Abstract

Kesalahan pengobatan pada layanan kesehatan memberikan dampak yang tidak menguntungkan kepada pasien dan pemberi layanan kesehatan, pada pelayanan farmasi hal ini dapat terjadi saat dibuatnya resep oleh dokter sampai dengan resep diterima oleh apoteker. Penulisan resep secara elektronik diyakini memiliki tingkat kesalahan yang lebih rendah dibandingkan resep yang ditulis tangan. Meski demikian penerapan resep elektronik belum dilaksanakan secara baik dan merata di fasilitas layanan kesehatan di Indonesia, hal ini melatarbelakangi dilakukan review jurnal dengan tujuan untuk mengulas pengaruh resep elektronik terhadap medication error sehingga dapat memberikan gambaran manfaat dari penggunaan resep elektronik. Studi ini menggunakan metode tinjauan literur dan terbagi menjadi empat tahap, yakni mengidentifikasi kata kunci, penentuan kriteria jurnal yang akan dimasukan maupun dikecualikan dalam ulasan, pemilihan jurnal, dan analisis data. Pengumpulan literatur dilakukan melalui basis data elektronik seperti Science Direct, PubMed, Google Scholar, dan Springer Link. Hasil dari tinjauan ini menunjukan bahwa resep elektronik berpengaruh terhadap beberapa penurunan kesalahan pengobatan seperti kesalahan penulisan resep dan penurunan kesalahan pemberian obat, namun untuk beberapa jenis kesalahan, resep elektronik menunjukan tidak ada perbedaan secara signifikan dibandingkan dengan resep manual seperti kejadian kesalahan pemberian obat, kesalahan duplikasi obat, dan salah obat.

Abstract

Glukokortikoid telah digunakan untuk pengobatan banyak penyakit inflamasi dan autoimun salah satunya adalah Systemic Lupus Erythemasous (SLE). Berbagai efek samping glukokortikoid telah diketahui, termasuk infeksi, osteoporosis, gangguan kejiwaan, cedera saluran cerna, katarak, hipertensi, arteriosklerosis, dan sebagainya. Di antara efek samping ini, hiperglikemia merupakan efek sistemik yang lazim dari pengobatan glukokortikoid. Kondisi hiperglikemik yang persisten akan berkembang menjadi diabetes melitus. Kajian literatur ini bertujuan untuk mengetahui faktor-faktor yang mempengaruhi terjadinya Glucocorticoid induced Diabetes Mellitus (GIDM) pada pasien SLE. Pencarian literatur pada studi ini dilakukan dengan database PubMed, DOAJ dan Sciencedirect yang diterbitkan 10 tahun terakhir. Ditemukan sebanyak 639 artikel dan 5 artikel yang memenuhi kriteria peneliti.  Total 5 jurnal yang diperoleh, seluruh jurnal membahas faktor risiko dosis, 2 diantaranya membahas faktor usia, 2 jurnal membahas faktor gangguan ginjal, 3 diantaranya membahas faktor komorbid hipertensi dan hiperlipid, dan 1 diantaranya membahas faktor ras. Secara keseluruhan jurnal, didapatkan kesimpulan bahwa usia lebih tua, penurunan fungsi ginjal, dosis, komorbid hipertensi dan hiperlipidemia, dan ras merupakan faktor risiko terjadinya GIDM pada pasien SLE.

Abstract

Disease cost analysis is an important element in the decision making process of chronic disease, because it can evaluate the cost of illness and illustrate diseases that require an increase in resources for the prevention or therapy. This study aims to determine the total cost of diabetes mellitus (DM), so it can be used as a health fund planning considerations related to the effectiveness and efficiency of medication costs for patients in Yogyakarta Regional Public Hospital. The method used is observational prevalence. The data were obtained retrospectively with patients primary diagnosed and secondary diagnosed of diabetes mellitus with or without complications in the period January to June 2014 in Yogyakarta Regional Public Hospital. The Financial perspective used is the hospitals perspective as health care providers to take into account the direct medical costs by using a bottom-up approach. Data analysis was performed using Kruskal-Wallis and Mann-Whitney Whitney to determine differences in the direct medical costs of outpatient diabetic patients in terms of age, the number and type of diabetes complications, as well as to determine the direct medical cost differences hospitalized diabetic patients in terms of age, the number of complications, type of diabetes, duration of hospitalization and inpatient classes.The results of the research showed that the patients who entered the inclusion criteria as much as 1.689 episodes of visits, which consisted of 1.531 episodes of outpatients visits and 158 episodes of inpatients visits. The total cost of DM based on hospital perspective Rp1.203.799.389,00 IDR within six months. There is a significant difference in direct medical costs in outpatients in terms of the number of factors of complications (p=0.004) and a significant differences direct medical costs inpatients in terms of the number of factors of complications, length of stay and inpatient class in hospitalization (p=0.000).

Abstract

Background: Congestive heart failure is a progressive disease that causes a decreased quality of life, increases the need for medical expenses, and has a long treatment time. The purpose of this study is to analyze the cost of treatment and see the utility index in patients with congestive heart failure at RSUD Dr. Moewardi Surakarta. Method: The research method used a cross-sectional design, data were collected retrospectively from medical records, financial data, and pharmaceutical installations to see data on direct medical costs and utility data using the EQ-5D-5L questionnaire during March-May. The subjects of the study were 78 outpatient heart failure patients at RSUD Dr. Moewardi Surakarta with inclusion criteria, namely heart failure patients aged > 40 years and complete medical record data including age, sex, comorbidities, degree of severity, and drug therapy. Results: Data were analyzed using the Man Whitney U test and the Kruskal Walis H test. The results of this study showed direct medical costs, namely pharmacy (Rp. 2,837,328.00), service fees (Rp. 150,000.00), medical procedures (Rp. 89,717.95), and administrative fees (Rp. 10,000.00). Total direct medical costs Rp. 3,087,045.95. The results of the EQ-5D-5L questionnaire based on each dimension were 78.2% no problems with walking ability, 97.4% no problems with self-care, 59.0% no problems with activities, 34.6% little had problems with pain, and 57.7% had no problems with anxiety/depression. The utility index value is 0.803. Conclusion: The conclusion is that the average direct medical cost is Rp. 3,087,045.95 and the utility index value is 0.803.

Abstract

Hepatitis A is a viral disease with a high potential for morbidity and mortality. It is also the second highest viral disease in Indonesia after hepatitis B. Community’s knowledge regarding Hepatitis A disease dan vaccine is required to support such program for eradication of Hepatitis A. The aims of this study was to measure the knowledge of Indonesian community about hepatitis A disease and its vaccine indicated from knowledge score of correct answer towards questionnaire, as well as to analyze the factors that can influence it indicated from difference of mean of knowledge scores among groups of respondents characteristics. This study used an observational survey research design applying a cross-sectional multi-center approach. The number of sample was 500 respondents located in 5 provinces in Indonesia, namely DIY, Lampung, West Java, Central Kalimantan, and Central Sulawesi. Data collection was carried out using a questionnaire that was modified from the questionnaire in a similar previous study. The results showed that public knowledge related to diseases and hepatitis A vaccine in Indonesia was considered sufficient with the mean score obtained was 75.2% ± 12.2%. Statistical analysis using Mann-Whitney test showed that there were significant differences in score of knowledge between groups on the characteristics of experience of hearing a disease (p=0,000), experience of hearing a vaccine (p=0,000) and the experience of the child being vaccinated (p=0,000). There are 2 items of knowledge that werelack understood by respondents, namely treatment of hepatitis A that cannot be treated with antibiotics and immunizations and hepatitis A is not included yet in the national immunization program in Indonesia. It is concluded that community knowledge regarding hepatitis A disease and vaccine was fair (mean knowledge score between 65 – 80%). However, the government still needs to conduct educational programs for the Indonesian community about disease prevention through the vaccination process and provide counseling about the types of vaccines that are not included in the national health insurance program in Indonesia.

Abstract

Severe acute respiratory syndrome coronavirus 2 merupakan virus pembawa corona virus disease 2019, yang terkait dengan tingginya insiden disfungsi multiorgan dan kematian menginfeksi manusia bergantung pada angiotensin converting enzyme 2 (ACE2) sehingga keamanan penggunaan pengobatan yang bekerja untuk meningkatkan ekspresi reseptor ACE2 seperti ACEI/ARB masih diperlukan pembuktian. Literature review ini dilakukan untuk mengetahui pengaruh ACEI/ARB terhadap kejadian mortalitas dan adverse outcome pada pasien COVID-19. Penelitian menggunakan literatur dari database Scopus, PubMed dan Science Direct. Pencarian literatur dengan kata kunci COVID-19, ACEI/ARB, Antihipertensi, Mortality dan outcome. Hasil penyaringan literatur diperoleh 19 artikel studi yang masuk kriteria inklusi. Ulasan ini menunjukkan hasil yang beragam terkait pengaruh ACEI/ARB terhadap perburukan klinis pasien COVID-19. Namun berdasarkan sejumlah artikel sistematik review dan meta-analisis dengan jumlah sampel yang cukup besar menunjukkan penggunaan ACEI/ARB tidak terkait dengan mortalitas dan perburukan klinis pasien (tidak dikaitkan dengan resiko admisi ke ICU, resiko penggunaan ventilasi mekanis invasif maupun non-invasif, peningkatan resiko rawat inap, serta resiko terinfeksi virus COVID-19). Kesimpulan: Penggunaan ACEI/ARB tidak berhubungan dan tidak mempengaruhi mortalitas serta perburukan luaran klinis pasien COVID-19, penggunaan ACEI/ARB sebelum terinfeksi direkomendasikan untuk dilanjutkan pada pasien COVID-19 untuk efek perbaikan pada kondisi pasien.

Abstract

Drug-drug interactions among children are a getting along concern in health care settings, specifically intensive care units, as sources of adverse drug events that may affect patient condition. Children admitted to pediatric intensive care unit are more prone to drug-drug interactions owing to the diseases and medications complexity. This condition could put the patient at high risk of harm, particularly with his critical condition, so need intense considerations from clinical practitioners to prevent adverse drug events caused by potential drug-drug interactions. This article’s review attempts to explore the important drug-drug interactions among children, including explaining the drug combination, mechanism, and related adverse drug events to help health practitioners recognize it earlier before prescribing the medication. This article’s review explored previous research results from PubMed and Google Scholar as literature resources and PRISMA flow chart as protocol for article selection process. A total of 9 articles discussed comprehensively about the type of drug combinations, mechanism of drug-drug interactions, and associated adverse drug events with significant drug-drug interactions that commonly occurred in children’s patient during the treatment. The drug-drug interaction including midazolam-phenobarbital, cannabidiol-clobazam, Paxlovid-tacrolimus, inhaled fluticasone propionate-lopinavir/ritonavir, rifampicin-warfarin, clofazimine-moxifloxacin, benzatropine-haloperidol, and enalapril-spironolactone. In conclusion, gaining a better understanding of drug-drug interactions among children will empower healthcare professionals to develop useful strategies to recognize, manage, and prevent various types of pharmacokinetic and pharmacodynamic interactions. Especially at different stages in terms of age, physiology, and complexity of the disease in children.

Abstract

Diabetes melitus merupakan salah satu penyakit kronis yang ditandai dengan kadar glukosa diatas normal secara menahun. Indonesia kini menduduki peringkat kelima yang sebelumnya peringkat ketujuh dengan jumlah diabetes melitus sebanyak 19,5 juta jiwa. Edukasi merupakan salah satu bentuk pelayanan kefarmasian oleh apoteker untuk meningkatkan pengetahuan dan kepatuhan pada pasien diabetes melitus. Tujuan penelitian ini adalah untuk melihat pengaruh edukasi dengan penambahan booklet terhadap pengetahuan dan kepatuhan pasien diabetes melitus di Puskesmas Kota Surabaya. Jenis penelitian ini adalah eksperimental dengan rancangan quasi experimental design dengan metode pretest and posttest design with control group. Metode pengambilan sampel menggunakan quota sampling. Penelitian ini dilakukan secara prospektif di Puskesmas Kota Surabaya pada periode Oktober 2024 hingga Januari 2025. Pretest dan intervensi pertama dilakukan pada mingggu ke-1 dan dua minggu berikutkan di minggu ke-3 akan dilakukan edukasi kembali untuk mengingatkan responsden, pada minggu ke-5 akan dilakukan Posttest. Instrumen yang digunakan kuesioner DKQ-24 untuk pengetahuan, kuesioner MARS-5 untuk kepatuhan. Penelitian terdiri dari 90 pasien dengan masing-masing kelompok 45 pasien. Hasil analisis deskriptif menunjukan tidak ada perbedaan antar kelompok pada data karateristik subjek (p>0.05). Hasil analisi bivariat menunjukan adanya pengaruh edukasi pada pengetahuan (p=0.000) dan kepatuhan (p=0.000) sehingga dapat disimpulkan pemberian edukasi menggunakan booklet dapat meningkatkan pengetahuan dan kepatuhan pasien diabetes melitus tipe 2.

Abstract

Background: Dexamethasone is a recommended therapy for severe COVID-19 patients who require hospital treatment. However, its use can affect blood glucose levels in patients, especially patients with diabetes mellitus comorbidity.Objectives: This study aims to determine the clinical outcomes for type 2 diabetes mellitus (DM) patients with Severe COVID-19 comorbidity through dexamethasone as COVID-19 therapy.Methods: This study employed the retrospective cohort an analytical observational research approach. Involving 60 patients as subjects, patients over the age of 18 received dexamethasone at RSUP Dr. Sardjito Yogyakarta between June 2021 and March 2022 and whose RT-PCR results showed severe COVID-19 with comorbidity of type 2 DM were included in the study. The blood glucose profiles derived from the patient's medical record data were the clinical outcome that already checked. The paired t-test was performed to analyze how dexamethasone affected the clinical results of the blood glucose profile.Results: The findings of the study were identified after using dexamethasone for four days, which showed that 78.30% of patients had an increased in blood glucose. Pre and post treatment of dexamethasone for four days had a significant impact on the patient’s blood glucose levels in this study, with an average change in blood glucose levels is 78.32 ± 116.77 (p<0.05).Conclusion: Dexamethasone therapy demonstrates a significant effect on blood glucose levels, resulting in a statistically difference between pre and post values for four days in patients with severe COVID-19 with type 2 DM comorbidity. Consequently, special monitoring needs to be carried out in severe COVID-19 patients with type 2 DM who receive dexamethasone to avoid worsening of the patient’s clinical condition.

Abstract

Background: Augmented renal clearance (ARC) is a common condition in critically ill patients, particularly in intensive care units, characterized by enhanced renal elimination. The accelerated elimination of drugs can significantly impact therapeutic efficacy, potentially resulting in suboptimal treatment outcomes. This review aims to provide a comprehensive overview of drugs impacted by ARC, offering a better understanding of the impact of ARC on drug therapy.Method: A narrative review was conducted to explore previous studies from Scopus, PubMed, and ScienceDirect. PRISMA flow chart was used to guide the article selection process.Result:  A total of 14 articles were comprehensively reviewed and discussed regarding drugs affected by ARC, the impact of ARC on the pharmacokinetic/pharmacodynamic properties of drugs, and their clinical outcomes. Classes of drugs affected by ARC include beta-lactam antibiotics, glycopeptide antibiotics, anticoagulants, and anticonvulsants.Conclusion: Antibiotics are the most frequently reported drugs to be impacted by ARC, followed by anticoagulants and anticonvulsants. The impact of ARC on anticoagulants is inconsistent. ARC reduces free drug concentration, requires a higher dose to achieve the therapeutic target, and is associated with a higher risk of treatment failure. However, no significant differences were found in clinical response and mortality compared to non-ARC.
