelayanan khusus, contohnya seperti pelayanan fertilitas. Selain itu juga terdapat berbagai pertimbangan yang dijadikan dasar dalam pengembangan klinik menjadi rumah sakit, antara lain kesulitan dalam mengakses pelayanan kesehatan tingkat lanjut, upaya pemerintah dan legislatif untuk meningkatkan kesejahteraan penduduk, kondisi demografi dan ekonomi penduduk, serta kemampuan sumber daya manusia dan keuangan. Klinik harus memperhatikan faktor internal dan eksternal dan saat hendak melakukan pengembangan menjadi rumah sakit. Kesimpulan penelitian ini adalah proses pengembangan klinik menjadi suatu rumah sakit bukanlah suatu proses yang mudah. Dalam proses perubahan ini membutuhkan kesiapan dan rencana strategis yang baik, baik secara internal seperti organisasi, sistem informasi, anggaran, sarana prasarana dan sumber daya manusia yang kompeten, maupun eksternal seperti geografi, demografi, aspek pemerintah, dan aspek pasar

Abstract

Penelitian ini bertujuan untuk menganalisis perkembangan inovasi dalam manajemen sumber daya manusia (SDM) di rumah sakit dalam kurun waktu 2019-2024, dengan fokus pada peran kepemimpinan dan pengembangan karyawan. Metode penelitian yang digunakan adalah analisis literatur sistematis dengan pendekatan kualitatif. Temuan menunjukkan bahwa rumah sakit telah mengadopsi berbagai inisiatif inovatif, seperti pemanfaatan teknologi canggih, program pelatihan berbasis kompetensi, dan transformasi budaya organisasi. Kepemimpinan transformasional dan program pengembangan karyawan yang komprehensif terbukti menjadi faktor kunci yang mendorong inovasi manajemen SDM. Selain itu, penelitian ini juga mengidentifikasi faktor-faktor keberhasilan implementasi, seperti dukungan kepemimpinan, keterlibatan karyawan, dan budaya organisasi yang kondusif. Hasil studi ini memberikan implikasi teoritis dan praktis bagi akademisi serta manajer SDM dan pembuat kebijakan di sektor kesehatan.

Abstract

Research Objectives: This study aims to analyze the implementation of strategic management in enhancing hospital competitiveness in the digital era, identify critical success factors, and explore the challenges faced by hospitals in their digital transformation process. Research Methods: The study employs a mixed-method approach, combining quantitative methods through surveys of 200 middle and senior managers from 50 hospitals in Indonesia, and qualitative methods through in-depth interviews with 20 hospital executives. Data analysis involves structural equation modeling (SEM) for quantitative data and thematic analysis for qualitative data. Research Results: Effective implementation of digital strategic management has been shown to improve hospital competitiveness through increased service innovation, improved quality of care, and enhanced patient satisfaction. Critical success factors include strong top management support, technological readiness, and a culture of innovation. Key challenges include resistance to change, budget constraints, and digital competency gaps. The study proposes a "Dynamic Digital Capability Framework for Hospitals" (DDCFH) as an adaptive model for digital transformation in healthcare settings. Conclusion: Successful digital transformation in hospitals requires a holistic approach that aligns digital strategies with organizational capabilities and environmental demands. The study emphasizes the importance of digital leadership, change management, and strategic partnerships in navigating the complexities of digital transformation in the healthcare sector.
